Abstract

Abstrak Pada tahun 2021, Brebes merupakan kabupaten dengan angka prevalensi stunting tertinggi ketiga di Jawa Tengah. Kecamatan Kersana merupakan salah satu lokus penanganan stunting di Kabupaten Brebes, dengan angka prevalensi mencapai 20,89%. Tujuan penelitian adalah untuk mengetahui determinan kejadian stunting pada balita di Kecamatan Kersana, Kabupaten Brebes. Jenis penelitian ini adalah kuantitatif observasional analitik, dengan desain cross-sectional. Penelitian ini dilaksanakan pada bulan September hingga Desember 2022. Jumlah sampel penelitian sebanyak 241 responden, diambil dengan proportionate stratified random sampling. Analisis data dilakukan secara univariat, bivariat dengan uji chi-square, dan multivariat dengan uji regresi logistik. Hasil penelitian menunjukkan bahwa sumber air minum utama (p= 0,018; AOR= 6,34; CI 95%= 1,37 – 29,33), kepemilikan jamban (p= 0,002; AOR= 8,75; CI 95%= 2,15 – 35,65), dan paritas (p= 0,012; AOR= 2,46; CI 95%= 1,21 – 5,00) memiliki hubungan yang signifikan dengan kejadian stunting pada balita. Faktor terkuat yang berhubungan dengan kejadian stunting pada balita di Kecamatan Kersana adalah jamban tidak layak. Abstract In 2021, Brebes is the third highest stunting prevalence rate in Central Java. Kersana District is one of the locus for handling stunting in Brebes Regency, with a prevalence rate of 20.89%. This study aimed to determine the determinants of stunting among toddlers in Kersana District, Brebes Regency. This study using quantitative observational analytic, with a cross-sectional design. This study was carried out in September to December 2022. The total sample size was 241 respondents, taken by proportionate stratified random sampling. Data analysis was performed univariately, bivariately using the chi-square test, and multivariately using the logistic regression test. The results showed that the main source of drinking water (p= 0.018; AOR= 6.34; 95% CI= 1.37 – 29.33), ownership of latrines (p= 0.002; AOR= 8.75; 95% CI= 2 .15 – 35.65), and parity (p = 0.012; AOR = 2.46; 95% CI = 1.21 – 5.00) has a significant relationship with the incidence of stunting in toddlers. The strongest factor related to the incidence of stunting among toddlers in Kersana District is inadequate latrines.

Abstract

Background: The COVID-19 pandemic has significantly altered children's daily routines, particularly increasing screen time, decreasing physical activity, and changing sleep duration, which may impact their physical and mental health. It is important to know how changes in physical activity and screen time can affect children's sleep duration before, during and after the COVID-19 pandemic. Methods: This study used a quantitative analytical observational cohort study method with 240 child samples aged 6–12 years in Semarang City. The sampling technique used simple random sampling. The research instrument used the SMALLQ® questionnaire. Data analysis techniques used normality tests, Spearman-Rank tests to determine correlations, and Kruskal-Wallis tests to compare data. Results: The correlation between screen time and sleep duration was not statistically significant (p>0.05), while physical activity showed a significant positive correlation with sleep duration (r = 0.253 on weekdays, r = 0.21 weekly, p<0.05). Significant differences were found in screen time, physical activity, and sleep duration across the three periods (p<0.001), with increased screen time and decreased physical activity and sleep duration persisting post-pandemic.  Conclusions: The pandemic led to lasting increases in screen time and decreases in physical activity and sleep duration among children. Promoting healthier routines is essential to mitigate these impacts in the new normal era.

Abstract

Equal access to health services remains a challenge in achieving Universal Health Coverage (UHC), including in Indonesia. One of the efforts to improve access and the quality of services at Primary Health Care Facilities (PHCFs) is the implementation of the Performance-Based Capitation (PBC) system, which includes indicators such as the Contact Rate, Non-Specialist Referral Ratio, and Controlled Prolanis Participant Ratio. However, the achievement of these indicators has not yet been fully optimized. In 2022, national data showed that the Controlled Prolanis Participant Ratio indicator reached only 3.78% of the ≥5% target. Meanwhile, in the first semester of 2024, the average Contact Rate achievement at BPJS Kesehatan Semarang was only 149.14‰, slightly below the ≥150‰ target. Efforts to optimize PBC performance require in-depth analysis of the factors that influence it, including the characteristics of PHCFs. This study aims to analyze the relationship between the characteristics of PHCFs and PBC indicators. This research is a quantitative analytical observational study with a cross-sectional design, using PBC indicators data from the second semester of 2024 obtained from BPJS Kesehatan Semarang. The sample includes all PHCFs that are contracted partners and receive capitation payments from BPJS Kesehatan Semarang. Data were analyzed using univariate, bivariate, and multivariate analysis with SPSS Statistics. PHCF characteristics that were significantly associated with PBC indicators (p < 0.05) included type of PHCF, ownership status, and number of registered participants

