Abstract

Patients with chronic kidney failure often experience anxiety before undergoing hemodialysis therapy, presenting symptoms such as palpitations, nausea, tremors, and nervousness. Anxiety is triggered by factors such as seeing the blood tubing, treatment costs, invasive procedures, and uncertainty about recovery. This community service initiative aims to address anxiety in hemodialysis patients at Bhayangkara Kediri Hospital through spiritual-based Guided Imagery therapy, aimed at providing calmness and reducing stress associated with the treatment. The community service activity was conducted from September 2 to 13, 2024, at the Hemodialysis Unit of Bhayangkara Kediri Hospital. The program was carried out in three phases: preparation, implementation, and evaluation. A total of 14 patients participated in this activity. To measure the level of anxiety before and after the therapy, the Zung Self-Rating Anxiety Scale questionnaire was used, a common tool for assessing anxiety symptoms. The results of this initiative showed a significant reduction in anxiety levels in hemodialysis patients at Bhayangkara Kediri Hospital after receiving spiritual-based Guided Imagery therapy. The therapy effectively alleviated anxiety symptoms such as palpitations and other physical tension related to anxiety before the hemodialysis procedure. Spiritual-based Guided Imagery therapy was proven to be effective in reducing anxiety in hemodialysis patients. This program can serve as a beneficial non-pharmacological approach in managing patient anxiety in the hemodialysis unit.

Abstract

Habits such as eating patterns, personal hygiene, and unhealthy snacking can affect students' health at school. To prevent these risks, health screening becomes an important step. ITSK RS Dr. Soepraoen Malang organized a health screening at SDN Dusun Sumber Bendo, Kucur Village, Dau District, Malang Regency, to detect issues with students' teeth, mouth, eyes, and ears. The activity was carried out in several stages. During the preparation phase, the community service team obtained permits from LPPM ITSK Soepraoen and the Head of SDN 9 Dusun Sumber Bendo. The team then divided tasks into two groups: one arranged the activity location, while the other prepared logistics, including borrowing equipment from the DIII Nursing Program laboratory and providing stationery and refreshments. The activity took place on May 11, 2024, and involved three groups: student guidance, administration, and screening. The guidance group managed the activity flow, the administration group recorded students' data, and the screening group conducted examinations at six stations. A total of 55 students from grade 1 of SDN 9 Dusun Sumber Bendo participated, from 08:00 to 11:00 WIB. The health screening results showed that only the tongue examination had a 100% normal result. The majority of students had cavities (51%) and caries (35%), with the highest cases found among female students (56%). For tonsil health, two students in grade 5 (4%) had tonsillitis. The eye examination revealed that 12 students (22%) had myopia, and 2 students (4%) had partial color blindness. Ear examinations often showed the presence of little (38%) or a lot of earwax (35%), with higher numbers in female students. These issues highlight the need for increased attention to dental and ear health, with education and routine examinations that can help prevent health problems that may impact children's learning process.

Abstract

Ineffective communication between pharmacy staff and nurses during the handover process of premedication drugs at the Oncology Pharmacy Depot of RSUD Provinsi NTB often causes discomfort for patients and their families. This issue impacts the efficiency of drug distribution services, requiring strategic solutions. This study aims to analyze the factors contributing to the suboptimal distribution of premedication drugs, develop Standard Operating Procedures (SOPs), and devise strategies to improve the quality of drug distribution services for chemotherapy patients. Methodology: The approach used was a SWOT analysis to evaluate strengths, weaknesses, opportunities, and threats in premedication drug distribution services. The WO (Weakness-Opportunity) strategy was selected to address internal weaknesses by utilizing external opportunities, such as implementing socialization programs and strengthening monitoring and evaluation efforts. The development of SOPs related to premedication drug distribution, the creation of a premedication drug handover logbook, and the implementation of socialization programs for pharmacy staff and nurses have significantly improved coordination among personnel. These measures contributed to a reduction in patient complaints regarding delays in drug distribution and enhanced service efficiency. The implementation of SOPs, a handover logbook, and socialization programs are effective steps in improving the quality of premedication drug distribution services in oncology wards. This intervention is expected to serve as a model for application in other pharmacy installations to enhance the quality of pharmacy services.

