Abstract

Pendahuluan: Rumah Sakit merupakan fasilitas pelayanan kesehatan masyarakat tingkat pertama. Sebagai fasilitas pelayanan kesehatan, dalam melayani pasien terdapat duplikasi penomoran rekam medis. Sistem penomoran merupakan hal utama dalam pengelolaan rekam medis. Berulang kali ditemukan duplikasi penomoran rekam medis pasien rawat jalan di RSU Bhakti Asih Tangerang, sehingga menyebabkan pelayanan menjadi terganggu, rak rekam medis menjadi terlalu penuh serta riwayat pasien yang tidak terkendali. Tujuan: Untuk mendapatkan output satu rekam medis untuk satu pasien, mengidentifikasi SPO Pemberian Penomoran Rekam Medis Rawat Jalan, mengidentifikasi kejadian duplikasi penomoran, dan faktor – faktor yang menyebabkan terjadinya duplikasi penomoran rekam medis rawat jalan RSU Bhakti Asih Tangerang. Metode: Metode yang digunakan penelitian ini yaitu metode deskriptif kuantitatif. 56 sampel pasien yang memiliki dua nomor rekam medis yang sama. Hasil: Faktor – faktor yang menyebabkan terjadi duplikasi penomoran rekam medis, yaitu latar belakang pendidikan petugas pendaftaran, pasien yang tidak membawa kartu identitas pribadi, petugas yang kurang teliti dalam melayani pasien. Kesimpulan: dari 7975 pasien baru rawat jalan RSU Bhakti Asih Tangerang terdapat 56 duplikasi penomoran rekam medis yang terjadi. Dengan persentase bulan April 5,5% dengan 218 pasien baru, Mei 9% dengan 107 pasien baru dan Juni 0,4% dengan 7650 pasien baru.  Saran petugas melakukan Cross Check data pasien pada master index pasien baik pada pasien baru maupun pasien lama, petugas pendaftaran perlu pelatihan dan seminar agar dapat meningkatkan wawasan tentang sistem penomoran rekam medis.

Abstract

Human resource planning is essential to ensure the availability of a workforce that meets current and future service needs. In the health sector, the health workload analysis method is used to objectively evaluate human resource needs. This study was conducted at Mekar Sari Bekasi Hospital to determine the ideal number of medical record and health information officers. Currently there are 9 medical record officers, consisting of 4 people with PMIK (Medical Recorder and Health Information) education backgrounds and 5 non-PMIK people. Conditions in the field show that officers often get additional workloads when colleagues are absent, and even have to work overtime to get the job done on time. This study uses descriptive quantitative methods through direct observation and interviews. The results show that the available work time (WKT) is 1201 hours or 72,000 minutes per year. With a Supporting Task Factor (FTP) of 8% and a Supporting Task Standard (STP) of 1.09, the ideal HR requirement is 10 people. This requires the addition of 1 officer with a PMIK background, who will be placed in the assembling section. Meanwhile, 1 officer each from the coding and filing sections will be transferred to the filing section.

Abstract

This research was conducted to evaluate non-academic services provided by the campus and which are not related to the curriculum in the form of supporting resources (responsiveness, neatness and politeness to officers), campus atmosphere, facilities on campus, and soft skills development. The aim of this research is to determine the characteristics and quality of non-academic services for RMIK Study Program students at Esa Unggul University, Jakarta using the servqual method introduced by Zeithaml, Parasuraman and Berry. The sample in this research was active students from the 2019-2023 class based on parallel and regular classes, totaling 74 students. The research method is quantitative with descriptive analysis using the normality test. The research results obtained from the description of characteristics based on gender are dominated by women, based on age dominated by an average age of 22 years, based on class year dominated by class of 2022 and based on class basis dominated by regular class base. Based on the description of the quality of non-academic services that have been carried out as a whole using the servqual method, students feel satisfied. Even though it was found that there were more Satisfied categories, there are several things that must be paid attention to, especially aspects that have low scores compared to others for the next Campus Image.

