Abstract

Introduction: Little do we understand about factors associated with functional disability in instrumental activities of daily living among the elderly living at home. This study aimed to explore the prevalence and determinant factors of functional disability in instrumental activities of daily living (IADL) among the elderly living in a rural area in Bali. Method: Using a cross-sectional design, 1,053 elderly aged 60 years and above living in a rural area in Bali were recruited. A structured interview by trained data collectors was conducted. Questionnaires used were the Lawton IADL Scale, the 5-item of Geriatric Depression Scale, and the adoption of questionnaires for vision, hearing, and communication problems from the Washington Group Short Set of Questions on Disability. A logistic regression model was applied to explore determinant factors. Results: Findings indicated that 26.1% of participants aged 75+ and 52.3% were female. Eight percent (8%) experienced functional disability in IADL, 1.9% lived alone, 11.8% indicated depression, 5.1%, 4.7%, and 1.6% of them had vision loss, hearing impairment, and communication problems, respectively. The strongest determinant factor for functional disability in IADL was depression (OR 7.869; 95% CI 4.657-13.296), followed by age (OR 4.602; 95% CI 2.764-7.663), and hearing impairment (OR 2.903; 95% CI 1.190-7.083). Conclusions: Depression is the strongest determinant factor for functional disability in IADL. Health professionals in rural areas should actively screen for depression to increase the ability of the elderly to fulfill their IADLs. Keywords: aged, depression, epidemiology, Indonesia

Abstract

ABSTRAK Lansia merupakan populasi rentan dan memiliki banyak masalah Kesehatan yang kompleks karena proses degeneratif. Manajemen kasus Kesehatan lansia dimasyarakat menjadi sangat penting dilakukan untuk memastikan bahwa lansia mendapatkan pelayanan Kesehatan yang baik. Keterlibatan kader Kesehatan lansia dalam manajemen kasus Kesehatan lansia dimasyarakat menjadi salah satu solusi yang dapat dilakukan untuk mencapai tujuan Kesehatan lansia. Kegiatan pengabdian kepada masyarakat ini bertujuan untuk memberikan pelatihan dan pendampingan kepada kader Kesehatan lansia dalam melaksanakan manajemen kasus Kesehatan lansia dimasyarakat. Kegiatan ini dilaksankan di Desa Melinggih, Provinsi Bali. Peserta dalam kegiatan ini adalah kader kesehatn lansia diwilayah desa setempat yang berjumlah 10 orang. Kegiatan PKM dilaksanakan dengan pendekatan PARE (Preparation, Action, Reflection dan Evaluation). Hasil kegiatan ini menemukan adanya peningkatan pengetahuan dan kemampuan kader Kesehatan lansia dalam manajamen kasus Kesehatan lansia dimasyarakat dengan nilai uji statistik Wilcoxon Signed Ranks Test didapatkan nilai Z= -2.675b dengan p value = < 0,001. Kegiatan pelatihan dan pendampingan kepada kader Kesehatan lansia dimasa mendatang harus terus dilakukan untuk membangun meningkatkan pelayanan Kesehatan yang berbasis peran serta masyarakat untuk Kesehatan lansia yang membutuhkan perawatan Kesehatan. Kata Kunci: Lanjut Usia, Gerontik, Kader, Manajemen Kasus, Perawatan Jangka Panjang  ABSTRACT Elderly people are a vulnerable population and have many complex health problems due to the degenerative process. Health case management for the elderly in the community is very important to ensure that the elderly receive good health services. The involvement of elderly health cadres in the management of elderly health cases in the community is one solution that can be done to achieve elderly health goals. This community service activity aims to provide training and assistance to elderly health cadres in implementing elderly health case management in the community. This activity was carried out in Melinggih Village, Bali Province. Participants in this activity were elderly health cadres in the local village area totally 10 people. PKM activities were carried out using the PARE approach (Preparation, Action, Reflection and Evaluation). The results of this activity found an increase in the knowledge and abilities of elderly health cadres in managing elderly health cases in the community with the Wilcoxon Signed Ranks Test statistical test value obtained Z = -2.675b with p value = <0.001. Training and mentoring activities for elderly health cadres in the future must continue to be carried out to build improvements in health services based on community participation for the health of elderly people who need health care. Keywords: Elderly, Geriatric, Cadres, Case Management, Long-Term Care

