Abstract

Building asset management is a strategic function that supports effective and efficient hospital services, particularly in regional general hospitals. This study aimed to describe building asset management at Prof. Dr. H. Aloei Saboe Regional General Hospital in Gorontalo City based on a corrective maintenance approach, viewed from the perspective of hospital administration, covering damage identification, handling procedures, repair implementation, administration and documentation, and monitoring and evaluation. A descriptive qualitative approach was employed, involving eight purposively selected informants comprising hospital management, technical staff, and service users. Data were collected through in-depth interviews, observation, and documentation, and analyzed using the Miles and Huberman interactive model. The findings show that damage identification is conducted participatively through reporting and routine inspection; handling procedures follow a structured standard operating procedure; repair implementation combines immediate and deferred corrective maintenance depending on severity; documentation supports accountability although it remains largely manual; and monitoring and evaluation are carried out through direct supervision and periodic audits. Overall, building asset management at the hospital reflects the principles of hospital administration and forms an integrated corrective maintenance cycle that supports continuous improvement, although strengthening human resource competence and digital documentation systems is still needed

Abstract

Keterlibatan suami saat persalinan dapat memperkuat dukungan emosional dan praktis bagi ibu, tetapi tidak semua suami memiliki pengetahuan dan kesiapan untuk menjalankan peran pendamping secara tepat. Penelitian ini menganalisis pengaruh promosi kesehatan menggunakan booklet terhadap pengetahuan, sikap, dan peran serta suami dalam mendampingi persalinan di UPTD Puskesmas Simpang Kiri Kota Subulussalam. Penelitian menggunakan desain kuasi eksperimen pretest-posttest dengan kelompok kontrol pada 40 suami dari ibu hamil usia kehamilan 36-40 minggu. Kelompok intervensi (n=20) menerima promosi kesehatan dan booklet, sedangkan kelompok kontrol (n=20) menerima promosi kesehatan tanpa booklet. Pengetahuan dan sikap diukur sebelum dan sesudah edukasi, sedangkan peran serta diukur setelah persalinan. Perubahan dalam kelompok dianalisis dengan uji Wilcoxon dan perbedaan posttest antarkelompok dengan independent samples t-test. Skor posttest pengetahuan lebih tinggi pada kelompok intervensi dibanding kontrol (7,95 vs 6,70; p=0,024), demikian pula sikap (27,85 vs 24,15; p=0,006) dan peran serta suami (26,95 vs 22,90; p<0,001). Promosi kesehatan berbantuan booklet efektif meningkatkan kesiapan dan keterlibatan suami dalam pendampingan persalinan.

Abstract

Effective English communication is increasingly required for pharmacists who deliver patient counseling in globalized healthcare settings, yet English for Pharmacy in Indonesia is often generic and rarely mapped to real counseling situations. This study aimed to identify and explore the English communication needs of pharmacy students in patient counseling and to determine the communication competencies that should be prioritized in pharmacy-related English instruction. A sequential explanatory mixed-methods design was applied at Universitas Tjut Nyak Dhien. In the quantitative phase, 120 pharmacy students completed a 20-item English Communication Needs Questionnaire using a 5-point Likert scale; in the qualitative phase, 12 students participated in semi-structured interviews analyzed thematically. Descriptive statistics indicated a very high overall need (M = 4.14, SD = 0.57). The medication information domain was the highest priority (M = 4.39), led by explaining medication instructions (M = 4.48), whereas general English conversation received the lowest priority (M = 3.43). Qualitative findings converged on difficulty simplifying technical information, limited spontaneous speaking, vocabulary and pronunciation challenges, and demand for authentic counseling practice. Integrated interpretation indicates that English for Pharmacy should emphasize profession-specific counseling communication supported by role-play and simulation rather than general English.

Abstract

Diabetes mellitus (DM) is a non-communicable disease with a continuously rising global prevalence and is frequently accompanied by hypertension as a complication that aggravates cardiovascular morbidity and mortality. Low physical activity and poor sleep quality have been proposed as modifiable behavioral risk factors, yet local evidence from primary-care settings in North Sumatra remains limited. This study analyzed the association between physical activity and sleep quality and the risk of hypertension among patients with DM at UPT Puskesmas Medan Johor in 2025. A quantitative analytical study with a cross-sectional design was conducted on 300 respondents selected through systematic random sampling. Physical activity was measured using the International Physical Activity Questionnaire (IPAQ), sleep quality was measured using the Pittsburgh Sleep Quality Index (PSQI), and blood pressure was measured using a sphygmomanometer. Data were analyzed using the Chi-square test and binary logistic regression at α = 0.05. Bivariate analysis showed that physical activity (p = 0.003) and sleep quality (p = 0.001) were significantly associated with the risk of hypertension, while treatment adherence also showed a significant association (p = 0.000). Multivariate analysis indicated that the joint model containing physical activity, sleep quality, and treatment adherence was significantly associated with hypertension (omnibus p = 0.003; Nagelkerke R² = 0.060), although no single variable remained significant after adjustment. Physical activity and sleep quality are behavioral determinants that should be integrated into hypertension prevention programs for DM patients in primary care.