Abstract

Emergency situations are sudden events that require immediate action. These conditions can arise from trauma or non-trauma incidents, with risks of respiratory arrest, cardiac arrest, organ damage, or bleeding. Emergencies can occur to anyone, at any time, and anywhere, characterized by their rapid onset and unpredictability. Quick, accurate, and careful management of emergencies is crucial to prevent death and disability, in line with the principle that "time is life." Health education is one of the strategic steps to enhance public awareness in facing emergency situations, especially through community service activities aimed at providing practical understanding of emergency management. This community service activity was conducted in Sari Bunga Mas Village and involved 26 participants, including community cadres and the general public. The methods used included a pre-test to assess participants' initial knowledge, lectures to provide information about emergencies, demonstrations to show practical emergency management procedures, distribution of leaflets as supplementary materials, and a post-test to evaluate the increase in participants' knowledge. Data from the pre-test and post-test were analyzed to measure the effectiveness of the intervention. The activity was attended by cadres and the community of Sari Bunga Mas Village, totaling 26 people. Data analysis showed the following results: before the intervention, 12% of participants had good knowledge, and after the intervention, there was an increase to 79.2% with good knowledge, with the remaining participants showing moderate knowledge. This activity successfully demonstrated that health education with an interactive approach significantly improves the community's knowledge about basic emergency management. The lecture provided a deep theoretical understanding, while the demonstration helped participants learn practical steps they can take in emergency situations. The distribution of leaflets served as a reminder and additional reference for the participants.

Abstract

Professional Nursing Care Management (PNCM) is a structured approach aimed at improving the quality of nursing services by emphasizing patient safety and professional standards. Despite its significance, many hospitals encounter barriers in implementing PNCM optimally due to uneven educational levels, limited nurse competencies, unsupportive management systems, and low staff motivation. These challenges can lead to suboptimal nurse performance and reduced care quality. This community-based project aimed to improve nursing care quality at Dayaku Raja Regional Public Hospital through the implementation of PNCM. A SWOT (Strengths, Weaknesses, Opportunities, Threats) analysis was conducted to identify internal and external factors affecting PNCM implementation. Based on the analysis, priority strategies were developed. Interventions included planning and organizing a care model, implementing care based on nursing standards, enhancing nurse competencies, promoting integrated documentation, and encouraging multidisciplinary collaboration. The effectiveness of implementation was evaluated using nursing quality indicators. The initial assessment led to the development and formalization of nursing care guidelines. Post-intervention evaluation showed improved compliance with nursing protocols such as standardized handovers and nursing conferences. There was a noticeable increase in patient satisfaction and quality indicators related to nursing services. The implementation of PNCM proved to be effective in enhancing nursing care quality. Strengthening nurse competencies, supporting organizational structures, and maintaining continuous quality evaluation are critical to sustaining improvements. The success at Dayaku Raja Hospital may serve as a model for similar healthcare facilities.

Abstract

Decreased appetite and irregular eating patterns among the elderly can lead to malnutrition or obesity, both of which negatively impact their quality of life. Educational interventions serve as a strategic approach to improve elderly knowledge regarding healthy eating and balanced nutrition. This activity employed a one-group quasi-experimental pretest-posttest design without a control group. It was conducted on January 29, 2025, at the Elderly Health Post (Posyandu Lansia) in Bumi Intan Permai Housing Complex, Sidoarjo. A total of 24 elderly participants were selected purposively. The education was delivered through interactive lectures and group discussions. Evaluation was carried out using pre- and post-education questionnaires to assess knowledge levels related to dietary patterns and balanced nutrition. Data were analyzed descriptively. The results showed that after the educational session, 70% of participants demonstrated a good improvement in understanding the concepts of healthy eating and balanced nutrition. All participants remained until the end of the activity and showed high enthusiasm during the discussion. Nutrition education based on lectures and discussions was effective in improving elderly knowledge about healthy and balanced diets. This activity may serve as a health promotion model in elderly community settings.

Abstract

One type of emergency problem that can cause sudden death is usually caused by cardiac arrest. Basic life support training provided to PMR cadres can improve the knowledge and skills of the younger generation in providing basic life support, especially in cases of cardiac arrest. The purpose of this community service activity is to optimize the role of PMR cadres so that they are able to educate about basic life support in cases of cardiac arrest through demonstrations and community service participants learn about how to provide basic life support in cases of cardiac arrest. The method of implementing this activity is carried out through lecture and demonstration methods involving 6 PMR cadres and 20 student participants from SMK YP 17 Pare. This activity was carried out on April 10-12, 2025. The implementation of the activity consisted of 2 sessions. The first session was empowering PMR cadres through cadre training activities on basic life support in cases of cardiac arrest and the second session was delivering material by trained cadres which was delivered to community service participants. The results of this community service activity were that trained cadres were able to deliver the material well, there was an increase in knowledge and ability of participants in providing basic life support in cases of cardiac arrest after demonstration education was carried out by trained cadres. Education through empowerment of cadres as a way to optimize the role of the younger generation can improve students' understanding of basic life support in cardiac arrest. As agents of change and possessing significant social influence, the younger generation is expected to be able to educate more people through creative and relevant methods.

