Folia Medica Indonesiana, as indicated by its name, focuses on publishing good quality articles about research and education on health science and medicine in Indonesia. However, due to the fast growth of science and knowledge in these fields, we also welcome submitted articles from around the world, especially the ones that contain related matters from lower-middle income countries. Folia Medica Indonesiana is an open-access, peer-reviewed journal that is published online at least four times a year. The scope covers various aspects of basic medical sciences includes anatomy, physiology, pathology, microbiology, pharmacology, and molecular medicine) and clinical medicine (covers specialties like internal medicine, surgery, pediatrics, oncology, psychiatry, etc). We highlight the pathology and potential treatment of metabolic syndromes and infectious diseases. Folia Medica Indonesiana also encourages the publication of articles about health education. The scope includes, but is not limited to, articles that emphasize on preventive education on certain diseases in a community, also research report of various materials and/or methods to develop medical education. We recognize the importance of this type of articles to be published alongside the assigned topic in each of our yearly issues, to provide our readers with updated information in medical sciences’ research and education simultaneously.

Abstract

Bone graft use in maxillofacial surgery is currently developing, though some problems still persist. One of promising solutions to the problems is application of platelet rich plasma which may accelerate soft tissue healing and bone formation. This study aims to provide basic understanding to the development of PRP application in bone healing where bone graft is applied on human maxillae. Bone defects were made on maxillae of albino rabbits, with an application of bone graft in control group and bone graft plus PRP in treatment group. Histopathological analysis was performed in both groups to assess osteoblast density and collagen tissue area. On day 21st, more osteoblast density and collagen area in treatment group are significantly observed (p<0.05, 95% confidence interval). Observations were made on hard callus formation. In conclusions, platelet rich plasma may increase density of osteoblast in rabbit maxillary bone graft.

Abstract

Cancer patients have a tendency to experience mental disorders, because the physical changes cause frustration and despair. It impacts on anxiety, depression, and social isolation. The purpose of this study was to analyze the influence of mindfullness meditation to stress levels and coping mechanisms in cancer patients. This study used pre experimental approach with one group pre-test post-test design. Samples were 32 cancer patients with composmentis condition in Indonesian Cancer Foundation East Java Branch, taken by total sampling. The instrument was Perceived Stress Scale and Coping Strategies Inventory. Statistics was verified with Wilcoxon Sign Rank Test and Paired T-test P < 0.05. Results showed the effect of Mindfulness Meditation on the level of stress p = 0.001, the adaptive of coping mechanisms p = 0.001 and maldaptive p = 0.003. In conclusion, Mindfulness meditation can cause the respondent to concentrate and focus on themselves, and also strengthen brain regions at the center of attention and executive function, interoception and mental flexibility.

Abstract

Menopausal period is the period of permanent cessation of menstruation due to reduced or lost ovarian activity. One of the effects of decline estrogen levels during menopause is atropic vulvovaginitis. Consumption of phytoestrogens can reduce menopausal symptoms. Phytoestrogen is found in vegetables and fruits such as tomatoes and red melon. The objective of this study was to compare the effectiveness of red melon juice and tomato on the increase of vaginal wall thickness in menopausal rats. This study revealed an alternative of phytoestrogen sources that is more effective to reduce menopausal symptoms, especially those related to vaginal atrophy to improve the quality of life of menopausal women. Subjects of this study were 27 ovariectomized Wistar strain Rattus norvegicus which were s divided into 3 groups randomly, one control group and two treatment groups with tomato juice and red melon juice at a dose of 330 mg/kgBW/day administered for 28 days. The variable of this study were the thickness of the vaginal wall which was precipitated by Hematoxiline Eosine. The highest vaginal wall thickness was found in the treatment group of red melon juice of 879.89 ± 70.52 µm and the lowest of the control group (K) of 643.3 ± 58.33 µm. The results of Anova test showed that there was a significant difference between control group and treatment groups, the tomato juice and red melon juice with a dose of 330 mg/kgBW/day, so it can be concluded that red melon juice with a dose of 330mg/Kg BW/day has the best effect in increasing the thickness of vaginal wall of menopausal rats as compared with tomato juice.