Abstract

Penyediaan rekam medis rawat jalan yang tepat waktu menghasilkan pelayanan rekam medis yang berkualitas. Waktu penyediaan rekam rawat jalan sesuai standar pelayanan minimal adalah ? 10 menit. Tujuan dari penelitian ini adalah untuk mengetahui lama waktu tunggu rekam medis pasien rawat jalan di RSAL dr. Mintohardjo. Metode dalam penelitian ini menggunakan metode deskriptif dengan pendekatan kuantitatif. Teknik pengambilan sampel menggunakan accidental sampling dengan besaran sampel berdasarkan estimasi proposi sebanyak 106 rekam medis. Hasil penelitian 14 rekam medis (13,2%) tepat waktu dan 92 rekam medis (86,8%) tidak tepat waktu, yang terbanyak tidak tepat waktu yaitu > 31-40 menit sebanyak 31 rekam medis (29,2%). Dalam mengidentifikasi penyebab keterlambatan penyediaan rekam medis rawat jalan, peneliti menggunakan faktor 5M. Faktor Man terjadinya missfile, terdapat rekam medis belum kembali dari poliklinik maupun ruang rawat inap, kurangnya pengetahuan dan pendidikan. Faktor Money belum adanya anggaran dalam perbaikan Roll O Pack. Faktor Material cetak bon pinjam rekam medis tidak otomatis pada aplikasi SIMRS dan hanya tersedia 1 mesin printer dari 7 komputer, jadi perlu ditambah jumlah printer. Faktor Machine terjadi kerusakan Roll O Pack dan gangguan pada software berupa downtime (waktu ketika suatu sistem, aplikasi dan layanan tidak dapat diakses/tidak berfungsi) pada aplikasi SIMRS. Faktor Method belum adanya SPO penyediaan rekam medis rawat jalan.

Abstract

One of the regional hospitals in Kembangan type D, West Jakarta, is District General Hospital Kembangan. The information management hospital (HMIS) is now used by the District General Hospital Kembangan to provide services.A number of issues exist, such as the fact that the Simrs server is still frequently prone to errors and slow service processes and reporting; data loss occurs when user data is transferred to another unit; insufficient computer resources; and a shortage of human resources to utilize the HMIS's features due to the hospital's lack of knowledge about how to run an effective HMIS. Analyzing system quality, information quality, and service quality in relation to customer happiness is the goal of this study.There are three independent variables and one dependent variable in this categorical study design.According to the findings, there is a correlation between user happiness and the quality system of (p = 0.031).Quality information and user happiness are related, as indicated by the statistic (p = 0,240).User happiness and quality of service are related, as indicated by the statistic (p = 0,021).The study's findings indicate that there is a correlation between customer satisfaction surveys and the quality of the system, information, and services.

Abstract

Alignment is the process of aligning medical records in the storage room according to the system used. The medical record storage system at Kembangan Hospital uses centralization, the medical record numbering system uses the Unit Numbering System (UNS) and the alignment system uses Terminal Digit Filing (TDF). The aim of this research is to determine the accuracy of medical record alignment. This research used a descriptive method with a quantitative approach, the population was 293 medical records using a saturated sample. From the results of research on the primary digit 07 storage shelf, it was found that there were inaccuracies in the alignment of 162 medical records (55%). This alignment inaccuracy was divided into three digits, namely 34 medical records (21%) had the 1st digit incorrect, 85 medical records (52%) had the 2nd digit incorrect and 43 medical records (27%) had the 3rd digit incorrect. . It was found that the cause of the alignment error was due to human error, the unavailability of a tracer and the map not using color coding. Medical records that cannot be found can be traced by looking at the patient's history of treatment at Simrs Khanza, then looking for the first digit number that is being searched for. When a medical record is not found, a search for the accuracy of the alignment of the medical record is carried out using the medical record tracking method. If a medical record cannot be found, a new medical record will be used temporarily.

Abstract

Penyelenggaran Sistem Informasi Manajemen Puskesmas (SIMPUS) yang diatur dalam Peraturan Menteri  Kesehatan Republik Indonesia No. 31 Tahun 2019 bahwa setiap puskesmas wajib menyelenggarakan Sistem Informasi Puskesmas (SIP). Puskesmas Kecamatan Cakung sudah mengimplementasikan epuskesmas pada tahun 2016 namun masih banyak kendala yang dihadapi ketersedian Standar Operasional Prosedure (SOP) tentang tata cara penggunaan SIMPUS yang tidak merata, ketersediaan SDM yang kurang, program pelatihan SIMPUS berkala yang tidak dijalankan, sering terjadi error pada aplikasi epuskesmas. Penelitian ini bertujuan untuk mengidentifikasi hubungan antar variabel metode HOT-Fit yaitu hubungan antara human-technology, human-organization, organization-technology. Analisis penelitian ini menggunakan uji korelasi pearson, dan uji korelasi rank-spearman dengan jumlah responden 25 petugas yang menggunakan sistem epuskesmas. Hasil identifikasi hubungan antara manusia (human) dengan teknologi (technology) memiliki hubungan yang signifikan, tingkat hubungan variabel kuat, dan memiliki arah hubungan yang searah (p-value : 0,002, r/rho : 0,593). Identifikasi hubungan antara organisasi (organization) dengan teknologi (technology) memiliki hubungan yang signifikan, tingkat hubungan kuat dan memiliki arah hubungan yang searah (Sig. : 0,000, CC : 0,700). Identifikasi hubungan antara manusia (human) dengan organisasi (organization) memiliki hubungan yang tidak signifikan, tingkat hubungan cukup kuat dan arah hubungan searah (p-value : 0,083, r/rho : 0,354).

