Abstract

Tuberculous lymphadenitis may cause weight loss and lead to malnutrition in children.This study analyzes the correlation between tuberculous lymphadenitis and nutritionalstatus in children. It was an analytic observational study with a cross-sectional design.The population was pediatric patients under 18 years old with lymphadenitis, outpatientand inpatient admitted from January 2018 to December 2020 in dr. Soebandi publichospital, Jember, Indonesia. The samples were 76 respondents with total sampling. There were 52 tuberculosis lymphadenitis patients in the case group, while 24 were non-tuberculosis in the control group. The body weight data was from the medical record when the first diagnosis of lymphadenitis. Instruments were weight-for-age z-scorecurves (WHO, 2007) for under 60 months old children, while CDC Growth Charts 2000for more than five-year-old children. Then, data analysis used the Chi-square test.Mostly, tuberculous lymphadenitis patients were girls (65.4%), 12-17 years old (67.3%),not underweight (61.5%), and lived in rural areas (73%). Furthermore, all of them weregiven isoniazid, rifampicin, and pyrazinamide. In addition, 100% of them had successfultreatment. Meanwhile, most of the non-tuberculous lymphadenitis patients were boys(54.2%), 6-11 years old (58.3%), lived in the rural areas (79%), and were notunderweight (66.7%). Treatment of the non-tuberculous lymphadenitis group consistedof antibiotics, analgesics, vitamins, and symptomatic medicine. The Chi-square testresults obtained p=0.667 (p>0.05). Abstract

Tuberkulosis (TB) adalah penyakit yang disebabkan oleh infeksi Mycobacterium tuberculosis. Kejadian ko-infeksi parasit di daerah endemik TB sering dilaporkan yang mengakibatkan kondisi penderita TB semakin parah dan sulit disembuhkan. Ko-infeksi cacing pada penderita TB diketahui berpengaruh terhadap respon imun, proses pengobatan, status gizi, dan prognosisnya. Tujuan penelitian ini adalah untuk mengetahui hubungan ko-infeksi soil-transmitted helminthiasis (STH) terhadap status gizi pada penderita TB di Kecamatan Puger, Kabupaten Jember. Studi ini adalah penelitian observasi dengan desain cross sectional yang dilakukan di Puskesmas Puger, Kabupaten Jember, Jawa Timur pada September 2019 sampai Januari 2020. Data ko-infeksi cacing pada penderita TB diperoleh dari pemeriksaan feses dengan metode sedimentasi dan flotasi sedangkan status gizi diperoleh dari pengukuran indek masa tubuh (IMT). Analisis data menggunakan uji Chi-square untuk mengethui adanya hubungan ko-infeksi STH terhadap status gizi penderita TB. Hasil penelitian menunjukkan bahwa 6 dari 32 pasien TB (18,72 %) terinfeksi STH, 4 (%) terinfeksi Ascaris lumbricoides, dan 2 (%) terinfeksi hookworms. Hasil pengukuran IMT adalah18 (56,25%) yang mengindikasikan bahwa penderita TB berstatus gizi kurang dan14 (43,75%) berstatus gizi normal. Hasil uji Chi Square menunjukkan tidak ada hubungan ko-infeksi STH terhadap status gizi penderita TB (p>0,05). Dengan demikian, perlu dilakukan edukasi pada penderita TB tentang gizi seimbang khususnya di wilayah puskesmas Puger Kabupaten Jember, Jawa Timur agar status gizi dapat meningkat.

