Abstract

Right ventricular dysfunction and failure receive much less attention than the left ventricular failure. Right ventricular dysfunction or failure is associated with increased mortality rates in cardiac surgery, surgical cases other than cardiac surgery and also in patients treated in the ICU. The purpose of this article review was to describe the anatomy, physiology and pathophysiology of right ventricular failure, its detection and diagnosis, and management considerations from anesthetic point of view, including preoperative, intraoperative and postoperative stages. Cardiac surgery may result in right ventricular failure. For example, 0.1% post cardiotomy patients experience severe right heart failure and require long-term inotropic support, and so do 2-3% of post-transplant patients, and 20-30% of patients installed with instrument in his left heart. Therefore, anesthesiologists play a major role in perioperative and postoperative intensive care and are obliged to comprehend the nature of right ventricular dysfunction and failure so as to carry out early detection, prevent and manage patients with right ventricular dysfunction.

Abstract

Tumor growth and metastasis in NPC patients suspected role of several molecular biomarkers that have been identified in tumor specimens of patients with NPC. Recent research states intracellular Hsp70 is a stress protein that is a main factor for the growth, invasion and metastasis of NPC. The purpose of this study is to analyze intracellular Hsp70 protein expression of NPC patients which correlate with staging of NPC as clinical manifestation. This study used cross sectional design. In this study, formalin-fixed paraffin-embedded biopsy specimens were obtained from 18 NPCs with all of histopathology and clinically divided into N0, N1, N2, N3. The expression of intracellular Hsp70 was obtained with immunohistochemistry using monoclonal antibody Anti Human Hsp70 antibodi from Santa Cruz Biotechnology, California, USA. Assessment of the staining was performed with Remmele methode by Histopathologies doctor Consultant. The Spearman's test was used to determine the relationship between expression of intracellular Hsp70 protein and enlargement cervical nodul of NPC. Statistical significance was defined as p <0,05. There were 18 patients NPC patients that met the inclusion and exclusion criteria. Showed negative intracellular Hsp70 expression by 16,67% for all sample in which consist 1 patient of N0, 1 patient of N1, 1 patient of N2, 0 patient of N3. Followed by 27,78% mild positive intracellular Hsp70 expression which consist 0 patient of N0, 2 patients of N1, 0 patient of N2, 3 patients of N3. Followed by 44,44% moderate positive intracellular Hsp70 expression which consist 0 patient of N0, 0 patient of N1, 1 patient of N2, 9 patients of N3. Followed by 11,11% strongly positive intracellular Hsp70 expression which consist 0 patient of N0, 0 patient of N1, 0 patient of N2, 2 patients of N3. Spearman test results test scores p = 0.01 with a correlation coefficient 0.606. Correlation of intracellular Hsp70 protein expression with enlargement cervical nodul of lymphe (N0, N1, N2, N3) in patients with NPC showed significant (p<0.05). In conclusion, there was correlation between increase of the intacellular Hsp70 expression and enlargement cervical nodul of lymphe in nasopharyngeal carcinoma.

Abstract

Unsafe act become the most cause of accidents. One of human factors causing accidents is work stress. The aim of this study was to analyze correlation between coping mechanism and safety behavior at workers of fabrication division in a steel construction company. The study was carried out with cross-sectional analytical observational methods. The study population was 200 workers of fabrication division in a steel construction company. By using simple random sampling method, the number of samples was 134 respondents. Data collection was performed by observation, interviews, and questionnaires. The independent variables were age, education level, K3 experience, coping mechanism, emotional focused coping, and problem focused coping, while the dependent variable was safety behavior. Intervariables correlation were tested by Chi-square test (α=0.05). Correlation between age and safety behavior was categorized on relation (p=0.016; r=0.301). Correlation between education level and safety behavior was not categorized on relation (p=0.260; r=0.315). Correlation between tenure and safety behavior was categorized on relation (p=0.001; r=0.422). Correlation between coping mechanism and safety behavior was not categorized on relation (p=1.0; r=0.015). The conclusion was age and tenure have unidirectional and strong relation with safety behavior, while the level of education and coping mechanism have no relation with safety behavior. Company should have given understanding to workers about safety behavior through K3 training and safety talk and held safety inspection and or safety patrol to supervise workers.

