Abstract

Background: Dermatomyositis (DM) is a chronic autoimmune disorder characterized by muscle and skin inflammation, a part of the idiopathic inflammatory myositis (IIM). Even though the disease is idiopathic, there are multifactorial factors related to dermatomyositis. This care report aimed to describe a DM case in a male patient to increase the knowledge and management of DM patients. Case description: A 44-year-old Filipino male was referred to the emergency department (ED) of Siloam Hospital Mataram complaining of muscle pain and weakness with skin rashes 3 weeks before admission. Before the skin rash started, he had enlarged femoral lymph nodes in both thighs. The patient was afebrile with normal vital signs and was prescribed ibuprofen and amoxicillin. After that, he experienced skin rashes around his neck and the back of his ears with minimal pruritus.  The symptoms worsened, making him unable to open his mouth and hard to breathe. In the ED, he also threw up dark-colored blood twice. Supporting examination showed elevated transaminase, increased LDH, and creatinine kinase. Biopsy results showed a histologic pattern of dermatomyositis. During hospitalization, he received a high-dose systemic steroid, antibiotic, and symptomatic treatment. He was discharged with a good outcome and planned to continue medical treatment in his country. Conclusion: Dermatomyositis is an idiopathic autoimmune disease involving skin and internal organs. It is a multifactorial disease yet with unclear etiopathogenesis. Specific treatment guidelines for DM are not yet established, but initial systemic corticosteroid and additional steroid-sparring agents may exhibit good outcomes.

Abstract

 Pendahuluan. Kanker payudara pada pria merupakan kondisi yang jarang ditemukan dan sering kali terabaikan, baik dalam hal deteksi maupun pengobatan, terutama di negara berkembang. Fine Needle Aspiration Cytology (FNAC) telah dikenal sebagai teknik diagnostik yang efektif dalam penegakan diagnosis lesi payudara, namun penggunaannya pada pria dengan lesi payudara masih terbatas, dan penelitian mengenai hal ini sangat terbatas, terutama di negara berkembang. Tujuan. Penelitian ini bertujuan untuk melakukan sistematik review terhadap penggunaan FNAC dalam diagnosis lesi payudara pria di negara berkembang. Metode. Proses seleksi studi dilakukan melalui pencarian artikel di Google Scholar, PubMed, dan database terkait lainnya, menghasilkan 7.220 artikel yang disaring menjadi 6 artikel yang memenuhi kriteria inklusi Hasil. Hasil review menunjukkan bahwa FNAC memiliki sensitivitas dan spesifisitas yang tinggi dalam mendeteksi kanker payudara pada pria, meskipun terdapat tantangan terkait keterbatasan fasilitas diagnostik dan rendahnya kesadaran akan kanker payudara pria. Penelitian ini juga menemukan bahwa kebanyakan studi dilakukan di negara berkembang dengan sistem kesehatan yang terbatas, yang mempengaruhi hasil dan aplikabilitas FNAC di wilayah tersebut. Kesimpulan. Studi lebih lanjut diperlukan untuk meningkatkan akses terhadap teknik diagnostik ini dan mengembangkan pedoman yang lebih efektif dalam penanganan kanker payudara pada pria di negara berkembang.

Abstract

Introduction: Intrapulmonary teratomas (IPT) are exceptionally rare, with only a handful of cases reported worldwide. The disease may lead to severe outcomes, such as massive hemoptysis, airway compression, and malignant transformation. Several international case reports on IPT focus on the diagnosis and surgical treatment approach. In Indonesia, only one case of intrapulmonary teratoma has been documented and analyzed for its radiological characteristics and anatomical pathology. Here, we present a case of mature IPT treated with surgery and adjuvant chemotherapy. To the best of our knowledge, this case is the second to be reported in Indonesia.Case Presentation: A 28-year-old man presented with a complaint of hemoptysis accompanied by clumps of hair (trichoptysis). Physical examination revealed no abnormalities. Chest X-ray and computed tomography (CT) thorax with contrast showed a right lung mass. The patient underwent lobectomy excision with the postoperative CT scan showing a residual mass. Furthermore, pathological findings confirmed a mature teratoma. Patients were then treated with adjuvant chemotherapy and showed clinical improvement.Conclusions: This case emphasizes the importance of including IPT in the differential diagnosis of hemoptysis, especially when trichoptysis–a rare but distinct symptom–is present. Although imaging tests like contrast-enhanced CT scans are crucial for identifying intrapulmonary masses, histological analysis is necessary for a conclusive diagnosis. The mainstay of treatment persists in surgical removal, and adjuvant chemotherapy may help control any remaining illness. This case emphasizes the importance of early detection and a multidisciplinary approach to enhance patient outcomes because it is one of the few documented IPT cases in Indonesia.

