Abstract

Abstract Perinatal deaths are babies who die at <28 days of birth. According to WHO, there are 2.7 million perinatal deaths worldwide in 2015. In 2016, the number of infant mortality at Jember is 50.19 per 1000 live births and become the second rank in Jawa Timur after Probolinggo. Infant mortality has been associated by maternal, gestational and neonatal factors. The aim of this study is to determine the risk factors of perinatal death age 0-28 days in RSD dr. Soebandi Kabupaten Jember. This research was analytic observasional with case control design doing in RSD dr. Soebandi Kabupaten Jember. The samples were 65 cases and 65 controls. Data analysis was done by Chi-square test and Fisher’s exact test with significance value < 0,05. The results showed APGAR Score (p <0.001, OR = 6.51), birth weight (p <0.001, OR = 9.66), body length (p <0.001 OR = 5.07), gestational age (p = 0.002; OR = 2.92), congenital anomalies (p = 0.013; OR = -) and maternal age (p = 0.001; OR = 4.51) were risk factors for perinatal death 0-28 days in RSD dr. Abstract

Recently, cancer is globally the second highest cause of death with a total death about 8.8 million in 2015. There are some therapy of cancer, one of them by using cyclophosphamid chemotherapy, but this regimen has side effects of bone marrow depression through its active metabolite of 4 hydroxy cyclophpsphamid and increase ROS which will effect on serum MDA elevation. Increased MDA serum can be prevented by giving an antioxidant or plants that have various antioxidants, such as kenitu leaves. The aim of this study was to determine the effect of antioxidants in kenitu leaves (Chrysophyllum cainito L.)) extract to MDA serum leves of cyclophosphamid-induced mice. This study used quasi experimental method with post test only controlled group design by using 1 normal control group, 1 negative control group (cyclophosphamid), and 5 treatment groups (dose of extract 50mg/kgBW; 100mg/kgBW; 200mg/kgBW; 400mg/kgBB; and 800mg / kgBW). The results of the treatment group dose 100mg/kgBB showed the average of MDA serum levels was 1.94 μg/mL which is the highest decrease in this study. The result of Kendall Tau correlation test showed the effect of kenitu leaves extract to MDA serum level had p = 0.013 and correlation coefficient = 0.431. Abstract

One of way to preventing bacterial resistance is a combination of natural plant products with antibiotics. Hibiscus rosasinensis leaf proved to have antibacterial activity against Shigella dysentriae. Ciprofloxacin is the first-choice antibiotic to treat Shigella dysentriae infection. The purpose of this study was to determine the effect of methanol extract of Hibiscus rosasinensis leaves and ciprofloxacin against S. dysentriae bacteria in vitro. The type of this study was quasi experimental design with posttest only control group design using nine treatment groups (with concentration variation of 0.5 μg / mL, 1 μg / mL, 2 μg / mL, 4 μg / mL, 8 μg / mL, 16 μg / mL, 32 μg / mL, 64 μg / mL, and 128 μg / mL with 5 μg / 5 μL ciprofloxacin) and one control group (only 5 μg / 5 μL ciprofloxacin). The method used was agar well diffusion. The data was the diameter of inhibition zone around the well. The results showed that the variant of hibiscus leaf concentration could inhibit the growth of Shigella dysentriae but formed smaller inhibition zone diameter than ciprofloxacin alone. Abstract

Abstract Low 25-hydrocyvitamin D (25(OH)D3) levels are usually found in patients with type 1 and type 2 diabetes mellitus. This study aims to compare the effectiveness of fasting blood glucose levels in hyperglycemic mice model with glimepirid single therapy, hyperglycemic mice model with vitamin D, and mice model of hyperglycemic with glimepirid therapy plus vitamin D. This study used the true experimental research design with the randomized posttest only control group design. There were 25 male mice (Mus musculus) strain Balb/C with 20-30 g body weight and 2-3 months old which were grouped into 5 groups. The negative control group (K1) was injected with placebo and the positive control group (K2) was injected with STZ 150 mg/kgBW i.p. Groups P1, P2, and P3 were given STZ injection of 150 mg/kgBW i.p and given glimepirid 0.26 mg/kgBW; vitamin D 6,5 ml/kgBW; glimepirid 0,26 mg/kgBW with vitamin D supplementation 6,5 ml/kgBW. The results of the Tukey post hoc tests obtained differences in the mean of delta KGD between groups P1 and K2 = 122,55; in group P2 with K2 = 81,60; and in group P3 with K2 = 74,40. From these data, it can be concluded that in the group given glimepirid plus vitamin D there was no additional effectiveness in decreasing fasting blood glucose levels of hyperglycemic mice model compared to the group given only glimepirid therapy and the group given vitamin D alone. Abstract

