Abstract

Abstract

While family support is widely acknowledged as a critical determinant of breastfeeding self-efficacy, research on its independent predictive effect remains sparse. This limitation is particularly pronounced among postpartum mothers in Indonesia, where context-specific evidence is currently insufficient. This study aimed to analyze the relationships and effects of family support, educational level, and parity status on postpartum mothers' breastfeeding self-efficacy. A multicenter cross-sectional study was conducted among 180 postpartum mothers recruited from nine primary health centers across urban, semi-urban, and rural areas using purposive sampling. Family support was measured using a Friedman-based questionnaire, and breastfeeding self-efficacy was assessed using the Breastfeeding Self-Efficacy Scale–Short Form (BSES-SF). Spearman correlation and multiple linear regression were performed. Family support showed a moderate positive correlation with breastfeeding self-efficacy (rs = 0.559, p < 0.01). In multivariate analysis, family support remained the strongest predictor of breastfeeding self-efficacy (β = 0.509, p < 0.01), followed by parity (β = 0.255, p < 0.001), whereas maternal education was not statistically significant. Family support is the primary determinant of breastfeeding self-efficacy among postpartum mothers. Interventions to improve breastfeeding outcomes should integrate family-centered support strategies, particularly for first-time mothers.

Abstract

Chronic kidney disease is a progressive condition characterized by a permanent decline in kidney function and requires renal replacement therapy, one of which is hemodialysis. Patients with chronic kidney disease undergoing hemodialysis often experience decreased physical function and quality of life due to activity limitations, fatigue, and psychological impacts. One non-pharmacological therapy that can be applied to address these problems is a physical exercise program. This study aims to determine the effect of a physical exercise program on physical function and quality of life among patients with chronic kidney disease undergoing hemodialysis at Sembiring General Hospital Deli Tua in 2026. This study used a quasi-experimental design using a one-group pretest–posttest approach. Sampling was conducted using a purposive sampling technique, resulting in a total sample of 54 respondents. Data analysis was performed using the Wilcoxon test with a significance level of α=0.05. The results showed that the mean score of physical function before the intervention was 54.56 and increased to 57.50 after the intervention. Similarly, the mean score of quality of life increased from 56.83 before the intervention to 58.93 after the intervention. The Wilcoxon test analysis revealed a p-value of 0.000<α=0.05 for both physical function and quality of life. The conclusion shows that the physical exercise program had a significant effect on improving physical function and quality of life among patients with chronic kidney disease undergoing hemodialysis at Sembiring General Hospital Deli Tua in 2026. It is recommended that physical exercise program can be implemented for patients with chronic kidney disease to enhance their physical function and quality of life.

Abstract

Hypertension is one of the non-communicable diseases that remains a major public health problem with an increasing prevalence, especially among the elderly population. This condition is influenced by unhealthy lifestyles, lack of physical activity, and limited public knowledge regarding the prevention and management of hypertension. This community service activity aimed to improve community knowledge and awareness regarding blood pressure control through the utilization of complementary therapy in the form of a combination of celery and cucumber juice. The activity was conducted in Dusun IV, Patumbak I Village, Patumbak District, Deli Serdang Regency, May 15, 2026. The methods used included preliminary surveys, observations, interviews, health education, health screenings, hypertension exercise programs, and community empowerment activities. The target participants were residents of Dusun IV using a participatory approach. The results showed that hypertension was identified as a priority health problem in the community. After the implementation of health education and counseling, the community demonstrated high enthusiasm and improved understanding regarding hypertension risk factors, the benefits of a healthy lifestyle, and the use of celery and cucumber juice as a non-pharmacological therapy to help reduce blood pressure. This activity is expected to increase community awareness in preventing and controlling hypertension independently through the adoption of healthy lifestyles and the use of natural ingredients that are easily accessible in the surrounding environment.

Abstract

Antenatal care (ANC) plays a critical role in reducing maternal and neonatal risks, yet its delivery in rural and remote areas remains constrained by geographic isolation, limited health resources, workforce shortages, and sociocultural barriers. This study explored the experiences and challenges of health workers in providing comprehensive ANC services in rural primary health centers in West Nias District, North Sumatra, Indonesia. A qualitative study with a grounded theory approach was conducted among 16 health workers, consisting of 11 midwives and five physicians, who were selected through purposive sampling. Data were collected through in-depth semi-structured interviews, supported by field notes, limited participant observation, and review of relevant service documents. Data were analyzed using open, axial, and selective coding. The findings identified professional resilience as the core category explaining how health workers sustain ANC services in resource-limited and culturally complex settings. This resilience was shaped by four interrelated themes: limited resources and infrastructure, human resource constraints, sociocultural influences, and adaptation strategies. Health workers maintained service continuity through manual examination methods, mobile services, home visits, informal counseling, community-based education, and collaboration with village leaders, religious leaders, and traditional birth attendants. These strategies helped strengthen community trust and improve access to ANC services. The study highlights the need to strengthen rural maternal health systems through improved infrastructure, adequate staffing, supportive supervision, and culturally responsive service planning Keywords: Antenatal Care, Health Workers, Maternal Health, Professional Resilience, Rural Health Services, Community Collaboration
