Abstract

Physiological changes in the elderly make them vulnerable to chronic diseases. The elderly are a vulnerable population to hypertension cases, as one of the chronic diseases in Indonesia. Efforts are needed to provide health services that can be provided by nurses through a community approach with the parties involved, such as the elderly. This community service is carried out through health education and counseling on hypertension management followed by knowledge evaluation. These efforts can be realized by holding counseling activities on hypertension. This community service activity involved 30 elderly people in Jetis Village, Sukoharjo, with 53% experiencing high hypertension. The activity took place from December 15-21, 2023, including preparation, intervention, and evaluation. The intervention was in the form of health education about hypertension and blood sugar for 2 x 60 minutes, with flip sheet media and group discussion. The evaluation was carried out by t-test on SPSS 22.0 to analyze the increase in knowledge. The measuring tools used were a glucometer, sphygmomanometer, and questionnaire (Cronbach's alpha = 0.80), with normal blood sugar values of 80-100 mg/dL and blood pressure of 130-140 mmHg (systolic) and 80-90 mmHg (diastolic). The results of the counseling activities on hypertension showed that the age of the participating people was mostly 50-75 years old with a total of 30 people (57%), the gender was mostly female, namely 25 people (83%) while 5 men (17%). A total of 16 people (53%) had high hypertension and normal hypertension (47%). A total of 18 people (39%) had good knowledge before counseling and 28 people (61%) had good knowledge after counseling. Health education and counseling on hypertension management effectively increased knowledge among older adults with hypertension who participated in this study. Providing health education is recommended to improve hypertension management.

Abstract

Pulmonary tuberculosis (pulmonary TB) is a curable condition, but often patients stop treatment before it is finished. This is due to the patient's misconception about healing; They think that their illness has been cured because their symptoms have disappeared or decreased. In fact, pulmonary tuberculosis therapy must be carried out for a minimum of six months. This study aims to identify how strategies to increase the discovery of new cases of BTA-positive tuberculosis in the community are applied.  The methods used include fishbone diagrams, Urgency, Seriousness, Growth (USG) methods, and Strength, Weakness, Opportunity, Threat (SWOT) analysis. Based on the analysis carried out, problems were found related to the strategy of increasing the discovery of BTA+ pulmonary TB cases at the Lalasan Health Center, Tanimbar Islands Regency, Maluku Province.  Based on the identification of the problem, the priority of the problem was determined using the Multiple Criteria Utility Assessment (MCUA) method, namely the low coverage of Pulmonary TB CDR at the Lingat Health Center, Tanimbar Islands Regency in 2024. Suggested solutions to address this problem include improving strategies for the discovery of new cases of BTA-positive tuberculosis. This study specifically examines the implementation of these strategies and determines the means and media to disseminate information about TB at the Lingats Health Center, Tanimbar Islands Regency, Maluku Province, by involving health workers and the community.

Abstract

The Plan-Do-Study-Act (PDSA) method is an approach to test the changes implemented to ensure continuous quality improvement. The four steps in the PDSA help guide the thinking process by breaking down the task into technical steps, then evaluating the results, improving them, and retesting them. The project to optimize the filling of ERM in the nurse education section requires the commitment of all nurses and the head of the Inpatient I room at Bali Royal Hospital. The concept in the search for the root of the problem uses a fishbone diagram, a scoring method to arrange the priority of issues that must be solved. The case study was carried out in Inpatient Room I of Bali Royal Denpasar Hospital with a total of 29 nurses and 1 room head. The implementation of optimizing the filling of nursing education at ERM began with a focused group discussion that resulted in an agreement/commitment between the nurse and the head of the room with his supervision function, then the implementation was evaluated using the P-D-S-A method at Bali Royal Hospital. The analysis of the implementation results obtained quite good results, with an achievement of 42% in the first week and increased to 51% in the second week. The application of the joint commitment model between the implementing nurse and the supervision of the head of the room through the P-D-S-A method approach in optimizing the filling of nursing education in ERM which is a form of effort to improve the quality in nursing is important to be carried out on an ongoing basis. The PDSA cycle that is carried out consistently and carried out in conjunction with the commitment of the nurse and the role of supervision from the head of the room will produce better results.