Human Immunodeficiency Virus (HIV) is a virus that decreased immunity and a set symptoms of diseases called Acquired Immune Deficiency Syndrome (AIDS). One of the major risk factors for HIV transmission is perinatal transmission about 2.8% during pregnancy, delivery, and postpartum. HIV positive mothers have a potential to give birth infants with low APGAR. APGAR Score was used as a reference to determine asphyxia in the first and fifth minutes of life. The purpose of this research were to determine the correlation between HIV/AIDS positive pregnant mother with infant APGAR Score and to determine the other factors that affect the infant APGAR Score in RSD dr. Soebandi Jember. This research used observational analytic survey method with cross sectional design using medical record of HIV positive and negative pregnant women from August 2014-July 2017 in RSD dr. Soebandi Jember as a subject that qualify the inclusion and exclusion criteria. This research used case group sampling technique by total sampling and control group by simple random sampling each 52 samples. Test result of the correlation between HIV/AIDS positive pregnant mother with infant APGAR Score using Chi-Square test obtained p value=1.000 (OR=1.13) that means there was no significant correlation. Test result of the correlation between the other factors that affect infant APGAR Score concluded that there were no significant correlation. Keywords: pregnant mother, HIV/AIDS, APGAR Score, infant

Abstract

Escherichia coli (E. coli) bacteria may contaminating uncooked food and cause food poisoning disease. Long beans (Vigna sinensis L) is one of the vegetable that consumed by people in raw (uncooked) and often traded in traditional markets. Many traditional markets do not meet sanitation requirements and many sellers do not perform good personal hygiene. These situation can potentially to cause bacterial contamination of vegetables sold in traditional markets. There has been no research examining E. coli contamination in long beans that sold at traditional markets in Jember.The purpose of this study to prove the contamination of E. coli bacteria in long beans at traditional markets in Jember. The research method uses an observational descriptive design with the cross sectional approach. Research places in eight traditional markets in Jember Regency and Microbiology Laboratory, Faculty of Medicine, University of Jember. The samples used amounted to 40 long beans purchased from 40 traditional market traders. The microbiological examination is performed using the most probable number (MPN) method, namely presumptive test and completed test. The results showed from the presumptive test all of 40 samples (100%) contaminated with coliform bacteria. After completed tests, 36 out of 40 samples (90%) were found contaminated by E. coli. The majority of long beans sold in traditional markets have been contaminated by E. coli bacteria.

Abstract

Tuberculosis is one of the world's health problems even in Indonesia. The immune response that needed to deal with TB can be influenced by other infections, such as helminth infection. The helminth infections induce Th2 immune responses and eosinophilia to eliminate these pathogen. Meanwhile, the Th2 response and eosinophilia can also suppress the Th1 immune response which is very important to eliminate the bacterium M. tuberculosis and make a negative impact on the success of TB therapy. Thus, the eosinophil profile can describe the immune response in TB patients with helminth co-infection. This study aims to determine the prevalence of helminthiasis in TB patients and determine the differences in eosinophil amount in tuberculosis patients with and without helminth co-infection. This study was an observational analytic study with a cross-sectional design conducted at Puskesmas Panti in August 2019-January 2020 using fecal and blood samples from 24 research subjects. Helminth co-infection status was obtained from fecal examination with sedimentation and flotation methods while the number of eosinophils was obtained from leukocyte counts. Data analysis used the Mann Whitney U test to determine differences in the number of eosinophils in the two groups. The results showed there was no difference in the number of eosinophils in TB patients with helminth co-infection and without helminth co-infection (p> 0.05). There is no typical picture of the number of eosinophils in both groups so that eosinophils cannot describe the immune response that arose in TB patients with helminth co-infections in Panti district, Jember.

