Abstract

Spontaneous pneumothorax is a rare but fatal complication of miliary tuberculosis. We report a 13-year-old boy presenting with shortness of breath. He was diagnosed with miliary tuberculosis with spontaneous pneumothorax, which showed significant improvement after the insertion of a thoracostomy tube and anti-tuberculosis drug therapy.

Abstract

Osteoarthritis is a degenerative disease characterized by progressive damage to joint cartilage. The prevalence of osteoarthritis will continue to increase and continue as the population and age increase. Cases of knee osteoarthritis are more common than other types of osteoarthritis. The high prevalence of osteoarthritis will have implications for the high treatment regimen given to patients. Irrational use of drugs can lead to drug-related problems (DRPs). This study used a descriptive design. The sample in this study were knee osteoarthritis patients at the Bina Sehat Jember Hospital. The sampling technique used is purposive sampling. The data in this study used secondary data from patient medical records for the period January 2020 - April 2021. A total of 3 knee osteoarthritis patients have the potential to experience drug-related problems in the drug interaction category.

Abstract

Lalapan is a fresh vegetable usually eat up with rice and other Indonesian dishes. However, fresh vegetables can potentially be contaminated with pathogenic bacteria, such as Enterobacteriaceae, harming consumers if not appropriately processed and cleaned. This study analyzes the correlation between food hygiene and sanitation in food vendors of Lalapan with Enterobacteriaceae contamination in fresh vegetables. It used an analytic observational design with a cross-sectional approach. In addition, the population was food vendors of lalapan in the Sumbersari District, Jember Regency. There were 30 respondents with a consecutive sampling method. Food hygiene and sanitation instrument was an observation sheet according to 16 points stipulated in the Regulation of the Minister of Health of the Republic of Indonesia number 1096/MENKES/PER/VI of 2011. In addition, we tested fresh vegetable samples to analyze Enterobacteriaceae contamination with a microbiological test using Salmonella Chromogenic Agar (SCA) media. Then, data analysis used Fisher's exact test with α=0.05. The observation showed that most respondents had good food sanitation hygiene (76.7%). The microbiological examination indicated 23 vegetable samples (76.7%) were contaminated with Enterobacteriaceae. Statistical analysis using Fisher's exact test Exact obtained p=1.000 (p>0.05). Thus, there was no significant correlation between food hygiene and sanitation in food vendors of lalapan with Enterobacteriaceae contamination in fresh vegetables. In conclusion, hygiene, and sanitation food among food vendors of lalapan do not correlate with Enterobacteriaceae contamination in fresh vegetables. Further research could analyze the vegetable's planting, harvesting, and distribution processes as risk factors for Enterobacteriaceae contamination in fresh vegetables.

