Abstract

Pulmonary TB is the leading cause of death among infectious diseases. Tuberculosis patients who do not comply with taking medication with the dosage rules set by health workers will result in drug resistance or rifampicin resistance, tuberculosis patients who are said to be compliant with taking medication are able to complete treatment for 6 months or until the end of treatment. This study aims to determine drug compliance in pulmonary tuberculosis patients in Pagar Alam City in 2024. The design in this study was an analytic survey with a cross sectional design. The analysis used was univariate and bivariate analysis. The population of this study amounted to 35 people. The sampling method uses total sampling.  The results showed the age variable p value 0.419, Gender p value 0.043 (OR 6,000) (CI 95% 1,274 - 28. 254), Education p value 0.031 (OR 4.667) (CI-95% 1.108 - 19.652), Employment p value 0.203, Knowledge p value 0.004 (OR 12.00) (CI95% 2.374 - 60.648), Family Support p value 0.062, Monitoring of Taking Medication (PMO) p value 0.041, Side Effects p value 0.016 (OR 7.429) (CI95% 1.555 - 35.479). The conclusion of this study is that there is an association between gender, education, knowledge, drug monitoring and side effects with drug compliance of pulmonary tuberculosis patients. On the other hand, there was no association between age, occupation, and family support with drug compliance of pulmonary tuberculosis patients. It is expected that tuberculosis patients pay more attention to their drug consumption compliance during treatment.

Abstract

Aging in the elderly is often accompanied by various health problems, one of which is cataracts. Cataract surgery is an effective solution but often causes anxiety in patients, which can affect their blood pressure before surgery. This study aims to determine the relationship between the anxiety level of the elderly and pre-operative blood pressure in cataract one day care. This research uses a quantitative method with a descriptive correlational design and a cross-sectional approach. The research sample consisted of 40 respondents selected using consecutive sampling techniques. Data were collected using the Amsterdam Preoperative Anxiety and Information Scale (APAIS) questionnaire and blood pressure measurements with a sphygmomanometer. The analysis results showed a significant and positive relationship between anxiety levels and pre-operative blood pressure in cataract one day care with a p-value of 0.000 and a correlation strength (r) of 0.955. High anxiety levels in elderly patients undergoing cataract surgery are associated with increased pre-operative blood pressure. It is crucial for the health professionals to manage patients' anxiety through psychological interventions and education before surgery to reduce the risk of elevated blood pressure that can affect surgical safety.

Abstract

Breast milk is the optimal method to fulfil the nutritional needs of infants for optimal growth and development. However, limited milk production and ejection in the first days after delivery is often a barrier to exclusive breastfeeding. Breast engorgement in breastfeeding mothers is also a common challenge, which can hinder breastfeeding. This study used a case study design with a nursing process approach. The subjects were two breastfeeding mothers over 25 years old who experienced breast engorgement. The interventions provided included a combination of breast care and acupressure, which was carried out for four consecutive days with a duration of 20-30 minutes per session. Data were collected through observation, interview, and cheque sheet. The results showed a significant decrease in the intensity of breast engorgement and an increase in milk production in both subjects. In client I, the frequency of breastfeeding increased from 5 times to 9 times per day, and the frequency of baby's BAK increased from 5 times to 7 times per day. In client II, the frequency of breastfeeding increased from 6 times to 10 times per day, and the frequency of baby's BAK increased from 5 times to 7 times per day. Both clients reported decreased pain and increased comfort while breastfeeding. The combination of breast care and acupressure intervention is effective in overcoming the problem of breast engorgement and increasing milk production in breastfeeding mothers. The researcher suggested the combination of breast care and acupressure as part of breast care management in breastfeeding mothers.
