Abstract

Background:A diastema is a gap or distance between two or more adjacent teeth. This condition can affect a person's appearance and confidence. Treatment in cases involving aesthetics can be treated by multidisciplinary approach, one of which is orthodontics.Case Report:A 13-year-old boy presented with chief complaints of gap at his front teeth. Intra oral examination showed multiple diastemas on the upper and lower teeth, minimal crowding on the mandibular anterior teeth, and flaring/proclination of the maxillary anterior teeth, class I molar relation on the right and left sides, class I canine relation on the right and left sides, low palate (34.5%), overbite 2.5 mm, overjet 5 mm and maxillary midline shifted to the left by 1 mm. Treatment was performed with Roth system fixed orthodontic appliances for diastema closure.Conclusion:The evaluation of the aetiology of the diastema is an important issue before starting any orthodontic correction. In caseof multiple diastema, treatment with fixed orthodontic is an excellent alternative for diastema closure, especially when the incisors are of normal color, shape and size.

Abstract

Background: Thermoplastic nylon is the denture base that dentists choose today. Thermoplastic nylon has a more aesthetic appearance in terms of color and does not use wire as a support. This thermoplastic nylon belongs to the polyimide class. This research aims to determine the effect of Arabica coffee drinks on changes in the base color of thermoplastic nylon dentures. Method: This type of research is laboratory experimental. The number of samples consisted of 24 samples divided into four treatment groups, two control groups in distilled water, and two immersion groups in Arabica coffee. The samples were immersed in the solution for 7 days and 14 days. The research data did not show changes in hue values, but there were changes in value and chroma values. The results: measuring value and chroma using the Kruskal-Wallis analysis test showed a significant color change of P = 0.00 (P 0.05). The results of the data analysis show that immersing thermoplastic nylon in an arabica coffee (Coffea arabica) environment on the value and chroma affects the color change of thermoplastic nylon denture elements. The conclusion: this study shows that there is an effect of changing the base color of thermoplastic nylon dentures when soaked in arabica coffee (Coffea arabica) solution.

Abstract

Background: Metabolic and bariatric surgery, such as laparoscopic sleeve gastrectomy (LSG), is effective for obese patients with type 2 diabetes mellitus (DM), proven to improve metabolic profiles, control hyperglycemia, and provide DM remission in 55-85% of patients, as well as reducing medication use and body weight (BW) within one year after surgery. Case Report: A 23-year-old female patient with grade II obesity (BMI 31.42 kg/m), DM, and cholecystolithiasis underwent laparoscopic cholecystectomy and LSG. Perioperative clinical nutrition therapy was performed to support the success of the surgery, including BW management, a preoperative diet through the Enhanced Recovery After Surgery (ERAS) protocol, and a gradual postoperative diet from liquid consistency. Evaluation 5 days after surgery showed decreased BW, waist circumference, body fat mass, visceral fat, and HbA1C without complications. The patient was discharged on the 6th day and was advised to adopt a transitional diet, multivitamins, and calcium supplementation. The mechanism of LSG involves calorie restriction, early satiety due to decreased gastric volume, and faster gastric emptying time. Conclusion: Short-term perioperative LSG nutrition therapy aims to meet nutritional needs, support weight loss, and improve metabolic profiles, thus contributing to clinical success for patients with obesity, type 2 diabetes, and metabolic syndrome.

