Abstract

Effective outpatient care is an indicator of hospital service quality and must meet the Ministry of Health’s Minimum Service Standards (MSS), particularly the waiting time requirement of ≤ 60 minutes. However, many healthcare facilities have yet to meet this standard. This study aims to analyze the implementation of Lean Hospital practices in improving service efficiency and compliance with MSS at the Internal Medicine Outpatient Clinic of Sabang City Regional General Hospital. The study employed a quantitative cross-sectional design using observation, time-motion studies, and the SERVQUAL questionnaire, with Spearman and Pearson correlation analyses. Findings revealed an average service time of 87 minutes, exceeding MSS (≤ 60 minutes), with non–value-added activities accounting for 55 minutes and waiting waste at 40%. The Process Cycle Efficiency (PCE) value of 36.7% indicates low efficiency. Patient satisfaction was high (mean 3.43), but higher among those meeting MSS (43.4%) compared to those exceeding it (6.7%). There was a significant relationship between waiting time and satisfaction (r = −0.612; p < 0.05), and between Lean implementation and process efficiency (r = 0.683; p < 0.05). Conclusion: Failure to meet MSS is associated with low efficiency and suboptimal Lean implementation. Improvements should focus on eliminating waiting waste and strengthening kaizen to enhance service quality and patient satisfaction

Abstract

Nurse performance and turnover intention are critical issues in hospital human resource management, influenced by leadership style, job satisfaction, and workload. Objective: This study aimed to analyze the effects of transactional and transformational leadership styles, job satisfaction, and workload on nurse performance and turnover intention at Sabang Regional General Hospital (RSUD Kota Sabang). Methods: A quantitative analytic design with a cross-sectional approach was employed. The study population and sample comprised all staff nurses in the inpatient units of RSUD Kota Sabang using total sampling (n=120). Data were collected using standardized Likert scale questionnaires (1–5), adapted from the Multifactor Leadership Questionnaire (MLQ) by Bass & Avolio, the Job Descriptive Index (JDI), and a validated turnover intention instrument. Multiple linear regression analysis was performed following normality, linearity, and multicollinearity testing. Results: Simultaneously, all four independent variables significantly influenced nurse performance (Sig.=0.037; R²=3.8%) and turnover intention (Sig.=0.032; R²=2.8%). In partial analysis, transformational leadership demonstrated the strongest positive effect on performance (B=0.211; Sig.=0.002) and the strongest negative effect on turnover intention (B=-0.111; Sig.=0.030). Workload was the most dominant predictor of performance decline (B=-0.186; Sig.=0.035), while job satisfaction was the most significant predictor of nurse retention (B=-0.110; Sig.=0.014). Conclusion: Transformational leadership and workload management represent priority interventions for improving nurse performance and reducing turnover intention. The low R² values indicate the need for further research integrating organizational culture, remuneration systems, and individual factors into the explanatory model.

Abstract

Prevalence of acute respiratory infections (ARI) in Indonesia among toddlers in 2024 was 52.7%, with West Jakarta Administrative City ranking second highest in DKI Jakarta Province. ARI is an upper respiratory tract infection influenced by infectious agents and various individual and environmental risk factors. Differences in risk factors have the potential to cause differences in ARI incidence between regions, requiring spatial analysis to identify the geographical correlation of disease incidence. This study aims to analyze the spatial correlation of ARI incidence in toddlers with risk factors between sub districts in the Kebon Jeruk Health Center working area in 2025. The study was conducted from August - September 2025, with a population of 16,344 children with ISPA and a sample of 109 children. Analyzed using Moran's I Global Index and Local Indicators of Spatial Association (LISA) using the GeoDa program. The global analysis revealed no spatial autocorrelation between the occurrence of ISPA in children and all the risk factors studied. However, local spatial analysis identified significant spatial autocorrelation between ISPA cases and the following risk factors incomplete immunization (p-value = 0,015) in Kebon Jeruk subdistrict, smoking habits of family members (p-value = 0,015) in Kedoya Selatan and Sukabumi Selatan subdistrict, and mother education level (p-value = 0,006) in Kelapa dua subdistrict.