Abstract

The interaction of mother and infant is a fundamental basis for the development of the infant. It can also determine the success of the baby in the future. About 4 out of 10 infants experience problems interacting with their parents. The NBO system was created to sensitize parents on their infant's competencies to help them understand their infant's behavior, and promote positive interactions between parents and their new infant. This study aimed to evaluate the effectiveness of the NBO system to improve mother-infant interactions and find out the mothers' knowledge about their infant's behavior when participating in this observation. This study was a cross-sectional study consisting of all the newborns and their mothers admitted to the Dr. Soetomo General Academic Hospital Surabaya from May 2019 to January 2020 who met the inclusion criteria. Mothers and infants participated in the NBO within 1-3 days after delivering their infants and one month postpartum for the second NBO. The NBO was carried out with 35 mothers and infants. The mothers' age was on average 28.5±5.98 years old, multiparous mothers accounted for 71.4%, most infants were male (65.7%), the median gestational age was 38 weeks with the median birth weight 2900 grams. The median score of mothers' knowledge about their infants before the NBO session was 2 (1-6). After the session, these median scores increased to 7 (5–10) significantly (p<0.001). The NBO system was significant to strengthen mother-infant interaction and increased the mothers' knowledge about their infants' behavior.

Abstract

Highlights: • Computer vision syndrome (CVS) that causes dry eyes becomes a health problem for college students. • Sodium hyaluronate eyedrops can be used as one of the CVS therapy strategies for students suffering from CVS. Abstract: The use of computers and mobile devices is increasing. Computers and mobile devices help our daily work or study. However, prolonged use of them may cause computer vision syndrome (CVS). Nowadays, CVS becomes a health problem for everyone working with computers or mobile devices including college students because it causes dry eyes. It may disrupt reading, doing professional work, or using a computer which is important for college students to complete academic tasks. Sodium hyaluronate can be used to overcome the dry eye problem due to CVS. To assess the effectiveness of sodium hyaluronate eyedrops on students suffering from CVS, pre- and post-administration of sodium hyaluronate was measured for two weeks. Inclusion criteria for this study were college students aged 20-35 years, using a computer for >2 hours a day, not using nonsteroidal anti-inflammatory drugs (NSAIDs), not consuming drugs, or having a disease that affects tear production and bearing no pregnancy. Parameters assessed include tear break-up time using the Tear Break-Up Time (TBUT) test, tear production using the Schirmer I test, the number of clinically subjective symptoms, and Ocular Surface Disease Index (OSDI) scores. Data were analyzed using student paired t-tests or Wilcoxon Rank Test. There were statistically significant differences before and after the TBUT (4.4 vs 6.7 seconds; p<0.0001); the Schirmer I Test (4 vs 6 mm; p<0.05), and the number of clinically subjective symptoms (3 vs 0 clinically subjective symptoms; p<0.0001). The OSDI scores did not show statistical differences before and after administration of sodium hyaluronate (27 vs 21; p>0.05), but there was a positive impact from moderate to mild dry eye. Sodium hyaluronate eyedrops can be used as one of the CVS therapy strategies for students suffering from CVS.

Highlights: • Medium- and high-risk STRONGkids scores are related with low total lymphocyte count and serum albumin, which are related to hospital malnutrition, albeit indirectly. • The use of STRONGkids with total lymphocyte count and serum albumin can detect the risk of hospital malnutrition in children. Abstract: Hospital malnutrition occurs in hospitalized patients who do not consume enough food while their nutritional requirements increase. It occurs particularly in children who have undergone gastrointestinal surgery. Despite the lack of a universal instrument for detecting hospital malnutrition, various parameters can be considered to assist in its identification. STRONGkids has demonstrated its efficiency in detecting malnutrition risk in children. Total lymphocyte count (TLC) and serum albumin are biochemical markers that are related to infection and protein leakage, which can worsen hospital malnutrition. The research objective was to analyze the correlation between STRONGkids and biochemical markers (TLC, serum albumin) to identify hospital malnutrition in children who underwent gastrointestinal surgery. This was a retrospective cross-sectional observational study utilizing medical records. The statistical analysis was conducted using SPSS 21. This study included 37 subjects, with a 24.32% hospital malnutrition incidence rate. The subjects were divided into two groups: hospital malnutrition (n=9) and non-hospital malnutrition (n=28). The STRONGkids of both groups at admission demonstrated a significant difference, while the albumin and TLC did not. The significantly different STRONGkids scores of both groups at admission correlated negatively with the length of hospital stay (LOS), body weight reduction, TLC, and albumin. Those parameters also did not correlate with hospital malnutrition. However, hospital malnutrition increased the risk of low albumin and TLC at discharge by 2.951 and 5.549 times, respectively. In conlusion, TLC and serum albumin cannot be used as independent markers for hospital malnutrition, but STRONGkids can be used in conjunction with TLC and serum albumin to identify hospital malnutrition risk.

