Abstract

XYZ merupakan perusahaan dalam bidang industry machining and engineering. Spesialisasi PT. XYZ adalah dalam manufacturing press parts, dies, jigs, machinery and construction. salah satu produknya yaitu Crank shaft. terdapat sejumlah jenis defect yang melebihi standar toleransi perusahaan. Selama periode Maret hingga Agustus telah ditemukan produk defect sebanyak 1,139 pcs. Six sigma adalah metode yang digunakan untuk pengendalian kualitas, memiliki tujuan untuk menghilangkan cacat produk dan mengurangi biaya. Terdapat lima jenis defect yang ditemukan yakni patah, baret, oversize, longgar, dan aus. Dengan diagram pareto diketahui bahwa ada empat jenis defect crankshaft paling dominan yaitu: baret 29,41%, patah 26.42% aus 16,06%, dan longgar 14,31%. Dengan menggunakan fishbone diagram dapat disimpulkan bahwa faktor utama dari terjadinya defect adalah bagian mata pisau yang digunakan pada mesin tidak sesuai standar, perawatan mesin kurang maksimal, kurangnya ventilasi udara, dan kualitas bahan baku yang rendah

Abstract

Occupational health and safety is one of the necessary things in the industrial world. The problems that arose in the industrial era were the need for workers and increased productivity to be able to produce quality products. Health conditions and the availability of employee safety protections can affect work productivity. So that the safety and work safety culture in the workplace can be realized and can increase the safety performance index, an approach is implemented in the form of methods that can be applied to all organizations in the enterprise, one of which is behavior-based safety methods. The analysis of the data obtained shows that the factors that cause work accidents due to behavior are taken from the validity test, namely the factors of perception, communication and environment, because these three factors are very easily ignored by employees and company management. Environmental factors and personal protective equipment have no effect on work accidents as management always provides periodic education to its employees and enforces strict rules on the use of personal protective equipment. The magnitude of the influence of the variables studied in Behavior-Based safety in an effort to improve safe behavior was 79.7% and the remaining 20.3% was influenced by other unexamined items. The value of 79.7% consists of perception, communication and environment.

Abstract

Perusahaan selalu berusaha memenuhi tuntutan pelanggan, sehingga harus mampu memberikan kualitas produk yang baik. Pengendalian kualitas selalu dilakukan oleh perusahaan manufaktur demi tercapainya zero defect. Collapsible tube merupakan salah satu jenis produk yang di produksi dari perusahaan manufaktur packaging alumunium tube yaitu PT XYZ. Tujuan penelitian ini, untuk mengetahui factor apa saja yang menyebabkan terjadinya reject khususnya pada mesin printing dengan metode Failure Mode and Effect Analysis (FMEA). Adapun problem yang dijadikan focus dalam perbaikan yaitu reject warna diluar standard. Dimana reject ini merupakan reject yang tertinggi dalam proses produksi collapsible tube selama bulan November 2019. Penelitian ini berkaitan dengan mengidentifikasi dan mencegah mode kegagalan sebanyak mungkin serta menemukan sumber dan akar penyebab masalah kualitas. Berdasarkan hasil data produksi selama bulan November 2019 presentase reject mencapai 7,6% sedangkan pihak Departement Quality Control menetapkan batas toleransi sebasar 5%. Maka jika dilihat dari presentase tersebut terdapat selisih 2,6% yang harus dikaji lebih dalam agar presentase reject turun dari jumlah sebelumnya.