Abstract

Job satisfaction among non-civil servant (non-PNS) health workers plays a crucial role in determining the quality of primary health services at community health centers. However, various challenges such as inadequate salary, dual workload, work fatigue, interpersonal relationships, and career uncertainty may influence job satisfaction levels. This study aimed to analyze factors associated with job satisfaction among non-PNS health workers at Primary Health Centers in Aceh Selatan in 2025. This research used a quantitative approach with a cross-sectional design involving 86 respondents selected through total sampling from six health centers. Data were collected using structured questionnaires and analyzed using Chi-Square and Fisher’s Exact Test at a 95% confidence level. The results showed that most respondents reported inadequate salary, high dual workload, work fatigue, and high career uncertainty. More than half of respondents were dissatisfied with their jobs. Statistical analysis indicated that salary, dual workload, work fatigue, and career uncertainty were significantly associated with job satisfaction (p<0.05), while interpersonal relationships were not significantly related. These findings highlight the importance of improving compensation systems, workload management, fatigue prevention, and career clarity to enhance job satisfaction and health service quality.

Abstract

Sexual Infectious Infection (IMS) is a health condition caused by infections that spread through sexual activity. IMS can befall anyone and potentially have serious consequences, especially in reproductive organs. Adolescents who are undergoing a developmental phase with high curiosity about sexual behavior, are susceptible to unsafe sexual practices. The data recorded that 59% of women and 74% of men had sexual intercourse before marriage. This study aims to deeply analyze adolescents’ lifestyles as well as the driving factors of risky sexual behavior with multiple partners in Samarinda City. The approach used is qualitative with a case study design involving 7 informants. Data collection was conducted through in-depth interviews. Research results indicate that multiple partner sex behavior among Samarinda youth has been integrated into their lifestyle, which was formed through a process of normalization by the social environment. Adolescents view sexual activity from physical contact to intimate relationships with various partners as normal in the context of modern relationships.

Abstract

Mental health is an important aspect of human life that often does not receive as much attention as physical health. One of the mental health problems found in society is anxiety disorders. One of its causes is surgery. Preoperative anxiety is caused by fear of surgical procedures and anesthesia. The purpose of this study was to describe of preoperative anxiety in elective surgery patients at AL-FATAH Hospital and Bhayangkara Hospital TK.III Ambon. This study was a quantitative descriptive study and used a cross-sectional approach, conducted from May to June 2025 with 96 respondents, and used a consecutive sampling technique. The research instrument used the APAIS (Amsterdam Preoperative Anxiety and Information Scale) questionnaire. The showed indicate that most elective surgery patients based on the age group 46-55 years experienced mild anxiety 46.7% (7 people), in the gender group women experienced mild anxiety 40.3% (25 people), in the education level group college education level experienced mild anxiety 50.0% (11 people), in the civil servant occupation group experienced mild anxiety 66.7% (10 people), in the elective surgery type group in general the highest percentage of respondents experienced mild anxiety 34.4% (33 people), in the anesthesia type group regional anesthesia type respondents who experienced mild anxiety were more 35.6% (26 people). Suggestions for hospitals are input for efforts to prevent and reduce the level of anxiety in preoperative patients about anesthesia procedures and surgical procedures. Future researchers is recommended to involve a larger sample size and provide more specific classification of elective surgery types.

Abstract

The oil and gas industry is characterized by capital intensity, advanced technology, and a high level of occupational risk. This study aimed to evaluate the effectiveness of Corporate Life Saving Rules (CLSR) training in improving workers’ knowledge at an oil and gas company located in Bojonegoro, East Java. A quasi-experimental design employing a non-equivalent pretest–posttest control group approach was utilized, involving 78 participants divided into an intervention group (39 individuals) and a control group (39 individuals). Statistical analyses were conducted using paired samples t-tests, independent samples t-tests, N-Gain analysis, and multiple linear regression. The findings revealed a significant increase in knowledge within the intervention group, with mean scores rising from 12.41 to 17.28 (p<0.05), while the control group exhibited no statistically meaningful change. Post-intervention comparisons demonstrated a significant difference between groups (p<0.05). The N-Gain value of 65.05% indicates that CLSR training achieved a moderately effective level of knowledge improvement. Categorically, all participants in the intervention group attained a “good” level of knowledge following the training. Regression analysis identified CLSR training as the primary determinant of knowledge enhancement, whereas age, educational background, and length of employment were not statistically significant predictors (p>0.05). In conclusion, CLSR training is empirically proven to be effective in significantly and consistently improving workers’ knowledge in high-risk industrial environments.

