Abstract

The aim of this community service activity is to empower the people of Padang Pelawi Village, Seluma Regency, in utilizing medicinal plants to improve family health. This activity was carried out through training in medicinal plant cultivation, herbal product processing, instant ginger production, and guidance on their use for everyday health. The implementation method used a participatory approach, involving the community directly in every stage of the activity. The first step was identifying local medicinal plants that could be used, followed by training on plant cultivation and how to process the harvest into beneficial products, such as instant ginger. Ongoing guidance was provided to ensure the sustainability of the activity and optimal management of the medicinal plants. The results of this activity show that the community has gained knowledge and skills in cultivating medicinal plants and processing them into useful products. Some families have already started growing medicinal plants in their yards and processing them into products that can be consumed or sold. Additionally, this activity has had an impact on increasing family income through the sale of medicinal plant products. This initiative successfully raised the community's awareness of the importance of utilizing medicinal plants as a natural and affordable health solution.

Abstract

e-retensi merupakan salah satu bagian pengelolaan rekam medis elektronik yang memiliki peran penting dalam menjaga keutuhan dan ketersediaan data rekam medis. Berdasarkan hasil penelitian yang telah dilakukan diketahui Rumah Sakit X Bengkulu masih menggunakan retensi konvensional, dimana data berkas rekam medis yang telah diretensi tidak dicatat dalam buku retensi, sehingga menyebabkan petugas filing memerlukan waktu cukup lama untuk ditelusuri berkas rekam medis pada saat pasien datang kembali berobat. Kegiatan Pengabdian Kepada Masyarakat ini bertujuan memberikan edukasi dan wawasan kepada petugas terkait pentingnya e-retensi di Rumah Sakit. Kegiatan ini dilaksanakan sebanyak 3 kali dengan metode sosialisasi dan diskusi interaktif mengenai pentingnya pemahaman petugas filing dalam mendukung upaya implementasi e-retensi. Dari hasil evaluasi, diketahui bahwa pemahaman petugas filing sebelum dan sesudah diberikan sosialisasi meningkat menjadi 70% dimana sebelum diberikan edukasi rerata pretest 56,00 dan setelah diberikan edukasi rerata nilai posttest menjadi 95,00. Kegiatan ini dilakukan sebagai upaya optimalisasi meningkatkan pemahaman petugas filing dalam meminimalisir rekam medis yang tidak tersimpan dalam e-retensi serta mendukung keberhasilan migrasi retensi konvensional ke elektronik.

Abstract

Rekam medis tebal (volumenious bulky) merupakan salah satu permasalahan yang sering terjadi pada penyimpanan dan penjajaran, diketahui terjadi peningkatan volumenious bulky setiap tahunnya ±10% dengan tingkat ketebalan yang berbeda antara 3,7 s.d 4,5 cm pada masing-masing rekam medis dengan kodisi map rekam medis yang mulai robek karena tidak mampu menopang isi rekam medis, hal ini disebabkan pasien gangguan jiwa sering kontrol rutin dan dirawat cukup lama, serta RSKJ Soeprapto Provinsi Bengkulu belum memiliki standar ketebalan rekam medis sebagai pedoman untuk memisahkan volumenious bulky. Dampak yang terjadi apabila volumenious bulky tidak dipecah menjadi beberapa bagian dapat mengakibatkan kerusakan map dan riwayat penyakit pasien tercecer/tidak utuh. Penelitian ini bertujuan mengetahui pengelolaan volumenious bulky di RSKJ Soeprapto Provinsi Bengkulu. Jenis penelitian ini adalah deskriptif. Populasi dalam penelitian ini adalah 693 volumenious bulky dengan sampel sebanyak 87 yang diambil secara random sampling. Data yang digunakan adalah data primer dengan teknik pengumpulan data dilakukan secara observasi dan wawancara yang diolah secara univariat. Pengelolaan rekam medis tebal (volumenious bulky) di RSKJ Soeprapto Provinsi Bengkulu dilakukan dengan cara dipecah menjadi beberapa map rekam medis Bengkulu dan diberi identitas pasien (nama dan No.RM) yang sama dengan map sebelumnya akan tetapi tidak diberi keterangan volume/bagian pada map rekam medis sebanyak 62 (71,3%), penempatan volumenious bulky yang telah dipecah menjadi beberapa map rekam ditempatkan terpisah antara volume 1 dengan volume 2 sebanyak 78 (89,7%) dan penggunaan sarana pengganti (tracer) pada saat salah satu bagian volumenious bulky yang keluar dari rak penyimpanan sebanyak 61 (70,1%).

