Abstract

ABSTRACT The health indicator that assesses the success of achievement of the SDGs is the nutritional status of children under five. Toddlers are a group that is vulnerable to malnutrition, one of which is stunting. Stunting is a condition that describes toddlers as having a body length or height that is less than their age; this condition uses a length or size that is <2 SD. Stunting toddlers in the future will experience difficulties, one of which is achieving optimal physical and cognitive development. This study determines the factors associated with stunting in toddlers aged 24-59 months in the Jambula Health Center Working Area in 2022. This type of research uses a quantitative analytic design with a cross-sectional study design. The population in this study was 285 toddlers aged 24-59 months. The sample size used was 74 toddlers. The sample used a stratified random sampling method with a systematic random sampling technique. The study's results showed a relationship between a history of infectious disease (p=0.002) and the incidence of stunting. There was no relationship between LBW (p=0.463) and exclusive breastfeeding (p=0.560) with the incidence of stunting. The recommendation to mothers with toddlers under five years old is to increase awareness about toddler health, especially about the fulfillment of toddler nutrition. Mothers can prevent stunting and routinely visit health services to get early detection of toddler health.

Abstract

The COVID-19 pandemic has heterogeneously affected the use of essential health services worldwide, with disruptions in some countries beginning in the early stages of the emergency in March 2020. These disruptions have occurred on both the supply and demand sides of healthcare and have often been related to resource shortages to provide care and lower patient turnout associated with mobility restrictions and fear of contracting COVID-19 at facilities. The goal of this paper, compare the HBsAg test before and after the COVID-19 pandemic to a pregnant woman's early detection with HBsAg to achieve the target of at least 80% in 2020. Method using a simple mathematical calculation modeling approach developed to predict health service use until the pandemic using routinely collected health information systems data (SIHEPI). Results show HBsAg reactive Proportion of total pregnant women in each Province is about 2,1% on 47,91% of pregnant women in Indonesia, highest in eastern Indonesia province (NTT, Papua, and West Papua). With a calculation of 80% targeted a pregnant woman in 2020, reactive Proportion predicted about 4,4% in Indonesia, West Papua becomes the highest Province with 21,38%, Papua about 12,63%, and NTT about 11,31%. In conclusion, refered to comparison of mathematical calculation approach from 80% targeted average become 4,4% reactive HBsAg test. Delaying early detection in pregnant women before and after the covid19 pandemic becomes a double burden in Indonesia. These phenomena become a high-risk factor for maternal-to-child transmission. Indonesia needs serious action to achieve Hepatitis B elimination in 2030.

Abstract

Obesity/overweight is a nutritional problem that has a long-term impact on health. An overweight person has a risk of developing diabetes, ischemic heart disease, and cancer. This study aims to analyze the relationship between factors of lack of vegetable and fruit consumption with overweight at the productive age (15-64 years) in the Parung Sub-district. This study uses secondary data, a quantitative type with a cross-sectional study design. The sample of this study is 142 people of productive age (15-64 years) using simple random sampling. Data collection was conducted in conjunction with vaccine surveys in Parung Sub-district conducted by officers and by implementing health protocols. The results showed that there was no relationship between a lack of vegetable and fruit consumption with overweight occurrence. Lack of fruit and vegetable consumption is not the main factor of overweight; many other factors can affect overweight such as genetic factors, lack of physical activity, drugs, or hormonal factors. Genetic factors usually affect 40-50% if one of the parents has a history of being overweight or obese, but if both parents have a history of obesity, it will affect 70-80% of the next offspring. Factors of lack of physical activity lead to less maximal energy expenditure; it is highly recommended to do physical activity at least 30 minutes a day. Drug factors such as steroid drugs that are often consumed in the long term to treat asthma, osteoarthritis, and allergies can lead to increased appetite and increase the risk of being overweight or obese. Hormonal factors commonly associated with obesity are hormones such as leptin, ghrelin, thyroid, insulin, and estrogen, and regular medications with hormonal are often associated with the use of contraceptives such as birth control pills or contraceptive injections.