Abstract

Background: Non-pharmacological management of hypertension through complementary medicine has now been developed in Indonesia. However, the interest of hypertensive patients in complementary medicine is not optimal because there are many factors that influence the behavior of patients in the use of complementary medicine. Methods: This study employed an analytic study design with a cross-sectional approach. This research was conducted at Puskesmas 1 East Denpasar for 1 month. This study involved 101 hypertensive patients using accidental sampling technique. Data collection was carried out using a questionnaire. Data were analyzed univariately and bivariately using SPSS version 20. Results: This study showed that most hypertensive patients had good knowledge of hypertension treatment with complementary therapies (64.4%). A total of 53.5% of hypertensive patients showed adequate behavior in the treatment of hypertension with complementary therapy. Based on the chi square test, it was found that there was a significant relationship between knowledge and behavior of hypertensive patients towards hypertension treatment with complementary therapy (p-value = 0.04). Conclusion: It is recommended for health workers to increase education and promotion about hypertension treatment with complementary therapy, especially for hypertensive patients with poor knowledge.

Abstract

Hipertensi tidak dikontrol khususnya pada lansia berisiko menyebabkan komplikasi seperti penyakit jantung, stroke dan gagal ginjal. Diet hipertensi dianjurkan bagi penderita untuk membantu mengontrol tekanan darah sebagai bagian dari perawatan jangka panjang. Penelitian ini bertujuan untuk mengidentifikasi determinan faktor yang berhubungan kuat dengan perilaku diet hipertensi pada lansia. Desain penelitian ini adalah cross-sectional. Sampel dalam penelitian sebanyak 162 orang lansia menderita hipertensi di Wilayah Kerja UPTD Puskesmas Sukawati II yang diambil dengan consecutive sampling. Pengumpulan data dilakukan dengan kuisioner. Uji Chi-square digunakan untuk menganalisis hubungan masing-masing variable penelitian dan regresi logistic dengan metode backward LR digunakan untuk menganalisis determinan faktor perilaku diet lansia. Data dianalisis mengunakan SPSS versi 20. Penelitian telah mendapatkan kelaikan etik dari Komisi Etik ITEKES Bali. Determinan faktor perilaku diet hipertensi lansia adalah pendidikan (p=0,002), berarti bahwa lansia yang memiliki riwayat pendidikan rendah berisiko 4,15 kali menunjukkan perilaku diet hipertensi yang buruk dibandingkan dengan lansia dengan riwayat pendidikan tinggi dan pengetahuan (p= <0,001), berarti bahwa lansia yang memiliki pengetahuan tinggi menurunkan risiko 0,18 kali melakukan perilaku diet yang buruk dibandingkan dengan lansia yang memiliki pengetahuan rendah. Riwayat pendidikan dan pengetahuan merupakan determinan faktor dari perilaku diet hipertensi lansia. Diperlukan upaya peningkatan pengetahuan penderita hipertensi melalui edukasi khususnya pada lansia dengan riwayat pendidikan rendah untuk merubah perilaku dietnya.

Abstract

Background: Burnout among operating room nurses negatively affects patient safety and quality of care, highlighting the need for effective and accessible interventions in high-intensity clinical environments. Digital media–based relaxation approaches, particularly smartphone video interventions, offer a low-cost, flexible, and scalable solution. Therefore, this scoping review aims to map and synthesize the existing evidence on interventions to reduce burnout among nurses, with a particular focus on digital relaxation strategies delivered through smartphone videos. Methods: This literature review examined previous research findings in published articles. Article searches were conducted through PubMed and Science Direct using the keywords "Burnout interventions and treatment" for English-language articles and "burnout interventions AND treatment" for Indonesian-language articles. Fifteen articles were extracted based on the inclusion criteria. Results: Of the 313 identified articles (271 PubMed; 42 ScienceDirect), 308 were screened after removing duplicates; 50 were read in full; and 15 studies were included. The majority reported a reduction in burnout or its components (emotional exhaustion and depersonalization) and/or an improvement in professional quality of life following the intervention. Technology-based interventions (video/smartphone, mHealth, SMS) have demonstrated consistent effects and ease of implementation; however, the heterogeneity of instruments and monitoring duration limits generalizability. Conclusion: The review indicates that digital and smartphone-based relaxation interventions effectively reduce burnout levels among nurses, particularly in high-stress environments such as operating rooms. These methods are flexible, accessible, and cost-effective, making them a promising complementary strategy for burnout management. However, differences in study design, instruments, and intervention duration suggest a need for more standardized and longitudinal research to ensure sustainable and generalizable outcomes.

