Abstract

Research related to education through home visits was carried out to see medication adherence, blood sugar control and quality of life of DM patients as well as to measure the cost of re-admission the cost of referral to a hospital. To measure the costs and benefits (in rupiah) of an intervention and its effect on health care outcomes, pharmacoeconomics Cost Benefit Analysis was used. This comparative study used a cross-sectional design. The research sample was taken using purposive sampling technique as many as 30 from the group at the clinic implementing the home visit and 30 from the group at the non-home visit clinic. Measuring patient medication adherence using the Medication Adherence Report Scale (MARS-5) questionnaire, measuring the patient’s quality of life using the European Quality of Life 5 Dimensions (EQ-5D-5L) questionnaire. Blood sugar control data were obtained from medical records. For cost analysis, it was obtained from the SIM Medical Clinic and Primary Care BPJS Health. The results of the Mann-Whitney test showed a significant value (p>0.05). There was no significant difference in medication adherence and quality of life between the two groups. The OR value of 1.43 shows that the level of patient compliance in the clinic completing the home visit is 1.43 x higher than the patient at the non-home visit clinic. The value of OR 1 on blood sugar control indicates that both groups have the same chance of controlling blood sugar.

Abstract

Several pharmaceutical companies have long complained that the price of medicines that win the e-catalogue tender is too low, in some cases even below the cost of production. However, it should not be a concern that only pharmaceutical products with regulation of Indonesian food and drug (BPOM) distribution licenses are eligible for the price tender, as they are of good quality. This study aims to determine the quality and potency of three antibiotic drugs based on their highest utilization (DU 90%) in pneumonia patients at two hospitals, A (ceftriaxone, azithromycin tablet) and B (ceftriaxone, levofloxacin infusion) compared to brand name. The quality of the samples was evaluated following the Indonesian Pharmacopoeia 6th Edition (FI-VI). Antibiotic potency was assessed using the Plate-Cylinder Method with K. pneumonia from human and S. pneumonia ATCC 10015 as the test bacteria evaluated following CLSI. All samples meet the criteria of FI-VI antibiotic content, weight uniformity, dissolution. Antibiotic potency all samples test S.pneumonia and K.pneumonia were sensitive but ceftriaxone test with K.pneumonia was resistance. All antibiotic tablets and injections studied met the requirements of the Indonesian Pharmacopoeia Edition 6 for active medicinal ingredient content, dosage weight uniformity, and dissolution. All drugs from hospitals (INN) have lower antibiotic potency than branded drugs. This highlight the importance of conducting microbiological testing on antibiotic preparations.

Abstract

Latar belakang  :Hemodialisis adalah suatu cara untuk memperbaiki kelainan fungsi ginjal,  dilakukan dengan menggunakan mesin hemodialisis. Terapi hemodialisis membutuhkan waktu yang lama dan biaya mahal yang dapat mempengaruhi kualitas hidup.Tujuan Penelitian Mengetahui manajemen profil pengobatan Eritropetin, biaya perspektif rumah sakit, dan faktor-faktor yang mempengaruhi kualitas hidup pasien hemodialisis rawat jalan di RSUP Fatmawati.Metode Penelitian : Metode yang digunakan adalah analisis Cross-Sectional  dengan rentang penelitian 2 bulan, pada pasien dewasa yang minimal 12-36 bulan menjalani hemodialisis Pengumpulan data dilakukan dengan retrospektif dari dokumen, untuk menilai kesesuaian manajemen profil pengobatan Eritropoetin dan secara prospektif dari pengisian kuesioner EQ-5D-5L dan VAS (Visual Analogue Scale) kemudian dilakukan analisis data secara deskriptif dan menggunakan analisis Regresi linier berganda untuk mendapatkan korelasi antara karakteristik terhadap kualitas hidup (VAS dan Utility).Hasil Penelitian : Selama periode 2 bulan jumlah pasien sebanyak 100 orang. Jumlah laki-laki lebih banyak (57 pasien), usia 50-59 tahun sebanyak 30%, 55% bekerja sebagai karyawan, 50% pendidikan SMA, 54% pendapatan kurang Rp 2.000.000,-. Hasil kesesuaian terapi Eritropoetin sudah 100% sesuai menurut pernefri. Biaya riil untuk tindakan hemodialisis sebesar Rp910.570,- lebih kecil dibanding biaya INA-CBGs Rp 982.400,- . Analisis regresi linier VAS menunjukan 3 variabel yang signifikan : depresi (p= 0,003), jenis kelamin (p=0,028), lama hemodialisis (p=0,035). Analisis regresi linier utility ada 1 variabel yang signifikan yaitu pendapatan tiap bulan (p = 0,023).Kesimpulan  : kesesuaian terapi untuk menejemen anemia di sesuai dengan standar terapi menurut pernefri. Biaya persepektif rumah sakit lebih kecil daripada biaya INA-CBGs. Ada tiga variabel yang mempengaruhi kualitas hidup VAS yaitu depresi, jenis kelamin dan lama hemodialisis sedangkan kualitas hidup dari aspek utility hanyak dipengaruhi faktor pendapatan per bulan. Â
