Abstract

Pulmonary tuberculosis is a disease with a high prevalence in Indonesia and in the world, and continues toincrease every year. Global prevalence reached 10 million cases in 2017 with 1.3 million deaths, and inIndonesia reached up to 1,017,290 cases. Pulmonary TB is one of the leading causes of death in the world.The cause of pulmonary TB continues to increase is easy transmission and vulnerable individual and socialfactors. Many problems are caused by pulmonary TB, especially the physical health problems of the patientwhich can cause various negative impacts. So, the treatment regimen is not enough in the prevention of thisdisease, but requires other interventions such as pulmonary rehabilitation programs. This study aims todescribe of the physical health of pulmonary TB patients with a literature review method of relevant studies,referenced from articles published and indexed internationally by Scopus for the period 2014-2019 inEnglish and can be accessed full text, with keywords the physical health status of pulmonary TB, pulmonaryTB rehabilitation, and pulmonary tuberculosis. Method of analysis by selecting articles according toinclusion criteria set by researchers, and obtained 11 articles. The analysis describes that in pulmonary TBpatients there are physical health problems that cannot be ignored like weight loss, pain and discomfort,fatigue and physical weakness, lack of energy and a decrease in the ability of daily activities, rest and sleepdisturbances, and a decrease in the mobility.

Abstract

Background: Human immunodeficiency virus (HIV), acquired immunodeficiency syndrome (AIDS), and tuberculosis are closely related and remain serious problems despite the implementation of various treatment strategies, but cases continue to increase. This research aimed to explore the perceptions and experiences of drug-resistant tuberculosis people with HIV/AIDS who experience loss of follow-up based on life values and stigma. Methods: This qualitative study employed a phenomenological approach and (COREQ) guidelines. Researchers purposively sampled participants with HIV/AIDS and tuberculosis in Lampung Province, Indonesia. Data were collected through interviews and analyzed using Colaizzi’s seven-step thematic analysis. Results: The results of the study described 7 themes related to the perceptions and experiences of drug-resistant tuberculosis HIV/AIDS sufferers who experience loss to follow-up based on life values and stigma, including 1) understanding of HIV/AIDS and tuberculosis; 2) causes of TB and HIV/AIDS clients experiencing loss to follow-up; 3) what happens after experiencing loss to follow-up: economic problems and drug availability; 4) adaptation after experiencing loss to follow-up: comfort; 5) use of herbal medicine as therapy; 6) barriers and support when it comes to control and treatment; 7) hope to recover and regularly taking medication and environmental dilemmas. Conclusion: HIV/AIDS and tuberculosis sufferers who experienced loss to follow-up provide an understanding of experience, including the process of disease transmission and spread as well as antiretroviral drug treatment, anti-tuberculosis drugs, and the side effects they experience. A further problem is the community’s assessment of the condition of the disease. The support and facilities provided affected the issue of needs, both material and psychological.

