Abstract

Pengujian Chemical Oxygen Demand (COD) merupakan parameter penting dalam evaluasi kualitas limbah cair industri. Ketidakakuratan hasil uji dapat menimbulkan ketidaksesuaian dengan standar lingkungan. Penelitian ini bertujuan menganalisis potensi kegagalan dalam proses uji COD menggunakan metode Failure Mode and Effect Analysis (FMEA) dan Fault Tree Analysis (FTA). Data diperoleh melalui observasi, wawancara, dan studi dokumen laboratorium. Hasil FMEA menunjukkan mode kegagalan utama meliputi tidak adanya panduan warna indikator (RPN=384), perbedaan persepsi visual antar analis (RPN=343), dan volume reagen tidak sesuai SOP (RPN=343). Analisis FTA mengungkap jalur kegagalan dominan akibat alat tidak terkalibrasi dan kesalahan interpretasi titik akhir titrasi. Kesimpulannya, integrasi FMEA–FTA memberikan analisis risiko yang komprehensif serta menegaskan perlunya kalibrasi alat, penyediaan color chart, dan peningkatan pencahayaan laboratorium untuk menjamin akurasi hasil uji COD. Abstract

Analisis dilakukan untuk mengidentifikasi potensi kecelakaan kerja pada aktivitas pemasangan pipa jalur distribusi N2 di PT. XYZ menggunakan metode HIRADC dan RCA 5 Whys. Berbagai potensi bahaya ditemukan pada setiap tahapan pekerjaan, antara lain masalah ergonomi, terpeleset, tersandung, terhirup gas, bising, terbentur, percikan api, kelelahan, tertimpa material, terjatuh, paparan panas, dan risiko kebakaran. Penilaian risiko dengan matriks likelihood dan severity menunjukkan bahwa sebagian besar bahaya berada pada kategori sedang hingga tinggi. Metode RCA 5 Whys digunakan untuk mengidentifikasi akar penyebab, yang mengungkap kelemahan pada sistem manajemen K3, kurangnya pelatihan, budaya kerja yang tidak disiplin, serta pengawasan yang lemah. Berdasarkan hasil analisis, disusun rekomendasi pengendalian risiko berdasarkan hirarki pengendalian untuk meminimalkan potensi kecelakaan serta meningkatkan keselamatan dan kesehatan kerja di lingkungan perusahaan. Abstract

Occupational Safety and Health (OSH) plays a critical role in maintaining safe and sustainable production, particularly in small-scale agro-industrial operations such as coffee processing. This study applies the Hazard Identification, Risk Assessment, and Risk Control (HIRARC) framework to evaluate workplace hazards at Kopi Lanange Jagad, a micro-scale coffee producer in Malang Regency, Indonesia. Field observations and structured interviews were conducted to identify potential hazards across four production stages roasting, grinding, sieving, and packaging. Quantitative measurements were also taken, including ambient temperature, noise level, and airborne dust concentration, to provide empirical data for risk evaluation. The findings identified a total of 12 hazards, classified into three risk categories: high (25%), moderate (50%), and low (25%). The roasting process exhibited the highest risk score (R = 18.4), with average workplace temperatures reaching 85°C–120°C, significantly exceeding the national occupational limit of 30°C for moderate workloads (Permenaker No. 5/2018). The grinding stage also presented considerable hazards, with airborne dust concentrations averaging 2.4 mg/m³, approaching the Ministry of Manpower’s threshold limit value (TLV) of 3 mg/m³. Manual packaging was associated with ergonomic strain, where workers lifted 15–20 kg sacks repeatedly, leading to potential musculoskeletal disorders. Risk evaluation revealed that existing control measures such as natural ventilation and sporadic use of personal protective equipment (PPE) were insufficient to ensure worker safety. To reduce these hazards, the study recommends implementing engineering controls such as local exhaust ventilation and machine insulation, combined with administrative measures including job rotation, scheduled rest breaks, and regular safety training. Ensuring consistent PPE use especially heat-resistant gloves, dust masks, and ear protection can further mitigate residual risks. Overall, the application of the HIRARC framework at Kopi Lanange Jagad demonstrates its effectiveness as a low-cost and practical tool for identifying, prioritizing, and managing occupational hazards in small-scale coffee industries. Implementation of the proposed control strategies is projected to reduce total workplace risk by 35–40%, improving both worker well-being and operational productivity.

Abstract

PT BSB is a company engaged in steel fabrication manufacturing. There are various kinds of projects carried out at the company, namely bridges, steel structures, buildings, towers, etc. and the irregular placement of steel in the warehouse results in problems in the steel storage warehouse at PT. BSB where the company does not have an arrangement regarding the layout of raw materials for steel retrieval has difficulty because the steel is stored randomly without paying attention to its type and is only placed in an empty place. Therefore, it is necessary to design a layout using the dedicated storage and class-based storage methods to improve the warehouse layout by designing a warehouse layout so that it can make it easier to find and minimize steel search time. Results of research carried out obtained that distance travelled using the dedicated storage method is amounting to 7790,85 metres with material handling time 15581,7 minutes, whereas with use class based storage method can be obtained distance 8382,05 metres with material handling time 16764,1 minutes. Concluded that design results Select the selected layout is with use possible dedicated storage method reduce distance travel and time at PT. BSB warehouse becomes more practice and efficient
