Abstract

Miliary tuberculosis is a form of tuberculosis characterized by widespread spread in the human body with small size lesions (1-5 mm). The word "miliary" comes from the typical pattern on chest x-rays which has many small spots distributed throughout the lung area with an appearance similar to millet seeds, so it is called 'miliaria' TB. In Indonesia, miliary tuberculosis is also an important form of tuberculosis to pay attention to, but specific data about its prevalence is often combined with data about general tuberculosis. Histology-based epidemiological studies from autopsies estimate miliary TB to account for less than 2% of all TB cases. Many patients initially experience a productive cough, difficulty breathing, chest pain, and hemoptysis. The combination of OAT given for miliary TB is the same as pulmonary TB, namely 2 RHZE / 4 RH. In certain conditions, corticosteroids can be given. The death rate from miliary TB is around 15% to 20% in children and 25% to 30% in adults. Factors generally associated with a poor prognosis of miliary TB are malnutrition, hypoalbuminemia, increased serum bilirubin levels, leukopenia, leukocytosis, lymphopenia, thrombocytopenia, and pleural or pericardial involvement.

Abstract

Trembesi (Samanea saman (Jacq.) Merr.) leaves contain various secondary metabolites, including flavonoids, alkaloids, saponins, tannins, phenolic compounds, and terpenoids, which are known to possess diverse biological activities. However, information regarding the safety profile of the 70% ethanol extract of S. saman leaves remains limited. Therefore, a preliminary toxicity evaluation is necessary. This study aimed to determine the toxicity profile of the 70% ethanol extract of S. saman leaves based on the LC₅₀ value using Artemia salina Leach larvae. This laboratory experimental study employed the Brine Shrimp Lethality Test (BSLT) with a post-test only control group design. The extract was prepared by maceration using 70% ethanol and tested at concentrations of 10, 100, 500, 1,000, and 5,000 ppm. Larval mortality was observed after 24 hours of exposure, and the data were analyzed using probit analysis to determine the LC₅₀ value. The results showed that larval mortality increased with increasing extract concentration, reaching 18.92%, 52.78%, 79.55%, 91.23%, and 97.22%, respectively. Probit analysis indicated an LC₅₀ value of approximately 72.66 mg/L, classifying the extract as toxic according to the toxicity classification proposed by Haryanto et al. (2023). These findings indicate that the 70% ethanol extract of S. saman leaves exhibits significant toxicity toward Artemia salina Leach larvae and may contain bioactive compounds with potential pharmacological applications. Further toxicity and pharmacological studies are recommended to evaluate its safety and therapeutic potential and is useful as a natural pesticide.

Abstract

Acne vulgaris is a chronic inflammatory skin condition affecting 80–85% of adolescents, with Staphylococcus aureus infection being one of its main triggers. Kirinyuh leaves (Chromolaena odorata) and bidara leaves (Ziziphus mauritiana) contain antibacterial bioactive compounds such as alkaloids, flavonoids, tannins, and saponins, making the combination of both potentially effective as a natural antiseptic cleanser in solid soap. This study aimed to formulate a solid soap combining kirinyuh and bidara leaf extracts that meets the physical quality standards of SNI 3532:2021, as well as to determine the concentration with optimum inhibitory activity against S. aureus ATCC 25923. This laboratory experimental study used 70% ethanol maceration and saponification of oil bases (VCO, olive, and palm oil) to produce four formulas (F0 as the base control; F1, F2, F3 with varying extract concentrations), which were subsequently evaluated for physical quality, hedonic properties, and irritation in 20 panelists. Antibacterial activity was tested using the Kirby-Bauer disc diffusion method, and the resulting data were analyzed using One Way ANOVA and Tukey HSD tests. All formulas met SNI criteria and caused no irritation, with Formula 2 (2.5% kirinyuh : 7.5% bidara) producing the largest inhibition zone (12.20 ± 0.67 mm, strong category), which was statistically comparable to the positive control, Asepso soap (p = 0.412). This soap effectively inhibited S. aureus with good physical quality, and Formula 2 was determined to be the most optimum concentration.

Abstract

Postoperative patients experience a metabolic stress response that may affect nitrogen balance and nutritional status, potentially leading to a prolonged length of hospital stay. This study aimed to analyze the relationship between nitrogen balance and nutritional status with the length of hospital stay among postoperative patients at Ulin Regional General Hospital, Banjarmasin. This study was an analytical observational study with a cohort prospektif design. The study sample consisted of 15 respondents selected using the total sampling technique. The data collected included respondent characteristics, nitrogen balance, nutritional status based on Mid-Upper Arm Circumference (MUAC), and length of hospital stay. The relationships between variables were analyzed using Spearman’s Rank correlation test. The results showed that 66.7% of respondents had a positive nitrogen balance, 53.3% had poor nutritional status before surgery, and 80% had a hospital stay of ≤7 days. The analysis indicated that nitrogen balance was not significantly associated with the length of hospital stay (p = 0.934; r = 0.023). Similarly, preoperative nutritional status (p = 0.936; r = 0.022) and postoperative nutritional status (p = 0.657; r = 0.125) were not significantly associated with the length of hospital stay among postoperative patients at Ulin Regional General Hospital, Banjarmasin. It is expected that hospitals, particularly healthcare professionals and nutrition departments, will enhance the monitoring of nutritional status and nitrogen balance while optimizing nutritional intake for postoperative patients.

Abstract

Breast cancer is the cancer with the highest incidence and mortality among women worldwide. In Indonesia, delayed diagnosis remains largely associated with low awareness of symptoms and limited access to early screening services at primary healthcare facilities. This study aims to design, develop, and evaluate the functionality of MAMAI-Check v1.8.5, an integrated breast cancer surveillance system based on a Progressive Web App (PWA) that combines self-education, symptom assessment, and risk screening with real-time data reporting for primary healthcare workers. The study employed a Research and Development (R&D) approach through four software engineering stages: requirements and system architecture analysis, user interface and user experience (UI/UX) design, system development, and black-box functionality testing. The application was developed using React 18 and TypeScript, with Tailwind CSS for layout design, and integrated with Firebase Firestore and Google Sheets for real-time data synchronization, supported by a service worker for offline access. The development resulted in eight integrated modules, including a home page, identity form and body mass index (BMI) calculator, breast self-examination (SADARI) and clinical breast examination (SADANIS) education center, interactive symptom checklist, self-administered risk screening questionnaire, automated recommendation and history module, FAQ, and a healthcare worker dashboard for population surveillance monitoring. Black-box testing demonstrated that all modules, input validation, risk assessment algorithms, and data synchronization operated according to the technical specifications and screening criteria established by the Indonesian Ministry of Health. These findings indicate that a lightweight PWA that can be installed without an application store can expand breast health awareness education and structured risk triage to primary healthcare facilities without replacing clinical diagnosis, making it a potential digital complement to early detection programs in resource-limited settings.