Abstract

Obstetric emergencies remain a leading cause of maternal mortality in Indonesia, most of which are preventable through early detection of pregnancy danger signs. One of the key factors in the success of early detection is the involvement of family members, particularly the husband as the primary support for pregnant women. However, the involvement of husbands in this aspect remains limited and suboptimal. This study aims to explore the role of husbands in the early detection of pregnancy danger signs as a preventive effort against obstetric emergencies in pregnant women. This research employed a descriptive qualitative approach through in-depth interviews with six husbands of pregnant women undergoing antenatal care at Sunggal Public Health Center, Medan, in January 2025. The study population consisted of husbands of pregnant women who met the inclusion criteria. Data were collected using a semi-structured interview guide, recorded, transcribed, and analyzed using thematic analysis. The main variable in this study was the role of husbands in identifying danger signs during pregnancy, with interviews serving as the primary instrument. Data validity was ensured through source triangulation and member checking. The results revealed three main themes: husbands' knowledge and awareness of danger signs, forms of involvement in preventing emergencies, and barriers to participation. Most informants recognized prominent danger signs such as bleeding and severe pain but were unaware of symptoms like preeclampsia or premature rupture of membranes. Forms of support included accompanying their wives to health facilities, assisting with household chores, and paying attention to complaints. The main obstacles included limited time, lack of direct information from healthcare providers, and cultural influences that consider pregnancy a woman's domain. In conclusion, although some husbands demonstrated concern, their involvement in early detection of pregnancy danger signs needs to be strengthened. Recommendations are directed at communities and healthcare providers to actively engage husbands in pregnancy education programs, including through interactive media, father classes, and community-based approaches, in order to significantly reduce the incidence of obstetric emergencies in pregnant women.

Abstract

Back pain is a common complaint among pregnant women, particularly in the third trimester, which can affect their quality of life and readiness for childbirth. One non-pharmacological method believed to alleviate this complaint is prenatal exercise. The aim of this study was to determine the relationship between the practice of prenatal exercise and the occurrence of back pain in third-trimester pregnant women. This study employed a descriptive-analytic design with a cross-sectional approach. The population consisted of all third-trimester pregnant women who visited healthcare facilities in Medan. Sampling was conducted using accidental sampling, resulting in 36 respondents. The independent variable was the practice of prenatal exercise, while the dependent variable was the occurrence of back pain. Data were collected using a closed-ended questionnaire and a subjective pain observation sheet. Data analysis was conducted using the Chi-square test with a significance level of 0.05. The results showed that out of 36 respondents, 61.1% did not participate in prenatal exercise, and 69.4% experienced back pain. Statistical analysis indicated a significant relationship between prenatal exercise and the occurrence of back pain in third-trimester pregnant women, with a p-value of 0.000 (<0.05). In conclusion, there is a significant association between prenatal exercise and back pain in third-trimester pregnant women. Therefore, it is recommended that prenatal exercise be integrated into antenatal care programs as a promotive and preventive intervention. Regular education from healthcare professionals is essential to increase pregnant women's awareness and encourage consistent participation in prenatal exercise to reduce the risk of back pain.

Abstract

ST-Elevation Myocardial Infarction (STEMI) is a cardiology emergency that requires prompt and accurate management to prevent mortality and serious complications. One of the main challenges in STEMI management is system delay, which is often influenced by two key factors: onset-to-door time and door-to-ECG time. This study aims to analyze the relationship between STEMI onset and door-to-ECG time with system delay in patients presenting to the Emergency Department (ED). This research was an observational quantitative study with a cross-sectional approach, conducted at H. Adam Malik General Hospital, Medan, during the period of September to December 2022. The sample consisted of 86 STEMI patients selected through total sampling. The independent variables were onset time and door-to-ECG time, while the dependent variable was system delay. Data collection instruments included medical record observation forms and a service time stopwatch. Data were analyzed using the Spearman Rank test with a significance level of <0.05. The results showed that 69.8% of patients experienced system delay (≥90 minutes). Most onset times were within the range of 6–10 hours (31.4%), while the fastest door-to-ECG time occurred within 0–3 minutes (44.2%). Statistical tests indicated that both onset time and door-to-ECG time were significantly associated with system delay, with p-values of 0.008 and 0.007, respectively. It can be concluded that the longer the STEMI onset and the slower the ECG is performed after the patient arrives at the ED, the greater the likelihood of system delay. Therefore, public education is necessary to improve early recognition of heart attack symptoms, along with enhanced triage efficiency and rapid ECG protocols in the emergency department.