Abstract

Male 61 years old who presented with stable angina since 1 month ago, with Risk factor of CAD: hypertension, Dyslipidemia and heavy smoker. The ECG showed inferior old myocardial infarction. Diagnostic coronary angiography found: bifurcatio lesion at distal LMCA with significant stenosis 60% at the distal LM and 85% at the osteal LCx (Medina score 1-1-0), high D1 and diffuse disease with maximal stenosis 85% at the distal LAD after D2, Significant stenosis 85% at the osteal LCx and CTO at the distal after OM1, diffuse disease with maximal stenosis 85% at the mid RCA. A 6-Fr JR 4.0 guiding catheter (Launcher, Medtronic) was engaged into the right coronary artery ostium via the femoral artery. GW pilot 50 (Hi-Torque Pilot 50) inserted to distal RCA. Perform Balloon support by Saphire II inserted to mid RCA and dilated, after that perform balloon to proximal RCA and dilated. Stent DES Firebird II (Rapamycin) to mid RCA and dilated. Stent BMS Apollo 3.0x36 mm inserted to proximal-mid RCA, overlapping with previous stent, but was loss or dislodged and insert to the guiding catheter. BMS stent was pulled out with small balloon ex stent. GC 6F 4.0 inserted to ascending Aorta and engaged at ostium RCA. BMS stent Arthos PICO 3.0x 34 mm inserted to proximal-mid RCA, overlapping with previous stent and dilated. Final angiography confirmed successful pull out of loss stent and dilation of the RCA.

Abstract

This study was to approve the increased secretion of Hsp 70, DNA damage, and inhibitor apoptosis protein in cisplatin therapy which influence apoptosis of oral cancer cell and to know mechanism of molecular pathology. This study was an in vitro experimental laboratory using Randomized Block Design. Cell culture of oral cancer divided from cisplatin resistance cancer cell and cancer cell never induce cisplatin. Two group of cancer cell would be given cisplatin therapy. Secretion of Hsp 70, DNA damage, Inhibitor of apoptosis protein, and apoptosis index would be examined. Cisplatin resistance cancer cell group showed lower apoptosis than never induce cisplatin cancer cell. Elevated secretion of Hsp 70 in cisplatin therapy group (p= 0.000, b=0.881). Lower secretion of DNA damage protein in cisplatin resistance cancer cell and it was not going apoptosis. In path regression analysis, cisplatin was significans through IAP pathway (p=0.000, b=0.726) to apoptosis. All type of cell cultures were also significans through IAP pathway (p=0.000, b=0.496) to apoptosis. Elevated IAP secretion influenced apoptosis (b= 1.000). In conclusion, cisplatin used IAP pathway to apoptosis. All type of cell cultures also used IAP pathway to apoptosis. Cisplatin resistance cell culture had stronger effect to IAP and IAP increased inhibition to apoptosis.

Abstract

Fracture is quite a common occurrence in the age group of children, with a fairly wide variety of epidemiology throughout the world, hence it could be considered as a major health problem. A retrospective study of all pediatric fractures presenting to Dr. Soegiri General Hospital Lamongan, Indonesiain 2015 was undertaken. It showed that 79.5% of children's fractures occurred in males and that 86.76% presented as a single fracture, whileelbow dislocation is the most prevalent dislocation in this study.Analysis of pediatric fractures shows that there is a trimodal distribution of single fracture with age, withdistal radius/ulna fracture as the most prevalent single fracture.The commonest cause of single fracture are road traffic accident, and the majority involve the upper limb. Those suggested that there should be more concern about road safety program in Indonesia.