Abstract

Introduction: Maintaining the functional capacity of older adults is essential to support independence, particularly in rural areas. Understanding the factors contributing to the reduced ability to perform activities of daily living (ADLs) and instrumental activities of daily living (IADLs) is essential for developing effective gerontological nursing strategies. This study identified the predictors of ADL and IADL impairment among older adults residing in rural communities. Methods: This cross-sectional study involved 318 older adults selected through simple random sampling. The Barthel Index and Lawton IADL Scale were used to measure ADL and IADL abilities. The Geriatric Depression Scale was used to assess depressive symptoms, whereas difficulties in vision, hearing, and communication were evaluated using the Washington Group Short Set. Chi-square tests and logistic regression were used to examine the associations and predictors. Results: ADL and IADL disabilities were identified in 4.4% and 10.1% of the respondents, respectively. Communication difficulties were a strong predictor of both ADL and IADL disabilities (AOR = 13.72; 95% CI = 3.26–57.76, p < 0.001 and AOR = 7.07; 95% CI = 2.34–21.34, p < 0.001). Depressive symptoms increased the risk of ADL disability (AOR = 4.17; 95% CI = 1.24–14.05, p = 0.021), and adults aged ≥ 75 years showed higher odds of IADL impairment (AOR = 4.39; 95% CI = 1.89–10.16, p < 0.001). Conclusions: Challenges in communication and depressive affect functional capacity among rural older adults. Maintaining independence and improving daily functioning by implementing early identification and intervention targeting communication and mental health are essential.

Abstract

Peningkatan derajat kesehatan masyarakat menjadi bagian integral dari upaya pembangunan nasional, sebagaimana tertuang dalam Sustainable Development Goals (SDGs), khususnya pada tujuan ketiga yaitu Good Health and Well-Being. Banyak tantangan yang dihadapi salah satunya adalah rendahnya kesadaran Masyarakat dalam melakukan pemeriksaan berkala mengenai penyakitnya. Di Banjar Yeh Gangga, Desa Sudimara merupakan daerah dengan penyakit kronis berupa hipertensi dan diabetes yang masih tinggi. Sehingga implementasi layanan kesehatan komplementer berbasis masyarakat masih terbatas, khususnya yang mengintegrasikan skrining kesehatan dan terapi akupuntur secara simultan. Pengabdian Masyarakat ini dilakukan bertujuan untuk memberikan pelayanan kesehatan komplementer melalui pemeriksaan kesehatan umum dan terapi akupuntur. Kegiatan ini diharapkan dapat menjadi model pelayanan kesehatan integratif di masyarakat yang menggabungkan pendekatan medis modern dan pengobatan tradisional untuk mencapai kesehatan holistik. Metode yang digunakan adalah pemeriksaan kesehatan dasar berupa pengukurun tekanan darah, pengecekan gula dan asam urat dengan mengintegrasikan terapi akupuntu, serta wawancara. Kegiatan ini diberikan kepada 40 orang lansia yang memenuhi kriteria inklusi melalui accidental sampling. Data didapatkan melalui wawancara dan pemeriksaan kesehatan sederhana. Secara umum Sebagian besar pesert a memiliki tekanan darah, kadar gula darah, dan asam urat normal. Selanjutnya seluruh peserta diberikan terapi akupuntur sebagai promotive, preventif, dan kuratif. Mayoritas peserta (80%) merasakan manfaat berupa  neri otot dan sakit kepala berkurang serta tubuh lebih rileks, sehingga kegiatan ini efektif dalam mendukung kualitas hidup lansia.    Kata kunci : Terapi akupuntur, pelayanan kesehatan komplementer, pemeriksaan kesehatan