Abstract

Stunting is a condition of failure to thrive in children under five years of age as a result of chronic malnutrition so that the child is too short for his age. The prevalence rate of stunting in Indonesia is still above 20%, meaning that it has not reached the target. Factors that influence stunting include birth length and birth weight. The purpose of this study was to determine the description of the factors that influence the incidence of stunting in children aged 0-59 months in the working area of the Balung Public Health Center in 2021. This research is descriptive in nature, which was conducted in the working area of Puskesmas Balung, Jember Regency in April 2021. The population is children under five with age stunting. 0-59 months recorded in the EPPGBM in February 2021. Sampling in this study was carried out by total sampling, namely as many as 639 stunting toddlers. Of the 639 stunted children under five, 347 were male and the majority occurred at the age of 12-23 months, namely 150 under five. From 639 toddlers, it describes 516 toddlers with normal body length and 123 toddlers with short birth lengths. Meanwhile, the birth weight of 639 children under five shows that 422 children under five were born with normal birth weight. From the data above, it can be concluded that the majority of children under five with stunting in the working area ofPuskesmas Balung were born with normal body length and weight. For this reason, it is necessary to carry out further research on the relationship of several risk factors using statistical tests. In addition, it is also necessary to look for other risk factors that can cause stunting in children.  Abstrak: Stunting adalah kondisi gagal tumbuh pada anak balita (bayi di bawah lima tahun) akibat dari kekurangan gizi kronis sehingga anak terlalu pendek untuk usianya. Angka prevalensi stunting di Indonesia masih di atas 20%, artinya belum mencapai target. Faktor yang mempengaruhi stunting diantaranya panjang badan lahir dan berat badan lahir. Tujuan penelitian ini adalah untuk mengetahui gambaran faktor yang mempengaruhi kejadian stunting pada balita usia 0-59 bulan di wilayah kerja Puskesmas Balung Tahun 2021. Penelitian ini bersifat deskriptif yang dilakukan di wilayah kerja Puskesmas Balung Kabupaten Jember pada bulan April 2021.  Populasi adalah balita stunting usia 0-59 bulan yang terdata pada EPPGBM pada bulan Februari 2021. Pengambilan sampel pada penelitian ini dilakukan dengan cara total sampling yaitu sebanyak 639 balita stunting. Dari 639 balita stunting, 347 berjenis kelamin laki-laki dan mayoritas terjadi pada usia 12-23 bulan yaitu sebesar 150 balita. Dari 639 balita menggambarkan 516 balita lahir dengan panjang badan normal dan 123 balita memiliki panjang badan lahir pendek. Sedangkan berat badan lahir 639 balita mengambarkan 422 balita lahir dengan berat bdan lahir normal. Dari data diatas dapat disimpulkan bahwa mayoritas balita stunting di wilayah kerja Puskesmas Balung ini lahir dengan kondisi panjang bdan dan berat badan normal. Untuk itu perlu dilakukan penelitian lebih lanjut mengenai hubungan beberapa faktor risiko tersebut dengan menggunakan uji statistik. Selain itu perlu juga dilakukan pencarian faktor risiko lain yang bisa menjadikan stunting pada anak.

Abstract

Pneumonia is an infection of the lung parenchyma caused by Klebsiella pneumoniae, resulting in a high mortality rate of millions each year. To reduce these deaths, one potential solution is to create a vaccine that utilizes virulence factors of this microorganism, such as pili. During the early phase of infection, these antigens have a crucial role and can stimulate the production of memory b cells. These cells are activated by IL-4 (interleukin-4) in lymphoid organs, such as the spleen. Pathogen exposure such as virulence factors can stimulate the secretion of IL-4 in the spleen organ. This study aims to investigate the role of Klebsiella pneumoniae pili in this process. The research conducted from May to December 2023 at the Microbiology Laboratory, Faculty of Medicine, University of Jember was purely experimental, using Balb/c mice with IL-4 levels in the spleen organ as variables. The study used 15 mice, divided into control (K1), adjuvant (K2), and antigen (K3) groups. The research data were analyzed using non-parametric tests, specifically the Kruskal-Wallis and Post Hoc tests. The Kruskal-Wallis test revealed significant differences (p=0.003). In the Post Hoc test, a significant difference was found between the control and antigen groups (p=0.002). The results conclusively demonstrate that induction of Klebsiella pneumoniae pili protein 65.5 kDa significantly increases IL-4 levels in the spleen. Future studies should consider adding serum specimens to provide additional information.

Abstract

Introduction: Diabetes mellitus (DM) comorbidity in drug-resistant (DR) pulmonary tuberculosis (TB) patients can be associated with the treatment outcome. In DR-TB patients with DM, the immune system is impaired, which will decrease the success of treatment. Sputum conversion time is an indicator used to predict the treatment outcome. However, there is still no further study related to the association between DM comorbidity and the sputum conversion time in DR-TB patients, especially in Jember. This study aimed to determine the association between DM and sputum conversion time in DR-TB patients at Dr. Soebandi Regional General Hospital, Jember. Methods: This was an analytic observational study with a cross-sectional design. A total of 122 samples of DR-TB patients were taken using the purposive sampling method in 2018-2023 at Dr. Soebandi Regional General Hospital, Jember. The data were analyzed using the Chi-square and logistic regression statistical test. Results: Chi-square analysis showed that DM (p = 0.015; OR = 2.604; 95% CI 1.195-5.674) and age (p = 0.021; OR = 0.377; 95% CI 0.162-0.878) were associated with sputum conversion time. Logistic regression showed that DM was the most associated variable with the sputum conversion time (p = 0.016; OR = 2.604; 95% CI 1.195-5.674) compared to gender, age, TB resistance type, and anti-TB regimen. Conclusion: DM is associated with prolonged sputum conversion time in DR-TB patients at Dr. Soebandi Regional General Hospital, Jember. DM was also the most associated variable with the sputum conversion time compared to gender, age, TB resistance type, and anti-TB regimen.

