Abstract

The rapid advancement of digital health technologies has transformed the delivery of primary health care (PHC) globally. Digital Health Literacy (DHL) has become a critical competency for PHC workers, particularly in Indonesia, alongside the implementation of SATUSEHAT and the Six Pillars of Health Transformation, which depend on the digital capacity of the health workforce. This scoping review aims to synthesize evidence on: (1) the role of DHL in strengthening PHC workforce capacity; (2) system-level benefits of investing in DHL; (3) barriers to its development; and (4) evidence-based policy recommendations for Indonesia and comparable LMICs. The review followed the Arksey & O’Malley (2005) framework, with searches conducted in PubMed/MEDLINE, Scopus, Frontiers, and WHO repositories. Keywords included digital health/eHealth literacy, primary health care, health workers, and capacity building, while grey literature from the Indonesian Ministry of Health and ASEAN health authorities was also included. Screening and data extraction were conducted independently by two reviewers. DHL improves clinical decision-making, health information management, patient engagement, interprofessional collaboration, and continuous professional development. At the system level, it enhances service quality, efficiency, equity, and health surveillance. Key barriers include limited infrastructure, low baseline DHL, unequal access to training, data privacy concerns, and fragmented training approaches. Five policy recommendations were proposed. Strengthening DHL requires a multi-level strategy integrating training, digital infrastructure support, ethical regulatory frameworks, and a national action plan. Investment in DHL is a fundamental requirement for achieving universal health coverage in Indonesia.

Abstract

Irrational use of antibiotics in the community remains a health problem that can increase the risk of antimicrobial resistance, Low levels of public knowledge are suspected to influence the formation of inappropriate attitudes toward antibiotic use. This study aimed to analyze the relationship between the level of knowledge and community attitudes toward antibiotic use in Bumi Ayu Village, Bengkulu City. This study used a quantitative method with an observational analytic design through a cross-sectional approach. The sample consisted of 110 respondents selected using a proportional cluster random sampling technique. Data were collected using questionnaires that had been tested for validity and reliability, with Cronbach’s Alpha values of 0.702 for the knowledge variable and 0.786 for the attitude variable. Data were analyzed using univariate and bivariate analyses with the Chi-Square test. The results showed that most respondents had low knowledge levels, with 50 respondents (45.5%), and poor attitudes, with 50 respondents (45.5%). The bivariate analysis showed a significant relationship between the level of knowledge and community attitudes toward antibiotic use, with a p-value of 0.000 (p < 0.05). Respondents with better knowledge tended to have better attitudes toward antibiotic use. Therefore, continuous health education through health workers and information media is needed to improve public understanding of rational antibiotic use in order to prevent antimicrobial resistance.

Abstract

Hypertension is a chronic disease commonly found among the elderly and requires long-term treatment with a high level of medication adherence. This study aimed to determine the relationship between family support, knowledge, and sociodemographic factors with medication adherence among elderly patients with hypertension at Idi Rayeuk Health Center. This study used a cross-sectional design. The population consisted of 694 elderly individuals, with a sample of 87 respondents selected using accidental sampling technique. Data were collected through interviews using a questionnaire administered via Google Forms. Data analysis was performed using SPSS version 25, with chi-square test for bivariate analysis and ordinal logistic regression for multivariate analysis (α = 0.05). The results showed that only 52.9% of respondents were adherent to antihypertensive medication. Bivariate analysis indicated that all variables were significantly associated with medication adherence. Multivariate analysis revealed that age, education level, family support, knowledge, and self-efficacy were significantly associated with medication adherence, while gender and marital status were not. In conclusion, family support, knowledge, and self-efficacy play an important role in improving medication adherence among elderly patients with hypertension.

