Abstract

Midwives are health providers influential in helping families to choose appropriate contraception for patients. Midwives must have competence in providing midwifery services in particular IUD insertion. This study’s aim was to evaluate the competency of midwives in the insertion of an intrauterine device Copper T 380A based on their age, education, experience of insertion and duration of practice in Padang City. This study was done in puskesmas and private midwivery practice in Padang from September to December 2014 using cross-sectional method on 24 samples who met the inclusion criteria and not exclusion criteria. We conducted interviews and observed how the respondents perform IUD insertion by using a check- list. From this study, there was no significant association between age (p: 0.540, p> 0.05), education (p: 0.439, p> 0.05), experience of insertion (p: 0.472, p> 0.05) and duration of practice (p: 0.505, p> 0.05) with competency of IUD insertion. Most respondents have a good competence in IUD insertion. Part of the checklist the respondents frequently missed out were conseling to the patients and prevention of infection.Keywords: Midwives Competency, IUD insertion, Copper T-380a

Abstract

There will be multiple organs changes in preeclampsia and eclampsia. One of them is a change in hemostasis system which is platelet activation, extrinsic and intrinsic cascade reaction and increasing of fibrinolytic activation. This is a cross sectional study conducted at Obstetric and Gynecologic Departement of Medical Faculty of Andalas University/ M Djamil Central Hospital in Padang on July 2014 with the number of samples are 44 persons. Samples are divided into 3 groups: Severe preeclampsia, eclampsia, and normal pregnancy. Platelet, PT,APTT, and D-Dimer counting were conducted and statistic analyzed was done with Anova dan Post Hoc Bonferoni. The more severe pregnancy, the lower platelet count and PT, but the dif- ference is not statistically significant between three groups: severe preeclampsia, eclampsia, and normal pregnancy (p < 0,05). Mean of APTT and D-Dimer is statistically significant due to condition of pregnancy. Post Hoc Bonferroni analysis showed a significant difference of APTT mean in the eclampsia, severe pre-eclampsia, and normal pregnancy (p <0,05 ). D-Dimer Mean shows a significant difference between normal pregnancy, severe preeclampsia, and eclampsia (p <0,05).Keywords: Pregnancy condition, severe preeclampsia, eclampsia, normal pregnancy, platelet, PT, APTT, and D-Dimer

Abstract

Pelvic organ prolapse is a condition that affects the quality of women life. Pelvic organ prolapse can be caused by injury until the birth process, the aging process, the composition of the tissue in a woman, a chronic cough, or often do heavy work. Early detection of prolapse associated with Prognosis of anatomy and functional pelvic organs recovery. So we need training and learning more about Pelvic Organ Prolapse Quantification (POPQ) are clearly. The study was conducted by the method of case control study in the department of OB polyclinic of Dr. M. Djamil Padang Hospital from September 2013 until the total sample of 98 patients with 49 control groups and 49 in the case group. Analyzes were connected to assess the association of age, parity, occupation and body mass index with the incidence of pelvic organs prolapse based on POPQ. Score data are presented in tabular form. Data were tested by t-test and chi square test. If p <0.05 indicates significant results. There is a significant relationship between age and the incidence of pelvic organ prolapse (p <0.05) and OR 27,871. there is a significant correlation between parity and the incidence of pelvic organ prolapse (p<0.05) and OR 52,970. From the statistical analysis of the work, it cannot be tested statistically. From the body mass index, there is no significant relationship to the occurrence of pelvic organ prolapse (P> 0.05) and OR 1:00.Keywords: age, parity, occupation, body mass index, pelvic organs prolapse

Abstract

Preeclampsia is a major obstruction complication with increasing incidence and is associated with maternal morbidity and mortality. One theory regarding preeclampsia is the development of oxidative stress due to the benefits of pro-oxidant and anti-oxidant functions that consequently results in free radicals, active oxygen, or reactive nitrogen. The deacrease of Zinc as a cofactor of anti-oxidant enzymes is reported to be associated with an increased risk of preeclampsia. Cross sectional comparative study was conducted in Dr. M. DjamilPadangHospital, SolokDistrict Hospital, PainanDistrict Hospital, BatusangkarDistrict Hospital, and Biomedicallaboratoryof Medical Faculty of Andalas University from September 2014 to February 2015. There were 40 samples with pregnancy beyond 20 weeks which were then divided into two groups; severe preeclampsia and normal pregnancy. Serum zinc wasexamined in both groups. Serum zinc in severe preeclampsia and normal pre-stressed demonstrated a significant difference (p <0.05). The mean concentration of serum zinc in severe preeclampsia and in normal pregnancy were 0.45 ± 0.09 μg / ml and 0.78 ± 0.55 μg / ml with p = 0.02. This difference is statistically significant with p <0.005. There was a significant difference between serum zinc concentration in normal pregnant women and that in severe preeclamptic women.Keywords: severe preeclampsia, normal pregnancy, zinc serum