Abstract

Background: Tuberculosis (TB) remains a major public health problem globally. TB in children is an important component in TB control. The diagnosis of TB in children until now is still challenging, underdiagnosis often occurs in primary health facilities in Indonesia. This study aims to describe the demographic and clinical features of pediatric tuberculosis patients hospitalized in a tertiary care center.Methods: This study reviewed medical records of children with TB who were hospitalized in a referral hospital in Jember, East Java, Indonesia from January 2018 through December 2019.Results: Among 96 patients, 33,3% happened in the age group of 15-18 years, 53.1% were females, 35,4% had severely underweight nutritional status, majority (84,4%) live in rural area, 79,2% without known TB contact history and 81.1% were new cases. Pulmonary TB was the most common diagnosis (80,2%). Only 4.1% had co-infection with Human Immunodeficiency Virus (HIV) and 4.2% were reported to be resistant to anti-TB drugs. The most common symptoms were fever (84.3%), cough (65.6%), and dyspnea (65.6%). The miliary lesion was the most common lesions on chest X-ray examination results. The successful therapy reached a percentage of 61.4% of all cases.Conclusions: The majority of pediatric TB patients were diagnosed clinically and chest X ray. Examination of HIV status and drug-sensitivity testing is low. The rate of successful therapy was still under WHO target.. Keywords: tuberculosis, children, characteristics

Abstract

A raw vegetable is one of the transmission factors in human food-borne infection. Contamination might start from the plantation, distribution, until at serving time in the dining table. A higher level of contamination will increase the risk of food-borne illness. The study aims to determine bacteria and parasite contamination in some raw vegetables that people usually consume in Jember Regency, East Java, Indonesia. In this study, we collected raw vegetable from eight traditional markets. We chose randomly at each market five samples of vegetables that usually directly consumed, like lettuce, tomato, cabbage, basil, long bean, and cucumber. Pathogens we identified limited on bacteria and parasite like helminth and protozoan group. This study showed that 91% contaminated by E. coli, 84% by Staphylococcus aureus, and 79% contaminated by Vibrio cholerae. Besides bacteria, 36% of samples contaminated by the helminth group, and protozoa contaminated 27%. The most vegetables contaminated was lettuce, even in bacterial or parasitic contamination. This study concluded that there is contamination in raw vegetables usually directly consumed in Jember Regency. It will be a potential risk factor for food-borne illness in the future.

Abstract

Latar belakang: Berdasarkan data Wilayah Kerja Puskesmas Patrang didapatkan sebanyak 2964 kasus diare pada tahun 2020 di Kabupaten Jember. Diare dapat terjadi akibat infeksi bakteri Salmonella spp. yang menular secara oral fecal. Bakteri Salmonella spp seringkali mencemari bahan pangan terutama sayuran. Sementara itu, masyarakat Indonesia sering mengonsumsi sayuran mentah yang disebut dengan lalapan mentah. Kontaminasi bakteri pada lalapan mentah dapat disebabkan oleh kondisi higiene penjual yang kurang baik yang dipengaruhi oleh beberapa faktor, seperti pengetahuan keamanan pangan penjual. Penelitian ini bertujuan untuk mengetahui hubungan pengetahuan keamanan pangan dengan higiene penjual dan kontaminan Salmonella spp pada lalapan mentah di Kecamatan Patrang.Metode: Penelitian ini menggunakan metode observasional analitik dengan desain penelitian cross sectional, dan data dianalisis menggunakan uji Spearman Rank. Sampel ditentukan dengan teknik simple random sampling dengan jumlah sampel 30 penjual.Hasil: Hasil penelitian menemukan bahwa 83,3% penjual memiliki pengetahuan keamanan pangan yang baik, 46,7% penjual memiliki higiene personal yang cukup, dan 80% lalapan mentah terkontaminasi Salmonella spp. Pada uji analisis Spearman Rank tidak didapatkan hasil yang signifikan antara pengetahuan keamanan pangan dengan higiene penjual (0,721>0,05) dan tidak ada hasil yang signifikan antara pengetahuan keamanan pangan dengan Salmonella spp (0,235>0,05).Simpulan: Dapat disimpulkan bahwa tidak ada hubungan pengetahuan keamanan pangan dengan higiene penjual dan Salmonella spp pada lalapan mentah di Kecamatan Patrang. ABSTRACT Title: Relation of Food Safety Knowledge, Seller Hygiene, and Salmonella spp on Raw Lalapan in Patrang DistrictBackground: Based on data from the Patrang Health Center Work Area, there were 2964 cases of diarrhea in 2020 in Jember Regency. Diarrhea can occur due to infection with Salmonella spp. which is transmitted oral fecal. Salmonella spp can contaminate food especially vegetables. Meanwhile, Indonesian people often consume vegetables salad called raw lalapan. Raw lalapan contamination can caused by seller poor hygiene that influenced by several factors, such as their food safety knowledge. This study aims to determine the relation of food safety knowledge with seller hygiene and Salmonella spp contaminants on raw lalapan in Patrang District.Method: This study used an analytical observational method with a cross sectional study design, and the data were analyzed using Spearman Rank test. The sample was determined by a simple random sampling technique with a sample of 30 sellers.Result:  The study found that 83.3% seller had good food safety knowledge, 46.7% seller had sufficient personal hygiene, and 80% of raw lalapan  were contaminated with Salmonella spp. In the Spearman Rank analysis test, there were no significant results between food safety knowledge and seller hygiene (0.721>0.05) and no significant results between food safety knowledge and Salmonella spp contaminants (0.235>0 ,05).Conclusion: So, it can be concluded that there was no relation of food safety knowledge with seller hygiene and Salmonella spp contaminants in raw vegetables in Patrang District.

