Abstract

Diabetic retinopathy is the common complication of T2DM, which leads to vision loss. The severity of retinopathy can be affected by HbA1c levels. Controlling the glycemic such as HbA1c, may prevent the severity of diabetic retinopathy. Objective: To identified the association between HbA1c levels and severity of retinopathy in Type 2 DM patients at Cicendo Eye Hospital, Bandung. Methods: This research was an analytic observational study and cross-sectional approach. Samples were taken using purposive sampling and consisted of 70 people. The secondary data were from the outpatient's medical records at the Cicendo Eye Hospital in Bandung for the period September-October 2022. The Chi-square test was used for data analysis. Results: According to HbA1c levels, the uncontrolled HbA1C levels were found in 52 patients (74,3%) and the controlled were found in 18 patients (25,7%). Most of them were diagnosed with PDR were found 62% and the rest of it were diagnosed with NPDR. A significant association is found between HbA1c levels and the degree of diabetic retinopathy with p-value = 0.018 (p <0.05). Conclusion: HbA1c levels in Type 2 Diabetes Mellitus patients have a significant association with the severity of diabetic retinopathy, which can be used as a parameter for controlling diabetes complications, particularly retinopathy.Keywords: diabetic retinopathy, T2DM, HbA1c, glycemic control

Abstract

Chronic rhinosinusitis (CRS) is defined as chronic inflammation of the nose and paranasal sinuses with two or more symptoms, one of which can be nasal congestion/obstruction/congestion or rhinorrhea (anterior/postnasal drip), and/or with facial pain, reduced or loss of smell. Elongated uncinate is one of the anatomical variants of the uncinate process where the uncinate process is curved or bent, which causes sinus problems and is one of the risk factors for CRS. It has been reported a 19-year-old man with the diagnosis of chronic rhinosinusitis with third-grade bilateral nasal polyp from computed tomography of the paranasal sinusitis. There was an impression of the sinonasal polyp with inverted papilloma as a differential diagnosis. The patient was treated through microdebrider-assisted functional endoscopic sinus surgery. This was chronic rhinosinusitis with grade III bilateral nasal polyps, and microdebrider-assisted fungsional endoscopic sinus surgery was performed with great results.Keywords: chronic rhinosinusitis, elongated unsinatus,  functional endoscopic sinus surgery, nasal polyp

Abstract

It is crucial for patients with hypertension to always control their blood pressure by consuming medicines for hypertension, the morbidity and mortality rates will be hopefully reduced. Medication adherence is necessary in order to reach the target of controlling blood pressure. There are a lot of causes affecting medication adherence, one of which is the consumption of herbal remedies. Objective: To investigated the correlation between the consumption of herbal remedies and medication adherence among patients with hypertension at Lempake Public Health Centre in Samarinda. Methods: The research design was analytical observational with a cross-sectional approach. The statistical analysis used Chi-Square test. The data were collected through interviews with 94 respondents. They were selected using convenience sampling and also asked to fulfill the identity form and MMAS-8 questionnaire. Results: Most respondents at Lempake Public Health Centre in Samarinda consumed herbal remedies (42.6%), but their medication adherence was low (82.9%). The significant p-value was 0.008 based on the statistical analysis. Conclusion: There is a significant correlation between the consumption of herbal remedies and medication adherence among patients with hypertension at Lempake Public Health Centre in Samarinda.Keywords:  herbal, hypertension, medication adherence, MMAS-8