Abstract

Abstract Nutritional imbalances can lead to malnutrition which can affect children's physical and cognitive growth and development. Advances in science and technology have created opportunities for innovation in the form of health applications to help monitor children's growth and development, nutritional status, and children's daily nutritional intake. The factor that influences the intention to use technology is the ease of use. This indicates that the easier the application is to use, the higher a person's interest and compliance will be in using the application regularly. This research aims to determine the effect of ease of use of the BalitaGrow+ application on compliance with filling in the daily nutrition column for children in Rambigundam Village, Jember. This research was an analytical observational research with a cross sectional approach with a sample size of 35 people who meet the criteria. Data analysis used univariate tests and bivariate used the Spearman Rank correlation test. The results obtained from 35 respondents showed that 34 people (97.1%) found it easy to use the BalitaGrow+ application and 31 people (88.6%) had good compliance in filling in the child's daily nutrition column in the application. The results of statistical analysis showed a significance value of p value >0.05 (0.726). So it can be concluded that there was an insignificant influence between the ease of use of the BalitaGrow+ application on compliance with filling in the daily nutrition column for children in Rambigundam Village, Jember. It is suggested to increase the sample size and investigate other influenced factors for future study. Keywords: health application, ease of use, compliance of filling, children’s nutrition

Abstract

Chylothorax is rare in neonates but can be fatal if not diagnosed and appropriately managed. We report a baby boy diagnosed with chylothorax, which showed significant improvement after the installation of indwelling pleural catheters (IPC).

Abstract

Dengue infection is a disease caused by dengue virus infection and transmitted by the Aedes mosquito, and is a major health problem in children, especially in developing countries. The Platelet to Lymphocyte Ratio (PLR) is a novel inflammatory biomarker that has recently been frequently studied and plays a role in the assessment of viral infectious diseases, and the use of PLR in viral infectious diseases has previously found greater potential compared with the Neutrophil-Lymphocyte Ratio (NLR). It is hoped that PLR can become a new biomarker for predicting the severity of dengue infection which is suitable for use in developing countries because PLR ​​is a cheap biomarker and is available in all clinical conditions. This research is a correlative analytical observational study using a cross-sectional approach. This research design was carried out by collecting data from children diagnosed with dengue infection as well as data on platelets and lymphocytes on the first day of admission to the hospital. This research uses stratified random sampling. Data were analyzed using Spearman correlation with a confidence level of 99%. The results of Spearman's correlation analysis showed a strong significant relationship with p<0.01 (p=0.000) and a correlation coefficient of -0.753. In conclusion, there is a significant and strong relationship between the Platelet-to-Lymphocy Ratio (PLR) value and the severity of dengue infection in pediatric patients in the Aster ward of RSUD Dr. Soebandi Jember. If the PLR ​​value increases, the severity of dengue infection will be lower or decrease and vice versa. Keywords: PLR; dengue infection; children; platelets; lymphocytes

Abstract

Indonesia  is a tropical country and  home to many infectious diseases. Two of them are filariasis and scabies. The coinfection of filarial infection and scabies may cause severe morbidity for the patient, especially children. There has been no reported coinfection or management between these two diseases. An 11-year-old boy was brought to the pediatrics division of Dr. Soebandi Hospital with diffuse enlargement of the left lower extremity and scrotum, along with non-pitting edema, itching, and nodular skin lesions all over the body. It is important to consider holistic approaches to prevent further morbidity and disability.
