Abstract

The rapid digitalization of healthcare has led to widespread adoption of Electronic Medical Records (EMR), driving improvements in data management, clinical efficiency, and patient-centered care. However, this transformation also presents critical challenges related to data security, privacy protection, and intelligent data utilization. This study conducts a Systematic Literature Review (SLR) of 30 peer-reviewed journal articles published between 2022 and 2025, focusing on three key research domains: EMR systems, smart data processing, and blockchain-based security. The review identifies that EMR serves as the foundation for healthcare data standardization and interoperability, while smart data processing—through artificial intelligence and machine learning—enables predictive analytics and clinical decision support. Concurrently, blockchain technology strengthens data integrity, transparency, and access control in distributed medical environments. Despite these advances, persistent challenges remain, including scalability limitations, interoperability gaps, and ethical concerns in data governance. The study concludes that integrating blockchain with intelligent data processing represents a promising path toward developing secure, interoperable, and intelligent EMR systems capable of supporting the next generation of digital healthcare.

Abstract

Delays in the return of inpatient medical records at Hospital X reached 25.01%, which is far below the required standard of 100% completion within 2×24 hours. This study aims to analyze the factors contributing to these delays using a qualitative approach. The main informants were medical record officers, and data were collected through interviews, observations, and documentation, with validity ensured through source and method triangulation. The findings show that predisposing factors include mismatched educational background, limited knowledge, and staff attitudes related to the completeness of signatures from attending physicians and nurses. Enabling factors involve the long distance between inpatient wards and the medical record unit, as well as the absence of elevator facilities for transporting records. Reinforcing factors include the lack of penalties for delayed returns and insufficient understanding of the physical form and standard operating procedures for medical record return. Improvement efforts include staff training, socialization of medical record completeness requirements, procurement of dedicated transport facilities for records, and regular revision and dissemination of standard operating procedures. The hospital is advised to strengthen monitoring, improve inter-unit coordination, provide efficient record transportation systems, and implement a reward and punishment mechanism to enhance the timeliness of inpatient medical record returns.

Abstract

Incomplete medical resume documentation remains a persistent issue, including at Bahagia General Hospital Makassar. In the Cempaka inpatient ward, the percentage of incomplete medical resumes in November, December, and January reached an average of 8.00 and showed an increasing trend. This study aims to analyze the factors contributing to the incompleteness of medical resume documentation based on Armstrong and Baron’s (1998) performance theory. The study employed a qualitative approach through interviews, observations, documentation review, and brainstorming. Research subjects consisted of physicians, nurses, and medical record staff. The findings indicate several contributing factors. Individual factors include the suitability of staff educational backgrounds, the absence of training related to medical resume completion, and limited knowledge regarding completeness standards (SPM). Leadership factors involve the lack of motivational strategies such as rewards for staff who consistently complete medical resumes, as well as insufficient involvement of all staff in completeness evaluations. System factors include suboptimal use of checklist forms, the absence of standardized operating procedures (SOP) for completing and assessing medical resumes, and the lack of SOP socialization. Situational factors are related to workload issues, particularly staff performing multiple roles (double jobs). Meanwhile, group factors were found to function well through effective teamwork among staff. In conclusion, incompleteness in medical resume documentation is influenced by individual, leadership, system, and situational factors. It is recommended to provide training for involved staff, develop and implement SOPs, and conduct SOP socialization to improve medical resume completeness.