Abstract

Introduction: Adrenocortical carcinoma (ACC) is a malignant tumor that develops in the adrenal cortex. This case involves a rare, aggressive disease that demands specialized knowledge and optimal treatment for a favorable prognosis. This study aims to present the case of a woman with a right-sided ACC extending into the inferior vena cava (IVC), managed through radical resection and IVC repair.Case Presentation: A 50-year-old female presented with a complaint of a lump in the upper right abdomen, which she noticed three months ago. Abdominal computed tomography (CT) scan with contrast revealed a solid, inhomogeneous mass in the right suprarenal region measuring 12 cm x 11 cm. However, three days before the scheduled operation, the patient was brought to the emergency department with complaints of weakness and muscle weakness throughout the body. During the surgery, an adrenalectomy on the right side revealed a tumor infiltrating the IVC, causing venous rupture. The histological examination of the right adrenal tissue revealed ACC, infiltration of tumor cells beyond the capsule, lymphovascular infiltration, and no evidence of tumor cell infiltration into the kidney tissue. The treatment approach for this ACC case involved a complete surgical resection with open adrenalectomy and vena reconstruction. Postoperative results indicated improved patient condition, including symptom relief and favorable biochemical evaluation after surgery.Conclusion: Our patient had a right-sided ACC with local vascular infiltration into the IVC.

Abstract

Breast cancer is the most prevalent cancer among women in Indonesia, including in Nusa Tenggara Barat (NTB). Breast Cancer 1 (BRCA1) plays an important role in DNA repair and cell cycle regulation. Altered BRCA1 expression has been associated with the biological characteristics of breast cancer, including estrogen receptor (ER) and progesterone receptor (PR) status, both of which are crucial markers in determining molecular subtypes and predicting responses to hormonal therapy. However, previous studies regarding the association between BRCA1 expression and ER/PR status have shown inconsistent results. This study employed a cross-sectional design involving 50 patients who met the inclusion criteria, namely invasive breast carcinoma patients who had not received chemotherapy. BRCA1 expression was assessed using immunohistochemical staining, with a cut-off of BRCA1 positivity defined as >20% nuclear staining. ER/PR status was obtained from patients’ medical records archived in the Anatomical Pathology Laboratory of NTB Provincial Hospital in 2022–2023. Data processing and analysis were performed using the Chi-square test with a significance level of p < 0.05. Breast cancer patients in NTB demonstrated a balanced distribution of BRCA1 expression, with 50% positive and 50% negative. The majority of patients had ER/PR-positive expression (50%). Among patients with negative BRCA1 expression, 32% were classified as ER/PR-positive subtype. There was no significant association between BRCA1 expression and ER/PR status in breast cancer patients in NTB (p = 0.842). There is no significant association between BRCA1 expression and ER/PR status in breast cancer patients in NTB.

Abstract

Breast cancer is the malignancy with the highest incidence among women in West Nusa Tenggara (NTB). The Breast Cancer 1 (BRCA1) gene is a tumor suppressor gene that plays a crucial role in DNA repair and genome stability. A decrease in BRCA1 expression is theoretically associated with more aggressive tumor characteristics, such as a low degree of differentiation (high grade). However, data regarding the relationship between BRCA1 protein expression and the degree of cell differentiation in the patient population in NTB remains limited. This study employed a cross-sectional design with an observational analytic approach on 50 paraffin block samples from Invasive Breast Carcinoma patients at the NTB Provincial General Hospital from 2022-2023. BRCA1 protein expression was assessed using immunohistochemistry with a positive cut-off defined as staining in >20% of tumor cell nuclei. Data on the degree of differentiation (Grade 1, 2, and 3) were obtained from medical records. The relationship between the two variables was analyzed using the Lambda, Pearson’s R, and Spearman Correlation tests with a significance level of p < 0.05. The majority of samples (84%) were grade 3 tumors. The distribution of BRCA1 expression in the total sample was balanced, with 25 cases (50%) showing positive expression and 25 cases (50%) showing negative expression. In the grade 3 subgroup, this distribution remained balanced. Statistical analysis consistently showed no significant relationship between BRCA1 expression and the degree of cell differentiation (p > 0.05).