Abstract

Abstrak: Tuberkulosis (TB) menjadi masalah kesehatan utama baik lingkup global maupun nasional. Tuberkulosis menyebabkan status gizi kurang baik. Status gizi yang kurang baik dapat disebabkan oleh infeksi kronis, salah satunya infeksi cacing usus terutama Soil-Transmitted Helminths (STH). Ko-infeksi STH pada pasien TB selain mempengaruhi status gizi, juga akan mempengaruhi respon imun pasien. Respon imun pada pasien ko-infeksi STH akan menghambat respon imun Th 1 dalam melawan TB serta akan didapatkan peningkatan eosinofil (eosinofilia). Tujuan utama dalam penelitian ini adalah untuk mengetahui jumlah eosinofil pada pasien tuberkulosis terhadap ko-infeksi STH di Kecamatan Kalisat, Kabupaten Jember. Jenis penelitian ini adalah penelitian observasional dengan desain analitik cross sectional. Populasi yang digunakan adalah pasien TB di Kecamatan Kalisat dalam periode waktu September – Desember 2019. Data yang digunakan dalam penelitian ini adalah data primer berupa hasil pemeriksaan darah dengan metode hitung jenis dan hasil pemeriksaan feses menggunakan metode sedimentasi dan floatasi. Analisis data dilakukan menggunakan uji Fisher. Hasil penelitian menunjukkan bahwa kejadian koinfeksi STH pada pasien TB di Kecamatan Kalisat Kabupaten Jember sebesar 3,6% dan disebabkan oleh Hookworm. Hasil uji Fisher menunjukkan tidak ada hubungan yang signifikan antara jumlah eosinofil dengan kejadian koinfeksi STH pada pasien TB di Kecamatan Kalisat Kabupaten Jember dengan nilai p-value 1,000. Abstract

ABSTRACT Ventilator-Associated Pneumonia (VAP) is pneumonia in patients who use mechanical ventilator. The empirical antibiotic therapy is given to VAP patients based on bacterial mapping and its antibiotic sensitivity. The aim of this study is to determine bacterial identification and antibiotic sensitivity of VAP patients at RSD dr. Soebandi Jember. This study is a retrospective study, the data collected from medical record of VAP patients from September to October 2019. All samples that is meet the inclusion and exclusion criteria was recruited in this study. Data analysis was use Microsoft Excel 2010. This study obtained 15 samples. The most frequents bacteria that causes VAP is Acinetobacter baumanii. It is also caused by Klebsiella pneumoniae, Pseudomonas aeruginosa, Enterobacter aerogenes, Burkholderia cepacia, Pseudomonas fluorescence, Salmonella arizonae, and Escherichia coli. The bacteria are most sensitive to amikacin, meropenem, and piperacillin-tazobactam and most resistant to cefixime, cefotaxime, and ceftriaxone.

Abstract

Background: The prevalence of Human Immunodeficiency Virus (HIV) infection in pregnancy were increase in developing countries. The existence of infection interferes with the absorption of nutrients due to accumulation of inflammatory cells in the placenta can cause the infant born with low birth weight. Objective: The purpose of this study was to determine the correlation of HIV infections in pregnancy and low birth weight infant. Methods: This research used an observational analytic design with a retrospective approach. The samples were positive and negative HIV mother with their infants that hospitalized at dr. Soebandi Hospital, Jember, from August 2014 - July 2017. The data were analysed by Fisher's Exact. Results: This study was found 52 positive HIV mother with their infants and 52 negative HIV mother with their infants. Nine from 52 infants (17.3%) who born from positive HIV mother were low birth weight. Only 3 from 52 infants (5.8%) who born from negative HIV mother were low birth weight. Data analysis using Fisher’s Exact was obtained p value = 0.06. Conclusion: There was no significant correlation of HIV infections in pregnancy and low birth weight infant.