Abstract

Nasopharyngeal carcinoma is a malignancy that originates from the epithelial tissues lining the nasopharynx and is considered one of the most common malignant cancers which cause mortality in Indonesia. Neutrophils and lymphocytes are the first lines to provide body immunity against foreign substances. Neutrophil-to-Lymphocyte Ratio (NLR) is a prognostic factor in various malignancies. Objective: To investigated the correlation between pre-therapy Neutrophil-to-Lymphocyte Ratio (NLR) and survival rate among patients with nasopharyngeal carcinoma at Abdoel Wahab Sjahranie Regional Public Hospital in 2018. Methods: The Neutrophil-to-Lymphocyte Ratio (NLR) data were obtained from outpatient medical records at Abdoel Wahab Sjahranie Regional Public Hospital in 2018. In addition, the data on mortality status were collected based on the information after directly contacting the patients. A purposive sampling technique was used to collect the data. Thirty-three data of patient medical records were obtained and analyzed using Kaplan-Meier survival analysis with a significant p-value of <0.05. Results: The analysis result showed p-value of 0.495. Conclusion: There is no correlation between pre-therapy Neutrophil-to-Lymphocyte Ratio (NLR) and survival rate among patients with nasopharyngeal carcinoma.Keywords:  nasopharyngeal carcinoma, neutrophil-to-lymphocyte ratio, survival rate

Abstract

Rheumatic Heart Disease (RHD) has high morbidity and mortality. Cardiac valve disorders after rheumatic fever are the leading cause of heart failure in children and young adults. A comprehensive understanding of RHD can help early diagnosis and provide optimal management in the future. It has been reported that a 55-year-old man came to the heart clinic of Bhayangkara Hospital Denpasar for routine control of heart disease. The patient has been diagnosed with rheumatic heart disease since 2018. The patient complains of shortness of breath when walking long distances and doing strenuous activities. The patient was diagnosed with CHF FC II ec RHD + MS moderate-severe (Wilkins score 8) + AF NVR (CHA2DS2VASc 3) and treated with heart failure and antiplatelet medication. Mitral stenosis due to RHD produces the typical pathological features seen in Transthoracic echocardiography (TTE), including fibrous thickening and calcification of the valve leaflets, fusion of the commissures, and thickening and shortening of chordae tendineae. Chronic stress overload on the LA due to MS can cause LA dilatation and trigger AF. RHD patients with AF are recommended for anticoagulation for stroke prevention. Patients with acute rheumatic fever clinical features should be evaluated further and given long-term therapy to prevent more severe heart problems.Keywords:  atrial fibrillation, mitral stenosis, rheumatic  heart  disease

Abstract

Meningoencephalitis is a pathological state characterized by concurrent inflammation of the meninges and the brain parenchyma. Liquor Cerebrospinal analysis (LCS) can help establish a diagnosis of diseases affecting the central nervous system, including meningoencephalitis. Objective: To identified the attributes of meningoencephalitis LCS analysis in pediatric patients at RSUD Abdoel Wahab Sjahranie Samarinda from 2019 until 2022. Methods: This study employs a descriptive research design to delineate the features of the LCS analysis. The data utilized in this study was acquired from the medical records of individuals who had experienced meningoencephalitis during their childhood and obtained as many as 39 patient data samples. Results: The findings revealed that most cases were observed in children aged 0-1 years (38.5%), with a higher incidence in males (59.0%). The LCS analysis parameters indicated that lymphocytic pleocytosis was the most common (76.9%), while clear  LCS was observed in 82.1%. The leukocyte count was within the normal range of 56.4%, and the polymorphonuclear ratio was negative (79.5%). The protein levels increased by 56.4%, while the glucose levels were within the normal range (59.0%). Conclusion: There is an increase in LCS  protein levels, which indicates inflammation.Keywords: LCS  analysis, LCS parameters, meningoencephalitis

Abstract

Cardiorespiratory endurance is the ability of the body's circulatory and respiratory system to meet the oxygen needs of the muscles as a source of energy production during sustained physical activity. One of the factors that influenced the level of cardiorespiratory endurance of a person was physical activity. Objective: To determined the relationship between physical activity and cardiorespiratory endurance. Methods: It was an analytic observational with a cross-sectional study. Data were taken from primary data that were measured directly on 97 students of the Medical Study Program at Mulawarman University who were selected using a purposive sampling technique. The data collected included physical activity using the Global Physical Activity Questionnaire (GPAQ) and cardiorespiratory endurance using Queen's College Step Test. The statistical analysis used was Spearman's correlation test with a significance value of p <0.05. Results: The statistical test showed that the relationship between physical activity and cardiorespiratory endurance was p = 0.003 and r = 0.295. Conclusion: There was a significant positive relationship between physical activity and cardiorespiratory endurance, and the relationship's strength was weak.Keywords:  cardiorespiratory endurance, global physical activity questionnaire, physical activity, queen’s college step test

