Abstract

Introduction: Diabetes mellitus is a chronic metabolic disorder with a high global prevalence and serious complications. The prediabetes stage is a critical target for prevention, often managed with α-glucosidase inhibitors. However, synthetic drugs such as acarbose are frequently associated with gastrointestinal side effects, highlighting the need for safer alternatives. Endophytic microbes from medicinal plants, including areca nut (Areca catechu), have the potential to produce bioactive compounds with antioxidant and antidiabetic activities. Objective: This study aimed to explore endophytic bacteria from fresh areca nut seeds as a source of metabolites with antioxidant and α-glucosidase inhibitory activity. Methods: Fresh areca nut seeds were sterilized using the surface sterilization method, followed by endophytic bacterial isolation and Gram staining. Cultures were fermented in Nutrient Broth, and supernatants were obtained by centrifugation. Antioxidant activity was assessed using the DPPH method, while α-glucosidase inhibition was tested in vitro with p-nitrophenyl α-D-glucopyranoside as substrate. Acarbose served as the positive control. Results: Seven Gram-negative, rod-shaped endophytic isolates (BP1-BP7) were obtained. Isolate BP1 exhibited the highest antioxidant activity with 45.7% inhibition. Furthermore, the BP1 supernatant demonstrated potent α-glucosidase inhibitory activity up to 93,6%, comparable to that of acarbose (91%). Conclusion: Endophytic bacteria from areca nut seeds possess dual bioactivity as antioxidants and α-glucosidase inhibitors, suggesting their potential as sustainable alternative antidiabetic agents worthy of further investigation.

Abstract

Hypertension is a common health problem among the elderly and carries a risk of increasing cardiovascular complications. Non-pharmacological efforts such as laughter therapy, a low-salt diet, exercise, and physical activity are important strategies in controlling blood pressure. This study aims to analyze the effectiveness of laughter therapy, a low-salt diet, exercise, and physical activity on reducing blood pressure in elderly patients at the Sei Kepayang Timur Community Health Center in Asahan Regency. The study used a quasi-experimental design with a pre-test and post-test control group approach. The number of respondents was 60 elderly people who were divided equally into an intervention group of 30 people and a control group of 30 people. Data were analyzed using comparative statistical tests (paired t-test and chi-square test) with a significance level of p < 0.05. The results showed that in the intervention group, there was a decrease in mean systolic blood pressure from 150.2 mmHg to 135.4 mmHg, with a difference of 14.8 mmHg (p = 0.000), and a decrease in diastolic blood pressure from 93.2 mmHg to 84.4 mmHg, with a difference of 8.8 mmHg (p= 0.000). Other interventions, including a low-fat diet, exercise, and physical activity, were also shown to be effective in reducing both systolic and diastolic blood pressure. In conclusion, laughter therapy, a low-salt diet, exercise, and physical activity are effective in reducing blood pressure in the elderly. Based on these study results, the elderly are advised to regularly engage in laughter therapy, maintain a low-salt diet, and increase physical activity and exercise. Health institutions need to integrate this non-pharmacological intervention program into promotive and preventive services. Future research is expected to examine long-term effectiveness and expand the sample size.

Abstract

Background: Street foods, particularly fried snacks, are highly popular among university students due to their convenience and affordability. However, the safety of such foods is a public health concern, as they are susceptible to contamination by coliform bacteria, which can cause gastrointestinal illnesses such as diarrhea. This study aimed to analyze the total coliform bacteria in fried snacks sold around the Universitas Prima Indonesia campus area in Medan as an indicator of food safety. Methods: This descriptive laboratory study analyzed fried snack samples of two types (rissole and filled tofu) collected from six street vendors on Jalan Ayahanda, Medan. Samples were obtained using a simple random sampling technique, resulting in a total of 12 samples. The presence of coliform bacteria was determined using the Most Probable Number (MPN) method with a 3-3-3 tube series, which included a presumptive test using Lactose Broth (LB) medium. Data were analyzed descriptively by comparing results with the standard set by the Indonesian Ministry of Health Regulation No. 2 of 2023 (maximum permissible limit <3.6 MPN/gram). Results: All 12 samples tested negative in the presumptive test, indicated by the absence of gas bubbles in the Durham tubes after 48 hours of incubation. The MPN value for all samples was <3 MPN/gram, confirming they met the required safety standard (<3.6 MPN/gram). Conclusion: Fried snacks sold around the Universitas Prima Indonesia campus were free from coliform bacterial contamination and are considered safe for consumption based on current regulations. Factors such as the high-temperature deep-frying process (>150°C) and adequately maintained vendor sanitation practices likely contributed to the absence of contamination.
