Abstract

The selection of referral hospitals is not solely determined by the administrative mechanism of the National Health Insurance (JKN) referral system but is also influenced by hospital marketing strategies and patient characteristics. This study aimed to examine the effects of the hospital marketing mix (7Ps) and personal patient factors on referral hospital selection at Segiri Primary Health Center, Samarinda. A quantitative cross-sectional design was employed involving 348 respondents selected through simple random sampling from 1,758 referred patients. Data were collected using a structured questionnaire that had been tested and confirmed to be valid and reliable. Statistical analyses included Chi-square and Fisher’s exact tests for categorical variables, as well as Kruskal-Wallis and Mann-Whitney tests for non-parametric comparisons. The results indicated that most personal characteristics, including age, gender, education, occupation, income, and JKN ownership, were not significantly associated with referral hospital selection (p>0.05), except for domicile (p<0.05). Clinical factors, represented by the destination polyclinic, showed a strong association with hospital choice (p<0.001). Among the marketing mix components, Product (p=0.026), Process (p=0.018), and Physical Evidence (p<0.0001) significantly influenced referral hospital selection, while Price, Place, Promotion, and People showed no significant effects (p>0.05). In conclusion, referral hospital selection was primarily influenced by service needs and the quality of patient experience rather than personal characteristics. Prioritizing improvements in physical facilities and optimizing service processes are essential to enhance the effectiveness of the referral system.

Abstract

Patients undergoing hemodialysis therapy often experience problems with excessive Interdialytic Weight Gain (IDWG) increase. The Information–Motivation–Behavioral Skills (IMB) model has been widely used in health behavior modification, but its application to fluid management of hemodialysis patients is still limited. Therefore, IMB-based interventions are needed to improve fluid adherence. This study aims to determine the influence of the IMB model on fluid adherence (IDWG) of hemodialysis patients. This study uses a quasi-experiment design with a two-group pretest-posttest approach, namely the intervention group and the control group. A total of 70 hemodialysis patients were divided into two groups, each of which was given different treatment. The intervention group received IMB-based education for 4 weeks, including information improvement, personal and social motivation, and behavioral skills. Data were collected from haemodialysis patients who met the inclusion criteria through observation sheets. Interdialytic weight gain (IDWG) was assessed by calculating the difference between post-dialysis and pre-dialysis body weight. Statistical analysis using Wilcoxon and Mann–Whitney tests. There was a significant effect on IDWG in the intervention group with p = 0.000, while the control group showed no significant change (p = 0.165). There was a significant difference between the two groups in IDWG after intervention with p = 0.003. The application of the Information Motivation Behavioral Skills (IMB) Model education has proven to be effective in improving fluid adherence of patients undergoing hemodialysis therapy. The application of the Information Motivation Behavioral Skills (IMB) model education is recommended as part of standard practice in fluid management of hemodialysis patients.

Abstract

Family Intensive Care Unit Syndrome (FICUS) is a cluster of psychological, emotional, and physical symptoms experienced by family members of critically ill patients in response to prolonged stress, medical uncertainty, and the burden of decision-making during ICU care. Although research exploring this condition has grown in recent years, available evidence remains fragmented, highlighting the need for a comprehensive synthesis of symptom manifestations, risk factors, and emerging patterns across studies and contexts. This scoping review aimed to map the symptoms of FICUS systematically reported in the literature over the past 10 years and to identify key characteristics and thematic patterns emerging across various research designs and contexts. This study followed the methodological framework of Arksey and O’Malley, refined by Levac et al., and was reported according to PRISMA-ScR guidelines. A comprehensive search was conducted across five databases (PubMed, Scopus, ScienceDirect, CINAHL, and ProQuest) using a combination of keywords and MeSH terms related to ICU, critically ill patients’ families, psychological distress, and FICUS symptoms. The search was limited to full-text articles published in the last ten years (2015-2025) in English or Indonesian. Of 2,977 identified records, 1,240 duplicates were removed. A total of 1,737 titles and abstracts were screened, 184 full texts were reviewed, and 15 studies met the inclusion criteria. Data were extracted and analyzed using thematic narrative synthesis. FICUS symptoms were categorized into three domains: psychological (anxiety, depression, PTSD), emotional (sadness, anger, worry, psychological threat), and somatic/functional (sleep disturbance, fatigue, impaired concentration). Anxiety and depression were the most consistently reported symptoms, while emotional constructs—such as uncertainty, role identity threat, and spiritual distress—were predominantly identified in qualitative studies. The characteristic Major risk factors included younger caregiver age, female gender, patient mortality, longer ICU stay, and poor communication quality. FICUS is a multidimensional phenomenon consistently observed across different countries and ICU care contexts. Findings highlight the urgency for routine psychological screening, improved communication strategies, and structured emotional support interventions for families of critically ill patients.

