Abstract

Type 2 diabetes mellitus is a chronic disease that requires long-term management and is at risk of causing acute complications such as hyperglycemia. The treatment of hyperglycemia depends not only on medications, but also on the patient's knowledge in managing the disease. This study aims to determine the relationship between the length of suffering and the knowledge of handling hyperglycemia in patients with type 2 diabetes mellitus in the working area of the Suwawa Health Center. This study uses a descriptive design correlative with a cross sectional approach. The total sample was 75 respondents who were selected using the Proportional Simple Random Sampling technique. The results showed that 44 people (58.7%) of respondents who had suffered for a long <time for 5 years (58.7%), the majority had less knowledge (49.3%), while 7 people (9.3%) were knowledgeable. Respondents with a 5-year ? of suffering were 31 people (41.3%), most of them had good knowledge (40.0%), and only 1 person (1.3%) was poorly knowledged. Based on the spearman rank test, a value of p = 0.000 (p < 0.05) was obtained, so that there was a significant relationship between the length of suffering and the knowledge of hyperglycemia management in patients with type 2 diabetes mellitus in the work area of the Suwawa Health Center. It is hoped that health facilities can improve education from the beginning of diagnosis and maintain continuous education programs so that patient knowledge continues to develop.

Abstract

Boarding time is an important indicator of the quality of emergency room services that describes the length of time from the time the patient is designated for hospitalization until he is transferred to the treatment room. Prolonged boarding time can lead to delays in further treatment, increase the risk of complications, and impact patient safety. This study aims to find out the description of the boarding time of patients in the Emergency Installation of Toto Kabila Hospital. The research design is descriptive quantitative with a single variable boarding time. The population amounted to 1132 emergency room patients, with a sample of 92 respondents using purposive sampling techniques. Data were collected through direct observation and verification of medical records, then analyzed in univariate analysis. The results of the study showed that 53 respondents (57.6%) experienced boarding time ? 6 hours, while 39 respondents (42.4%) experienced boarding time > 6 hours. The main cause of boarding time is the limited number of beds in the inpatient room. Other contributing causes are extra work of nurses, administration and documentation and patient or family factors. These findings show that patient transfers have gone quite well, although some patients are still experiencing delays. Toto Kabila Hospital is recommended to improve boarding time monitoring and improve the bed availability information system. Further research is suggested to develop other variables to gain a more comprehensive understanding of boarding time.

Abstract

This study analyzes the effectiveness of the Polri Super App in improving the quality of public services in Gorontalo City, particularly in digital services such as SKCK, SIM, and STNK administration. Using a qualitative descriptive approach supported by interviews, observations, and documentation, the research applies Duncan's effectiveness indicators—goal achievement, integration, and adaptation—to assess the performance of the application. The findings show that the Polri Super App has facilitated faster, more practical, and easily accessible services, contributing to improved public service effectiveness. However, its implementation remains suboptimal due to persistent obstacles, including additional administrative requirements during SKCK collection, limited public knowledge stemming from inadequate socialization, and unequal digital adaptation among community groups. Younger and digitally literate users adapt more quickly, whereas older, rural, and low-literacy users experience difficulties accessing digital services. Overall, the Polri Super App partially fulfills its intended goals but requires enhanced socialization efforts, improved digital literacy support, and service procedure refinement to optimize its role in strengthening public service delivery in Gorontalo City.

Abstract

Gout arthritis triggers acute pain that interferes with mobility. At the Central City Health Center, there were 459 cases with a tendency for patients to avoid chemical drugs. Warm ginger water foot soaking therapy comes as a nonpharmacological alternative that takes advantage of the thermal and phytochemical effects of ginger. This Quasi-Experimental quantitative research with Non-Equivalent Control Group Design involved 36 respondents who were selected by purposive sampling. Respondents were divided into intervention groups (warm ginger water) and control (warm water). The pain scale was measured using the Numeric Rating Scale (NRS) and analyzed using the Wilcoxon and Mann-Whitney tests. The majority of respondents were men (56%) with an age range of 41-50 years (44%). Both groups showed a significant reduction in pain after the intervention (p=0.000). Although the decline in the ginger group was slightly higher, the intergroup test showed no significant difference in effectiveness (p=0.318). Soaking your feet in warm ginger water and regular warm water are equally effective in relieving gout arthritis pain. This therapy is recommended as a safe and economical independent pain management for the community.

