Abstract

Abstract One of the fundamental steps toward a healthy and productive life is breastfeeding, which is in line with the Sustainable Development Goals (SDGs) or Sustainable Development Goals 2030 (Bennett et al., 2020). Low rates of exclusive breastfeeding lead to various infant health problems. The North Sumatra Provincial Government has recorded 299 cases of infant mortality until July 2021. Records from the Ministry of Health of the Republic of Indonesia, as many as 31.36% of 37.94% of children are sick, because they do not receive exclusive breastfeeding. The prevalence of stunting in Indonesia is currently at 27.67%. The low coverage of exclusive breastfeeding is influenced by various obstacles to optimal breastfeeding, one of the biggest obstacles is working women, especially those who work in the informal sector. The purpose of this study was to determine "The perspective of women informal workers in exclusive breastfeeding in the working area of the Deli Tua Health Center. This research is a qualitative study with a descriptive phenomenological approach. The participants in this study were mothers of traditional market traders as many as 7 participants who were obtained using nonprobality sampling technique with purposive sampling type. The instrument used in this research is an interview guide. In collecting data, this study used the in-depth interview method to collect data. The location of this research is Deli Tua Tax in the working area of the Deli Tua Health Center. The results of the research conducted obtained the results of a description of the factors that influence the perspective of traditional market trader mothers in exclusive breastfeeding, namely knowledge, work, husband support and socio-culture.

Abstract

Pregnant women should pay close attention to their health and the health of their fetuses. To prevent maternal and infant mortality, antenatal care visits are one of the actions that must be adhered to by pregnant women. This study aims to explore the perspectives of pregnant women in antenatal care visits in Padang Lawas Regency. The study was qualitative with a grounded theory approach. This study involved five mothers using purposive sampling. Data was collected through in-depth interviews and analyzed by content analysis. The results emerged namely: 1) support from husband and family, 2) accessibility, 3) health providers, and 4) mother’s behavior. Health providers play a very important role in making antenatal care visits a success routinely and completely.

Abstract

The coverage of Antenatal Care (ANC) visits in Indonesia has not yet reached the national target of 95%. Low awareness among pregnant women regarding the importance of ANC remains a challenge, influenced by factors such as education level, knowledge, family support, and limited access to health information. To address this issue, appropriate and effective educational approaches are needed to increase pregnant women's understanding and adherence to routine ANC visits. One effective educational tool is the flip chart, a visual aid composed of sequential pages containing concise and structured health information, supported by illustrations and diagrams. Flip charts promote two-way communication between healthcare providers and clients, delivering information in a gradual and visually engaging manner. This Community Service Program (PKM) aims to raise pregnant women’s awareness and understanding of ANC through flip chart-based education. The flip chart serves not only as an information medium but also as an interactive tool to support informed health decisions. It is expected that this intervention will enhance knowledge and positively influence pregnant women’s behavior in complying with ANC schedules, ultimately contributing to better maternal and fetal health outcomes.

Abstract

Abstract

The low utilization of antenatal care services has an impact on the condition of the pregnancy not being monitored which will cause danger signs of pregnancy to go undetected and can lead to maternal death. Padang Lawas is the area with the lowest coverage of antenatal care visits in north Sumatra province. The majority of pregnant women in Padang Lawas district do not receive standard antenatal care services. Efforts to increase coverage rates for Antenatal Care visits have been carried out by the Health Service but have not produced any real impact. Objective: This study aims to find out more about the perspective of antenatal care visits for pregnant women. Method: This study was qualitative with a grounded theory approach. The informants in this study were 5 mothers, 5 husbands, 3 midwives, and 1 community figure who were recruited using purposive sampling. Data were collected using semi-structured interviews. Data were analyzed using the Content Analysis. Results: The study emerged 4 themes: (1) Culture about pregnancy, (2) Infrastructure in antenatal care, (3) Economic issues, and (4) Mother's behavior in carrying out antenatal care. Conclusions: The findings show that there are several barriers of antenatal care visits in rural maternal communities, so the government needs to pay attention to them in following up on the barriers found in these areas.

Abstract

The maternal mortality rate (MMR) remains a significant public health concern in Indonesia and serves as a key indicator in achieving the targets of the Sustainable Development Goals (SDGs). One of the major contributing factors to the high MMR is the delay in recognizing and managing pregnancy danger signs, which is often due to a lack of knowledge among pregnant women. Additionally, the suboptimal coverage of antenatal care (ANC) visits exacerbates this issue, as fewer women receive critical information during pregnancy. Providing education to pregnant women is a strategic effort to enhance their understanding and foster positive attitudes throughout pregnancy. One educational medium that can be utilized is the flipchart an interactive, visual, and communicative tool designed to deliver health information in an engaging and easy-to-understand manner. This study aimed to analyze the effect of flipchart-based education on improving pregnant women’s knowledge of pregnancy danger signs. A quasi-experimental design with a one-group pre-test–post-test approach was employed. The sample consisted of 17 primigravida women receiving antenatal care at Juliana Dalimunthe Clinic, selected through purposive sampling. A validated and reliable knowledge questionnaire was administered before and after the educational intervention. Data were analyzed using a paired sample t-test. The results showed a significant increase in knowledge among pregnant women regarding danger signs of pregnancy following the educational intervention, with a significance value of p = 0.004. These findings indicate that flipchart media is an effective educational tool in maternal health promotion. Not only does it improve maternal understanding of danger signs, but it is also expected to enhance awareness and motivation to attend regular ANC visits. Therefore, it is recommended that healthcare providers optimize the use of flipcharts in health education and counseling sessions for pregnant women, as part of preventive efforts to reduce complications.