Abstract

Lettuce is a vegetable that is usually consumed in raw condition or unprocessed by people. Unprocessed vegetables can cause food-borne diseases. Lettuce that is not processed and being consumed directly can cause contamination by Coliform bacteria. The purpose of this study is to know the correlation between Greengrocer’s personal hygiene and Coliform bacteria contamination in lettuce in Jembertraditional markets. This research used a descriptive observational design with the cross-sectional approach. It took place in eight traditional markets in Jember Regency and Microbiology Laboratory, Faculty of Medicine, University of Jember. The samples used were 41 lettuces purchased from 41 traditional market sellers. The microbiological examination was performed in the laboratory using the most probable number method. The results showed that lettuce sellers had bad personal hygiene (92,7%) and the data from the most probable number test found 97,6% contaminated by Coliform bacteria. The bivariate analysis was used to know the relationship between personal hygiene vegetable seller and contamination Coliform bacteria in lettuce. Thep-value was 0,072, which means there was no relationship between personal hygiene vegetable seller and contamination Coliform bacteria in lettuce. Abstract

Human Immunodeficiency Virus (HIV) infectionin pregnant women was increasing in Indonesia. HIV infection will lead complication risk to the mother and their babies. Objective: This study aims to determine the association of HIV infection in pregnant women and preterm birthat RSD Dr. Soebandi Jember.Method:An analytics descriptive with retrospective approach study was conducted. The subjects were medical records of HIV-positive pregnant women group and HIV-negative pregnant women group at RSD Dr. Soebandi Jember from August 2014 to July 2017. Result:Each of groups had 52 samples. In the HIV-positive pregnant women group, there were 19 preterm and 33 aterm birth, on the other hand, in the HIV-negative pregnant women group, there were 15 preterm and 37 aterm birth. The Chi Square test analysis was obtained p value = 0.403 (OR = 1.42). Conclusion: There was no significant correlation between HIV infection in pregnant women and preterm birth.

Abstract

Klebsiella pneumoniae merupakan bakteri genus Klebsiella dalam famili Enterobactericeae. Klebsiella pneumoniae menyebabkan infeksi serius. Faktor virulensi penting dalam perlekatan awal bakteri untuk menginfeksi inang pili dan Outer Membrane Protein (OMP). Pili sebagai faktor virulensi dapat melekat kuat secara spesifik dan mampu menghindari pembersihan yang dilakukan oleh sel inang. Kemampuan adhesi suatu bakteri dipengaruhi oleh kemampuannya dalam mengagglutinasi eritrosit. Berdasarkan studi pendahuluan yang telah dilakukan, berat molekul protein pili yang muncul dari hasil elektroforesis SDS-PAGE yaitu 96,4 kDa memiliki pita tebal dan berkonsentrasi tinggi serta belum ada penelitian  membuktikan bahwa protein pili 96,4 kDa merupakan protein hemaglutinin dan adhesin.Tujuan penelitian ini untuk mengetahui peran protein pili 96,4 kDa bakteri Klebsiella pneumoniae sebagai protein hemaglutinin dan adhesin.  Penelitian ini dilakukan dengan mengisolasi protein pili Klebsiella pneumoniae kemudian dilakukan uji aglutinasi menggunakan sel eritrosit mencit dan uji adhesi menggunakan sel enterosit mencit. Hasil yang diperoleh dianalisis menggunakan Saphiro-Wilk. Hasil uji hemaglutinasi dengan menggunakan sel eritrosit mencit didapatkan titer hemaglutinasi paling tinggi untuk berat molekul protein pili 96,4 kDa adalah pada titer 1. Indeks adhesi dengan titer pengenceran memiliki hubungan yang bermakna yaitu  0,016 (p 0,05) dengan nilai r = -0,847, sehingga disimpulkan protein pili 96,4 kDa Klebsiella pneumoniae merupakan protein hemaglutinin dan adhesin.