Abstract

Diabetes mellitus is a chronic disease characterized by an increase in blood glucose levels in the long term due to insulin disorders and is a growing public health problem. Prolonged hyperglycemia can lead to a variety of complications, including diabetic neuropathy, through peripheral nerve damage and microcirculatory disorders. Therefore, the length of suffering from diabetes mellitus needs to be studied as a factor that plays a role in the occurrence of diabetic neuropathy to support prevention and early detection efforts. This study aims to determine the relationship between the length of time suffering from diabetes mellitus and the incidence of diabetic neuropathy in patients who are treated at the South Bulango Health Center. The study used a quantitative approach with a cross sectional design and involved 116 respondents who were selected through proportional stratified sampling techniques. The research instrument used was the Diabetic Neuropathy Symptom Score (DNS) questionnaire. The results showed that respondents had diabetes mellitus for a long time <5 years, as many as 48 respondents (41.4%) had diabetic neuropathy and 37 respondents (31.8%) did not experience diabetic neuropathy. Meanwhile, in respondents with diabetes mellitus for a long time ?5 years, most of them had diabetic neuropathy, namely 27 respondents (23.2%), and only 4 respondents (3.4%) did not experience diabetic neuropathy. The results of the statistical test using the chi-square test obtained a p-value of 0.002 (<0.05), which shows that there is a significant relationship between the length of suffering from diabetes mellitus and the incidence of diabetic neuropathy in patients with diabetes mellitus at the South Bulango Health Center.

Abstract

Type 2 diabetes mellitus is a chronic disease that often causes complications of peripheral neuropathy in the form of decreased sensitivity of the legs. This condition increases the risk of diabetic leg injuries to amputation if prevention is not carried out early. One of the nonpharmacological interventions that can be carried out is Active Leg Stretching. This study aims to determine the effect of Active Leg Stretching on foot sensitivity in type 2 diabetes mellitus patients at the Tilongkabila Health Center. This study uses a pre-experimental design with a one group pretest–posttest approach. The study sample was a type 2 diabetes mellitus patient who experienced decreased sensitivity of the legs and was selected using the Purposive Sampling technique. Active Leg Stretching interventions were administered according to standard operating procedures during the study period. Leg sensitivity was measured using monofilament tests before and after the intervention. Data analysis was carried out using the Wilcoxon Signed Rank Test. The results showed an increase in foot sensitivity after being given an Active Stretching Leg intervention with a statistically significant difference (p-value < 0.05). This increase occurs because leg stretching can improve blood flow, reduce muscle stiffness, and improve nerve function, so the sensitivity of the legs increases. It can be concluded that Active Leg Stretching has a significant effect on increasing foot sensitivity in patients with type 2 diabetes mellitus. This intervention is recommended as a simple, safe, and easily applied nonpharmacological therapy in primary health care.

Abstract

This study examines the epistemological challenges in defining Public Administration as an independent discipline amid the dominance of contemporary Public Management paradigms. Since the rise of New Public Management (NPM) and New Public Governance (NPG), the discipline has faced an identity crisis concerning its academic legitimacy and conceptual boundaries. Using a qualitative-descriptive approach, this paper analyzes the paradigm shift of Public Administration in Indonesia and its implications for academic orientation and governance practices. The findings reveal that Indonesia's Public Administration faces challenges at three levels: ontological (the shift from state administration to managerial governance), epistemological (disciplinary legitimacy crisis), and axiological (value transition from public ethics to efficiency). To address these issues, it is necessary to recontextualize Public Administration as a value-driven discipline rooted in public interest, social justice, and civic responsibility. Such an approach would restore the normative foundation of Public Administration while maintaining its relevance alongside Public Management in contemporary governance systems.