Abstract

Hypertension remains a major public health issue in Indonesia. The 2018 Riskesdas reported that 34% of Indonesians suffer from hypertension, with 13.2% occurring in young people. Both genetic factors and unhealthy lifestyles are significant risk factors for hypertension among adolescents. These data indicate that adolescents are at high risk of developing hypertension. Hypertension is a serious condition that requires long-term care due to complications such as stroke, kidney failure, and heart disease. Providing education on hypertension and identifying its risk factors are among the key strategies for prevention in adolescents. This Community Service Program (PKM) aimed to provide information about hypertension and identify risk factors among adolescents. The activity was carried out with 50 students from SMA Negeri 1 Abiansemal, Badung Regency, Bali. The method used was PARE (Preparation, Action, Reflection, and Evaluation). The results of the program showed an increase in adolescents’ knowledge about hypertension after receiving education, with the Wilcoxon Signed Rank Test indicating a z-value of -6.122 and a p-value < 0.001. Identification of risk factors revealed that most adolescents practiced unhealthy lifestyles such as low physical activity, frequent consumption of fast food and fatty foods, insufficient intake of vegetables, caffeine consumption, and smoking. Although only a small proportion of adolescents had a family history of hypertension as the main risk factor, prevention efforts are still necessary through lifestyle modification

Abstract

ABSTRAKHipertensi semakin sering terjadi pada masyarakat di Indonesia dan dianggap sebagai salah satu penyebab utama morbiditas dan mortalitas. Meskipun upaya pengelolaan hipertensi dengan pendekatan farmakologi dan nonfarmakologi termasuk pendekatan pelayanan kesehatan tradisional telah dilakukan, namun prevalensi hipertensi dan kematian akibat hipertensi masih tetap tinggi. Tujuan: Penelitian ini bertujuan untuk mengidentifikasi perawatan mandiri hipertensi dan pemanfaatan pelayanan kesehatan tradisional pada pasien hipertensi di Denpasar. Metode: Penelitian ini merupakan penelitian deskriptif dengan pendekatan cross sectional. Responden adalah 100 pasien yang direkrut melalui consecutive sampling di 4 Puskesmas di Denpasar. Kuesioner yang divalidasi tentang manajemen diri dari diet, aktivitas fisik, manajemen stres, konsumsi alkohol, merokok, kepatuhan pengobatan dan penggunaan layanan kesehatan tradisional, digunakan untuk mengumpulkan data. Data dianalis secara univariate mengunakan SPSS versi 20.0. Hasil: Hasil menunjukkan bahwa usia rata-rata responden adalah 55 tahun (berkisar 17-80 tahun), dan 50,4% adalah perempuan. Dalam perawatan mandiri hipertensi, sebagian besar responden menunjukkan perawatan mandiri yang memadai di 5 domain (72% untuk perawatan mandiri diet, 74% untuk perawatan mandiri stres, 97% membatasi konsumsi alkohol, 95% menghindari merokok dan 89% patuh dengan konsumsi obat). Namun, 63% responden menunjukkan perawatan mandiri yang buruk pada aktivitas fisik. Selain itu, hanya 25% responden yang menggunakan pelayanan kesehatan tradisional untuk mengelola hipertensinya. Kesimpulan: Meskipun perawatan mandiri hipertensi sudah baik, penanganan pada aktivitas fisik dan penggunaan pelayanan kesehatan tradisional perlu tingkatkan. Data were analysed for univariate statistics using SPSS version 20.0. Results: The results showed that the average age of the respondents was 55 years (range 17-80 years), and 50.4% were women. In hypertension self-care, most respondents indicated adequate self-care in 5 domains (72% for diet self-care, 74% for stress self-care, 97% limiting alcohol consumption, 95% avoiding smoking, and 89% adhering to drug consumption). However, 63% of respondents indicated poor self-care in physical activity. In addition, only 25% of respondents used traditional health services to manage their hypertension. Conclusion: Although hypertension self-care is considered adequate, the management of physical activity and the utilization of traditional health services should be improved. Abstract

