Abstract

Juvenile granulosa cell tumour (JGCT) is a subtype of granulosa cell tumour that predominantly affects young females, particularly those who are premenarchal, with a mean age at diagnosis of 13 years. Although rare, this tumour can occur across a wide age range, from 10 months to 67 years. Histologically, JGCT resembles the adult-type granulosa cell tumour but presents with distinct features, including aggressive growth and the absence of FOXL2 mutations. Common clinical manifestations include an abdominal mass, abdominal pain, precocious puberty in prepubertal girls due to estrogen secretion, and menorrhagia or amenorrhea in premenopausal women. Macroscopically, the tumour typically exhibits a dominant solid component, frequently accompanied by hemorrhagic and necrotic areas. High mitotic activity and variable degrees of cytological atypia are characteristic findings. The diagnosis is supported by positive immunoreactivity for inhibin, calretinin, and CD99. In early-stage disease, the primary treatment is unilateral oophorectomy with fertility preservation. In contrast, advanced stages often require total hysterectomy and adjuvant chemotherapy. Prognosis is highly dependent on disease stage; patients with stage I disease have a survival rate of up to 97%, whereas advanced-stage disease is associated with a poorer outcome. This review aims to provide a comprehensive overview of the epidemiology, pathogenesis, clinical manifestations, diagnostic approaches, and management strategies of Juvenile Granulosa Cell Tumour (JGCT). It also underscores the importance of early detection and the development of more effective therapeutic options to enhance clinical outcomes.

Abstract

Vitamin D, traditionally known for its role in bone health, is increasingly recognized as a crucial regulator of immune balance and inflammation. This review explores the mechanisms by which vitamin D influences both innate and adaptive immune responses, with a focus on immune cells like macrophages, dendritic cells, T cells, and B cells. Vitamin D, through its active form, calcitriol, modulates immune cell function by binding to the Vitamin D receptor (VDR), which impacts cytokine production and inflammatory pathways. Notably, vitamin D promotes anti-inflammatory effects by shifting immune responses towards a regulatory phenotype, reducing pro-inflammatory cytokines while enhancing anti-inflammatory signals. This regulatory potential highlight vitamin D’s therapeutic value for inflammatory and autoimmune diseases. Further research is essential to determine optimal vitamin D dosing and its implications across diverse populations.

Abstract

Diabetes mellitus (DM) is a chronic metabolic disorder characterized by persistent hyperglycemia due to impaired insulin secretion, insulin resistance, or both. Poor glycemic control in DM leads to complications such as cardiovascular disease and nephropathy. Vitamin D has been suggested to influence glucose metabolism by improving insulin secretion and sensitivity thorugh mechanisms involving pancreatic β-cells and anti-inflammatory pathways. This study evaluates the effect of vitamin D supplementation on blood glucose levels in diabetic rats. An experimental study was conducted using 25 males Sprague Dawley rats, divided into five groups: Group A (negative control), Group B (non-DM + 415 IU/kgBW vitamin D), Group C (DM without vitamin D), Group D (DM + 415 IU/kgBW vitamin D), and Group E (DM + 1100 IU/kgBW vitamin D). Diabetes was induced using streptozotocin (50 mg/kg BW), and rats with random blood glucose levels >200 mg/dL were considered diabetic. Vitamin D supplementation was given via gavage for 30 days. Serum vitamin D and random blood glucose levels were measured 30 days after supplementation. Statistical analyses included ANOVA and Pearson correlation tests. Vitamin D supplementation significantly increased serum vitamin D levels in diabetic rats, especially in Group E (DM + 1100 IU/kgBW vitamin D; (p < 0.05). Random blood glucose levels decreased significantly in diabetic groups and Group E showed the largest glucose reduction (p < 0.001). A moderate negative correlation (r = -0.47, p = 0.017) between vitamin D levels and glucose changes was observed. Vitamin D supplementation reduces blood glucose levels in diabetic rats, with higher doses showing greater efficacy. These findings highlight vitamin D’s potential as an adjunctive therapy for diabetes management. Further research is needed to explore underlying mechanisms.

Abstract

Introduction: Cardiovascular fitness is one of the components that can be used to evaluate health-related physical fitness. Body mass index (BMI) is a factor that can influence cardiovascular fitness. This is due to the strong correlation between BMI and cardiovascular fitness. Aims: To assess the correlation between BMI and cardiovascular fitness among medical students at Universitas Baiturrahmah. Method: The research was conducted at the Physiology Laboratory, Faculty of Medicine, Universitas Baiturrahmah. We assessed each subjects’s BMI and evaluated their cardiovascular fitness using the Harvard Step Test. The data were processed using Spearman rho correlation analysis. Results: Among the 50 subjects who participated in this study the mean BMI was 23.66±0.67 kg/m2, with approximately 68% (34) of subjects categorized in the normal BMI group. Out of the total number of subjects, around 34% (17) subjects had good cardiovascular fitness, with a mean value around 75.48±2.17. The results showed a significant correlation between BMI and cardiovascular fitness (r = -0.436, p<0.05), indicating that a higher BMI is related to lower levels of cardiovascular fitness. Conclusion: The results of this study indicate a significant negative correlation between BMI and cardiovascular fitness among medical students at Universitas Baiturrahmah.