Abstract

Degenerative lumbar spondylolisthesis is a common spinal problem seen in everyday practice, especially in older adults, and it often presents with ongoing low back pain, neurogenic claudication, and radicular symptoms. Although decompression surgery is generally considered effective for relieving neural compression, whether fusion should be added at the same time is still controversial, particularly when long-term results and resource use are taken into account. This meta-analysis followed PRISMA guidelines. We systematically searched PubMed, the Cochrane Library, and Google Scholar for randomized controlled trials involving adults with degenerative lumbar spondylolisthesis. The main outcomes were functional disability measured by the Oswestry Disability Index (ODI), along with separate assessments of low back pain and leg pain. Data were pooled in Review Manager 5.4 using fixed- or random-effects models as appropriate, and results were reported with 95% confidence intervals. Across the three included trials, the two surgical strategies performed similarly overall. There were no meaningful differences in ODI scores or in reported back and leg pain. Decompression alone was linked to a slightly higher reoperation rate, but the difference was not statistically significant. In contrast, adding fusion led to longer operative times and longer hospital stays, without providing a clear clinical benefit. Taken together, these findings suggest that decompression without fusion may be a simpler, more resource-efficient choice for carefully selected patients with degenerative lumbar spondylolisthesis who do not have instability or major deformity. That said, the evidence is still limited by the small number of trials and relatively short follow-up. Larger, well-designed studies with longer observation are needed to better determine how durable the outcomes are and what the true reoperation risk is.

Abstract

Cervical cancer ranks among the most common cancers and is a primary contributor to female mortality worldwide, especially in countries with lower and middle-income economies, such as Indonesia. A lasting infection with human papillomavirus (HPV), especially the cancer-causing types HPV-16 and HPV-18, significantly increases the risk of developing cervical cancer. A positive HPV DNA test result indicates a high risk of cervical cell transformation into precancerous lesions and invasive cancer, making HPV DNA testing an important indicator for assessing cervical cancer risk. Therefore, HPV DNA detection serves as a key indicator for identifying the risk of developing cervical cancer. This study aims to describe the determinants of HPV DNA test results among women of reproductive age at Puskesmas (Community Health Center) Kasihan II, Bantul Regency. This study employed a descriptive quantitative design with a cross-sectional approach. The sample consisted of 48 women of reproductive age who underwent HPV DNA testing and were selected using a purposive sampling technique. Secondary data were obtained from medical records and included age at first marriage, smoking habits, number of sexual partners, family history of cervical cancer, parity, and use of hormonal contraception. Data were analyzed using frequency distributions. The results showed that the majority of respondents were HPV-negative across variables such as age at first marriage, smoking habits, number of sexual partners, family history, parity, and hormonal contraception use. However, both negative and positive results require careful attention through strengthened routine screening and reproductive health education as preventive measures against cervical cancer in primary healthcare settings.

Abstract

Lymphatic filariasis remains one of the most neglected tropical diseases, characterized by complex transmission dynamics involving parasites, vectors, hosts, and environmental determinants. Understanding the periodicity of Brugia malayi and the molecular confirmation of potential vectors are crucial for optimizing control and elimination strategies in endemic regions. Objective: This study aimed to characterize the transmission dynamics of Brugia malayi by analyzing its microfilarial periodicity and confirming vector species through molecular identification. Methods: A cross-sectional field study was conducted from April 2017 to May 2018 in Banyuasin District, South Sumatra. Peripheral blood samples from humans and domestic cats, as well as adult mosquitoes, were collected and examined microscopically for microfilariae. Polymerase chain reaction (PCR) targeting the yACA gene of Brugia spp. was used for molecular confirmation. Microfilarial periodicity was assessed through hourly finger-prick sampling over 24 hours and analyzed using the Aikat and Das formula. Results: Two of six human subjects in Sedang Village were positive for Brugia malayi microfilariae with an average density of 150.3 mf/mL, while one cat from Muara Sugih was positive for Dirofilaria repens. PCR detected Brugia malayi DNA in two Mansonia spp. samples (I4 and I8) with 93–95% identity to reference strain MG030487.1. The periodicity exhibited a dual pattern diurnal (peak at 15:00) and subperiodic nocturnal distinct from the typical nocturnal rhythm reported elsewhere in Indonesia. Conclusion: The detection of dual periodicity and molecular confirmation of Brugia malayi in Mansonia spp. highlight potential adaptive transmission patterns influenced by local ecological conditions in Banyuasin’s wetland ecosystems.