Abstract

Anemia selama kehamilan merupakan masalah kesehatan yang cukup prevalen di Indonesia dan dapat menimbulkan komplikasi serius bagi ibu dan janin, seperti persalinan prematur dan berat badan lahir rendah. Kekurangan zat besi (Fe) merupakan penyebab utama anemia pada ibu hamil. Salah satu upaya pencegahan yang efektif adalah dengan rutin mengonsumsi tablet Fe selama kehamilan. Namun, tingkat pengetahuan ibu hamil tentang pentingnya konsumsi tablet Fe masih rendah di wilayah PMB Eka Sriwahyuni. Melalui kegiatan edukasi dan sosialisasi, diharapkan peningkatan pengetahuan dan sikap positif terhadap konsumsi tablet Fe dapat tercapai. Metode yang digunakan meliputi penyuluhan langsung, distribusi brosur, dan diskusi interaktif kepada ibu hamil di posyandu. Hasil dari kegiatan ini menunjukkan adanya peningkatan pengetahuan dan kesadaran ibu hamil mengenai manfaat tablet Fe, serta peningkatan jumlah ibu hamil yang rutin mengkonsumsi tablet Fe. Diharapkan, kegiatan ini dapat berkontribusi dalam menurunkan angka anemia selama kehamilan dan meningkatkan kesehatan ibu dan janin di wilayah tersebut.

Abstract

Low birth weight (LBW) remains a significant public health concern in Indonesia, contributing to neonatal morbidity, mortality, and long-term developmental challenges. Although numerous studies have explored LBW, evidence on maternal risk factors in low-resource settings particularly within independent midwife practices remains scarce. This study examined the associations between maternal age, parity, antenatal care (ANC) visits, history of anemia, and gestational weight gain with LBW incidence in Deli Tua Sub-District, Deli Serdang Regency. A case-control design was employed from December 2024 to March 2025, involving 95 postpartum mothers (35 cases, 60 controls) selected through purposive sampling. Data were collected using validated questionnaires (Content Validity Index = 0.82; Cronbach's α = 0.85) and maternal health records, and analyzed using Chi-square tests and multivariate logistic regression (SPSS version 22). Multivariate analysis identified maternal age at risk (<25 or >35 years) as the sole significant predictor of LBW (OR = 6.69; 95% CI = 3.21-25.22; p = 0.006). Parity (OR = 2.76; p = 0.077), ANC visits (OR = 1.22; p = 0.797), history of anemia (OR = 0.57; p = 0.421), and gestational weight gain (OR = 1.20; p = 0.773) were not significantly associated with LBW. The model demonstrated acceptable fit (Hosmer Lemeshow p = 0.49; Nagelkerke R² = 0.29). Maternal age emerged as the most influential determinant of LBW in this population. These findings underscore the importance of targeted surveillance for high-risk age groups and the enhancement of promotive and preventive strategies including improved ANC service quality, nutritional counseling, and family engagement in maternal care to reduce LBW incidence in resource-limited settings. Abstract

Postpartum visits by independent midwives play a critical role in initiating and sustaining exclusive breastfeeding. However, operational and institutional barriers may compromise their effectiveness, particularly in semi‑urban and rural settings such as Deli Tua District. This study examined the association between these barriers and exclusive breastfeeding practices among mothers in Deli Tua. A quantitative cross-sectional study was conducted in 2025, involving all independent midwives in the district (n = 85) and a purposive sample of 120 postpartum mothers (1-6 weeks post-delivery). Data were collected using structured questionnaires and analyzed through Chi-Square tests and multivariate logistic regression at a 95% confidence level. Key barriers negatively associated with exclusive breastfeeding included lack of government incentives (OR = 0.23; 95% CI: 0.09-0.61; p = 0.004), excessive workload and fatigue (OR = 0.33; 95% CI: 0.14-0.78; p = 0.013), high operational costs (OR = 0.38; 95% CI: 0.17-0.83; p = 0.015), and limited time for visits (OR = 0.43; 95% CI: 0.19-0.96; p = 0.043). Conversely, institutional support from health centers significantly increased the likelihood of breastfeeding success (OR = 2.01; 95% CI: 1.01-4.01; p = 0.046). Socio-cultural factors such as stigma and customary practices showed no significant association. To enhance the effectiveness of postpartum care and promote exclusive breastfeeding, the study recommends implementing performance-based incentives, reducing workload burdens, providing financial subsidies, and establishing standardized visit-time guidelines. Strengthening cross-sectoral collaboration and resource allocation is essential to improve service delivery and breastfeeding outcomes.