Abstract

Physiological and psychological problems can arise from sleep disorders. The majority of older people experience a decrease in sleep quality with age, so it is necessary to do morning physical activity such as walking to improve sleep quality. Objective: To determine whether walking in the morning exercise can improve older people's sleep quality. Methods: It was a quantitative research with a quasi-experimental type using a one-group pre and post-test design. The sample consisted of 28 respondents selected by purposive sampling. Measurement of sleep quality using the Pittsburgh Sleep Quality Index (PSQI) questionnaire to evaluate a person's sleep quality. The walking morning exercise intervention was carried out thrice a week for up to 4 weeks. Results: Wilcoxon test for bivariate analysis showed a significance level of 0.008 (p < 0.05). Conclusion: Walking in the morning exercise improves sleep quality in older people.Keywords:  older people, sleep quality, walking morning exercise

Abstract

The increased production of beta-amyloid marks Alzheimer's disease. PPAR-γ shows involvement in Alzheimer's disease. PPAR-γ Gene is suspected to reduce beta-amyloid plaques and reduce inflammation and oxidative stress in Alzheimer's disease. Mesenchymal Stem Cells Wharton's Jelly (MSC-WJ) is expected to increase the PPAR-γ Gene expression. Objective:  To observed the PPAR-γ Gene expression in Alzheimer's Mice. Methods: The mice are induced using AlCl3 and were given MSC-WJ. This study was an experimental study with a post-test-only control group design on 18 samples of RNA from Alzheimer's mice that were divided into 3 groups. Negative control group (K-), positive control group (K+), and experimented group (P). The mean of PPAR-γ gene expression was obtained by comparing the PPAR-γ gene with the GAPDH gene. This study used semiquantitative methods using ImageJ. Data analysis was used with the Kruskal-Wallis test. It is significant if the value is <0,05. Results: The mean ratio of PPAR-γ gene expression that was obtained in K-, K+, and P were 0.12, 0.06, and 0.08, respectively. There were significant differences between each group, with a p-value of 0,023 (p<0,05). Conclusion: Giving MSC-WJ could increase the PPAR-γ gene expression. Further study using real-time PCR is highly needed to increase the MSC-WJ treatment in patients with Alzheimer's disease.Keywords:  Alzheimer disease, MSC-WJ, PPAR- γ

Abstract

Purple sweet potato (Ipomoea batatas L.) has a secondary metabolite that contains a lot of antioxidants. Some of the secondary metabolites in purple sweet potato have a hepatotoxic effect. Objective: To described the toxic effects of purple sweet potato water extract on steatosis in rats. Methods: This descriptive study was conducted on 29‒30 June 2019 in the Biomedical Laboratory of the Faculty of Medicine, Islamic Bandung University, with an in-vivo experimental design with random allocation techniques and using the proposed new method on 11 rats. One of the control rats was not given extract, and ten other rats were given a dose of purple sweet potato water extract 50, 200, 400, 800, 1000, 1500, 2000, 3000, 4000, and 5000 mg/KgBW/PO. The liver organ was harvested after 24 hours, and then slides were made at the Pathology Anatomy Laboratory of Padjajaran University. Results: The histopathological view did not find any steatosis either in macrovesicular or microvesicular, but other features were found in ballooning degeneration, sinusoidal dilation, and inflammation. Conclusion: Steatosis was not found because purple sweet potato contains anthocyanins and tannins.Keywords: purple sweet potato, steatosis, tannin