Highlight:• The PCC counseling effect using Poedji Rochjati Score Card (PRSC) on premarital knowledge about high-risk pregnancies was studied. • PRSC for preconception counseling is effective for women to improve knowledge about high-isk pregnancy. Abstract: Preconception care (PCC) is a health approach that includes promotive and preventive activities to detect risk factors and interventions carried out to expectant mothers by considering biological, behavioral, and social aspects that affect their health. This study studied the effect of PCC counseling using Poedji Rochjati Score Card (PRSC) on premarital knowledge about high-risk pregnancies. This study was quasi-experimental with 52 respondents divided into 4 groups, namely intervention and control groups, 2 groups with partners, and 2 groups without partners that were chosen through total sampling by applying inclusion and exclusion criteria. Knowledge was measured using pretest and posttest questionnaires. The intervention given was in the form of counseling using PRSC for ± 20 minutes. There were differences in knowledge between the intervention and control group among respondents without partners. Preconception counseling using PRSC was effective for women attending premarital health checks. Applying PRSC as media for counseling could improve knowledge in high-risk pregnancy that could be avoided.

Highlight: • Sepsis and septic shock cause morbidity and mortality in pediatric patients. • The accuracy of pediatric sequential organ failure assessment and anion gap (pSOFA+AG) was compared with AG and pediatric logistic organ dysfunction-2 (AG+PELOD-2). • The mortality assessment of pediatric septic patients showed that pSOFA was more sensitive than PELOD-2, while pSOFA+AG was not more sensitive than PELOD-2. Abstract: Sepsis and septic shock are some of the causes of morbidity and mortality (50-60%) in pediatric patients treated in intensive care rooms. This study aimed to compare the accuracy of pediatric Sequential Organ Failure Assessment (pSOFA) score combined with anion gap (AG) score to Pediatric Logistic Organ Dysfunction-2 (PELOD-2) score in the assessment of mortality in pediatric septic patients at the Resuscitation Room of Dr. Soetomo Geeneral Academic Hospital, Surabaya, Indonesia. This was a retrospective observational cohort study using pediatric sepsis diagnosis guidelines based on the 2016 Pediatric Sepsis Consensus and medical records between January-December 2018. All data of patients aged 1 month to 16 years with suspected infection at the Resuscitation Room were collected based on predisposing infections, signs of infection, and warning signs. Organ dysfunction was assessed by calculating the pSOFA+AG scores, PELOD-2 scores, and corrected anion gap (cAG) in the first 24 hours. Sepsis mortality was assessed by comparing the results of the pSOFA, pSOFA+AG, and PELOD-2. The results showed 94.9% sensitivity and 70.0% specificity (p<0.0001) in the pSOFA, 89.9% sensitivity and 71.3% specificity (p<0.0001) in the PELOD-2, 79.7% sensitivity and 65% specificity (p<0.0001) in the AG, 79.7% sensitivity and 73.8% specificity (p<0.0001) in the cAG, and 79.3% sensitivity (p<0.0001) in the pSOFA+AG. In conclusion, pSOFA was more sensitive than PELOD-2, while the use of pSOFA+AG was not more sensitive than PELOD-2 in assessing the mortality of pediatric septic patients.

Abstract

Sepsis is a major problem causing high morbidity and mortality in patients who hospitalized in intensive care. In sepsis, there is a change in the distribution of albumin in the intravascular and extravascular compartment, so hipoalbumin is occured. Using albumin in septic patients from several studies that have been done are still controversial. Analyzing albumin infusion with changes in levels of albumin, cytokines and SOFA score in septic patients in intensive care dr. Soetomo. The design of this study is a prospective longitudinal observational. The study was conducted in the intensive care unit or ICU dr. Soetomo in February 2015. The inclusion and exclusion criteria were determined that obtained 15 research subjects. They were examined with serum albumin and proinflammatory cytokines as well as evaluating the SOFA score. Then, it measure with ANOVA test methods. Infusion of albumin positively correlated with increased levels of serum albumin and a negative correlation with the levels of IL6, TNF-a and SOFA score. It also found a correlation between changes in levels of albumin with changing levels of IL6 (p <0:01) and changes in SOFA score (p <0.05).