Abstract

Abstrak: Latar belakang: Dengan adanya pandemi ini segala kegiatan seperti pekerjaan, pendidikan dilakukan dirumah dengan cara online dan beribadah pun dilakukan dari rumah. Sehingga pembelajaran dilakukan secara online melalui zoom meeting,google meet, classroom, dan website. Dengan menggunakan media cetak (modul) dan non cetak (audio/video) serta menggunakan komputer dan internet. Tujuan: Mengetahui motivasi belajar mahasiswa Rekam Medis dan Informasi Kesehatan Universitas Esa Unggul pada pembelajaran online di masa pandemi covid 19 melalui lima indikator motivasi belajar yaitu ketekunan menyelesaikan tugas, kegigihan menghadapi kesulitan, ketajaman perhatian serta minat belajar, memiliki prestasi belajar, dan kemandirian pada belajar. Metode penelitian: Metode penelitian yang digunakan adalah deskriptif dengan pendekatan kuantitatif. Sampel dalam penelitian ini adalah 203 mahasiswa dari tiga angkatan berbeda yang diambil dengan teknik stratified random sampling. Hasil penelitian: Hasil penelitian dari 5 indikator yaitu Ketekunan Menyelesaikan Tugas 58% kurang baik, Kegigihan Menghadapi Kesulitan 59% kurang baik, Ketajaman Perhatian Serta Minat Belajar 60% kurang baik, Memiliki Prestasi Belajar 73% kurang baik, dan Kemandirian pada Belajar 59% kurang baik. Kesimpulan: Motivasi belajar mahasiswa rekam medis pada pembelajaran online di masa pandemi 47% motivasi tinggi dan 53% motivasi rendah.

Abstract

Medical record is a file containing records and documents regarding patient identity, examination, treatment, actions and other services that have been provided to patients. where one of the services provided is the management of patient medical record documents that contain coding of diagnoses and actions given to patients. The implementation of coding disease diagnoses in hospitals is a very important activity, namely by classifying disease diagnoses into several groups for the benefit of reports that the hospital does every month, both for internal reports and external reports and plays an important role in the financing system at the hospital itself. The purpose of this study was to determine the accuracy of the diagnosis code for typhoid fever in inpatients at the Karawang Islamic Hospital. The research methodology was carried out using quantitative descriptive methods, the population was 200 medical records of typhoid fever patients in 2020, with a total sample of 67 samples. The sampling technique used is simple random sampling. How to collect data is done by direct observation. The accuracy of the code obtained is 31 (46%) correct codes and 36 (54%) incorrect codes, the inaccuracy of the code is caused by the medical record professional staff in charge of the coding section who are still not focused and not careful with laboratory results to determine the results of the coding. It is correct and only codes for the H titer, while in determining the diagnosis of typhoid fever, it is seen from the O titer. Suggestions for coding the diagnosis carried out at the Karawang Islamic Hospital are expected that the coding is carried out correctly according to the diagnosis, history, and laboratory results.

Abstract

The hospital is one of the health care facilities with various services helping people who have health probems. Medical record is a file that contains records and data related to patient identity, examination results, treatment history that has been given, other actions and services that have been carried out to patients. This study aimed to describe the completeness of filling the initial polyclinic assessment form. The methode of this study was descriptive analysis. The population of this study were the initial assessment of the physiotherapy clinic 312 medical records, surgery poly 177 medical records, and internal medicine clinic 457 medical records. And the sample of this research is 91 medical records. The results showed that from 91 medical records, 70 (77%) of forms is complete and 21 (23%) of forms is incomplete, which consisted of complete patient identification 80 (88%) and incomplete 11 (12%), completeness of important reports/notes 54 (59%) and incomplete 37 (41%), completeness of author authentication 73 (80%) and incomplete 18 (20%), and completeness of good records 72 (79%) and incomplete 19 ( 21%).