Abstract

Competition among hospitals required improvements in service quality and the strengthening of hospital image to retain patients. Good service quality and a positive hospital image influenced patients’ trust and their decision to return for treatment. Patient loyalty became an important indicator of service success, making it necessary to analyze the factors that affected.  it. This study aims to analyze the association of service quality, hospital image with the loyalty of inpatients at I.A Moeis Samarinda Public Hospital. The study population was all inpatients in the treatment room with an average number of visits per month of 835 patients. With stratified sampling technique, a sample of 156 respondents was obtained. Questionnaire instruments were used to collect data on respondent. he results of the validity test showed that all Corrected Item Total Correlation values exceeded 0.1562, while the reliability test yielded a Cronbach’s alpha coefficient of 0.980, indicating that the questionnaire was valid and reliable. Data analysis using regression logistic biner. The results of the three independent variables test simultaneously show that the technical quality variable is positively and significantly associated with loyalty (p value = 0.016), and hospital image is positively and significantly associated with loyalty (p value = 0.000). The most dominant variable associated is hospital image with OR = 9.589 (95% CI OR = 3.064-30.006), which means that a good hospital image has a 9.5 times chance of creating patient loyalty. The conclusion of this study is the technical quality and the hospital image are positively and significantly associated with loyalty and the hospital image is the most dominant variable associated with the loyalty of inpatients.

Abstract

Elderly individuals with dementia experience a decline in cognitive function, which affects their independence and daily quality of life. Uno Block play therapy is a form of cognitive and social stimulation that involves arranging and moving numbered and colored blocks according to specific rules. This activity stimulates memory, attention, fine motor coordination, and social interaction. This study aims to analyze the effect of Uno Block play therapy on the cognitive function of elderly patients with dementia at PSTW Budi Mulia 3 Jakarta. Method: The study used a quasi-experimental design with a pretest–posttest control group involving 32 elderly participants. They were selected using purposive sampling based on the following inclusion criteria. Participants were divided into an intervention group (n = 16) and a control group (n = 16). Cognitive function was measured using the Mini-Mental State Examination (MMSE), and data were analyzed using the Wilcoxon test for non-normally distributed paired data. The results showed a significant difference in cognitive function scores before and after the intervention in the intervention group (p = 0.005), while no significant difference was observed in the control group (p = 0.157). It can be concluded that Uno Block play therapy has a significant effect on improving the cognitive function of elderly individuals with dementia.

Abstract

The postpartum period is a crucial phase for the success of exclusive breastfeeding, marked by various physiological and psychological changes in the mother. One common issue during this period is the difficulty in expressing breast milk, which can affect the baby's growth and overall health. Contributing factors include stress, fatigue, poor nutritional intake, and a lack of knowledge about proper breastfeeding techniques. Breast care is a non-pharmacological intervention that has been scientifically proven to stimulate the hormones prolactin and oxytocin, thereby enhancing milk production and expression. This study aims to evaluate the effectiveness of the breast care technique in improving breast milk production and expression in postpartum mothers. It also seeks to describe the characteristics of the respondents and compare their condition before and after the intervention. This research used a descriptive qualitative approach with a case study design involving five postpartum mothers. Respondents were selected based on specific criteria using purposive sampling. Data were collected through direct observation during a four-day breast care intervention. Before the intervention, all respondents experienced difficulties in milk expression, with scores categorized as “very low” to “low.” After four days of intervention, a significant improvement was observed. All respondents achieved the maximum score (10/10) based on maternal indicators and a good score (5/6) based on infant indicators. Breast care proved effective in enhancing milk flow through hormonal stimulation and its relaxing effect.