Abstract

Background: Post-extraction wound healing usually proceeds but sometimes has problems and causes complications in the form of a dry socket. Streptococcus viridans bacterial infection is one of the causes of dry socket, therefore prevention of infection can be done by providing antibiotic or antibacterial (antiseptic) therapy. Noni fruit (Morinda citrifolia) contains many phenols, saponins, and alkaloids that can potentially be antibacterials. Objective: This study aims to determine the effectiveness of noni (Morinda citrifolia) extract against Streptococcus viridans bacteria in post-extraction sockets. Methods: This type of research is a laboratory experiment with Post test only control group design, where this study uses the Kirby-Bauer diffusion method with Streptococcus viridans bacteria samples and uses noni fruit extract (Morinda citrifolia) with concentrations of 3.125%, 6.25%, 12.5%, 25%, and 50%; Positive control Chlorhexidine digluconate 0.2%; And negative control DMSO. Results: The data analysis using Kruskal-Wallis showed a p-value of 0.000 on inhibition, which showed that noni fruit extract could inhibit the growth of Streptococcus viridans bacteria. Conclusion: Noni fruit extract (Morinda citrifolia) effectively inhibits the growth of Streptococcus viridans bacteria with an effective dose of 50%, but the antibacterial activity is still below the activity of Chlorhexidine gluconate 0.2%.

Abstract

Background: Benign tumors are collections of cells, tissues or organs that grow slowly and usually enlarge with peripheral expansion, pushing on surrounding structures. Squamous odontogenic tumor (SOT) is a very rare benign epithelial intraosseous tumor, which can cause arising from the neoplastic transformation of Malassez epithelial cells on the lateral surface of the periodontal ligament. Objective : To discuss the management of odontogenic squamous tumors using conservative surgical excision. The goal of treatment for oral myxoma was surgical excision. Case management : A 51 year old female patient came to the SULTAN FATAH Hospital Polyclinic, Demak Regency on May 26 2023 with complaints of a lump on the upper left gum of the jaw. The patient felt the lump about 1 year ago. The patient does not feel pain in the lump. Clinical examination showed a lump in the upper jaw on the front of the patient's left side.Conclusion : Management of the patient undergoing surgical excision of the tumor with general anesthesia and infiltration in the case of odontogenic squamous tumor, based on subjective examination the patient complained of a lump with a tumor mass measuring 10 x 8 cm, redder than the surrounding tissue, round, not moving, attached to the gums of the upper jaw.

Abstract

Periodontal disease is a global prevalent burden on well-being and health-related quality of life. Adequate plaque control on a daily routine basis is the most critical thing to achieve periodontal health. Using toothbrushes in combination with dentifrices is one of the most common methods of oral hygiene maintenance. However, natural chewing sticks such as Salvadora persica are still used in many developing countries due to their low cost and religious background. Salvadora persica displays precious biotherapeutics capacity for antimicrobial, antiviral, antioxidant, and anti-inflammatory effects. Recently, a lot of Salvadora persica extract has been found in dentifrices, mouthwash, chewing Gum, and even local delivery drugs such as periodontal chips, which are still in the research stage.The present review aims to summarize the evidence of various pharmacological and therapeutic aspects of the effectiveness of Salvadora persica in promoting periodontal health and the treatment of periodontal disease.

Abstract

Background: Root canal treatment is an effort to maintain the function and aesthetics of teeth. The smear layer is formed from the remains of instrumentation composed of organic and inorganic components. A smear layer in the tooth root canal will benefit the adhesion and colonization of microorganisms, block the action of disinfectant solutions, and prevent medicaments in the tooth root canal from penetrating the dentin tubules in the tooth root canal walls. Irrigation can be used for cleaning and disinfection in areas of the root canal that cannot be reached using instruments. Starfruit contains alkoloids, saponins, tannins, flavonoids, phenols, and triterpenoids. Objective: To determine the effectiveness of star fruit extract (Averrhoa bilimbi L) as a smear layer cleaning agent. Materials and Methods: This researcher used laboratory or experimental testing called Post Test Only Control Design. Samples were taken using the purposive sampling technique involving four treatments and six repetitions. The statistical test used is the Mann-Whiteney test. Results: Based on the Mann-Whiteney test, the test results obtained a p-value of 0.093 which is more significant than 0.05 (p-value 0.05). The results showed no significant difference between the 17% EDTA and 35% extract treatment groups regarding smear layer cleanliness. The 35% extract group is as good as the 17% EDTA group. Conclusion: star fruit extract (Averrhoa bilimbi L) is 35% effective in cleaning the smear layer.