Abstract

Until recently, the exact etiology and pathophysiology of preeclampsia have not been discovered yet, but based on the clinical symptoms and the defect that appeared, researchers submitted some ways as assumption or as early detection of preeclampsia and eclampsia. Some researchers have suggested the examination of calcium / creatinine excretion ratio in urine from preeclampsia patients as the result of kidney's function changes. This study has been performed with a pre and post test group design experimental method at Obstetric and Gynecology Polyclinic in RS. Dr. M. Djamil Padang and several midwife private practices in Padang since July 2013 until the samples reached the quantity up to 40 samples. Analysis has been done to describe the relationship between calcium intake and blood pressure. Furthermore, we analyzed the differences of systolic - diastolic and MAP before and after calcium intake. Data has been shown on the table and analyzed by Pearson correlation, Wilcoxon test and Paired T test. if p <0.05, it shows a significant result. The mean of systolic after calcium intake was the same for both of the groups (121.5 + 8.02: 121.5 + 6.71). Mean of diastolic after calcium intake in controls group was lower than trials group (75.9 + 4.32: 75.9 + 4.32). Mean of MAP after calcium intake for controls group was lower that trials group (91,088 + 4.47: 91,956 + 6.08). There is an influence of calcium intake on decreasing maternal blood pressure. Abstract

Objective : To determine the effect of giving prophylactic antibiotic ceftriaxone and cefazolin and giving ceftriaxone before and after surgery to the risk of postoperative wound infection in postoperative patients.Method: This study was an experimental study with a post test control group design that looked at the differences in the effect of administration of ceftriaxone, cefazolin, and ceftriaxone before and after surgery on the risk of postoperative wound infection. The population in this study were patients planned for surgery in the Obstetric and Gynelologic Departement of Dr. M. Djamil General Hopital, Padang. The number of samples used by 30 people with a group of 10 people each group. The study began in August until the number of samples was fulfilled. Univariate analysis was used to see the frequency, percentage, mean, and standard deviation. Bivariate analysis using Chi-square test with 95% CI (α <0.05) was used to see differences in the effect of the three antibiotic procedures.Results: There were no cases of postoperative wound infection based on the three procedures used. There was no difference in the effect of prophylactic antibiotics in postoperative infections. Abstract

Background : Overactive bladder (OAB) is a group of urgent symptoms, with or without urgent incontinence. Research shows that some Lactobacillus spp. can be a sign of a women bladder’s good health; and found a decrease number of Lactobacillus spp. in patients with OAB. This study aims to assess the correlation of Lactobacillus spp. colony with OAB using the OABSS score at Pauh Primary Health Center, Padang.Method : This research is an analytic study with a cross sectional comparative design. The research was conducted from December 2019 to February 2020 at Pauh Primary Health Center. All women aged 20-40 years who came to Pauh Primary Health Center during the study period were included in the study. Pregnant women, having pelvic abnormalities and a history of other urinary tract diseases or having a history of hypertension and diabetes mellitus were excluded in the study. Numerical data are presented in the form of central tendency. Bivariate analysis was performed using the t-independent test if the data distribution was normal and the Kolmogorov-Smirnov test if the distribution of the data was not normally distributed.Result: There were 42 samples consisting of 21 OAB respondents and 21 normal respondents. The age of the respondents in the OAB group was 28 ± 6.8 years, while the normal group was 32 ± 7.3 years (p> 0.05). The number of Lactobacillus spp colonies in the normal group was higher than the OAB group, namely 16,389,670 ± 74,380,427.9 CU / ml compared to 15,229,634 ± 67,553,932.9 CU / ml (p> 0.05).Conclusion: There was no correlation between the number of Lactobacillus Spp colonies and the incidence of Overactive Bladder. It is necessary to do further research regarding other risk factors associated with the incidence of OAB and the causes of the decrease number of Lactobacillus spp colonies in OAB patients and the presence of other microorganisms, especially pathogenic microorganisms. Keywords: overactive bladder, Lactobacillus, OABSS score