Abstract

The national strategy for accelerating the reduction of stunting in Indonesia reaches 14% in 2024. One of the provinces with stunting prevalence exceeding the national figure is West Java Province (31%). Stunting toddlers experience failure to thrive when their height is less than their age standard. Nutrition and sanitation factors are key indicators as essential criteria that must be achieved at the family level to prevent stunting. This study aimed to examine the relationship between nutrition and sanitation aspects on the incidence of stunting in children under five in Bogor District. The study used a cross-sectional study on 100 childrens aged 0-59 months living in the Bogor district selected by proportional random sampling technique. Stunting was measured using an anthropometric index (body length/height according to age. Measurement of nutrition and sanitation aspects included: 1) Nutrition indicators, namely balanced nutrition practices, quantitative food intake (food frequency questionnaire), 2) sanitation indicators, including personal hygiene practices and availability sanitation facilities. Sociodemographic characteristics were measured using a structured questionnaire. Data were analyzed univariately and bivariate using SPSS version 22.0. The results showed that practicing balanced nutrition was related to stunting in children under five (p = 0.046). Meanwhile, carbohydrate intake, protein intake, and sanitation were not associated with stunting (p=>0.05). Health promotion related to the four principles of balanced nutrition for mothers of children under five is expected to reduce the prevalence of stunting.

Abstract

People living with HIV and AIDS (PLWHA) are a vulnerable population with psychosocial problems, including changes in status in society and stigma. PLWHA is prone to mental health disorders, including stress and depression, therefore a positive coping mechanism with resilience is needed. This study aims to identify the relationship between sex and marital status with resilience in PLWHA. This research was conducted in Bogor City using a cross-sectional design. The population is PLWHA in Bogor City. Samples were taken randomly from 98 PLWHA. Data were collected using an online questionnaire and analyzed bivariately with the Chi-square test. The results showed that 48% of PLWHA had negative resilience. Negative resilience is shown on the points of ability to cope with stress (36%), belief in finding love (39%), and belief in having children (38%). Meanwhile, high positive resilience is shown in the ability to respect others (79.5%) and believe in their own religion (74.4%). There is a significant relationship between gender (p-value, 0.005; OR=5.5) and marital status (p-value, 0.018; OR=2.9) with resilience in PLWHA. PLWHA who are female and married has a positive chance of resilience compared to male and unmarried sex. Psychosocial assistance activities are needed to increase resilience in PLWHA, especially for PLWHA who are not married and female.

Abstract

Since the Faculty of Medicine and Health Sciences, University of Warmadewa, launched community-oriented medical education (COME), the stunting prevalence in Gianyar Residence decreased from 12.1% (2018) to 11.1% (2019). In 2021, it decreased to 5.1% which was lower than Bali province (10.9% in 2021) and much less than the Republic of Indonesia (24.4%). The PentaCOME (Pentahelix Community-Based Medical Education) project, the modification of COME, was the collaboration of the Indonesian Ministry of Education, Culture, Research, and Higher Education, the Warmadewa University, as well as PT. Sido Muncul is implemented to further lower stunting prevalence via the “Stunting Primordial Prevention Through Prospective Brides Advisory Service” initiative in Payangan District. The present study was intended to figure out barriers to implementing such initiatives. This is a qualitative study using in-depth interviews and focus group discussions (FGDs) in obtaining data. Purposive sampling was used to select informants who represented academicians (3), businessmen (2), community (9), government (3), and media (3) (i.e., Penta helix components). Twenty in-depth interviews and two FGDs involving academician, community, government, and media were conducted during the 1 September 2022 – 15 October 2022 period. Two coders transcribed and coded independently. The social-ecological model was used to categorize the codes. Informants described that they never heard about the ELSIMIL application (ELSIMIL stands for Electronic Siap Nikah dan Hamil which means ‘prepared to get married and pregnant application’) at the personal level. The “No pregnancy, no marriage” motto (i.e., social pressure to get pregnant before marriage among Balinese) was found a barrier at the interpersonal level. At the institutional level, key informants stated the prospective bride’s advisory service and ELSIMIL application campaign were not adequate. At the community level, the family support team community has been established, however, they could not fulfill their function properly due to the “no pregnancy, no marriage” motto. At the policy level, the traditional village assembly (i.e., “Majelis Desa Adat”) has not issued regulations regarding the prospective bride’s advisory service.  A lack of cross-sector collaboration was also reported as a barrier to this initiative. A well-planned promotion could socialize ELSIMIL application among prospective brides. Advocacy to the traditional village assembly might diminish the “no pregnancy, no marriage” motto. Pentahelix collaboration needs to be established to achieve the “zero stunting for golden generation” goal.