Abstract

Stunting remains a public health problem in Indonesia, including in Gowa Regency. The Genting Program is a collaborative innovation involving Foster Parents (OTA) to assist families at risk of stunting. However, the effectiveness of this program has never been comprehensively evaluated. This study aims to evaluate the implementation of the Genting Program using the CIPP Model (Context, Input, Process, and Product). This study used qualitative research with a case study approach. 15 informants consisted of program managers, local government officials at the village level, cadres, OTAs, and target families. Data were collected through in-depth interviews, observation, and documentation, then analyzed thematically. The results of the study on the context aspect indicate that the program is in accordance with national policies and local needs, supported across sectors with 37 target families. The input aspect shows that there is support from cadres, OTAs, and the government with voluntary funding and adequate infrastructure. The process aspect shows that mentoring activities, nutrition education, and child growth monitoring are running, but are hampered by low community interest in becoming OTAs and limited funds. The product aspect shows that the program improves nutritional knowledge, parenting patterns, awareness of growth monitoring, and children's nutritional status, although sustainability is not yet optimal. This study concludes that the Gentinf Program makes a positive contribution to stunting prevention, but still faces obstacles in terms of community participation and funding.

Abstract

The use of thermoplastic nylon denture bases produces waste that causes environmental pollution. To overcome this environmental pollution, the remaining nylon can be recycled into denture bases. The purpose of this study was to determine the effect of adding recycled thermoplastic nylon to pure thermoplastic nylon on the modulus of elasticity of denture bases. The laboratory experiment research method was carried out in the laboratory of the Dental Engineering Study Program D III, Universitas Megarezky, Makassar and the Mechanical Laboratory of the Ujung Pandang State Polytechnic. This study used Thermoplastic Nylon material, in this case using Thermoset material which was divided into 4 sample groups, namely: pure Thermoplastic Nylon (Sample A), a combination of 75% pure Thermoplastic Nylon with 25% recycled Thermoplastic Nylon (Sample B), a combination of 50% pure Thermoplastic Nylon with 50% recycled Thermoplastic Nylon (Sample C), 100% recycled Thermoplastic Nylon (Sample D). The results of the study showed that the modulus of elasticity with an average value for the pure Thermoplastic Nylon group was 832.97 MPa, the 75% pure Thermoplastic Nylon 25% Recycled Thermoplastic Nylon group was 831.29 MPa, the 50% pure Thermoplastic Nylon 50% Recycled Thermoplastic Nylon group was 830.11 Mpa and the highest was the 100% Recycled Thermoplastic Nylon group, which was 851.44 MPa. In conclusion, the addition of recycled thermoplastic nylon to pure thermoplastic nylon can cause changes in the modulus of elasticity in the form of a lack of flexibility of the denture base.

Abstract

Diabetic ketoacidosis (DKA) is one of the most serious and life-threatening complications of diabetes mellitus (DM). DKA is characterized by the presence of hyperglycemia, metabolic acidosis, and ketosis. One of the precipitating factors of DKA is excessive alcohol consumption. Alcohol intake can adversely affect glycemic control by increasing the production of reactive oxygen species (ROS). Elevated ROS levels may lead to dysfunction and apoptosis of pancreatic beta cells, resulting in glucose intolerance and insulin resistance. This case report describes a 19-year-old female who presented to the emergency department with decreased consciousness, Kussmaul respiration, and shock. Random blood glucose level was 725 mg/dL. Arterial blood gas analysis revealed metabolic acidosis (HCO₃ 1.4 mmol/L, PCO₂ 13.4 mmHg, PO₂ 214.6 mmHg, tCO₂ 1.8 mmol/L, pH 6.64, and SaO₂ 97.8%). Additional laboratory findings included urea of 35.0 mg/dL and creatinine of 1.10 mg/dL. Urinalysis showed urine ketones +2, glucose +3, and leukocytes 5–10 per high-power field. The patient was diagnosed with diabetic ketoacidosis, acute kidney injury, and complicated urinary tract infection. Recommended management of DKA includes correction of dehydration, insulin therapy, correction of electrolyte imbalances, and identification of precipitating factors. Appropriate management can lead to a favorable prognosis in patients with DKA.

