Abstract

Background: According to a report by the Indonesian Renal Registry (RII) in 2017, the number of patients with chronic kidney disease increased drastically. It shows more patients who may have hemodialysis. Patients with hemodialysis use AVF Access to ease the therapy process. Education based on family support is needed to help patients in self-care, using AVF Access, and improving the quality of patients. Purpose: This research aims to know the impact of education based on family support toward self-care and the life quality of patients with hemodialysis. Methods: The methodology of research used is total sampling. There are 22 samples of patients with hemodialysis with AVF Access that fulfill inclusive criteria (living with family, having components awareness, and doing regular hemodialysis twice a week). The instruments to collect data are questionnaires of self-treatment with ACF Access and KDQL SFTM-36. The data is analyzed with the validating technique of the T Dependent Test with a significance level of 0,05. Result: The result shows that most respondents have good self-treatment for 14 patients (63,6%), proven by p-value = 0,000. The life quality of most respondents is good, proven by p-value = 0,00. Conclusion: In conclusion, there is a significant, substantial, and one-way correlation between education based on family support toward self-care AVF Access and the life quality of patients with hemodialysis. Family is obliged to give more support to patients with hemodialysis to do self-care well so the life quality of patients may improve.

Abstract

Introduction: Asthma is a chronic inflammatory disease of the respiratory tract characterized by coughing, shortness of breath, wheezing, and a feeling of heaviness in the chest, which is generally reversible both with and without treatment. Giving Buteyko breathing exercise action is more effective than before being given the action. Objectives: To analyze the application of Buteyko breathing exercises to improve respiratory rate (RR) and oxygen saturation (SpO2). Methods: Descriptive analysis in the form of case studies with total sampling based on inclusion and exclusion criteria of 3 patients under management. The Buteyko breathing exercise was applied, and vital signs were observed before and after the Buteyko breathing exercise. Results: Nursing problems are ineffective breathing patterns with Respiratory Rate (RR) > 26 x/minute, and two patients were found to have mild hypoxia with SpO2 <95%. After applying the Buteyko breathing exercise, the three patients experienced an improvement in their condition with RR 18-20 x/minute and SpO2 > 98%. The author describes the application of Buteyko breathing exercises to asthma patients, which improved the condition of RR and SpO2. They were supported by Lisavina's research (2019) on the difference between controlling asthma before and after implementing Buteyko breathing. Conclusions: The application of Buteyko breathing exercises in assessing improvement in respiratory rate (RR) and oxygen saturation (SpO2) did not harm patient conditions, hospital costs, time and effort, or infrastructure.

Abstract

Introduction: Diabetes Mellitus (DM) is a severe problem with a rapidly increasing incidence rate. DM can affect almost all segments of society worldwide. The number of individuals with DM continues to rise yearly due to the sedentary lifestyle and unhealthy eating habits prevalent in contemporary society. Objectives: This study aims to compare the effects of soymilk and red dragon fruit consumption on fasting blood glucose levels in individuals with type 2 diabetes mellitus. Methods: The research design employed in this study is quasi-experimental with a pre-test-post-test approach involving two treatment groups. The study subjects were 54 individuals with type 2 diabetes mellitus from Bukittinggi. The participants were divided into three groups: an intervention group of 18 individuals who consumed red dragon fruit daily for seven days, a second intervention group of 18 individuals who consumed soymilk daily, and a control group of 18. Comparison analysis is done using the ANOVA test. Results: The research findings indicate that the p-value is 0.035, and the calculated t-value (2.198) is greater than the tabulated t-value (2.110. Furthermore, the consumption of red dragon fruit impacted fasting blood glucose levels and blood pressure, as evidenced by a p-value of 0.018 for fasting blood glucose and a calculated t-value of 2.198, which exceeds the tabulated t-value of 2.110. The ANOVA results indicate that the effectiveness of red dragon fruit consumption is superior, with an average decrease in blood glucose levels of 32.56, compared to the effectiveness of soymilk consumption, which resulted in an average decrease of 22.50. Conclusions: Red dragon fruit is more effective in reducing blood glucose and blood pressure levels in patients with type 2 diabetes mellitus.