Abstract

Background: Tuberculosis (TB) remains one of the biggest health problems in society. TB is caused by bacteria (Mycobacterium tuberculosis) and most often attacks the lungs. TB is transmitted through the air, when someone with pulmonary TB coughs, sneezes, or spits. A person only needs to inhale a few germs to become infected. Each year, 10 million people are infected with TB. Although the disease is preventable and curable, 1.5 million people die from TB each year, making it the world's biggest infectious killer. Purpose: To determine the effectiveness of the structured physical exercise program in improving the quality of life of tuberculosis patients. Method: Literature search in several databases Science Direct, Proquest, Scopus, and francis & taylor. The articles used are the results of research published between 2020 - 2024 with the search technique using the PRISMA method. Results: This review highlights the importance of a planned and targeted physiotherapy protocol in supporting the recovery of tuberculosis patients. Protocols that include breathing exercises, muscle strengthening, mobility, and patient education have shown effectiveness in reducing physical symptoms, increasing functional capacity, and improving quality of life. Evidence-based physiotherapy protocols show that a structured, targeted, and patient-tailored approach is very effective in accelerating physical and psychosocial recovery in tuberculosis patients. This systematic review also underlines the need for comprehensive management to ensure long-term recovery and improved quality of life. Conclusion: Evidence-based physiotherapy protocols show that a structured, targeted, and patient-tailored approach is very effective in accelerating physical and psychosocial recovery in tuberculosis patients. Suggestion: Hospital-based or community-based pulmonary rehabilitation programs need to be improved, especially in areas with high tuberculosis prevalence, by conducting professional training for health workers, especially on specific protocols for tuberculosis patients to ensure high quality of care.   Keywords: Quality of Life; Structured Physical Exercise; Tuberculosis Patients.   Pendahuluan: Tuberculosis (TB) masih menjadi salah satu masalah kesehatan terbesar di masyarakat. TB atau TBC disebabkan oleh bakteri (Mycobacterium tuberculosis) dan paling sering menyerang paru-paru. TBC menular melalui udara, ketika penderita TBC paru-paru batuk, bersin, atau meludah. Seseorang hanya perlu menghirup sedikit kuman untuk terinfeksi. Setiap tahunnya, 10 juta orang terjangkit TB. Meskipun penyakit ini dapat dicegah dan disembuhkan, 1.5 juta orang meninggal karena TBC setiap tahunnya, menjadikannya penyakit menular pembunuh terbesar di dunia. Tujuan: Untuk mengetahui efektivitas program latihan fisik terstruktur dalam meningkatkan kualitas hidup pasien dengan tuberkulosis. Metode: Penelusuran literatur pada beberapa database Science Direct, Proquest, Scopus, dan francis & taylor. Artikel yang digunakan adalah hasil penelitian yang terbit antara tahun 2020 – 2024 dengan teknik pencarian menggunakan metode PRISMA. Hasil: Tinjauan ini menyoroti pentingnya protokol fisioterapi yang terencana dan terfokus dalam mendukung pemulihan pasien dengan tuberkulosis. Protokol yang mencakup latihan pernapasan, penguatan otot, mobilitas, dan edukasi pasien menunjukkan efektivitas dalam mengurangi gejala fisik, meningkatkan kapasitas fungsional dan meningkatkan kualitas hidup. Protokol fisioterapi berbasis bukti menunjukkan bahwa pendekatan yang terstruktur, terarah, dan disesuaikan dengan kebutuhan pasien sangat efektif dalam mempercepat pemulihan fisik dan psikososial pasien tuberkulosis. Systematic review ini juga menggaris bawahi perlunya pengelolaan yang komprehensif untuk memastikan pemulihan jangka panjang dan peningkatan kualitas hidup. Simpulan: Protokol fisioterapi berbasis bukti menunjukkan bahwa pendekatan yang terstruktur, terarah, dan disesuaikan dengan kebutuhan pasien sangat efektif dalam mempercepat pemulihan fisik dan psikososial pasien tuberkulosis. Saran: Program rehabilitasi pulmoner berbasis rumah sakit atau komunitas harus ditingkatkan, khususnya di daerah dengan prevalensi tinggi tuberculosis, melakukan pelatihan profesional kesehatan terhadap tenaga kesehatan terlebih tentang protokol khusus untuk pasien dengan tuberkulosis guna memastikan pelayanan berkualitas tinggi.   Kata Kunci: Kualitas Hidup; Latihan Fisik Terstruktur; Pasien Tuberculosis.

Abstract

The increasing incidence of falls in the elderly has become a global health issue. The development of wearable electronic device technology is able to monitor daily activities, measure step counts, physical activity levels, and even sleep patterns of the elderly, while providing early warnings when changes occur that may increase the risk of falls. The aim is to see the effectiveness of using wearable electronic devices in preventing the risk of falls in the elderly in the community. PICO in this study is Population: Community-dwelling elderly, Intervention: wearable electronic device, Comparisons: none, and Outcome: fall prevention. Researchers used 3 search data sources, namely Embase, Pubmed, and Science Direct. The inclusion and exclusion criteria of this study are literature published in electronic data sources from 2018-2023, is a type of quantitative research, the elderly population in the community, English language and available in full text. This literature review includes ten articles that discuss the utilization of wearable electronic device technology, the types of wearable devices used include Inertial Measurement Units (IMU), biosensors, and accelerometers to evaluate the mobility, gait, and balance of the elderly. This literature review shows that wearable electronic device technology can effectively detect falls and motorize the incidence of falls in the elderly

