Abstract

Introduction: Maintaining a universal balance is one goal of environmental cleanliness. Environmental cleanliness is difficult to handle in Bali, especially in Tabanan. The government involves the village to solve this problem. The goals are to explore the leading and inhibiting factors influencing people's behavior to maintain environmental health and realize the balance of tri hita karana. Methods: This study employed qualitative and explorative. Ten participants were interviewed using in-depth interviews. Data was validated by source triangulation (community, community leaders, health workers) and analyzed thematically. Results: The themes were determinant factors that lead to community behavior, inhibiting factors of community behavior, and community perceptions in environmental health maintenance. The categories were the leading factors that influence community behavior, community awareness and motivation for the importance of environmental health, the role, support, and outreach of the village and community leaders, and the availability of facilities in the form of waste disposal sites (TPS). The inhibiting factors were that many people had vacant land to dispose of garbage, access to information media needed to be improved, and facilities and infrastructure required to be added. The public's perception of environmental cleanliness associated with the tri hita karana concept was good and supported this effort.Conclusion: Several factors influence people's environmental behavior toward tri hita karana. These factors include leading, inhibiting, and influencing people's perceptions of environmental cleanliness when associated with tri hita karana. When connected with efforts to tri hita karana, keeping the environment clean is expected to help create harmonious human relations with nature, which can directly or indirectly impact human-to-human relations and human relations with God.

Abstract

Introduction: Little do we understand about factors associated with functional disability in instrumental activities of daily living among the elderly living at home. This study aimed to explore the prevalence and determinant factors of functional disability in instrumental activities of daily living (IADL) among the elderly living in a rural area in Bali. Method: Using a cross-sectional design, 1,053 elderly aged 60 years and above living in a rural area in Bali were recruited. A structured interview by trained data collectors was conducted. Questionnaires used were the Lawton IADL Scale, the 5-item of Geriatric Depression Scale, and the adoption of questionnaires for vision, hearing, and communication problems from the Washington Group Short Set of Questions on Disability. A logistic regression model was applied to explore determinant factors. Results: Findings indicated that 26.1% of participants aged 75+ and 52.3% were female. Eight percent (8%) experienced functional disability in IADL, 1.9% lived alone, 11.8% indicated depression, 5.1%, 4.7%, and 1.6% of them had vision loss, hearing impairment, and communication problems, respectively. The strongest determinant factor for functional disability in IADL was depression (OR 7.869; 95% CI 4.657-13.296), followed by age (OR 4.602; 95% CI 2.764-7.663), and hearing impairment (OR 2.903; 95% CI 1.190-7.083). Conclusions: Depression is the strongest determinant factor for functional disability in IADL. Health professionals in rural areas should actively screen for depression to increase the ability of the elderly to fulfill their IADLs. Keywords: aged, depression, epidemiology, Indonesia

Abstract

ABSTRAK Lansia merupakan populasi rentan dan memiliki banyak masalah Kesehatan yang kompleks karena proses degeneratif. Manajemen kasus Kesehatan lansia dimasyarakat menjadi sangat penting dilakukan untuk memastikan bahwa lansia mendapatkan pelayanan Kesehatan yang baik. Keterlibatan kader Kesehatan lansia dalam manajemen kasus Kesehatan lansia dimasyarakat menjadi salah satu solusi yang dapat dilakukan untuk mencapai tujuan Kesehatan lansia. Kegiatan pengabdian kepada masyarakat ini bertujuan untuk memberikan pelatihan dan pendampingan kepada kader Kesehatan lansia dalam melaksanakan manajemen kasus Kesehatan lansia dimasyarakat. Kegiatan ini dilaksankan di Desa Melinggih, Provinsi Bali. Peserta dalam kegiatan ini adalah kader kesehatn lansia diwilayah desa setempat yang berjumlah 10 orang. Kegiatan PKM dilaksanakan dengan pendekatan PARE (Preparation, Action, Reflection dan Evaluation). Hasil kegiatan ini menemukan adanya peningkatan pengetahuan dan kemampuan kader Kesehatan lansia dalam manajamen kasus Kesehatan lansia dimasyarakat dengan nilai uji statistik Wilcoxon Signed Ranks Test didapatkan nilai Z= -2.675b dengan p value = < 0,001. Kegiatan pelatihan dan pendampingan kepada kader Kesehatan lansia dimasa mendatang harus terus dilakukan untuk membangun meningkatkan pelayanan Kesehatan yang berbasis peran serta masyarakat untuk Kesehatan lansia yang membutuhkan perawatan Kesehatan. Kata Kunci: Lanjut Usia, Gerontik, Kader, Manajemen Kasus, Perawatan Jangka Panjang  ABSTRACT Elderly people are a vulnerable population and have many complex health problems due to the degenerative process. Health case management for the elderly in the community is very important to ensure that the elderly receive good health services. The involvement of elderly health cadres in the management of elderly health cases in the community is one solution that can be done to achieve elderly health goals. This community service activity aims to provide training and assistance to elderly health cadres in implementing elderly health case management in the community. This activity was carried out in Melinggih Village, Bali Province. Participants in this activity were elderly health cadres in the local village area totally 10 people. PKM activities were carried out using the PARE approach (Preparation, Action, Reflection and Evaluation). The results of this activity found an increase in the knowledge and abilities of elderly health cadres in managing elderly health cases in the community with the Wilcoxon Signed Ranks Test statistical test value obtained Z = -2.675b with p value = <0.001. Training and mentoring activities for elderly health cadres in the future must continue to be carried out to build improvements in health services based on community participation for the health of elderly people who need health care. Keywords: Elderly, Geriatric, Cadres, Case Management, Long-Term Care