Abstract

Background: Cancer is the second leading cause of death in the world. Cervical cancer is in the 9th position out of 35 types of cancer causing the most deaths in the world. Cervical cancer is the second highest type of cancer after breast cancer in women, which affects more than 1.4 million women worldwide. Every year more than 460,000 cervical cancer incidents occur and about 231,000 women die from the disease.Objectives: This study aims to determine the relationship between husband's support, level of knowledge and motivation, with the behavior of IVA examination in fertile aged women in the city of Padang.Methods: This research is an analytic observational study with  cross-sectional comparative study approach that looks at the relationship between knowledge, motivation, and husband's support with IVA examination behavior in women of childbearing age.Results: The percentage who did not perform IVA examination was higher among respondents with low motivation compared to those with high motivation, namely 54.7% versus 45%. Statistically this difference was not significant (p> 0.05). The number of respondents who did not perform IVA examinations was higher for respondents with less husband's support, namely 32 respondents (42.1%) and more than half of respondents who did IVA examinations, namely 44 respondents (57.9%) who received less support from their husbands. Statistically this difference was significant (p <0.05).Conclusion: There is a relationship between the level of knowledge and the behavior of the IVA examination, there is no relationship between motivation and the behavior of the IVA examination and there is a relationship between the level of knowledge and the behavior of the IVA examination.Keywords: IVA, women of childbearing age, education

Abstract

Background: Preeclampsia is a multi-organ disorder that occurs in pregnant women with high blood pressure which is still difficult to predict. Until now, many theories have put forward the causes of preeclampsia, one of which is much debated is the lack of nutrition in pregnant women which can lead to preeclampsia. Several studies suggest that there is a relationship with the lack of micronutrient intake, especially magnesium, which causes preeclampsia, especially in developing countries. Magnesium deficiency is one possible factor causing severe preeclampsia. Magnesium plays a role in normal neuromuscular function, regulation of calcium, potassium and transport of sodium in the membrane as a regulation of the activation of ATP. Magnesium also plays a role in blood pressure regulation. Magnesium deficiency can lead to endothelial malfunction, hypertension and failure of the cardiovascular system. The role of magnesium is in enzyme reactions that bind to anaerobic glucose metabolism, the krebs cycle, fatty acid oxidation and reactions between coenzyme and fatty acids. Magnesium deficiency will result in changes to the nerves, muscles and blood vessels.Methods: Analytical observational study with cross sectional design in 17 women with preeclampsia and 17 normal pregnant women who met the inclusion and exclusion criteria. Research subjects were collected at Dr M Djamil Padang General Hospital, November 2019 to January 2020. Magnesium levels were checked by enzymatic methods. The difference in the mean magnesium preeclampsia and normal pregnancy was analyzed using independent t test.Results: Mean magnesium in preeclampsia was lower than normal pregnancy (1.61 ± 0.59aµg / ml vs 2.20 ± 0.18, p = 0.001).Conclusion: Mean magnesium level in preeclampsia was significantly lower than normal pregnancy..Keywords: Average levels of magnesium, magnesium, preeclampsia, normal pregnancy

Abstract

Introduction : Albumin main function is maintaining the plasma colloid oncotic pressure. Preeclampsia cause a decrease in serum albumin in the pregnant woman. Low albumin levels increase mortality, longer wound healing and longer hospital stays. The intake of oral albumin is expected to be effective in increasing the level of albumin in the blood.Objective : To determine the effect of freeze dryer method intake compared to liquid snakehead fish extract in patients with preeclampsia and hypoalbuminemia.Material and methods : Using pre and post-test control group design.The total sample was 38 patients with severe preeclampsia with hypoalbuminemia, divided into control and intervention groups. Control was given the intake of liquid snakehead fish extract, in the intervention was given the Freeze dryer method. The research data were analyzed using the Mann Whitney test method and the Wilcoxon test.Results : In the intervention group the mean pretest albumin level was 2.66 ± 0.17 gr / dl and on the 4th day it was 3.13 ± 0.16 gr / dl (p = 0.000). In the control group the mean pretest albumin level was 2.72 ± 0.15 gr / dl and on the 4th day it became 3.06 ± 0.14 gr / dl (p = 0.000). On the 4th day after treatment, the increase in serum albumin levels in the intervention group was 0.47 ± 0.19 gr / dl; whereas in the control group was 0.34 ± 0.10 gr / dl with a value of p = 0.031 (p <0.05).Conclusion : The freeze dryer method (freeze dryer method ) for the Cork Fish Extract resulted in a higher increase in albumin levels than the liquid snakehead fish extract.Keywords: Preeclampsia, Hypoalbuminemia, freeze dryer method of Snakehead Fish Extract
