Abstract

Introduction: People with low self-esteem usually judge themselves negatively, feel guilty, feel useless, and feel insignificant. If this is not handled properly and quickly it will cause problems such as committing suicide, injuring others, and damaging the environment. Objective: The purpose of this study is to describe the analysis of nursing care in schizophrenic patients with the application of occupational therapy (free time) to overcome the problem of low self-esteem at Menur Hospital in Surabaya. Methods: The case study design with the subject Mrs. D with low self-esteem problems, was carried out 2 times in occupational therapy (free time) for 30 minutes in 2 days. Data collection techniques were used through direct observation and interviews using mental nursing care sheets. Before and after the application of occupational therapy, an assessment was carried out through direct observation and interviews. Results: The results of the evaluation after occupational therapy showed that there was a decrease in signs and symptoms of low self-esteem and an increase in the ability to increase self-esteem, such as increasing positive self-assessment, feeling worthless, and feelings of being unable to do anything decreased. Conclusions: This case study shows that leisure occupational therapy (flower arranging) can reduce signs and symptoms of low self-esteem. The conclusion from this study is that the application of leisure time occupational therapy can be used to overcome low self-esteem in schizophrenic patients. The role of the nurse is to provide nursing care and education to the patient's family about occupational therapy in their spare time.

Abstract

Introduction: Hemodialysis is a medical procedure that often causes significant physical changes, such as weight loss, swelling, and skin changes that can affect the patient's body image. The long-term process of hemodialysis also has the potential to interfere with psychological conditions, especially patient self-acceptance. Objective: This study aims to determine the relationship between body image and self-acceptance in patients undergoing hemodialysis. Methods: This type of research is quantitative research with a cross-sectional approach. The population of this study were all patients undergoing hemodialysis at Sidoarjo Regional Hospital as many as 200 respondents. The sample size was 133 respondents selected using a simple random sampling technique. The independent variable is body image and the dependent variable is self-acceptance. The instrument of this study was a questionnaire. Analysis using the Spearman Rank test. Results: The results of this study indicate that 54.9% experience negative body image and 41.4% experience low self-acceptance. Based on the results of the analysis obtained a correlation coefficient of 0.601 which means it has a strong relationship with, p-value = 0.000. Conclusions: Patients who have a positive body image tend to show a higher level of self-acceptance of their physical condition.

Abstract

Introduction: Nurses have encountered significant stress, especially during the COVID-19 pandemic. While previous meta-analyses evidenced the efficacy of mindfulness in addressing negative mental states among nurses, the positive effects remain less clear. Objective: This study aims to systematically review previous literature on mindfulness interventions and examine their impact on stress, burnout, mental health, and quality of life among nurses. Design: Systematic literature review following PRISMA guidelines. Data Sources: Seven databases (Web of Science, MEDLINE, Science Direct, CINAHL, Cochrane, Ichu-shi, J-STAGE) were searched for randomized control trials (RCTs), non-RCTs, and qualitative studies published before October 2021 in English or Japanese. Review Methods: The review protocol was registered in PROSPERO (CRD42021269307). Studies were screened, selected, and appraised for quality using Joanna Briggs Institute checklists. Data were extracted and synthesized narratively. Results: Sixty studies met the inclusion criteria. Interventions were heterogeneous in format, duration, and delivery. While overall study quality was moderate, results showed significant correlations with psychological variables. Both positive (e.g., well-being, self-compassion) and negative (e.g., stress, burnout) outcomes were reported. Conclusions: Mindfulness interventions offer potential benefits for nurses, though further research with standardized designs and outcomes is needed. Qualified instructors and structured implementation are essential to ensure effectiveness and safety.

Abstract

Introduction: Chronic obstructive pulmonary disease (COPD) is one of the leading causes of death worldwide. This disease attacks the respiratory tract, resulting in chronic symptoms such as narrowing of the airways, shortness of breath, and coughing. However, the management of treatment for this disease is still very lacking. Therefore, the best treatment needs to be understood. Objective: This review article aims to explain the best treatment for COPD sufferers. Design: This study employed a systematic review design guided by the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) framework. Data Sources: A comprehensive literature search was conducted using four electronic databases—Scopus, PubMed, Science Direct, and Google Scholar—focusing on articles published between 2020 and 2025. Keywords used included "Non-Pharmacology", "COPD", "Chronic Obstructive Pulmonary Disease", "Therapy", "Intervention", and "Nursing". Studies were included if they used Pre-Experimental, Quasi-Experimental, Randomized Controlled Trial, or Qualitative research designs. Review Methods: Two reviewers performed data extraction and risk of bias assessment independently using the Joanna Briggs Institute (JBI) checklist, with discrepancies resolved through consensus or a third reviewer. Results: A review of 19 articles showed that non-pharmacological interventions, such as telerehabilitation, physical exercise, and breathing techniques, have been shown to help improve the functional capacity and quality of life of COPD patients. Conclusions: The combination of physical and psychological therapies has shown more optimal results, especially with the support of technology. However, further research is needed to evaluate long-term effectiveness and ensure wider and more targeted implementation.

