Abstract

Background: Midwifery services are a key determinant in improving maternal and neonatal health outcomes. Midwives’ behaviors, including empathy, communication, professionalism, and responsiveness, have been shown to influence the quality of nursing and midwifery care perceived by patients. At Hospital X Boyolali, internal data revealed persistent complaints regarding insufficient empathy and delayed responsiveness, despite compliance with technical medical standards. This highlights the need to examine the contribution of midwives’ behaviors to the quality of midwifery care. Aims: This study aimed to analyze the relationship between midwives’ behaviors and the quality of midwifery care as perceived by patients in Hospital X Boyolali. Methods: A quantitative study with a cross-sectional analytic design was conducted involving 70 midwives selected through proportional random sampling. Data were collected using a structured questionnaire consisting of 15 items on midwives’ behaviors and 20 items on quality of care. Instrument reliability was confirmed with a Cronbach’s alpha of 0.872. Data were analyzed using the chi-square test with a 5% significance level. Results: Most midwives demonstrated good behaviors in empathy (64.3%), communication (60.0%), professionalism (68.6%), and responsiveness (57.1%). Patients’ perceptions of care quality were predominantly rated as high, with reliability showing the highest proportion (71.4%). The chi-square test indicated a significant association between midwives’ behaviors and the quality of midwifery care (p = 0.002). Conclusion: Midwives’ behaviors play a crucial role in determining the quality of midwifery services. Efforts to improve service quality should prioritize strengthening empathetic, communicative, and professional behaviors through continuous soft skills training and professional development programs.

Abstract

Background: Acute Respiratory Infection (ARI) remains a leading cause of morbidity among children under five, particularly in agrarian areas characterized by exposure to kitchen smoke, limited ventilation, and agricultural activities that potentially increase ARI risk. Community health cadres for under-five children, as the front line of community empowerment, often have limited knowledge regarding the relationship between household environment and ARI prevention. Aims: To assess the effect of Clean and Healthy Living Behavior (CHLB/PHBS) education on changes in the knowledge of child health cadres in preventing ARI among under-five children in agrarian areas. Methods: A pre-experimental study with a one-group pretest-posttest design was conducted to evaluate the impact of a 90-minute CHLB education session. The intervention combined interactive lectures, discussions, and visual media. Knowledge was measured using a validated 15-item questionnaire. Data were analyzed using the Wilcoxon Signed-Rank Test. Results: The mean knowledge score significantly increased from 6.20 ± 0.97 (pretest) to 12.97 ± 1.03 (posttest). Statistical analysis showed p = 0.000, indicating a significant difference. Before the intervention, 83.3% of cadres were in the low-knowledge category, whereas after the education, 90% achieved the high-knowledge category. Conclusion: CHLB education significantly improved the knowledge of child health cadres in preventing ARI among under-five children in agrarian areas. This approach proved to be effective and contextually relevant, with potential to serve as a model for cadre empowerment in ARI prevention.

Abstract

Background: Because of its high recurrence risk and extended treatment duration, drug-resistant tuberculosis (DR-TB) presents a serious public health concern. Poor treatment adherence, financial difficulties, immunocompromised conditions, bacterial mutations, and irregular treatment regimens are some of the factors that lead to recurrence. Aims: To describe smoking cessation intervention in drug-resistant tuberculosis patients at Hospital X. Methods: A qualitative research study explored the challenges of providing Smoking Cessation (SC) interventions in a hospital setting, specifically focused on patients with drug-resistant tuberculosis, hospital staff, and structural elements, with a total sample size of 18. Conducted at a DR-TB management hospital in Surabaya. In-depth interviews lasting approximately thirty minutes each utilized open-ended questions regarding the dangers of smoking, barriers to cessation support, and resource availability. Interviews were recorded, transcribed verbatim, and thematically analyzed using an inductive coding approach following participant consent. Results: The research offers subthemes that were outlined under each of the three main themes (barriers to quitting smoking) that were identified. Individual DR-TB inpatients' personal factors are obstacles to quitting smoking, whereas hospital staff members' personal aspects are obstacles to quitting smoking. The government and hospital administration are in charge of addressing the institutional impediments to quitting smoking. Conclusion: The health of drug-resistant tuberculosis inpatients is adversely affected by cigarette smoking. Mandatory screening and counseling for smokers among DR-TB and TB patients are essential for developing effective treatment strategies in healthcare.

Abstract

Background: Hypertension is a common condition in older adults that requires effective self-care management. This management includes behavioral skills for self-management of the disease, which is essential for controlling hypertension. However, levels of self-care management in older adults tend to be low, and they often experience difficulties that can worsen their health condition. Many older adults perceive poor self-care management as normal, even though it can increase the risk of complications. Aims: to describe the characteristics of the elderly and self-care management in the elderly with hypertension Methods: This study was a descriptive design. The sample consisted of 100 elderly people with hypertension selected using quota sampling. The Self-Management Behavior Ability of patients with hypertension questionnaire was used. Results: The results showed that the majority of elderly people were aged 56-65 years (84 people). The majority of elderly women (86 people). Most respondents were unemployed/retired (69%). The highest level of education was higher education graduates (29 respondents). The duration of hypertension for the majority of elderly people (66 people) was 1-5 years. In addition, the majority of respondents (68 people) had poor (negative) self-care management. Conclusion: Optimal self-care management needs to be implemented to improve the elderly's ability to manage hypertension and maintain quality of life.

Abstract

Background: Healthcare-associated infections (HAIs) constitute a significant global health challenge, exacerbating morbidity, mortality, and economic burdens. The hospital environment encompassing air quality, water systems, surface contamination, and building design serves as a potential pathogen reservoir and a primary transmission pathway. With HAI prevalence ranging from 3.0% to 20.7% and incidence rates between 5% and 10%, Aims: this study aims to synthesize scientific evidence from observational studies regarding the influence of environmental factors on the incidence of nosocomial infections.. Methods: This systematic review was conducted in adherence to the PRISMA guidelines. A comprehensive literature search was performed across ScienceDirect, ProQuest, JSTOR, Scopus, Sinta, PubMed, Google Scholar, and Portal Garuda databases. The inclusion criteria prioritized observational studies addressing environmental factors and HAIs. Study quality was evaluated using relevant critical appraisal instruments, followed by a qualitative synthesis of the data. Results: Thirteen articles met the inclusion criteria. The synthesis indicates that poor air quality, high levels of surface contamination, unmanaged water systems, and suboptimal ward design are significantly correlated with an increased risk of nosocomial infections. Conclusion: Environmental factors are critical determinants in the transmission of infections within hospital settings. The implementation of strict disinfection protocols, improvements in ventilation systems, and facility designs grounded in infection prevention principles are paramount to enhancing patient safety and ensuring cost-efficiency in healthcare.