Abstract

Tuberkulosis (TB) dan cacingan merupakan penyakit infeksi dengan jumlah penderita yang banyak di Indonesia. Tingginya prevalensi cacingan di Indonesia memungkinkan terjadinya koinfeksi STH pada pasien TB paru yang menyebabkan imunitas anti M. tuberculosis menurun sehingga respon terhadap pengobatan tuberkulosis menjadi tidak maksimal. Cacingan salah satunya dipengaruhi oleh higiene perorangan. Tujuan penelitian ini adalah untuk menganalisis hubungan antara higiene perorangan dan kejadian koinfeksi cacingan pada penderita TB di Kecamatan Tempurejo Kabupaten Jember. Jenis penelitian ini adalah penelitian observasional dengan desain analitik cross sectional. Populasi yang digunakan adalah pasien TB di Kecamatan Tempurejo dalam periode waktu September – Oktober 2019. Data yang digunakan dalam penelitian ini adalah data primer berupa hasil wawancara dan hasil pemeriksaan feses. Pemeriksaan feses dilakukan menggunakan metode sedimentasi dan floatasi. Analisis data dilakukan menggunakan uji Fisher. Hasil penelitian ini menunjukkan bahwa kejadian koinfeksi cacingan pada pasien TB di Kecamatan Tempurejo Kabupaten Jember sebesar 9,67% dan disebabkan oleh dua spesies STH yaitu A. lumbricoides (66,7%) dan Hookworm (33,3%). Higiene perorangan responden terdiri atas higiene baik (64,5%) dan higiene buruk (35,5%), dimana 66,7% kejadian koinfeksi cacingan terjadi pada responden dengan higiene perorangan buruk. Hasil uji Fisher menunjukkan bahwa tidak ada hubungan yang signifikan antara higiene perorangan dengan kejadian koinfeksi cacingan pada pasien TB di Kecamatan Tempurejo Kabupaten Jember (p-value sebesar 0,281).

Abstract

Background: Children are known to be less infected by Coronavirus disease 2019 (COVID-19) rather than adults. However, children are still a vulnerable group and the incidence of patients admitted to hospital continues to increase. As Indonesia’s district has limited resources during pandemic wave, the data on pandemic cases was still underreported especially cases of children.Methods: This retrospective observational study collected data on pediatric case aged 0 – 18 years old, who were hospitalized at dr. Soebandi Hospital, Jember, Indonesia and confirmed to have COVID-19, from March 2020 to the end of March 2021.Results: 40 children were hospitalized and confirmed COVID-19 during the study period, the majority aged 15 – 18 years (40%), male (65%), and had a history of contact (67.5%). The most common symptoms were cough (85%), fever (67.5%), and shortness of breath (40%). Laboratory results showed that the majority had decreased neutrophils and increased lymphocytes, while most of the patient had normal chest X-rays (52.5%). From 5 pediatric patients who had multimorbidity, 4 patients died. All four had common underweight clinical morbidity, while acute respiratory distress syndrome was dominated the cause of death. The mortality rate in this study reached 10%.Conclusion: The majority of pediatric patients were found to have mild to moderate symptoms with good outcomes. Case mortality rate in this study was lower than national data. Meanwhile, the underweight comorbid in children with COVID-19 was worsening the outcome in this study.