Abstract

Average Length Of Stay (LOS) is an important indicator in determining the success of therapy for a disease and is also related to the cost of care incurred by the patient. the ideal value of AvLOS is 6-9 days. The level of AvLOS is influenced by the complications experienced by the patient. Typhoid fever is an endemic disease in Indonesia that knows no age and gender boundaries, and can cause various complications in the form of bleeding and intestinal perforation which can often lead to death. Based on the initial survey, the visit of typhoid fever patients in the last three years at Harapan Doa Hospital, Bengkulu City has increased, namely 2017 (11.02%), 2018 (13.47%) and 2019 (17.935) of the total visits/year, however, in 2020 it decreased (15.08%) due to the Covid-19 pandemic. This affects the AvLOS value and the cost received by the hospital. The purpose of this study was to determine the AvLOS value of typhoid fever patients at Harapan Doa Hospital, Bengkulu City. The type of research used is descriptive with a cross sectional approach. The sample of this study used a total population of 121 patients who were analyzed univariately using the AvLOS formula. The results of the univariate analysis, from the calculation results obtained that the total length of stay (LD) of typhoid fever patients was 395 days/year with AvLOS 3.18 days/year, the number of typhoid fever patients was out of life 124 (100%) patients, AvLOS of typhoid fever patients based on age group. The highest was found in the age group >1-5 years, namely 5.17 days compared to the age group >44-65 years, namely 3.43 days and >65 years, namely 3 days, where typhoid fever was accompanied by complications/companies. AvLOS of typhoid fever patients based on gender was found in female sex at 3.31 days with a total of 59 patients compared to male sex at 3.23 days with a total of 5 people. It is hoped that the hospital will formulate a clinical pathway for Typhoid Fever Treatment so that the quality of service for typhoid fever cases is in the efficientcategory.

Abstract

The daily inpatient census is a collection of patient data entering and leaving the inpatient room. The daily census data obtained from each treatment room will be processed, so that later it will get the parameters BOR (Bed Occupancy Rate), AvLos (Average Length Of Stay), TOI (Turn Over Interval), BTO (Bed Turn Over). Present in the formulir of Barber Johnson Graph. The purpose of this study was to determine the calculation of the Barber Johnson chart indicator based on the daily census of inpatients at RSKJ Soeprapto Bengkulu in 2017. The type of research used was descriptive observational with a cross-sectional design, the population in this study was inpatient data recorded at the SHRI. in January-March 2017, totaling 560 patients, with a total sample of the existing population, total sampling. Using secondary data, processed univariately. This research was conducted from July to August 2017 at the medical record unit of RSKJ Soeprapto Bengkulu. The results of univariate analysis, from the calculation of hospital service indicators, the BOR value of IPC room was (60.9%) according to standard, Rajawali (27.38%) not according to standard, Orchid (60.33%) according to standard. The AvLos value of IPC space (2.57 days) was according to the standard, Rajawali (5.52 days) was according to the standard, Orchid (14.22 days) was not up to the standard. BTO value of IPC room (18.8 times) was not up to standard, Rajwali (3.15 times) was not up to standard, Orchid (3.32 times) was not up to standard. TOI value of IPC room (1.9 days) was up to standard, Rajwali (24.57 days) is not up to standard, Orchid (11.42 days) is not up to standard. It is expected that health workers, especially medical records, fill out the Inpatient Daily Census (SHRI) form completely so that Barber Johnson charts can be made so that the efficiency of hospital management is known.