Abstract

Background: Rising demand for efficient chemotherapy services calls for improved hospital processes. Lean Hospital applies Lean principles to reduce waste and enhance service quality. Objective: This study aims to assess the available quantitative evidence regarding the impact of lean methodologies on key performance indicators (KPIs) such as cost efficiency, service quality, and patient satisfaction in the chemotherapy unit. Design: A systematic literature review methodology was adopted to collate evidence related to the implementation of lean hospital practices in chemotherapy units using PRISMA guidelines for Scopus, Web of Science, and PubMed searches. Data Sources: This systematic review synthesizes peer-reviewed studies published between 2015 and 2023, retrieved from Scopus, Web of Science, and PubMed databases. Review Methods: A systematic literature search was conducted in Scopus, Web of Science, and PubMed using a combination of keywords and MeSH terms related to lean hospital practices and chemotherapy services. Boolean operators and truncation were applied to refine results, with the search limited to peer-reviewed articles published between 2015 and 2023. Title, abstract, and full-text screenings were conducted based on predefined inclusion criteria. Results: This study included a total of 48 full-text studies for analysis. The findings indicate statistically significant improvements in process efficiency and service quality metrics following lean interventions, with cost savings ranging from 10% to 35% observed across studies. Conclusion: This systematic review shows that Lean Hospital practices effectively improve chemotherapy unit services. Lean interventions significantly reduce costs, enhance service quality, and boost patient satisfaction. Despite some methodological variations, the evidence supports lean methods as a cost-effective strategy for optimizing care delivery.

Abstract

Introduction: Diabetes mellitus (DM) is a metabolic disorder characterized by hyperglycaemia due to abnormalities in insulin secretion, insulin action, or both. Insomnia is a common complication among DM patients and may worsen their overall condition. Objective: This study aimed to examine the effect of sleep hygiene on insomnia in 36 patients with diabetes mellitus at RSU Royal Prima Medan in 2024. Methods: A quasi-experimental one-group pre-test post-test design was employed. A total of 36 DM patients were selected using purposive sampling. Data were collected through questionnaires and analyzed using the Wilcoxon signed-rank test. Results: Before the intervention, 77.8% of patients experienced severe clinical insomnia. Following the sleep hygiene intervention, 97.2% of patients reported moderate insomnia, indicating a significant reduction in severity (p < 0.05). Conclusions: Sleep hygiene intervention significantly reduced insomnia levels in patients with diabetes mellitus. However, the study was limited by its small sample size and single-site setting, which may affect the generalizability of the results. Further research involving larger, multicenter samples is recommended.

Abstract

Introduction: Hypertension and diabetes mellitus are degenerative diseases that increase due to risk factors such as obesity, aging, poor diet, and unhealthy lifestyle. Objective: The research was to assess the effectiveness of the DASH diet in reducing blood pressure and blood glucose levels in hypertensive and diabetic patients. Methods: A single-factor quasi-experimental study with a randomized pretest-posttest comparison group design was conducted on 36 respondents selected through simple random sampling from 305 Posbindu residents in Setonorejo Village. Data collection involved observation sheets, sphygmomanometers, glucose test kits, and the DASH diet SOP. The Wilcoxon Signed-Rank Test and the Kruskal-Wallis Test were used for data analysis. Results: The DASH diet significantly reduced systolic blood pressure (p = 0.006) and blood glucose levels (p = 0.008). However, only the 2600-calorie DASH diet significantly impacted diastolic blood pressure (p = 0.025), while 1600 and 2000-calorie diets did not (p = 0.083; p = 0.059). The Kruskal-Wallis test showed no significant differences between diet groups for systolic blood pressure (p = 0.319), diastolic blood pressure (p = 0.618), and blood glucose levels (p = 0.128). Conclusions: The DASH diet effectively lowers systolic blood pressure and blood glucose levels, suggesting its potential as a non-pharmacological strategy for managing hypertension and diabetes mellitus.

