Abstract

Background: Workplace accidents remain a major concern in the energy sector. Unsafe acts and conditions are the leading cause of accidents, with over 85% of incidents related to unsafe behavior. PT. PLN Indonesia Power's Payo Selincah Jambi Power Plant Unit, with 144 workers, recorded 99 workplace accidents in 2019–2021.Purpose: To identify factors underlying unsafe acts and unsafe conditions among field workers.Method: Qualitative research using a case study approach. Six informants were purposively selected: the unit manager, the OHS team leader, the head of operations, the head of maintenance, and two field workers. Data were collected through in-depth interviews, observation, and documentation, then analyzed thematically.Results: Two main themes were identified: human behavioral factors, low PPE compliance due to unfamiliarity and discomfort, "cat and mouse" behavior toward supervisors, and neglect of procedures during emergencies. Environmental factors are generally well managed through routine housekeeping, waste management, and annual PPE replacement, but outdoor work in extreme weather conditions remains risky. Conclusion: The main challenge lies in the genuine internalization of a safety culture. It is recommended to review the ergonomics of personal protective equipment (PPE), strengthen the reward-punishment system, implement emergency simulation training, and formalize safe work practices across all units. Abstract

Background: Data from the 2023 Indonesian Health Survey (SKI) showed a prevalence of 887,531 respiratory tract infections in Indonesia, while in Jambi Province the prevalence was 11,588 cases. According to data from the Indonesian Ministry of Health in 2024, four of the ten most common diseases are related to the respiratory system: COPD, lung cancer, pneumonia, and asthma. Upper respiratory tract infections are infectious diseases that infect the initial part of the respiratory tract, from the nose to the alveoli, as well as structures such as the sinuses, middle ear cavity, and pleura. Purpose: To determine factors associated with respiratory complaints in street sweepers. Method: This quantitative study used a cross-sectional design. The sampling technique used was random sampling, so that the entire population meeting the inclusion criteria, totaling 84 respondents, was selected as the study sample. Data collection was conducted using a questionnaire through direct interviews and analyzed univariately and bivariately using the chi-square test. Results: Bivariate analysis showed a significant relationship between respiratory complaints in street sweepers with gender (p value = 0.007; PR 0.44; 95%CI 0.24-0.76), smoking habits (p value = 0.020; PR 0.521; 95%CI 1.128-3.377), and use of personal protective equipment (p-value = 0.032; PR 1.46; 95%CI 0.30-0.94). There was no significant relationship between respiratory complaints in street sweepers with age (p-value = 0.313; PR 0.21; 95%CI 0.58-7.65), and length of service (p value = 0.127; PR 0.625; 95%CI 0.354-1.104). Conclusion: Gender, smoking habits, and use of PPE are associated with respiratory complaints in street sweepers.   Keywords:  Respiratory Complaints, Street Sweeper

Abstract

Backgroung: The implementation of Occupational Safety and Health (OHS) in the electricity sector is crucial given the high occupational risks. The PLN Unit, which operates a 100 MW gas-fired power plant, employs 14 field workers exposed to various potential hazards. Initial observations indicate a gap between the applicable OHS policy and the practice of using PPE in the field. Purpose: To describe the implementation of OHS among field workers at the PLN Unit, covering aspects of occupational health (weather and physical-mental conditions) and occupational safety (people, supervision, and equipment availability). Method: Qualitative research with a descriptive design was conducted from August 2025 to March 2026 at the PLN Payo Selincah Unit with six purposively selected informants. Data were collected through in-depth interviews, observation, and documentation. Analysis used the Miles and Huberman model, supported by NVivo software. Results: Institutionally, OHS has been well-implemented, supported by the OHS Management System (SMK3) policy, the Zero Accident Award, IP-Safe, annual PPE replacement, JSA (Workplace Safety Assessment), LOTO (Lotto of Action), and emergency response simulations. However, critical challenges in the field include inconsistent use of PPE during emergencies, limited OHS personnel, suboptimal leadership exemplars, and workload pressures that impact OHS compliance. Conclusion: OHS implementation at PLN Units is quite good institutionally but requires strengthening leadership exemplary behavior, additional OHS personnel, emergency PPE protocols, technical solutions for cramped engine rooms, and a sustainable OHS culture program to foster compliance through self-awareness.   Keywords: Occupational Health And Safety; Field Workers; PLN; Safety Culture; PPE Compliance