ABSTRACT Uncontrolled hypertension has been identified as a leading cause of death due to its complications. Identifying effective strategies to assist patients in managing blood pressure remains a significant public health challenge in Indonesia. Although hypertension self-management is recommended to improve blood pressure control, the prevalence of uncontrolled hypertension has been reported to be increasing. Multiple factors influence self-management behaviors among individuals with hypertension. This study aimed to identify the determinants of hypertension self-management. This study employed an analytical design with a cross-sectional approach. A total of 120 elderly individuals with hypertension residing in the working area of South Denpasar III Primary Health Center, Bali Province, were recruited using consecutive sampling during the period of July–October 2025. Data were collected using structured questionnaires. Bivariate analysis was conducted using the Chi-square test, with Fisher’s exact test applied as an alternative when appropriate. Multivariate analysis was performed using logistic regression with the backward likelihood ratio (LR) method to identify determinant factors of hypertension self-management. Data analysis was conducted using SPSS version 20. Ethical approval was obtained from the Ethics Committee of ITEKES Bali.The results showed that three variables were significantly associated with hypertension self-management: knowledge (p 0.001; PR = 4.352; 95% CI: 1.985–9.540), educational level (p = 0.048; PR = 8.444; 95% CI: 1.985–9.540), and family support (p = 0.037; PR = 0.400; 95% CI: 0.180–0.888). In contrast, age (p = 0.359), sex (p = 0.769), occupation (p = 0.939), economic status (p = 0.139), and duration of hypertension (p = 0.396) were not significantly associated with hypertension self-management. The main determinant of hypertension self-management among the elderly was knowledge (p 0.001; adjusted PR = 4.322; 95% CI: 1.941–9.626), indicating that elderly individuals with poor knowledge of hypertension were 4.322 times more likely to exhibit poor hypertension self-management. These findings highlight the need for targeted educational interventins by primary health centers, particularly for elderly individuals with limited knowledge, to enhance effective hypertension self-management. Keywords: Hypertension, Elderly, Self-Management.  ABSTRAK Hipertensi tidak terkontrol dinyatakan sebagai penyebab utama kematian akibat komplikasinya. Menemukan strategi yang tepat dalam membantu penderita mengelola tekanan darah masih menjadi masalah kesehatan yang signifikan di Indonesia. Meskipun self management hipertensi direkomendasikan untuk membantu mengelola tekanan darah namun prevalensi hipertensi tidak terkontrol dilaporkan mengalami peningkatan. Banyak faktor yang mempengaruhi perilaku self management penderita hipertensi. Penelitian ini bertujuan mengidentifikasi determinan faktor self management hipertensi. Metode yang digunakan adalah studi analitik dengan pendekatan cross-sectional. Sampel dalam penelitian sebanyak 120 orang lansia yang menderita hipertensi di wilayah kerja Puskesmas Denpasar Selatan III Provinsi Bali yang diambil dengan consecutive sampling selama periode Juli-Oktober 2025. Pengumpulan data dilakukanmenggunakan kuisioner. Uji bivariat Chi-square dengan uji alternatif Fisher exact test digunakan untuk menganalisis hubungan masing-masing variabel penelitian dan uji multivariat regresi logistic dengan metode backward LRdigunakan untuk menganalisis determinan faktor self management hipertensi. Data dianalisis menggunakan SPSS versi 20. Penelitian ini telah mendapatkan kelaikan etik dari Komisi Etik ITEKES Bali. Hasil penelitian ini menunjukkan ada tiga variable yang berhubungan dengan self management hipertensi yaitu pengetahuan (p=0,001: PR=4,352: CI95%:1,985-9,540), pendidikan (p=0,048: PR:8.444, CI95%:1,985-9,540) dan dukungan keluarga (p=0,037: PR:0,400: CI95%:0,180-0,888) sedangkan usia (p=0,359), jenis kelamin (p=0,769), pekerjaan (p=0,939), status ekonomi (p=0,139), dan lama menderita hipertensi (p=0,396) tidak memiliki hubungan dengan self management hipertensi. Determinan faktor self management hipertensi pada lansia adalah pengetahuan dengan nilai (p=0,001: APR=4,322: CI95%: 1,941-9,626). Hal ini berarti pengetahuan yang buruk tentang hipertensi berisiko 4,322 kali bagi lansia melakukan self management hipertensi yang buruk. Temuan ini menekankan diperlukan upaya peningkatan pengetahuan oleh puskesmas melalui edukasi khususnya bagi lansia dengan pengetahuan yang buruk agar mampu melakukan self management hipertensi yang baik. Kata Kunci: Hipertensi, Lansia, Self-Management.