Abstract

Elderly is a problem that may disrupt the balance of various molecules in the body, especially fat molecules, which may result in the emergence of various diseases so that blood fat and adipose level examinations need to be done regularly and periodically as an effort of early detection. This study aimed to find correlation between Low Density Lipoprotein (LDL), High Density Lipoprotein (HDL), and triglyceride with body fat percentage among professors in Universitas Padjadjaran (Unpad) who were mostly in elderly age. This study was a linear regression analytical study conducted on 52 Unpad professors (♂ 75%, ♀ 25%) based on data of LDL, HDL, and triglyceride levels obtained through laboratory examination, as well as body fat percentage data by using Bio Impedance Analysis (BIA). The correlation between low density lipoprotein (LDL), High Density Lipoprotein (HDL), and triglyceride with body fat percentage in male professors group was R2=0.072; P>0.05 and in female was R2=0.028; P>0.05. As a conclusion, there was no correlation between Low Density Lipoprotein (LDL), High Density Lipoprotein (HDL), and triglyceride with body fat percentage in Unpad professors.

Abstract

Intra Uterine Fetal Death (IUFD) is one contributor to infant mortality. Human Leukocyte Antigen-E (HLA-E) and Natural Killer Cells (NK cells) are believed to play an important role towards IUFD associated with immune maladaptation. This study aimed to determine the expression of HLA-E and NK cells on trophoblast on IUFD and normal pregnancy. The study used cross sectional approach undertaken at Department of Obstetrics and Gynecology, Dr. Moewardi Hospital, Surakarta, networking hospitals, and Anatomic Pathology Laboratory of Faculty of Medicine, Sebelas Maret University. The number of samples were 32 subjects consisting of 16 subjects with normal pregnancy and 16 subjects with IUFD. The expression of HLA-E and NK cells on trophoblast of subjects who met the inclusion and exclusion criteria were examined using immunohistochemistry method and t-test statistical analysis. The mean value of HLA-E expression in the trophoblast of IUFD group was 17.30±6.69, in normal pregnancy was 57.06±32.04, with p=0.00 (p<0.05). The mean value of NK cell expression in trophoblast in IUFD group was 78.62±36.43, in normal pregnancy was 19.87±6.43, with p=0.00 (p<0.05). This study concluded that the expression of HLA-E was lower and NK cells wass higher in IUFD compared to those in normal pregnancy.

Abstract

This study aimed to determine the effects of arak bali on the fragmentation of spermatozoa and testosterone in rats (Rattus norvegicus). This study used 24 rats (170-200 g), divided into four groups: one control and three treatments (receiving arak bali containing 40% alcohol as much as 0.1 and 0.5 mL and synthetic alcohol (40%) as much as 0.1 mL, for 45 days). The observation of DNA fragmentation was done using acridine orange staining and the measurement of testosterone level used ELISA method. The results showed that the provision of arak bali in experimental animals increased the occurrence of spermatozoa DNA fragmentation. The higher the volume of arak bali given, the higher the fragmentation of spermatozoa DNA. The administration of arak bali also decreased testosterone level. The higher the volume of arak bali given, the lower the rats' testosterone level.

Abstract

Monitoring childhood activities especially for toddlers is important to review because this period is a golden period for chilhood development. The aim of this research to review determine the prevalence of irregularities childhood development for toddler that aged 1-3 years in District of Bulak, Surabaya. This research is a descriptive cross-sectional study. The study subjects were 98 children aged 1-3 Years that obtained by cluster random sampling. The examination conducted using a screening tool prospective development of the Capute scale (CAT test - SHELL). The result, total of value development (FSDQ) average of 97.88 ± 12.26, visuomotor development value (CAT) average 92.58 ± 16.10 while value language development (CLAMS) average 102.67 ± 13.63. Based on FSDQ score, 84.2% of child had > 85 (normal). The remaining 2.1% including developmental disorders had <75 and 13.7% had score 75-85 (suspected). So, the prevalence of irregularities childhood development for toddlers that aged 1-3 years in District of Bulak around 15.8%.