Abstract

Aims: This study aimed to evaluate the effect of the Caregiver Care for TB Program on improving the knowledge, attitudes, and caregiving skills of mothers as primary caregivers of children diagnosed with tuberculosis in Depok, Indonesia. Methods: A quasi-experimental study was conducted with 80 mothers divided equally into intervention and control groups. The program included four structured educational sessions delivered in small support groups at a health center over four consecutive weeks. Data were collected at three points: before the intervention (pre-test), immediately after the completion of the sessions (post-test 1), and 10 days after the intervention (post-test 2). Knowledge, attitudes, and skills were measured using the Knowledge, Attitude, and Practice (KAP) questionnaire for Tuberculosis care. Results: Significant improvements were observed in the intervention group compared to the control group in knowledge (p=0.000) and attitudes (p=0.000) and skills (p=0.000), as well as improvements in knowledge, attitude, and skills 10 days later (p < 0.000). Conclusion: The Caregiver Care for TB Program effectively enhanced caregivers' knowledge, attitudes, and caregiving skills in preventing TB transmission among children. This program should be implemented as part of TB control strategies in high-burden areas

Abstract

Background: Tuberculosis not only affects the physical condition of sufferers but also causes psychological problems such as anxiety and depression in tuberculosis patients. These psychological disorders will have an impact on treatment that is still undergoing treatment. The efforts that can be given are non-pharmacological therapy, namely Cognitive Behavioral Therapy (CBT), this therapy is carried out by changing the wrong way of thinking about the disease experienced by the patient. Purpose: To determine the effectiveness of Cognitive Behavioral Therapy (CBT) in reducing psychological stress and increasing medication compliance in pulmonary tuberculosis patients. Method: A systematic review with article selection guided by the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines. The systematic review process begins with the formulation of clinical questions relevant to the topic. Before this, the authors establish PICOS criteria, which stand for: P (problem, patient, or population), I (intervention, prognostic factor, or exposure), C (comparison or control), O (outcome), and S (study design). For this article, P: Patients diagnosed with pulmonary tuberculosis who experience psychological stress, I: Cognitive Behavioral Therapy (CBT), C: Standard care or conventional treatment without CBT, O: Reduction in psychological stress (levels of stress, anxiety, or depression), S: Randomized Controlled Trials. The article search was conducted systematically using the keywords: "Tuberculosis" AND "Cognitive Behavioral Therapy” AND “Cognitive Therapy” AND “Medication Adherence” AND “Medication Compliance” AND “Psychological“AND “Depression” AND “Anxiety”. Results: This study shows that Cognitive Behavioral Therapy (CBT) is effective in reducing psychological stress and increasing medication compliance in pulmonary tuberculosis patients. Conclusion: CBT is effective in increasing medication adherence in pulmonary tuberculosis patients.

Abstract

Instrumental support is concrete and tangible, for instance, helping to provide food or accompanying patients to health care facilities. However, its impact on medication adherence among tuberculosis patients in Indonesia needs to be explored. This study aimed to examine the impact of instrumental support from family on tuberculosis patients’ adherence to medication in Bogor City, West Java Province, Indonesia. A cross-sectional study involving 106 participants with an age average of 39.7 (SD = 14.9) was conducted in 12 primary health care (PHC). The independent variables (age, sex, education, employment status, wealth index, and the distance between home and PHC) of instrumental support from family was assessed using MMAS-8. In contrast, the dependent variable of medication adherence was assessed using a self-reported instrument. Multivariate binary logistic regression was used in the analysis and indicated that participants receiving family support were more likely to adhere to medication protocol (95% CI = 1.1–6.3; p-value = 0.029). Instrumental support from family was associated with medication adherence among tuberculosis patients in Bogor City, West Java Province, Indonesia. It is necessary to design further comprehensive interventions in the community setting to encourage the family to support tuberculosis patients following medication protocol.