Abstract

Introduction: Venous blood sampling is always uncomfortable and painful. Especially for neonates. Several impacts, such as rapid pulse, tissue damage, psychological stress, and altered behavior. Improper intervention results in further complications like hyperalgesia and severe pain experience. Objectives: Identify the effects of vanilla extract on neonatal pain and comfort levels of neonates during venous blood sampling. Methods: Within three months of data collection in a private hospital, the quasi-experiment post-test-only with a control group design used quota sampling with 40 neonates. The Neonatal Infant Pain Scale (NIPS) is used to assess the pain level in newborns, and the Comfort Scale is used to assess their comfort level. Results: More than half of neonates (64%) were 24 hours old, and 60 % were boys. As many as 76% of them were diagnosed with umbilical cord infection, PROM, Etc. There was no significant difference in the pain scales of the two groups (p=0.51), but there was a significant difference in the results of the comfort level (p= 0.00). These results follow previous research that found that the vanilla concentrations of 0.64% and 2% could not significantly reduce pain. Conclusions: The nerve response of neonates has not developed correctly, or the asphyxia problem during the intervention, resulting in the smell of vanilla aromatherapy 0.64% applied not being able to overcome pain significantly. It is further recommended to vary the dose to see the impact of pain.

Abstract

Background: Gastric ulcer is an excoriation area of the stomach caused by the destruction of mucosal defense against the predominant factors. One of the efforts that should be made to reduce the incidence of gastric ulcers is a precautionary measure to use ethanol extract of celery (Apium graveolens L). Celery contains phenolic compounds, such as apigenin which acts as an antioxidant. Antioxidant has the potential as an agent of gastric mucosal protection against cell damage caused by excessive activation of free radicals (ROS). Objectives: This study aimed to determine the effect of celery ethanol extract in preventing gastric ulcers through a reduction of ulcer lesion length.  Method: Research method Post-test Only Randomized Controlled Group Design conducted on Wistar strain rats. Experimental animals were divided into 5 groups: healthy, normal wound with induction indomethacin, and celery ethanol extract administered orally at a dose of 200 mg/kg, 300, mg/kg, and 400 mg/kg. Observations were made on the length of gastric ulcer lesions. The length of the gastric ulcer was measured using a vernier caliper and statistically analyzed by the One-way ANOVA test. Result: The result showed that the ethanol extract of celery at 300mg/kgBB was the best dose in preventing the formation of gastric ulcer by decreasing the length of the gastric ulcer. Conclusion: Based on these results, the researcher suggests comparing other various parameters regarding the effect of celery ethanol extract for the prevention of gastric ulcers.

Abstract

Introduction: Home visits are a primary health care program targeting families with chronic health problems to promote healthy behavior changes and improve the quality of life of patients. Objectives: This study aims to determine the effect of nurse home visit interventions on knowledge, dietary salt adherence, and blood pressure in hypertensive patients at the Primary Health Care (Puskesmas) in Dau District. Methods: This quantitative research uses a quasi-experimental method with a one-group pre-test post-test design. The sample consisted of 69 individuals from Kunci and Princi hamlets who suffered from hypertension. The sampling technique was purposive sampling with inclusion criteria including families with hypertension, living with family members, being able to read and write, and willing to participate as respondents. The home-visit interventions were conducted over 7 weeks and included counseling on hypertension concepts, diet, medication, physical activity, and stress management, along with ongoing monitoring during the home-visit period. Results: Statistical test results on the knowledge variable using the paired t-test showed a significance of 0.01, indicating a significant change post-intervention. The dietary salt adherence variable, assessed using the Wilcoxon rank test, showed an average increase of 24.00 from pre-intervention to post-intervention. Both systolic and diastolic blood pressure variables exhibited significant changes, with a 2-tailed significance of 0.01. Conclusions: The results of this study conclude that nurse home-visit interventions have a positive effect on knowledge, dietary salt adherence, and blood pressure in hypertensive patients.

Abstract

Introduction:  The sociodemographic conditions of Jember Regency can shaped the role attainment of mothers in preventing stunting. The Maternal Role Attainment theory, explains that the mother's role attainment was influenced by 3 factors, namely the macrosystem, mesosystem and microsystem. Objectives:  The purpose of this study was to analyze differences in maternal role attainment in rural and urban areas of Jember Regency. Methods: The research design used was  descriptive analytical with a cross sectional approach.  Total sample of 215 mothers who met the inclusion criteria was selected using the multistage random sampling technique. Data were analyzed using  Mann Withney U Test with α = 0.05. Results:  there was a difference between the role attainment of mothers in rural and urban areas (0.032<0.05). Conclusions:  Based on the theory of maternal role attainment, differences in maternal role attainment in rural and urban areas were influenced by macrosystems, mesosystems, and microsystems as well as components of the mother's self. Further research can analyze the determinant factors in the differences in role attainment.