Abstract

Extreme environments across land, sea, and air domains expose individuals to challenges such as hypoxia, extreme temperatures, pressure changes, isolation, and physical and psychological stress that may affect health and performance. Human adaptation to these conditions involves complex interactions among biological, psychological, social, and spiritual dimensions. However, adaptation mechanisms are often examined separately, requiring a more comprehensive understanding. This study aimed to review human adaptation mechanisms in extreme environments and their implications for military medicine, preventive medicine, occupational health, and operational settings. This study employed a literature review approach to identify, analyze, and synthesize scientific evidence regarding human adaptation mechanisms in extreme environments. The review included original research articles and relevant scientific reports discussing biological, psychological, social, and spiritual aspects among human populations. Only full-text articles in English or Indonesian were included. Biological adaptation includes ventilation regulation, erythropoiesis, thermoregulation, and cardiovascular-respiratory adjustments. Psychological adaptation through coping strategies and emotional regulation reduces stress, while communication, team cohesion, and leadership support social adaptation. Spirituality enhances meaning, hope, and resilience. Human adaptation in extreme environments is a multidimensional process requiring a holistic biopsychosocial-spiritual approach to improve preparedness, health, and resilience among individuals and groups.

Abstract

Premarital sexual behaviour among adolescents has become a serious public health challenge, particularly with the rising cases of sexually transmitted infections and unintended pregnancies in Samarinda. The objective of this study is to analyse the factors associated with premarital sexual behavioural intentions among students at State Senior High School X Samarinda, considering both demographic aspects and current psychological conditions. The present study employed a quantitative approach with a cross-sectional design, and involved 125 respondents who were selected through a multistage random sampling technique. The data were collected through face-to-face interviews using the DASS-21 instrument, the Self-Harm Inventory, as well as scales for intention and peer pressure. The subsequent analysis employed multiple linear regression. The findings of the study indicated that intentions regarding sexual behaviour in the premarital context were found to be significantly influenced by grade level (p = 0.029) and peer pressure (p = 0.011). In the eleventh grade, students demonstrated a decline in intentions, whilst high levels of peer pressure resulted in an escalation in sexual intentions. It is noteworthy that psychological variables, including depression, self-control, and self-esteem, did not demonstrate a substantial impact in this model. The findings of this study indicate that sexual intention among adolescents in Samarinda is more situational-social in nature, rather than being merely an internal individual issue. It is recommended that educational institutions consider the implementation of enhanced peer education programmes and assertiveness skills training as a means of mitigating the potentially detrimental influences of the social environment.

Abstract

Anemia is a condition when the hemoglobin (Hb) level in the blood is lower than the normal limit. Adolescent girls are at high risk of experiencing anemia due to regular blood loss during menstruation, and iron (Fe) deficiency is the main factor causing iron deficiency anemia, which is a condition resulting from insufficient iron for hemoglobin synthesis in erythrocytes that function to transport oxygen. The aim of this study is to determine the serum iron (Fe) levels and the number of erythrocytes in anemic adolescent girls who receive a free nutritious meal program. The research method used is an analytical observational approach with a cross-sectional design. The population in this study consisted of 80 anemic femal students, with a sample size of 40 anemic female students. The research results show that the iron levels in anemic adolescent girls who received the MBG program averaged 97.14 ± 91.67, and the average erythrocyte count was 4.87 ± 0.54. The results indicate that the average of both variables is still within the normal range. Meanwhile, the results in the group that has not received free nutrition show an average erythrocyte count of 4.73 ± 0.31, while the average iron level is at 47.50 ± 19.39.

Abstract

Due to the growing prevalence of psychiatric conditions over the past few years, the mental well-being of adolescents has emerged as a significant public health challenge. Findings from the I-NAMHS 2023 reveal that over 33% of the adolescent population encountered mental health difficulties during the past 12 months. This condition is influenced by various interrelated determinants, both at the individual and environmental levels. The objective of this research was to examine the various factors influencing adolescent mental well-being in Indonesia through a systematic literature review. A comprehensive search was carried out across Google Scholar, ScienceDirect, and PubMed to identify academic papers written between 2020 and 2025, focusing on quantitative cross-sectional studies involving adolescents aged 10–19 years. Guided by PRISMA protocols, the methodological rigor of the selected literature was assessed via the JBI Critical Appraisal Checklist. Ultimately, from a pool of 1,825 retrieved articles, 21 met the inclusion standards and were subsequently analyzed. The findings indicate that adolescent mental health is influenced by internal factors such as sex, history of chronic illness, genetic predisposition, personality type, and lifestyle, as well as external factors including parenting style, social support, academic stress, experiences of bullying, socioeconomic status, digital media use, and educational environment. This study underscores that teenager mental well-being is determined by multi-level interactions, necessitating multifaceted interventions at personal, family, and community levels.