Abstract

Rekam medis volumenious bulkymerupakan salah satu permasalahan yang sering terjadi dipenyimpanan, dan keadaan dimana rekam medis pasien dengan penyakit tertentu yang rutinberobat jalan atau dirawat cukup lama dan rutin menjadi sangat tebal karena seluruh riwayatkesehatan pasien tersimpan dalam satu map rekam medis. Berdasarkan observasi di RSKJSoeprapto Provinsi Bengkulu ditemukan rekam medis volumenious bulky yang mengalamikenaikan setiap tahunnya ± 10% dengan tingkat ketebalan yang berbeda antara 3,7 s.d 4,5 cmpada masing-masing rekam medis dengan kodisi map rekam medis yang sudah mulai robekkarena tidak mampu menopang isi rekam medis. Hal ini dikarenakanpasien gangguan jiwarawat jalan yang sering kontrol rutin atau lama dirawat yang panjang dengan didukung masasimpan (retensi) aktif berkas rekam medis jiwa rawat jalan 10 tahun dan rawat inap 5 tahunmenyebabkan rekam medis mejadi tebal.RSKJ Soeprapto Provinsi Bengkulubelum memilikiprosedur kerja sebagai pedoman pengelolaan dan penyimpanan dalam rekam medisvolumenious bulky, sehingga berdampak pada kerusakan map rekam medis yang dapatmengakibatkan riwayat penyakit pasien yang bersangkutan bercecer/tidak utuh.Penelitian inibertujuan merancang prosedur kerja rekam medis volumenious bulky di RSKJ SoepraptoProvinsi Bengkulu. Jenis penelitian adalah deskriptif kualitatif dengan teknik pengumpulandata dilakukan secara observasi dan wawancara tentang proses pengelolaan rekam medisvolumenious bulky. Tahapan dalam penelitian ini meliputi tahap analisis, tahap perancangan,tahap testing dan tahap pemeliharaan prosedur kerja.Hasil penelitian adalah rancanganprosedur kerja rekam medis volumenious bulky sesuai dengan kebutuhan rumah sakit yaituprosedur kerja rekam medis volumenious bulky dengan penempatan bagian rekam medisterpisah.

Abstract

Ministry of Health of the Republic of Indonesia Number HK.02.02/Menkes/514/2015 states that pneumonia is one of the diseases that often causes medical disputes due to disagreements between BPJSKesehatan and Advanced Health Facilities regarding medical issues that are not in accordance with the Clinical Practice Guidelines. This is due to incompleteness and inaccuracy of the results of physical examinations and medical support (thorax radiographs) recorded in the patient's medical record, so that the process of coding the diagnosis of pneumonia becomes inaccurate. This Community Service Activity aims to provide an overview to casemix officers about the effect of incompleteness and inaccuracy of medical information reviewed from the results of physical examinations and medical support on the accuracy of pneumonia diagnosis codes at Bengkulu Hospital. The method used is socialization and interactive discussion targeting the Head of Casemix and 9 casemix officers. The results of the evaluation through pretest and posttest showed an increase in knowledge and understanding of casemix officers before and after being given socialization by 61% with an average pretest of 56.00 and posttest to 90.00. This activity needs to be carried out as an effort to improve the quality of health services and increase the awareness and understanding of casemix officers regarding the importance of completeness and accuracy of physical examinations and medical support in supporting code accuracy.

Abstract

The completeness of visual acuity (visual acuity) examination results, the completeness of slit lamp examination results, and the accuracy of diagnosis are very important in supporting the accuracy of diagnostic codes and action codes. The initial survey results of 10 e-RMs obtained 6 (60%) accurate and 4 (40%) inaccurate. This is because the results of the visual acuity examination and slit lamp examination are incomplete, resulting in inaccurate diagnostic codes and action codes that impact the quality of data, information, and reports, as well as the accuracy of INA-CBG's rates, which can result in a decrease in the quality of hospital services. This study aims to determine the accuracy of cataract diagnosis and treatment codes based on the completeness of e-RM-based medical information at Hospital X. This type of study is an observational study with a quantitative descriptive design. The study population was 548 e-RMs, with a sample of 85 e-RMs taken by simple random sampling. This study uses secondary data measured by observation sheets, and then the data is processed and analyzed univariately using frequency distribution. The results of this study obtained completeness of visual acuity examination results of 74 (87%), completeness of slit lamp examination results of 77 (91%), accuracy of diagnosis codes of 79 (93%), and accuracy of action codes of 72 (85%). The need to conduct periodic control of the completeness of medical information through medical record audits, establish communication if there is an inaccurate diagnosis, and improve coder skills through coding classification training.