Abstract

Background: Soursop leaves (Annona muricata L.) contain bioactive compounds including acetogenins, flavonoids, alkaloids, and tannins, which possess antioxidant, anti-inflammatory, and antitumor activities. Transdermal patch delivery systems offer significant advantages by bypassing hepatic first-pass metabolism, enabling controlled drug release, and improving patient adherence. Two independent formulation strategies were pursued: (1) a hydrophilic–hydrophobic PVP–Ethyl Cellulose (EC) matrix system, and (2) a chitosan-based biopolymer matrix system. Objectives: This study aimed to formulate and comparatively evaluate transdermal patches of soursop leaf ethanol extract using PVP K-30/ethyl cellulose (Study 1) and chitosan (Study 2) as polymer systems, and to determine the optimal formulation for each system based on physicochemical characteristics and in vitro diffusion performance. Methods: Ethanol extracts were prepared by maceration with 96% ethanol. Study 1 evaluated three PVP:EC ratios (F1: 1:1, F2: 1:2, F3: 1:3). Study 2 evaluated three chitosan concentrations (F1: 250 mg/50%, F2: 225 mg/45%, F3: 200 mg/40%). Both systems incorporated PEG 400 as plasticizer and oleic acid as penetration enhancer. Patches were evaluated for organoleptic properties, thickness, pH, folding endurance, and in vitro diffusion using Franz diffusion cells with phosphate buffer pH 7.4, measured by UV-Vis spectrophotometry at 266 nm. Results: Both polymer systems produced patches meeting pharmacopoeial physical requirements. Study 1: pH 5.26–5.47, thickness 0.1 mm, folding endurance >300; optimal formula F3 (PVP:EC = 1:3) achieved the highest cumulative diffusion of 119.087 µg/cm² at 360 min (R² calibration = 0.99909). Study 2: pH 4.30–4.59, thickness 0.35 mm, folding endurance >300; optimal formula F3 (chitosan 40%) achieved the highest cumulative diffusion of 327.44 µg/cm at 360 min with cumulative release of 0.655%. A clear inverse relationship between chitosan concentration and diffusion rate was established. Conclusion: Soursop leaf ethanol extract can be successfully formulated into transdermal patches using both PVP–EC and chitosan polymer systems. The chitosan-based system (F3: 40%) demonstrated superior total drug diffusion, while the PVP–EC system (F3: 1:3) produced a thinner, more uniform patch with controlled diffusion. Both formulations are promising candidates for further in vivo and stability studies.

Abstract

Background: Nasopharyngeal carcinoma (NPC) is a type of epithelial cancer arising from the nasopharyngeal mucosa, with tumors most commonly found in the fossa of Rosenmüller. NPC ranks as the fourth most prevalent cancer in Indonesia, after cervical, breast, and skin cancers. However, its exact pathogenesis remains unclear. Aims: This study aimed to determine the relationship between PTEN gene expression and histopathological types in NPC patients, and to explore its potential as a diagnostic and therapeutic target. Methods: This was an analytic study with a cross-sectional design. Samples were obtained from paraffin-embedded tissue blocks, followed by RNA isolation and synthesis. RT-PCR was subsequently performed and analyzed using a Rotor-Gene system. Statistical analysis was conducted using SPSS software. Results: Of 38 NPC patients, the majority were male (57.9%), with the most common age group being 41–60 years (71.1%). Based on histopathological type, 60.5% were diagnosed with non-keratinizing squamous cell carcinoma (SCC) and 39.5% with undifferentiated carcinoma. Statistical analysis of the relationship between PTEN expression levels and histopathological type yielded a p-value of 0.413 (p > 0.05). Conclusion: B There was no statistically significant relationship between PTEN expression levels and the histopathological type of NPC.

