Abstract

Type 2 diabetes mellitus (T2DM) is a chronic disease with high prevalence and contributes significantly to the burden of inpatient care in Indonesia. The severity of T2DM-related hospitalization as classified under the INA-CBGs system, is an important indicator for assessing disparities in access to care and the health system’s response to vulnerable populations. This study aims to analyze the relationship between socio-demographic characteristics and the severity of T2DM-related hospitalization visits based on 2023 Social Security Administering Body for Health (BPJS Health) data. This study employed a cross-sectional design using secondary data from the 2023 BPJS Health Sample Data. The unit of analysis was hospitalization visits. The study included inpatient hospitalization visits with a diagnosis of T2DM (ICD-10: E11). Univariate and bivariate methods were conducted, including chi-square tests and ordinal logistic regression was estimated using cluster-robust standard errors. Hospitalization severity was categorized based on the INACBGs classification (mild, moderate, severe). Among 431 T2DM-related hospitalization visits, age >45 years (OR = 2.17; p = 0.035), divorced status (OR = 2.33; p = 0.021), and lower middle socioeconomic status (OR = 1.54; p = 0.046) were significantly associated with higher INA-CBGs hospitalization severity. Gender, inpatient care class, and type of healthcare facility ownership were not statistically significantly associated with hospitalization severity. These findings highlight the importance of the health system's response to the social determinants reflected in INA-CBGs–based hospitalization severity among T2DM-related visits. Targeted and multisectoral interventions are needed to reduce disparities and improve service quality for socially vulnerable groups. Abstract

Type 2 diabetes mellitus (T2DM) is a common chronic comorbidity that exacerbates the clinical course of COVID-19, leading to higher complication rates and mortality. Patients with T2DM experience impaired immunity, persistent inflammation, and metabolic dysfunction, making them more vulnerable to severe viral infections. Despite extensive global research, evidence from Indonesia remains scarce, highlighting the need for local data to guide public health strategies. This study aimed to analyze the survival of COVID-19 patients with T2DM and identify factors associated with mortality in Padang City. A retrospective cohort study using surveillance data from the Padang City Health Office between March 2020 and March 2021 included 209 randomly selected patients. Survival analysis was conducted using Kaplan–Meier methods, and Cox regression models identified mortality predictors. The study found that 6.7% of patients died, with an incidence rate of 0.0058. T2DM patients had nearly four times the risk of death compared to non-diabetic individuals (HR = 3.9; 95% CI: 1.2–14.2). Other predictors included older age, male sex, multiple comorbidities, symptomatic status, and hospitalization. These findings emphasize the need to prioritize T2DM patients in vaccination, early detection, and integrated diabetes management to strengthen health system resilience.

Abstract

Introduction: The implementation of distance learning due to the COVID-19 pandemic has impacted not only the students but also parents especially mothers. This study was aimed to provide an overview of mothers’ mental health status (stress and anxiety) in Jabodetabek and its association with mother’s education level and occupation status, number of children, and number of children doing distance learning during the COVID-19 pandemic.Method: This was a cross-sectional study using an online questionnaire targeted to married women who have greater than 1 children in elementary school-high school doing distance learning in Jabodetabek. The data was collected in the period of 2-20 December 2020. Instruments used to measure stress and anxiety levels were PSS-10 and GAD-7.Results: There were 311 respondents aged between 27-61 years old. The average respondents had moderate stress and mild anxiety. The risk of severe stress increased significantly in mothers with elementary-middle education level (crude RR 2,1; 95%CI 1,04-4,4) and increased risk of moderate-severe anxiety in mothers with greater than 3 children (crude RR 1,9; 95%CI 1,1-3,1).Conclusion: More than 50% of mothers with children in elementary school-high school in Jabodetabek had moderate stress and 31% showed signs of mild anxiety. The mother’s sociodemographic factor associated with the risk of severe stress was education level. The factor associated with moderate-severe anxiety risk was the number of children living under one roof.

Abstract

Epithelial ovarian cancer (EOC) is a leading cause of gynecological cancer mortality and is often diagnosed at an advanced stage, resulting in poor survival outcomes. Access to specialized gynecological oncology services has been associated with improved prognosis; however, data on its impact in Indonesia remain limited. This study aimed to evaluate the association between gynecologic oncologist (GO) services and disease-free survival (DFS) in patients with EOC. This retrospective cohort study included 76 patients with EOC who were treated at a national cancer hospital in Jakarta between 2015 and 2018. Patient data were obtained from medical records and followed for three years after the primary treatment. Survival outcomes were analyzed using the Kaplan-Meier curve and Cox proportional hazards model. The overall 3-year DFS rate was 41.6%. Patients treated by GO specialists had a higher DFS rate (54.3%) than those treated in hospitals without GO services (24.5%). Multivariate analysis showed a higher recurrence risk in patients treated without GO services (HR 2.75; 95% CI: 1.32 - 5.74). This study’s findings highlight the potential clinical benefits of facilitating early access to specialized GO care for patients with EOC.