Abstract

Background: Traditional caries removal techniques, like using a vibrating drill, have several drawbacks, including unpleasant sensations, heat that can irritate dental pulp, the need for local anesthesia, high energy consumption, and environmental concerns. A new method, CMCR (chemomechanical caries removal) with Brix 3000, aims to address these issues. It is minimally invasive, painless, and comfortable, promoting a positive attitude towards dental care, especially for pediatric patients, those with anxiety or fear, and high-risk medical patients. Objective: This literature review examines the use of Brix 3000 gel for chemomechanical caries removal. Methods: The study uses a scoping review method, conducting a comprehensive literature search on PubMed, EBSCO, ScienceDirect, Scopus, ClinicalTrials.gov, and Google Scholar databases. The search focused on articles from the last 10 years using keywords like "chemomechanical caries removal," "Papain Based Gel," "Brix 3000," and "Dental caries." Results: Brix 3000 is the latest effective, efficient, and non-cytotoxic CMCR method. It has a shorter application time and lower cytotoxicity compared to other methods. It is the only CMCR agent using Encapsulating Buffer Emulsion (EBE) technology, enhancing proteolysis to remove degraded collagen from infected dentin. Brix 3000 is easy to use, requires no special instruments, offers patient comfort, does not need local anesthesia or drills, and does not irritate healthy tissue. Conclusion: Brix 3000 gel is an effective and efficient chemomechanical agent for caries removal.

Abstract

Background: Myocardial infarction (MI) and hypertension are cardiovascular conditions that frequently coexist and influence each other. Hypertension not only increases the risk of MI but also complicates its clinical management. Therefore, it is crucial to understand the complications arising from this condition and the effective management strategies. Objectives: This systematic review aims to identify and analyze common complications in hypertensive patients experiencing MI and evaluate the optimal management approaches based on the latest literature. Methods: A literature search was conducted in electronic databases such as PubMed, Scopus, and Google Scholar using relevant keywords like "myocardial infarction," "hypertension," "complications," and "management." Articles published in the last five years were selected to ensure the most current information. Inclusion criteria included observational studies, clinical trials, and systematic reviews relevant to the topic. Data were extracted and analyzed descriptively. Results: Of the 1500 articles identified, 45 studies met the inclusion criteria. Major complications found in hypertensive patients with MI included heart failure, arrhythmias, stroke, and left ventricular dysfunction. The most effective management approaches included appropriate use of antihypertensive medications, early reperfusion interventions, and a multidisciplinary approach to controlling cardiovascular risk factors. The use of ACE inhibitors, beta-blockers, and aldosterone antagonists proved to have significant benefits in reducing morbidity and mortality. Conclusions: Complications of myocardial infarction in hypertensive patients require comprehensive and coordinated management. Effective management involves a combination of pharmacological and non-pharmacological therapies focusing on optimal blood pressure control and prevention of further complications. Further research is needed to explore more specific and personalized management strategies in this population.

Abstract

Background: Anterior crossbite is an abnormality in the sagittal direction of one or more maxillary anterior teeth against the mandibular teeth. Although the incidence of this case is small, it can cause aesthetic problems, tooth abrasion, caries, gingivitis, bone resorption, and gingival recession. Case Report: A 24-year-old female patient presented with her upper front teeth positioned behind her lower teeth and a noticeable gap between her upper front teeth. She exhibited an anterior crossbite involving teeth 22 to 32, a central diastema, and crowding of the upper and lower front teeth. Treatment was carried out using a fixed orthodontic with an MBT system and the bracket placement of 22 in an inverted position to get a labial torque effect on the roots. Conclusion: The use of pre-adjusted brackets with the MBT system and the placement of the lateral incisive bracket in an inverted position successfully corrected the anterior crossbite in these teeth both at the crown and root without additional wire bending or auxiliary spring.