Abstract

Cardiac arrest is a leading cause of death worldwide, including in the United States and Indonesia, with most cases triggered by coronary heart disease. Successful resuscitation largely depends on rapid interventions such as cardiopulmonary resuscitation (CPR) and defibrillation, as survival chances decrease by approximately 10% for every minute without intervention. Basic Life Support (BLS) has been proven to improve resuscitation outcomes; however, knowledge and skills related to BLS remain limited, including among medical students. This study aimed to analyze the effect of providing BLS training using audiovisual methods on the knowledge and skills of students who are members of the Medical Assistance Team (Tim Bantuan Medis/TBM), Faculty of Medicine, Universitas Hang Tuah Surabaya, in performing cardiopulmonary resuscitation (CPR). A cross-sectional study was conducted at the Faculty of Medicine, Universitas Hang Tuah, Surabaya, Indonesia, from June to September 2025. A total of 21 TBM students were selected using a total sampling technique. The dependent variables were knowledge and skills in performing CPR, while the independent variable was BLS training delivered using audiovisual methods. Data were collected using questionnaires that had been tested for validity and reliability. Data analysis was performed using the Wilcoxon signed-rank test for knowledge levels and the McNemar test for CPR skills. The Wilcoxon signed-rank test showed that knowledge levels were significantly improved after the provision of BLS training using audiovisual methods (p = 0.020). The McNemar test demonstrated that CPR skills were also significantly improved after the audiovisual BLS training (p = 0.008). There is a significant effect of audiovisual-based Basic Life Support (BLS) training on improving both CPR knowledge and skills among TBM students.

Abstract

Chronic Energy Deficiency in pregnant women is a serious health problem because it increases the risk of complications during pregnancy, childbirth, and fetal growth. Various studies have shown that the prevalence of chronic energy deficiency remains high and is influenced by multidimensional factors such as nutritional status, socioeconomic status, knowledge, and access to health services. The purpose of this study was to conduct a literature review to identify factors of chronic energy deficiency to strengthen evidence-based prevention strategies, specifically dietary patterns, maternal knowledge, and husband or family support. The study used a literature review by searching articles in PubMed, ScienceDirect, and Google Scholar. The inclusion criteria were articles published between 2021 and 2025, cross-sectional design, full papers, and discussing variables related to chronic energy deficiency in pregnant women. The MeSH (Medical Subject Heading)-based keywords were "Chronic Energy Deficiency" AND "Pregnant Women" AND "Food Consumption Patterns" OR "Husband/Family Support" OR "Mother's Knowledge". A total of 21 articles met the criteria and were analyzed. Most studies found a significant association between maternal knowledge and the incidence of chronic energy deficiency. Consumption patterns were also associated with the incidence of chronic energy deficiency, especially in mothers with low energy and protein intake. Husband or family support has been shown to be a reinforcing factor that helps mothers meet nutritional needs through emotional, informational, and financial support. Prevention of chronic energy deficiency requires a multidimensional approach that focuses on increasing nutritional knowledge, improving eating habits, and strengthening family support.

Abstract

In Indonesia, the prevalence of dysmenorrhea reaches 60 to 70%, with three-quarters of the number of adolescents experiencing mild to severe pain and as many as 7-15% do not go to school. The incidence of dysmenorrhea in Indonesia is around 54.89% primary dysmenorrhea, the remaining 45.11% secondary dysmenorrhea. Adolescents who experience dysmenorrhea tend to reduce their activities, especially activities at school and often decide not to go to school. Pharmacological therapy that is too frequent can cause several side effects such as stomach irritation, intestinal colic, diarrhea and asthma attacks. Non-Pharmacological Therapy is an alternative for dysmenorrhea, one of which is the red ginger hug pillow (BAPER). Red ginger is an option because it contains a lot of oleoresin, a bioactive component consisting of gingerol and shogaol which works as an anti-inflammatory and provides a warming effect so that prostaglandins can be blocked and reduce the intensity of pain during menstruation. This type of research uses a quasi-experimental research design with a pretest posttest control group design. In early September 2025, researchers began collecting data on dysmenorrhea history from students at SMA Negeri 2 Kisaran. They were assisted by homeroom teachers and OBGYN doctors to better understand the students' condition. They also conducted interviews with both students and homeroom teachers. During this data collection phase, 60 students with a history of dysmenorrhea were identified. The sampling technique used was purposive sampling with a sample of 60 middle-aged adolescents (aged 15-17 years) at SMAN 2 Kisaran. The statistical test results obtained a P value = 0.000 < 0.05, so it can be concluded that non-pharmacological therapy of red ginger hug pillows can reduce dysmenorrhea. Red ginger hug pillows (BAPER) can be a solution for adolescents who experience dysmenorrhea.