Abstract

Introduction: The trend of increasing hypertension tends to increase and shift to a younger age. Self-management of hypertension is a strategy to help individuals with hypertension change their negative behaviour. The application of hypertension self-management is influenced by predisposing factors such as the level of knowledge and motivation. Objective: This study aimed to analyse the relationship between knowledge and motivation in hypertension self-management with the blood pressure of hypertensive patients. Methods: This study used a correlational study with a cross-sectional approach. Respondents in this study were 146 hypertensive patients who were recruited using the purposive sampling technique. The independent variables in this study were the level of knowledge and motivation in hypertension self-management. The dependent variable in this study was blood pressure. The instruments used were questionnaires and blood pressure observation sheets. Analysis used Kendall’s tau-b statistical test with a significance value of p < 0.05. Results: The results of this study are the relationship between the level of knowledge (p=0.000, r=-0.591) and motivation in hypertension self-management (p=0.000, r=-0.601) with blood pressure. Conclusions: Patients with hypertension correlate with their blood pressure and their level of motivation and knowledge about self-management of their condition. The level of knowledge and motivation is an important value that can help them achieve a more normal blood pressure value.

Abstract

Introduction: Stunting remains a significant public health issue in low- and middle-income countries, including Indonesia, with long-term impacts on child development and national productivity. Family engagement has emerged as a critical factor in transforming maternal health behaviors to prevent stunting. Objective: This integrative review aimed to explore the role of family engagement in promoting behavioral change and maternal motivation for stunting prevention, and to identify evidence-based strategies supported by relevant theoretical frameworks. Design: An integrative review approach was employed to synthesize findings from diverse study designs (quantitative, qualitative, and mixed-methods) following the five-stage framework of Whittemore and Knafl (2005). Data Sources: Relevant literature covering the period from 2018 to 2024 was retrieved from PubMed, Scopus, CINAHL, Google Scholar, and ScienceDirect. Review Methods: A total of 248 records were initially identified. After applying inclusion and exclusion criteria and quality appraisal using the MMAT 2018, 21 studies were included in the final synthesis. Thematic analysis categorized findings based on behavioral outcomes, intervention types, and contextual factors. Results: Four key domains emerged from the synthesis: (1) enhancement of maternal motivation and self-efficacy through family involvement; (2) knowledge transfer via inclusive health education; (3) emotional and social support mechanisms; and (4) culturally adapted interventions. Theoretical models such as the Health Belief Model, Self-Determination Theory, and Theory of Planned Behavior helped explain the motivational processes underlying behavior change. Conclusions: Family engagement strategies are essential in transforming maternal behaviors and preventing stunting. Effective interventions should be context-specific, theory-driven, and inclusive of family and community dynamics. These findings provide valuable insights for designing sustainable, family-centered public health programs.

Abstract

Background: Sexual abuse remains a pressing concern among adolescents, particularly in Indonesia, where rising cases indicate both systemic and educational gaps. Adolescents are developmentally vulnerable and often lack the necessary knowledge to identify, resist, or report abusive behavior. Objective: This study aimed to assess the level of knowledge about sexual abuse among female adolescents attending a public junior high school in Medan, Indonesia. Methods: A descriptive quantitative design was employed, involving 106 female students selected from a population of 376 through proportionate stratified random sampling. Data were collected using validated questionnaires measuring demographic characteristics and knowledge of sexual abuse. The sexual abuse knowledge instrument consisted of 26 items, demonstrating strong internal consistency (Cronbach’s Alpha = 0.879). Data were analyzed using univariate analysis to determine frequency distributions. Result: The results revealed that 79.2% of respondents possessed good knowledge about sexual abuse, while 20.8% had sufficient knowledge. Most participants had received information from parents, schools, peers, and digital media. However, 21.7% reported having experienced sexual abuse, highlighting a significant gap between awareness and real-life prevention. Knowledge was highest regarding physical forms of abuse and lowest concerning non-verbal forms, such as inappropriate gestures or stares. Conclusion: Adolescents demonstrate generally good knowledge about sexual abuse; notable weaknesses persist in recognizing non-verbal abuse and translating awareness into protective behavior. These findings underscore the need for comprehensive, multi-source educational interventions emphasizing practical prevention skills and reporting mechanisms.
