Abstract

Breast milk is the optimal method to fulfil the nutritional needs of infants for optimal growth and development. However, limited milk production and ejection in the first days after delivery is often a barrier to exclusive breastfeeding. Breast engorgement in breastfeeding mothers is also a common challenge, which can hinder breastfeeding. This study used a case study design with a nursing process approach. The subjects were two breastfeeding mothers over 25 years old who experienced breast engorgement. The interventions provided included a combination of breast care and acupressure, which was carried out for four consecutive days with a duration of 20-30 minutes per session. Data were collected through observation, interview, and cheque sheet. The results showed a significant decrease in the intensity of breast engorgement and an increase in milk production in both subjects. In client I, the frequency of breastfeeding increased from 5 times to 9 times per day, and the frequency of baby's BAK increased from 5 times to 7 times per day. In client II, the frequency of breastfeeding increased from 6 times to 10 times per day, and the frequency of baby's BAK increased from 5 times to 7 times per day. Both clients reported decreased pain and increased comfort while breastfeeding. The combination of breast care and acupressure intervention is effective in overcoming the problem of breast engorgement and increasing milk production in breastfeeding mothers. The researcher suggested the combination of breast care and acupressure as part of breast care management in breastfeeding mothers.

Abstract

Background: Diastasis rectus abdominis (DRA) is a common condition in postpartum women characterized by abdominal muscle separation, which may lead to functional disorders and reduced quality of life. One of the effective interventions is postpartum exercise, a simple physical activity designed to strengthen the abdominal muscles and accelerate recovery. Objective: This study aimed to describe the implementation of education and postpartum exercise to reduce DRA and increase knowledge among postpartum mothers. Methods: A descriptive case study design with a nursing care approach was applied to two multiparous postpartum mothers with DRA >2.5 cm. Interventions consisted of health education and postpartum exercise training conducted over seven consecutive days. Data were collected through interviews, observations, questionnaires, and DRA measurements using a simple caliper. Results: The findings showed a reduction in DRA size, from 2.96 cm to 2.78 cm in client I and from 2.86 cm to 2.64 cm in client II. Knowledge scores also increased significantly, with correct responses on the questionnaire improving from 4–5 to 10 out of 10 items. Conclusion: Education and postpartum exercise with a nursing care approach were proven to reduce DRA size and enhance maternal knowledge. Postpartum exercise can be recommended as a simple, cost-effective, and practical nursing intervention to support abdominal muscle recovery in postpartum mothers.

Functional nocturnal enuresis is a condition in which children over the age of 4 without physical abnormalities or organic disease experience involuntary urine output during the day or night. Bedwetting can result in loss of self-confidence, damage relationships, and affect children's social developmen. There are several students who wet the bed when they wake up at night and parents have never been educated about enuresis. The aim of this activity is to increase parents' knowledge, attitudes and abilities about Enuresis. Methods used: lecture, question and answer, and self and group reflection. It was found that the level of insufficient knowledge before education was 100%. After education, the level of knowledge was good 64.28%, sufficient 32.14% and poor 3.57%. There was an increase in the percentage of knowledge of 96.42%. Proactive actions taken by parents to understand bedwetting and actively participate in dealing with bedwetting. Of the 10 children who still wet the bed after evaluation between 2 - 4 months, it was found that 90% of the children no longer wet the bed. The forms of behavioral intervention are as follows: walking (24.13%), lifting (3.44%), reward system (10.34%), limiting drinking before bed (24.13%), limiting playing time (10.34) and Urination before bed (31.03%).