Abstract

Recently, the use of digital media has increased significantly. The role of parents is very important in the use of digital media for children. The aim of this study is to determine the correlation between parental digital media and physical play habits with time spent on digital media by preschool children aged 2-6 years in Semarang City, Indonesia. The benefits of this research are as a reference for the use of digital media for parents in child care. The novelty of this research is an analysis of the use of digital media on children's physical activity. It was cross sectional study to 360 parents who had preschool children and voluntarily complete the validated online questionnaire. Data were collected using the Surveillance of digital-Media hAbits in earLy chiLdhood Questionnaire (SMALLQ®). We analyzed the data collected with Spearman test. The result showed that the time spent on digital media by parents positively correlated with time spent on digital media by children on weekday (p value<0.01) and on weekend (p value<0.01). Digital media engagement with children negatively correlated with time spent on digital media by children on weekday (p value: 0.03) and on weekend (p value: 0.02). However, physical play engagement with children do not correlate with time spent on digital media by children on weekday (p value: 0,71) and on weekend (p value: 0,35). Conclusion, time spent on digital media by parents and digital media engagement correlated with time spent on digital media by preschool children aged 2-6 years old.

Abstract

Children under the age of five (the under-five) from low-income families are more vulnerable to experience underweight. This nutritional vulnerability is evident in the preliminary study, where 35.1% of the under-five experience underweight, and 28.48% are low-income families. This study aimed to explore Positive Deviance (PD) behaviors in preventing underweight among the under-five. The study applied a qualitative approach with a case study design. Data collection took place in July-August 2022, focusing on low-income families in the Gunung Brintik area. Data were collected through two focus group discussions, seven in-depth interviews, and five key informant interviews. Coding, subtheme, and theme development were facilitated using the ATLAS.ti trial package. Triangulation of the study results yielded four major themes influencing the prevention of underweight in the under-five: interventions by health workers and cadres, community awareness and concern, family support, and maternal nutrition literacy. The identified PD behaviors practiced by the community in the study location have been shown to contribute to the prevention of underweight in the under-five. Strengthening these identified PD behaviors and their implementation in other areas is essential to support efforts to combat underweight in the under-five.

Abstract

Maternal mortality is a global-scaled health problem to indicate how good a health system is in a region. Indonesia reached 305 per 100.000 live birth as per 2017. That rate is still far from SDGs 2030 which suggests that every country has accomplished 70 per 100.000 livebirth. There are factor risks that caused maternal mortality. Those factors can be divided into individual character-related factors, socioeconomy-related factors, and healthcare-related factors. This study is an analytical study using cross-sectional design data from the DHS program 2017. Data were analyzed using bivariate analysis and multivariate analysis to fiigure out which variables that associate with  maternal mortality. Preceding birth interval, succeeding birth interval, and twin birth status are the three main factor risks that associate with maternal mortality. It is recommended that Indonesian family needs to participate in family planning, including birth control in order to control the factor risks of maternal mortality.

Abstract

Background: The increase in the dengue mortality rate in Indonesia and its risk factors which must be addressed immediately. Brebes Hospital, a hospital that is the primary referral for health centers and clinics in Brebes Regency, has a dengue mortality rate (Case Fatality Rate) of 7.88%, contributing to the first death rate in Brebes Regency. Data related to dengue deaths, such as comorbidities and hematocrit levels in dengue patients, were unavailable at the provincial and Brebes district levels, especially at Brebes Hospital. This study aims to determine the determinants of dengue death in inpatients at Brebes Hospital. Methods: This cross-sectional study used medical record data of dengue inpatients from September 2021 to August 2022, with as many as 138 respondents using a simple random sampling technique. The research instrument is an observation sheet, including age, gender, incidence of thrombocytopenia, hypotension, hematocrit levels, hemoglobin levels, diabetes mellitus status, hypertension status, comorbidities, and LOS. The data were analyzed in univariate, bivariate with the chi-square test, and multivariate with a logistic regression test. Results: The bivariate analysis results showed that the variables of hypertension status, DM status, thrombocytopenia, hematocrit levels, and the presence of comorbid diseases were associated with dengue mortality (p<0.01). Multivariate analysis showed that after controlling for other variables, the risk of comorbid diseases was significant 27.54 times (p-value <0.001; AOR= 27.54; 95% CI= 7.21-105.07), and hematocrit levels were at risk of 4.96 times increasing dengue mortality (p-value = 0.005; AOR= 4.69; 95% CI= 1.59-13.83). Conclusion: Hematocrit levels and the presence of comorbid diseases are critical determinants of dengue case mortality. The central comorbid diseases are hypertension, diabetes, and stroke. These two factors must be the primary concern of the clinicians in charge of patients in anticipation of an emergency.