Abstract

Introduction: Elderly people experience a decline in the function of their reproductive organs, which will affect intimate relationships with their partners, thus having an impact on physiological problems regarding sexual needs, so they are expected to be able to adapt to changes by improving their sexual function by doing Kegel exercises. Objectives: to determine the effect of Kegels exercises on sexual function in the elderly in the East Jakarta area. Methods: The research uses a quasi-experimental design with a pre-post test design without a control group, namely observations carried out before treatment and after treatment without using a control group, with a total of 27 respondents starting from May to August 2023 using the 19-item FSFI questionnaire. Results: The results showed that sexual scores increased by an average of 2 after Kegel exercises were carried out: sexual desire was 2, sexual arousal was 2.63, lubrication was 1.37, organism was 1, sexual satisfaction was 1.70, pain decreased by 1.19, p value = 0.000, and FSI score ≤ 26.55 for 21 people (77.8%) who did not experience sexual dysfunction. Conclusion: Sexual function will increase if you do light exercise such as gymnastics, namely Kegels exercises. This can also reduce sexual dysfunction.

Abstract

Introduction: Urinary incontinence refers to an individual's inability to restrain or control the discharge of urine due to weakness of the pelvic floor muscles and bladder muscles that are unable to control urine. This causes contamination of skin diseases in the genital area, quality of sleep, and decubitus. Prevention efforts can be provided with non-pharmacological therapy by doing Kegel exercises to stretch the pelvic floor muscles. The mechanism of action strengthens the capacity of the bladder sphincter. Objectives: This study aimed to determine the effect of Kegel exercises on urinary incontinence in postmenopausal women in the Banyuwangi Hindu elderly community in 2023. Methods: This research employed a pre-experimental research design with a one-group pre-test post-test research design of 18 respondents using a purposive sampling technique. Statistical analysis using the Wilcoxon test. Results: The study's results before the intervention were those who experienced urinary incontinence in the elderly with a minimum value of 13, a median value of 17.50, and a maximum value of 22. After the intervention, those experiencing urinary incontinence had a minimum value of 4, a median value of 7.44, and a maximum value of 12.  The findings of this research, analyzed using the Wilcoxon Test with SPSS, yielded a significance value of p = 0.000, which is less than 0.05. This indicates that Kegel Exercise has an impact on Urinary Incontinence in Menopausal Women in the Banyuwangi Hindu Elderly Community in 2023. Conclusions: Kegel exercises contract, increase muscle tone, improve healing by up to 50%, and form a supporting structure for the pelvis, vagina, urethra, and rectum to affect the inability to urinate under control.

Abstract

Introduction: The roles of nurses in implementing continuing care in Coronary Artery Bypass Graft (CABG) patients have an essential part in improving patients' quality of life. Objectives: This study aims to determine nurses's roles in continuity of care implementation for CABG patients. Methods: This study employed a descriptive research design. The sample of this study was all nurses working at the CABG wards at the Arifin Achmad Hospital, who were selected using a total sampling technique. The variables measured in this study are the role of nurses, including caregivers, communicators, educators, client advocates, and counsellors. Data were collected using a questionnaire developed by the researcher that had been tested before use. The data analysis used is descriptive analysis, which presents the percentage of variables measured. Results: Demography characteristics from 37 nurses depict the majority of respondents in the age range of 36-45 years (45.4%), with the majority of women (81.1%), most nurses' education background is registered nurse (56.8%), and the majority of work experience is ≥ 12 years (64.9%). This study's findings showed the role of nurses as good caregivers (56.8%), good communicators (75.5%), good educators (67.6%), good client advocacy (73%), and good counsellors (51.4%). Conclusions: It can be concluded that most nurses perform an adequate role in implementing continuity of care in CABG patients. Then, it will reduce re-hospitalisation risk and improve the quality of life for patients with CABG.
