Abstract

Pineapple peel (Ananas comosus L. Merr.) contains bioactive compounds such as flavonoids, tannins, and saponins with antioxidant and UV absorption potential. Its utilization as an active ingredient in sunscreen is expected to provide a natural, effective, and eco-friendly alternative. This study aimed to formulate a sunscreen cream containing pineapple peel ethanol extract and to evaluate its physical properties and SPF activity. The extraction method used was maceration using 70% ethanol as the solvent. The extract was formulated into four creams: F0 (without extract), F1 (10%), F2 (20%), and F3 (30%). Evaluations included organoleptic test, homogeneity, pH, spreadability, viscosity, and in vitro SPF determination using a UV-Vis spectrophotometer at 290–320 nm. The results showed that all formulations met the physical requirements of the Indonesian National Standard (SNI 16-4399-1996), with homogeneous texture, pH of 4.5–8.0, spreadability of 5.00 ± 0.05 cm, and viscosity of 30.631 mPa·S. SPF values increased with extract concentration, with Formula 3 (32.98%) showing the highest value and classified as maximum protection. The pineapple peel ethanol extract cream formulation meets physical evaluation standards and has potential as a sunscreen.

Abstract

Pineapple peel extract contains bioactive compounds, such as flavonoids and tannins, with photoprotective properties, making it a potential active ingredient in sunscreen formulations. Adding vitamin C, known for its antioxidant properties, increases the SPF value. The aim of this study is to examine the effect of adding vitamin C to the SPF value of pineapple peel extract cream. The method used is a laboratory experimental design, creating 4 cream formulas with 30% pineapple peel extract and varying concentrations of vitamin C in F1 (5%), F2 (7%), and F3 (9%), compared with a control cream that did not contain vitamin C (F0). Analysis using ANOVA showed a significant difference in the SPF values of the pineapple peel extract creams, which were influenced by the varying concentrations of vitamin C, with a p-value of <0.001 (p < 0.05). Duncan’s test further confirmed that the cream formula containing 9% vitamin C achieved the highest SPF, indicating excellent protection against UV exposure. The addition of vitamin C has an effect on increasing the SPF activity of pineapple peel extract cream.

Abstract

Metformin is the most common first-line therapy used in the management of type 2 diabetes mellitus (T2DM). However, therapeutic response to metformin varies between individuals, which is largely influenced by genetic factors, especially polymorphisms in the SLC22A1 gene. This gene encodes the organic cation transporter 1 (OCT1), which plays a role in the transport of metformin into hepatocytes. This literature review used a bioinformatics approach to explore the impact of SLC22A1 polymorphisms on the efficacy of metformin. Genetic and pharmacogenomic data were collected from two major databases, namely PharmGKB and GTEx Portal. Relevant polymorphic variants were identified based on data that were associated with drug response. Tissue expression analysis of SLC22A1 in GTEx was used to evaluate the biological relevance of gene expression in metformin target organs. The results showed that SLC22A1 gene expression was significantly highest in liver tissue, which plays a key role in the efficacy of metformin in T2DM patients. SLC22A1 functions as a major transporter in transporting metformin into hepatocytes, where its pharmacological action occurs. Genetic variations such as rs622342, rs594709, and rs628031 can significantly affect the expression and function of these transporters, which ultimately have a direct impact on the efficacy of metformin therapy. This study emphasizes the importance of pharmacogenomic approaches in personalized medicine for T2DM patients.

Abstract

Information regarding the characteristics of emulsion multivitamin products available in Indonesian community pharmacies remains limited, highlighting the need for evaluation to provide useful information for healthcare professionals and consumers. This study aimed to analyze the characteristics of commercial emulsion multivitamin products based on price, composition, dosage form, and packaging type. A quantitative descriptive study with a cross-sectional observational design was conducted. Ten oral emulsion multivitamin products were selected using purposive sampling and evaluated through direct observation of primary and secondary packaging using a structured observation instrument. Data were analyzed descriptively and presented as percentages. The results showed variations in product prices accompanied by differences in formulation composition and packaging type. Vitamins were the primary components of all products, supplemented with minerals and other supporting ingredients. Emulsion dosage forms were commonly used because of their ease of administration and potential to enhance vitamin bioavailability. Plastic packaging was more frequently used than glass packaging due to its practicality and cost efficiency, although it provides less protection against environmental factors. This study provides baseline information on the characteristics of emulsion multivitamin products available in community pharmacies. In conclusion, these products exhibit considerable variation in terms of price, composition, dosage form, and packaging type, providing a basis for future studies on product quality and stability.

Abstract

Typhoid fever, an infection caused by Salmonella typhi, remains an endemic public health problem in Indonesia, with an estimated 11–20 million cases reported globally each year. Antibiotics are the mainstay of treatment; however, irrational use may lead to bacterial resistance and increased complications. This study aimed to evaluate the rationality of antibiotic use in patients with typhoid fever at TK IV 02.07.04 Hospital, Bandar Lampung, in 2023 using the Gyssens method. An analytical observational study with a cross-sectional design was conducted using 121 medical records of typhoid fever patients from 2023, selected through purposive sampling based on predefined inclusion criteria. Data were analyzed descriptively using the Gyssens method to classify the rationality of antibiotic use (categories 0–VI) and processed using Microsoft Excel. The majority of patients were male (71%), aged 20–35 years (83%), and had a body weight of 61–90 kg (69%). Ceftriaxone was administered to all patients (100%), predominantly at a dose of 1 gram every 12 hours (98%) and for a duration of 5–6 days (76%). Gyssens evaluation revealed that 75% of antibiotic use was classified as rational (category 0), while 25% was irrational, mainly due to excessively short duration of therapy (24%, category IIIB) and inappropriate dosing (1%, category IIA). The use of ceftriaxone was generally consistent with the Indonesian Ministry of Health Regulation No. 28 of 2021; however, deviations related to treatment duration of less than five days and suboptimal dosing frequency contributed to irrational use. Male patients exhibited a higher risk, potentially associated with increased outdoor activities and lower hygiene practices. These findings highlight the need for stricter monitoring to minimize antimicrobial resistance and optimize therapeutic outcomes. In conclusion, most antibiotic use in typhoid fever patients was rational (75%); nevertheless, a substantial proportion (25%) remained irrational due to noncompliance with recommended duration and dosing. Enhanced adherence to national guidelines and improved hygiene education are recommended to support prevention efforts and control bacterial resistance.

Abstract

Cytotoxic compounds are substances that can inhibit the growth of cancer cells, whereas antibacterial compounds inhibit microbial growth. This study was conducted to evaluate the cytotoxic and antibacterial effects of Scurrula ferruginea Jack Danser. The cytotoxic activity was assessed against Artemia salina larvae using the Brine Shrimp Lethality Test (BSLT), while antibacterial activity was evaluated against P. acnes using the agar diffusion method. The results showed that S. ferruginea exhibited cytotoxic activity against A. salina larvae. The tested concentrations were 50, 100, 250, 500, and 1000 ppm. The LC₅₀ value of S. ferruginea against A. salina larvae was 19.49 ppm. The antibacterial activity of the n-hexane extract of S. ferruginea, tested at concentrations of 0.5%, 2%, 5%, 7.5%, and 12%, showed no inhibitory effect against P. acnes (positive control inhibition zone: 20.66 mm). GC-MS characterization revealed that the n-hexane extract of S. ferruginea contained 14 compounds, with lupeyl acetate (44.99%) identified as the dominant compound.