Abstract

Background: Diabetes mellitus is a metabolic disorder characterized by hyperglycemia caused by abnormalities in insulin secretion, insulin action, or both. Tomato juice contains lycopene, antioxidants, and fiber which are believed to help reduce blood glucose levels in patients with diabetes mellitus. Purpose: To determine the effect of tomato juice on reducing blood sugar levels in patients with diabetes mellitus. Method: This study used a quasi-experimental method with a two-group design. The study population was 21 patients with type 2 diabetes mellitus, with a sample of 20 respondents using the total population technique. Data analysis used the t-dependent test. Results: The average reduction in blood sugar levels in the intervention group was 50.60 mg/dl, while in the control group it was 25.20 mg/dl. Statistical analysis showed a p-value of 0.026, indicating a significant effect of tomato juice on reducing blood sugar levels. Abstract

Background: Hypertension is a chronic disease often accompanied by headaches due to elevated blood pressure and increased vascular tension. This condition requires appropriate treatment, both pharmacologically and non-pharmacologically. One non-pharmacological intervention is Progressive Muscle Relaxation Therapy (PMR), a gradual muscle-relaxation technique that aims to reduce muscle tension, increase comfort, and lower blood pressure. Purpose: To determine the effectiveness of Progressive Muscle Relaxation Therapy (PMR) in reducing blood pressure in hypertensive patients with headaches. Method: The method used is a case study with a pretest-posttest approach. The PMR intervention was given for three consecutive days to hypertensive patients. Blood pressure and pain scale measurements using a Visual Analog Scale (VAS) were carried out before and after the intervention. Results: Blood pressure and headache intensity decreased gradually. On the first day, blood pressure decreased from 202/100 mmHg to 188/102 mmHg with VAS remaining 6, the second day from 180/98 mmHg to 164/98 mmHg with VAS decreasing from 6 to 4, and the third day from 174/100 mmHg to 150/88 mmHg with VAS decreasing from 4 to 2. Conclusion: Progressive Muscle Relaxation Therapy (PMR) is an effective complementary therapy for lowering blood pressure and reducing headaches in hypertensive patients.   Keywords: Blood Pressure; Headache; Non-Pharmacological Therapy; Hypertension; Progressive Muscle Relaxation Therapy (PMR).

Abstract

Background: Social isolation is a common cause of death in schizophrenia patients and is characterized by withdrawal, poor communication, and difficulty interacting with others. Interaction training is a surgical intervention that can help improve patients' social skills during treatment. Purpose: To determine the effect of interaction training on changes in signs and symptoms of social isolation in patients. Method: This study used a case study design with a pre-test and post-test approach. The interaction training intervention was delivered over 14 days through therapeutic communication and gradual social interaction training. Measurements were conducted using an observation sheet for social interaction skills, which included communication, eye contact, cooperation, and social participation. Data were analyzed descriptively based on changes in scores before and after the intervention. Results: The study showed an improvement in the patient's social interaction skills after the interaction training. The social interaction score increased from 18 (poor) on the first day to 34 (good) on the third day. The patient began to make eye contact, speak more clearly, participate in group activities, and demonstrate a friendly and cooperative attitude toward others. Conclusion: Interaction training can help reduce signs and symptoms of social isolation in patients with mental illness. This intervention can be implemented as a self-help measure to improve communication skills and social relationships in patients during treatment in a psychiatric hospital.   Keywords: Interaction Training; Mental Illness; Schizophrenia; Social Interaction; Social Isolation.

Abstract

Background: Hemorrhagic stroke has a very high mortality and disability rate. According to the World Stroke Organization, there will be 11.9 million new cases of stroke and 7 million deaths annually by 2025. Hemorrhagic stroke accounts for 28.8% of stroke cases globally and approximately 32.9% in Indonesia. Age and gender are known to play a role in the epidemiology of hemorrhagic stroke, but their relationship to severity based on hematoma volume and supratentorial hemorrhage location remains understudied. Purpose: To determine the relationship between gender and age and hemorrhagic stroke severity based on hematoma volume and supratentorial hemorrhage location on head CT scans at Dr. H. Abdul Moeloek Regional General Hospital (RSUD) from 2022 to 2025. Method: This analytical observational study used a retrospective cross-sectional approach, involving 71 participants using a total sampling technique. The study found that the largest age group was 45-65 years old (24 patients (33.8%), and 36 patients were male (50.7%). Hematoma volumes <30 ml were found in 51 patients (71.8%), and deep cerebral hemorrhage in 49 patients (69.0%). Results: The chi-square test showed no association between gender and age with hematoma volume or supraterritorial bleeding location (p>0.05). Abstract

