Abstract

Drug services in Public Healthcare are an important component whose availability influences the success of health efforts. District / City Health Office is a regional government work unit in the health sector, one of the tasks and authorities of which is to provide and manage public medicines for the district/city area. Effective pharmaceutical management in health service facilities is very important for patient welfare so risks must be identified and controlled. One of the efforts made by the City Health Office is to procure drugs using other methods that are legal according to government procurement of goods/services besides e-purchasing, which is called non-e-purchasing of drugs. Risks in the pharmaceutical supply chain are related to product discontinuity, product shortages, poor performance, patient safety, expense errors, and technological errors, all of which can result in system disruption. This study aims to determine the suitability of the plan with the realization of e- purchasing of drugs, identify problems that hamper the implementation of e-purchasing of drugs, determine the impact of obstacles on procurement, cost efficiency of drug procurement and determine the potential savings of drug procurement by e-purchasing for the period 2015 to 2019 by using the FMEA method, the e-purchasing data was collected at the Wonokromo Public Health Center in Surabaya. Study Failure Mode Effect (FMEA) in the procurement of drugs and UHC health supplies in Surabaya Wonokromo Health Center for the period of 2015-2019, it can be concluded that the absorption of capitation funds is more considered using the availability variable compared to the uptake of funds based on the rupiah value because the availability of more guarantees the need for treatment in the sustainable health centre. In this case, the perception of procurement actors in the analysis of UHC procurement process obstacles there are 41 obstacles, the role of E-purchasing is a very helpful system even though the implementation has not been perfect.

Abstract

Corona Virus Disease2019 (COVID-19) is a disease that shocked the world at the end of 2019. Based on data, positive cases of Covid-19 in Indonesia on July 29, 2021 reached 3,331,206 people, with 3,240,654 Covid-19 patients recovering and 90,552 Covid-19 patients dying. Meanwhile in the city of Surabaya there were 52,782 confirmed cases with 50,894 patients recovered and 1,888 patients died. The high death rate shows that this virus is dangerous, but the cure rate is also high. A survival analysis is needed to determine the patient's survival time (cured) for Covid-19. This research was conducted non-reactively using secondary data in the form of medical records of Covid-19 patients at the Surabaya A. Yani Islamic Hospital in January-July 2021. The results of the study using the Kaplan-Meier method found that the survival time based on age was 12,423 days, the survival time based on gender was 12,078 days, the survival time based on Early Symptoms was 11,461 days and the survival time based on SpO2 capacity was 16,787 days. The conclusions of this study indicate that, age affects the survival time of respondents where the 27-36 year interval has a better survival time than the other respondents' age intervals. The female sex has a better survival time than the male. Initial symptoms determine the severity and duration of recovery for respondents. SpO2 capacity is directly proportional to the degree of patient safety, the higher the percentage of SpO2 capacity, the greater the chance of survival and the lower the percentage of SpO2 capacity, the lower the expectation.

Abstract

The application of tariff applied in handling BPJS patients references the INA- CBGs and the payment model used by BPJS Kesehatan to replace the total bill by the hospital. Hospitals receive payments based on the INA- CBGs rate, which is the average cost spent by a group of diagnoses. It is expected to improve the quality and efficiency of hospitals. The benefit of implementing INA -CBGs in JKN is the tariffs in the form of packages cover all components of hospital costs. Cost efficiency efforts must be made. That is no deficit from the applicable INA-CBGS tariff. Quality and cost control efforts are very important in the implementation of ENT specialist poly services. This study aims to analyze the demographic characteristics of the patient, the patient's diagnosis, the difference in rates between INA CBGS payments and RSIS rates, the composition of financing and the Unit Cost of ENT Polyclinics. The research type is quantitative observational with cross-sectional design. The research location is at the Surabaya Islamic Hospital with BPJS TXT data, processing in January-December 2019. The results showed, the demographic characteristics of most patients were > 50 years old, and most of them were diagnosed with minor chronic diseases. The difference between Ina-CBGS payments and RSIS rates is Rp. 60,174 which means that each patient contributes a profit of Rp. 60,174. The composition of the financing for implementation of the ENT Polyclinic is the cost of consulting services. The unit cost of ENT Polyclinic patients is Rp. 132,774 per patient.

Abstract

Nursing staff is a priority in providing services to patients. If the number of nurses and patients is not balanced, this will result in workload problems and the lack of staff will result in less than optimal service to patients. In this study, the method used to calculate staffing needs was WISN (Workload Indicators of Staffing Need) for Mekkah inpatient nurses (adult inpatients). The WISN method has five stages: finding available work time, finding work units and human resources categories, making work load standards, making allowance standards, and calculating labor requirements for each work unit. The aim is to analyze the need for nursing staff based on the workload in the Mekkah inpatient unit (adult inpatient unit). The time motion study method is to be able to understand more deeply the phenomena that occur and are felt by the informants through observations and interviews. The results of research on staff requirements using the WISN method were 35 nurses with real conditions of 20 nurses so there was a shortage of 10 nurses.

Abstract

One of the serious challenges currently faced by XYZ Hospital in Sidoarjo, a class C hospital, is the employee turnover rate which continues to increase significantly. The increasing number of employee transfers and resignations has become a critical internal concern, as the turnover rate has exceeded the hospital management’s established threshold of 5%. field conditions indicate that this threshold has been surpassed over the past several periods. The objective of the study was to analyze the effect of Quality of Work Life (QWL) on nurses' intention to leave, in order to understand how various aspects of the work environment influence nurses’ tendency to resign, and to provide a basis for developing effective nurse retention strategies within the hospital. This observational study employed a cross-sectional design, in which researchers conducted observations without providing any intervention on the variables under study. A total of 182 participants were selected from a population of 402 employees using the Slovin formula, followed by simple random sampling. Located at XYZ Hospital in Sidoarjo. Data analysis was conducted using simple linear regression to examine the causal relationship between the independent variable, Quality of Work Life (QWL), and the dependent variable, intention to leave. Quality of Work Life was measured through three dimensions: job satisfaction, organisational culture, and work climate. A relationship was considered statistically significant if the significance value (p-value) was less than 0.05. Based on the results of the study conducted at XYZ Hospital, Quality of Work Life—encompassing organisational culture, work climate, and job satisfaction—was found to be at a moderate level. Similarly, employees’ intention to leave was also categorised as moderate. The regression analysis demonstrated that Quality of Work Life had a significant effect on intention to leave. These findings indicate that higher perceived Quality of Work Life is associated with a lower tendency among employees to leave the organisation.  Keywords: Organizational Culture, Intention To Leave, Quality Work Of Life, Job Satisfaction