Abstract

Abstrak Data Dinas Kesehatan Kota Semarang tahun 2019 menunjukan bahwa masih banyak kasus TB di Kota Semarang yaitu sekitar 43,38%. Data Puskesmas Halmahera Kota Semarang menunjukkan bahwa pada tahun 2019 pasien TB paru yang sembuh sebanyak 38%, Puskesmas Bangetayu sebanyak 57%, dan di Puskesmas Genuk sebanyak 43%. Tujuan penelitian ini adalah untuk mengetahui hubungan riwayat asupan vitamin A dan D dengan kesembuhan Tuberkulosis di Semarang. Jenis penelitian ini merupakan penelitian observasional dengan rancangan case control dengan teknik two stage cluster sampling. Sampel penelitian sebesar 38 responden kasus dan 38 responden kontrol. Instrumen penelitian menggunakan kuesioner, alat antropometri (timbangan injak dan microtoice), serta lembar food frequency questioner (ffq). Analisis data penelitian dianalisis menggunakan uji deskriptif, uji Chi Square, dan regresi logistik. Hasil penelitian menunjukan bahwa ada hubungan antara riwayat asupan vitamin A (p = 0,002), riwayat asupan vitamin D (p = 0,005), dan kepatuhan minum obat (p = 0,001) dengan kesembuhan tuberkulosis.

Abstract

Proporsi BBLR di Kabupaten Kendal mengalami tren kenaikan sejak tahun 2016 yakni 4,16% pada 2016; 4,2% pada 2017; dan 4,4% pada 2018. Riwayat BBLR diduga berkaitan dengan tingkat aktifitas fisik yang lebih rendah di kemudian hari yang diukur dengan perkembangan motorik kasar pada anak usia dini. Penelitian ini bertujuan ini mengetahui hubungan BBLR dengan masalah perkembangan motorik kasar anak usia 12-24 bulan. Sampel dalam penelitian ini dibedakan menjadi 2 kelompok, yaitu kelompok kasus dan kontrol dengan perbandingan 1:3. Analisis data dilakukan secara bivariabel, dengan uji Chi-square dan multivariabel, dengan uji regresi logistic. Hasil menunjukan bahwa variabel berat badan lahir dan variabel status ibu bekerja berhubungan dengan perkembangan motorik kasar dengan nilai p berturut-turut 0,001 (OR: 5,66, CI 95%: 2,028-15,791) dan 0,007 (OR: 4,12, CI 95%: 1,472-11,527). Anak yang memiliki riwayat berat badan lahir rendah berisiko 5,66 kali lebih besar mengalami masalah perkembangan motorik kasar dibandingkan dengan anak yang memiliki riwayat berat badan lahir normal, setelah mempertimbangkan status ibu bekerja. Abstract

Background: Tuberculosis is an infectious disease which is the main cause of death worldwide caused by the bacterium Mycobacterium tuberculosis. This bacterium can infect children to adults. According to data from the Semarang City Health Office for 2022, Semarang City is one of the regions in Indonesia that has played a role in realizing national tuberculosis cases with a cure rate that is still far from the global target, namely 78,95% in 2020 and 71,72% in in 2021. The purpose of this study was to determine the factors that can affect recovery in pediatric tuberculosis patients in Semarang City.Methods: The type of research used was analytic observational with a retrospective cohort study design. This study used secondary data from the SEMAR BETUL system from January 2021-September 2022.Results: The results showed that ownership of the supervisor swallowed the drug (PMO) (p<0.001; AOR= 13.997, 95% CI= 8.728-22.446) and house humidity variable (p= 0.002; AOR= 2.059. 95% CI= 1.290-3.287) effect on the recovery of pediatric tuberculosis patients in the city of Semarang.Conclusion: It is known that the supervisor ownership variable for ingesting drugs is the strongest factor influencing the recovery of pediatric tuberculosis patients in Semarang City.