Background: The reliability of laboratory diagnoses is significantly influenced by the performance and readiness of medical equipment. In healthcare facilities with remote geographic locations such as Wedabay Medical Center (WMC) in North Maluku, preventive maintenance of medical equipment is a crucial strategy for maintaining service quality and avoiding service disruptions due to equipment failure. Purpose: To formulate a managerial strategy for preventive maintenance to improve diagnostic reliability. Method: The study used a qualitative approach with a case study design. Data were obtained through in-depth interviews, observations, maintenance document reviews, and focus group discussions (FGDs). Informants were selected using purposive sampling from laboratory staff, medical personnel, and management. Data analysis was conducted descriptively using a reduction, presentation, and verification model, and was supported by a SWOT analysis. Results: Preventive maintenance has been implemented routinely with the support of management commitment and a regular schedule. However, challenges remain, such as limited technical resources, vendor dependence, remote location, and suboptimal documentation. Conclusion: Recommended strategies include strengthening the documentation system, optimizing vendor collaboration, and developing a more structured maintenance management system. Abstract

Background: Implementing an Occupational Safety and Health Management System (SMK3) is crucial for preventing workplace accidents and creating a safe work environment. The increasing number of workplace accidents in Indonesia indicates that implementing occupational safety remains a challenge in various sectors, including the electricity sector, which faces high risks such as electrical hazards, construction workers, and workers at heights. Purpose: To determine the implementation of SMK3 based on Government Regulation Number 50 of 2012. Method: Qualitative research with a descriptive approach. Research informants were selected using a purposive sampling technique. Data collection was conducted through in-depth interviews, observation, and document review. Data were then analyzed through data reduction, data presentation, and conclusion drawing. Results: The implementation of SMK3 at PT PLN (Persero) Sumbagteng 3 Project Implementation Unit has been running well in accordance with Government Regulation Number 50 of 2012. The establishment of OSH policies, OSH planning, implementation of OSH plans, monitoring and evaluation of OSH performance, and review and improvement of OSH performance have been carried out effectively. Furthermore, the implementation of HIRARC, Job Safety Analysis (JSA), and Safe Place has also been optimal in supporting a work safety culture. Conclusion: The implementation of the Occupational Health and Safety Management System (SMK3) at PT PLN (Persero) Sumbagteng 3 Project Implementation Unit complies with Government Regulation Number 50 of 2012, supporting the creation of a physically and psychologically safe work environment. The combined implementation of SMK3, Safe Place, JSA, and HIRARC plays a crucial role in strengthening the work safety culture and supporting a safe, healthy, and productive work environment.   Keywords: HIRARC; Job Safety Analysis; Occupational Safety and Health Management System; Occupational Safety; Safe Place.

Abstract

Background: The Contribution Assistance Recipients (PBI) program is a national health insurance scheme in which premiums are fully subsidized by the government for low-income and impoverished populations. Muaro Jambi Regency records the highest proportion of PBI participants (84.73%) in the BPJS Jambi Branch area, with Muara Kumpeh Puskesmas holding the highest PBI enrollment (22.45%) among all puskesmas in the regencyPurpose: To analyze the impact of PBI membership on household consumption patterns, including food consumption, non-food consumption, education expenditure, and tobacco expenditure. Method: Data analysis was performed using the Wilcoxon Signed Rank Test to determine differences in household consumption before and after PBI enrollment, with a significance threshold of p < 0.05.Results: Significant differences (p-value = 0.000 < 0.05) were found across all household consumption variables before and after PBI enrollment. (1) Food consumption increased from an average of IDR 1,061,263 to IDR 1,668,895 per month. (2) Non-food consumption increased from an average of IDR 790,105 to IDR 1,110,874 per month. (3) Education expenditure slightly decreased from an average of IDR 437,404 to IDR 421,011 per month (Z = -4.811), influenced by seasonal factors and inter-household variation. (4) Tobacco expenditure increased from an average of IDR 545,426 to IDR 628,316 per month (Z = -6.837). Conclusion: PBI membership significantly impacted household consumption patterns in the working area of Muara Kumpeh Puskesmas. Food and non-food consumption increased substantially, reflecting the economic benefit of reduced direct health expenditures. However, the increase in tobacco expenditure indicates that not all additional financial capacity was allocated to productive needs.   Keywords: Household Consumption; National Health Insurance; PBI.
