Abstract

Handover implementation can provide clear information on patient care during the previous shift so that nurses know about efforts to continue the care provided and improve patient safety. The aim of this activity is to be able to implement the handover process using SBAR communication techniques optimally in the inpatient room. This service activity occurred at Jayapura District Hospital in April - August 2023. The target of this activity was nurses in the Inpatient Room. The method of this activity is through socialization, role play, determining the person in charge, preparing formats and technical instructions, preparing management cases, determining implementation schedules, and documenting results. Based on the results of the activities carried out, 24 nurses experienced an increase in knowledge and there were 4 nurses who did not experience a change in their scores. Optimization of patient handover activities can be realized if leadership supervises and ensures weighing activities comply with operational standards.

Abstract

Based on data from the Sarmi District Health Office in 2023, the prevalence of stunting among children under five reached 22.5%, spread across 11 community health centers. This figure remains far above the national target of 14%, necessitating intensive efforts to reduce it. In 2024, a study conducted by nursing students at Cenderawasih University revealed that in the Betaf health center area, 21.2% (55 out of 259) of children under five were stunted. Additionally, the Betaf health center reported that Kampung Komra itself recorded 15 cases of stunting in 2023. The high prevalence of stunting in this area has motivated the team to implement a community partnership program aimed at empowering Posyandu mothers, who play a crucial role in health and nutrition education for pregnant women and young children. This community service program aims to reduce stunting rates in Kampung Komra through health education, strengthening food independence with the implementation of Sustainable Food House Areas (KRPL), aquaponics-based fish farming, and the use of information technology in stunting prevention. Nutritional status measurements of children under five in the Posyandu of Kampung Komra indicate that 35% are stunted, underscoring the need for continuous interventions to meet the nutritional needs of pregnant women, breastfeeding mothers, and young children.Given the high stunting rate and the community's limited ability to independently meet their food needs, this program should be continued periodically. Additionally, the program should be supported with intensive training for Posyandu mothers and access to affordable food resources, considering the community's preference for hunting over livestock farming, low economic conditions, and high commodity prices.

Abstract

Structured nursing conferences, both pre and post, can contribute to improving patient outcomes. Conferences have been shown to enhance patient safety in care units compared to teams that do not conduct similar conferences. This activity was carried out from April to August 2024. The partners in this activity were nurses in the inpatient ward. The results of this activity included the achievement of several outcomes, such as the appointment of responsible individuals for conferences in each room, the development of shared conference techniques, and the implementation of pre and post-conferences led by the team leader/primary nurse. The socialization results also demonstrated that the conference optimization program in the Inpatient Ward of Bhayangkara Level II Hospital Jayapura successfully improved nurses' understanding and skills. Pre-conference activities allowed nurses to better plan care and align perceptions within the team, while post-conference provided space for evaluation and reflection, which are essential for continuous improvement.

Abstract

Kegiatan pengabdian masyarakat di Kampung Tobati, Jayapura Selatan, bertujuan menekan tingginya angka stunting yang meningkat dari 29% pada 2024 menjadi 33% pada 2025. Program ini menekankan pendekatan edukatif, partisipatif, dan teknologi, melibatkan ibu rumah tangga dan remaja sebagai mitra utama serta memberi pengalaman praktis bagi mahasiswa dan dosen. Kegiatan meliputi sosialisasi, pelatihan, penerapan teknologi, pendampingan, dan evaluasi berkelanjutan. Inovasi yang diterapkan mencakup penggunaan alat ukur digital, website untuk deteksi status gizi, media pembelajaran digital, dan pengolahan sampah organik menjadi pupuk melalui komposter. Mitra dilatih mengolah pangan lokal berbasis ikan dengan alat sederhana untuk meningkatkan gizi dan nilai tambah produk. Hasilnya, pengetahuan, keterampilan, dan kesadaran masyarakat terkait kesehatan, lingkungan, dan pangan meningkat, dengan harapan program berkelanjutan melalui dukungan pemerintah, penguatan kader, dan replikasi ke wilayah lain.
