Abstract

Rheumatoid Arthritis or RA disease is a chronic inflammatory and systemic disease associated with broad synovitis resulting in erosion of the articular cartilage and marginal bone causing joint damage. RA is an autoimmune disease with the discovery of an autoantibody, namely rheumatoid factor. The relevant antibody is anti-citrullinated protein (ACPA) antibody. Citullination on the introduction of several proteins such as fibrin, vimentin, fibronectin, collagen type II, which is expressed in the synovial membrane during inflammation by ACPA. In a recent study increased HDAC activity in the synovial tissue of RA patients has increased. Histone deacetylase or abbreviated as HDAC is an enzyme that is important in regulating gene transcription by altering the acetylation of histone proteins which results in an important regulation of repression in the implementation of inflammation. HDAC enzyme inhibitor (HDACi) is an HDAC enzyme inhibitor that can provide benefits for the treatment of various diseases including malignancy and inflammation. In inflammatory disease HDACi overcomes inflammatory cytokines such as TNF-α, IL-6 and IL-1β. One of the HDAC classes is HDAC1 which is highly expressed in the synovial fibroblasts of RA patients. HDACi can burden swollen joints, reduce mononuclear cell infiltration, request pannus orders, inhibit bone and cartilage damage.

Abstract

Background: Adult-Onset Still's Disease (AOSD) is a rare multisystemic autoinflammatory disorder with an unknown etiology, and the diagnosis is difficult due to various differential diagnoses. AOSD diagnosis is getting better because medical developments and therapeutic strategies from various studies have benefited from advances in understanding autoinflammatory and autoimmune diseases. Case presentation. A woman patient has a complaint of periodic high fever with joint pain for more than 1 month and reddish spots on the skin, fatigue, and menstrual disorders. Patients had been treated several times and performed a blood transfusion, but complaints still occur frequently. The patient is diagnosed with Adult-Onset Still’s Disease with secondary amenorrhoea. Conclusion: This case is rarely found, so it requires quite difficult therapy and diagnostics. Therefore, this case was presented as a case report in order to get better therapy.

Abstract

Anemia is a medical problem that occurs throughout the world, especially in developing countries such as Indonesia. One type of anemia that is often asymptomatic (silent anemia) is iron deficiency anemia, which mostly occurs in the productive age group. This condition in addition to causing a decrease in work productivity also affects the body's immunity which is very important in the current state of the COVID-19 pandemic. The purpose of this activity was to determine the pattern of anemia and the prevalence of iron deficiency anemia in health workers who work in the COVID-19 infection service room who are at high risk for infection if they have a weak immune system. The results showed that 11 people (14.47%) of health workers had absolute or relative iron deficiency anemia. Although this figure is relatively lower than the national average, the management of this condition is very important because it is related to the services that will be provided to the community.

Abstract

. Osteoporosis is a worldwide health issue. The microarchitecture of the bone is deteriorating, which increases the risk of fracture and disability. As the Indonesian people's life expectancy increases, the incidence of osteoporosis rises, making it more difficult to manage. This community service is carried out as a form of preventive treatment in the adult community in the village of Ibul Besar III Pemulutan Ogan Ilir, South Sumatra, which is one of Sriwijaya University's guided villages, with counseling methods to increase knowledge and health checks for early detection of osteoporosis, as well as calcium supplementation. There were 81 participants in this activity, with numerous dominant risk factors for osteoporosis identified, including female gender, menopausal state, and age above 50. Musculoskeletal symptoms, such as pain in the waist and back, which can be a consequence of osteoporosis, are more common than non-musculoskeletal symptoms.

Abstract

Systemic Lupus Erythematosus (SLE) is a chronic autoimmune disease that can affect various organs. Clinical manifestations vary from mild to severe depending on the organs involved. Manifestations of SLE in the kidney known as lupus nephritis can occur in 60% of the SLE population and are more common in Asian, Hispanic, Native American, and Black populations, especially in women of childbearing age, involvement of other organs such as neurology is referred to as neuropsychiatric lupus, as well as other organs can manifest such as mucocutaneous lupus, arthritis, pericarditis and other organs. This community service is carried out as a form of preventive treatment with counseling methods to increase knowledge and health checks for early detection of SLE, as well as promotive efforts for the SLE community in the disease polyclinic at Dr. Mohammad Hoesin Palembang, South Sumatra which is one of the teaching hospitals of the Faculty of Medicine, Sriwijaya University. This activity was attended by 35 people with SLE with several risk facts such as female gender and middle adulthood which is quite common, and some of the systemic clinical manifestations experienced are almost fatigue and fever, for musculoskeletal manifestations the most commonly experienced is arthritis, while the most common manifestations of other organs are mucocutaneous disorders. Abstract

BACKGROUND: Vitamin D has a role in downregulating the proinflammatory cytokines as well as promoting the antiinflammatory pathway in rheumatoid arthritis (RA). Seluang fish (Rasbora sp.) has potency as a new source of vitamin D. Previous study had proven Seluang fish oil efficacy in systemic lupus erythematosus. However, there are no trials that prove its efficacy in RA yet. Hence, this study was conducted to find out the ability of Seluang fish oil to improve proinflammatory cytokines, vitamin D levels, and disease activity in RA.METHODS: A clinical trial with a randomized and double-blind method was done in two groups, each one consisting of 17 RA subjects. One group was given 500 mL of a Seluang fish oil capsule (contains 665 IU cholecalciferol), while the other group was given a placebo daily, for 12 consecutive weeks. Measurements of the RA disease activity score 28 erythrocyte sedimentation rate (DAS28-ESR) and DAS28 C-reactive protein (DAS28-CRP), as well as measurement of interleukin (IL)-6, IL-17, and vitamin D levels by using immunoassay method were performed before and after the supplementation.RESULTS: Significant alterations in the lower levels of IL-17 were observed in the Seluang fish oil group (p=0.031), but not in the placebo group (p=0.320). Reduction of DAS28-ESR (p=0.000) and DAS28-CRP (p=0.000) score demonstrated that the Seluang fish oil supplementation was useful in reducing RA disease activity. No significant shift was observed in either vitamin D (p=0.967) or IL-6 levels (p=0.076) after Seluang Fish Oil supplementation.CONCLUSION: Seluang fish oil is effective in lowering IL-17 levels, DAS28-ESR, and DAS28-CRP, but not in improving vitamin D level or lowering IL-6 level in RA patients.KEYWORDS: rheumatoid arthritis, seluang fish oil, interleukin-6, interleukin-17, vitamin D, DAS28

Abstract

Gout is a heterogeneous, often familial, metabolic disease associated with abnormal deposits of uric acid in tissues and initially characterized by recurrent acute arthritis, usually monoarticular, and later by chronic deforming arthritis. Urate deposition occurs when serum uric acid is saturated (that is, at greater than 6.8 mg/dL [404.5 mcmol/L]). Hyperuricemia is caused by excess or underexcretion of uric acid, sometimes both. The disease is especially common in the Pacific islands, for example, the Philippines and Samoa. Acute gouty arthritis is sudden in onset and often occurs at night. It may develop without a clear precipitating cause or may follow a rapid increase or decrease in serum urate levels. Common precipitants are excess alcohol (especially beer), changes in medications that affect urate metabolism, and, in hospitalized patients, fasting before medical procedures. This literature review presents gout arthritis, symptoms and signs in general to the prognosis of this disease.