Abstract

Diabetes mellitus is a chronic disease caused by the body's inability to produce or use insulin effectively. The increasing prevalence of diabetes mellitus in Indonesia, reaching 10.9%, makes health promotion an important intervention to improve public knowledge in prevention efforts. This study aimed to determine the effectiveness of health promotion through direct counseling, video, and leaflet media in improving public knowledge about diabetes mellitus prevention, and to identify the most effective medium among the three. This was a quantitative study using a quasi-experimental method with a Pretest-Posttest Multiple Group Design. The study was conducted in Tangkahan Village, Namorambe District, Deli Serdang Regency in February 2026. The population consisted of 125 people with diabetes mellitus, and a sample of 96 respondents was obtained, divided into three intervention groups of 32 each. Data were collected using a validated and reliable knowledge questionnaire. Data analysis used the Wilcoxon Signed Rank Test and Kruskal-Wallis Test. Results showed a significant difference in knowledge before and after intervention in all groups with p-value = 0.000 (p < 0.05). Mean knowledge scores in the direct counseling group increased from 15.8 to 27.8 (difference 12.00), the video group from 13.3 to 22.0 (difference 8.7), and the leaflet group from 13.9 to 19.22 (difference 5.32). Based on the Kruskal-Wallis test, direct counseling showed the highest mean rank (70.75), followed by video (49.91) and leaflet (24.84), with H = 43.832 and p = 0.000. It was concluded that all three media were effective, with direct counseling being the most effective. Healthcare workers and village governments are recommended to integrate direct counseling-based health promotion as a routine diabetes mellitus prevention program.

Abstract

Nutritional knowledge is often assumed to shape food-related decisions, yet its translation into daily menu choices among Culinary Arts students remains underexamined. This study aims to determine the level of nutritional knowledge, identify the tendency of healthy menu choices, and analyze the relationship between nutritional knowledge and healthy menu choices among students of the Culinary Arts Concentration, Department of Family Welfare Education, Faculty of Engineering, State University of Manado. A quantitative approach with a correlational design was employed. The population comprised 17 active students, all of whom were recruited through total sampling. Data were collected using a nutritional knowledge test and a healthy menu choice questionnaire, and analyzed using the Kendall's Tau-b correlation test. Results showed that 16 students (94.1%) had good nutritional knowledge and 1 (5.9%) had sufficient knowledge, while healthy menu choices were categorized as good in 4 students (23.5%) and sufficient in 13 students (76.5%). The Kendall's Tau-b analysis yielded a correlation coefficient of -0.030 with a significance value of 0.883 (p > 0.05), indicating no statistically significant relationship between the two variables. It is concluded that although the students possess good nutritional knowledge, this knowledge is not necessarily reflected in their healthy menu choices.

Abstract

Maternal and infant mortality rates in Indonesia remain high, with one contributing factor being the low rate of exclusive breastfeeding. In the working area of UPTD Puskesmas Plered, Cirebon Regency, the exclusive breastfeeding coverage has not yet reached the national target. Oxytocin massage is a natural method that can stimulate the oxytocin hormone to improve breast milk production. This study aims to determine the effect of oxytocin massage on the smoothness of breast milk production in breastfeeding mothers. This research used a pre-experimental method with a one-group pre-test–post-test design and a quantitative approach. A total of 20 postpartum breastfeeding mothers were selected using purposive sampling. Data were collected using checklists and questionnaires. The Wilcoxon test with a significance level of 0.05 was used to assess the effect of oxytocin massage on breast milk production. The results showed that before the oxytocin massage intervention, most mothers experienced poor milk production (65%). After the massage, the majority of mothers experienced smooth milk production (95%). The Wilcoxon test showed a significant result with a p-value of 0.000 (<0.05), indicating that oxytocin massage significantly affects the smoothness of breast milk production. Based on these findings, it can be concluded that oxytocin massage is effective in improving breast milk production. Therefore, midwives are encouraged to actively provide education to breastfeeding mothers regarding the benefits of oxytocin massage. Mothers are also expected to better understand the importance of this method in supporting the success of exclusive breastfeeding. Future researchers are recommended to use larger sample sizes and more varied research designs.

Abstract

Hypertension is a public health problem with high prevalence whose management is strongly influenced by self care behavior. This study aims to determine the level of success in changing self care behavior in maintaining blood pressure among hypertensive patients. The study used a quantitative design with a one-group pretest-posttest approach involving 60 respondents selected through purposive sampling. Data were collected using a closed questionnaire administered face to face before and after the intervention, then analyzed using the Wilcoxon Signed Rank Test and Spearman Correlation. The results showed a significant increase in the proportion of respondents in the Good self care category from 53.3% to 81.7% (p=0.000), accompanied by a decrease in systolic blood pressure from 137.9 mmHg to 134.7 mmHg and diastolic blood pressure from 81.4 mmHg to 80.5 mmHg (p<0.05). Self care was significantly associated with systolic blood pressure at pretest (p=0.020), but this association weakened and became non-significant at posttest (p=0.450). This study concludes that self care education intervention is effective in improving self care behavior and reducing blood pressure among hypertensive patients. Primary healthcare services are recommended to integrate ongoing self care education to support optimal blood pressure control.