Abstract

Background: Non-pharmacological management of hypertension through complementary medicine has now been developed in Indonesia. However, the interest of hypertensive patients in complementary medicine is not optimal because there are many factors that influence the behavior of patients in the use of complementary medicine. Methods: This study employed an analytic study design with a cross-sectional approach. This research was conducted at Puskesmas 1 East Denpasar for 1 month. This study involved 101 hypertensive patients using accidental sampling technique. Data collection was carried out using a questionnaire. Data were analyzed univariately and bivariately using SPSS version 20. Results: This study showed that most hypertensive patients had good knowledge of hypertension treatment with complementary therapies (64.4%). A total of 53.5% of hypertensive patients showed adequate behavior in the treatment of hypertension with complementary therapy. Based on the chi square test, it was found that there was a significant relationship between knowledge and behavior of hypertensive patients towards hypertension treatment with complementary therapy (p-value = 0.04). Conclusion: It is recommended for health workers to increase education and promotion about hypertension treatment with complementary therapy, especially for hypertensive patients with poor knowledge.

Abstract

Hipertensi tidak dikontrol khususnya pada lansia berisiko menyebabkan komplikasi seperti penyakit jantung, stroke dan gagal ginjal. Diet hipertensi dianjurkan bagi penderita untuk membantu mengontrol tekanan darah sebagai bagian dari perawatan jangka panjang. Penelitian ini bertujuan untuk mengidentifikasi determinan faktor yang berhubungan kuat dengan perilaku diet hipertensi pada lansia. Desain penelitian ini adalah cross-sectional. Sampel dalam penelitian sebanyak 162 orang lansia menderita hipertensi di Wilayah Kerja UPTD Puskesmas Sukawati II yang diambil dengan consecutive sampling. Pengumpulan data dilakukan dengan kuisioner. Uji Chi-square digunakan untuk menganalisis hubungan masing-masing variable penelitian dan regresi logistic dengan metode backward LR digunakan untuk menganalisis determinan faktor perilaku diet lansia. Data dianalisis mengunakan SPSS versi 20. Penelitian telah mendapatkan kelaikan etik dari Komisi Etik ITEKES Bali. Determinan faktor perilaku diet hipertensi lansia adalah pendidikan (p=0,002), berarti bahwa lansia yang memiliki riwayat pendidikan rendah berisiko 4,15 kali menunjukkan perilaku diet hipertensi yang buruk dibandingkan dengan lansia dengan riwayat pendidikan tinggi dan pengetahuan (p= <0,001), berarti bahwa lansia yang memiliki pengetahuan tinggi menurunkan risiko 0,18 kali melakukan perilaku diet yang buruk dibandingkan dengan lansia yang memiliki pengetahuan rendah. Riwayat pendidikan dan pengetahuan merupakan determinan faktor dari perilaku diet hipertensi lansia. Diperlukan upaya peningkatan pengetahuan penderita hipertensi melalui edukasi khususnya pada lansia dengan riwayat pendidikan rendah untuk merubah perilaku dietnya.
