Abstract

The prevention and control of dengue fever with the eco-health approach are crucial especially at the household level. However, the implementation is still limited and has continued to experience several obstacles. This study aimed to analyze the eco-health-based dengue vector control at the household level, identify the factors influencing decision-making, and assess household readiness towards the implementation. The study was conducted in Bandarharjo Village, the coastal area of Semarang City and the samples were residents who had settled for at least six months, selected using the purposive sampling technique. Furthermore, the household survey used a mixed method with quantitative and qualitative approaches. Data were collected through a Google Form, which consists of a questionnaire, an observation checklist by respondents, and interview guidelines with open- ended answer questions. The data were further analyzed with Univariate Analysis and qualitative data were processed with the Uwe Flick method. The results showed that the majority or 65.1% of the community is ready to carry out prevention and control with the eco-health method as indicated by the excellent and good readiness criteria of 14.7% and 50.5% respectively. Social capital and support from stakeholders, as well as community leaders, were found to strengthen community readiness for sustainable eco-health prevention and control of dengue fever.

Abstract

ARI incidence at Pasar Kemis Public Health Center is the highest infectious disease case in last 2 years. The prevalence of ARI at Pasar Kemis Public Health Center was 9.2% for all age groups and 10.9% for children under fives, while in 2020 it was 8.9% for all age groups and 5.3% for children under fives. This shows that ARI cases in 2020 decreased compared to 2019. The purpose of this study was to determine the factors associated with ARI incidence in children under five. This type of research is analytic observational with a case-control design by taking primary data in working area of Pasar Kemis Public Health Center. The dependent variables in this study are nutritional status, exclusive breastfeeding, immunization status, family members smoking behavior, use of mosquito coils, garbage burning behavior, ventilation area, residential density and economic status and the independent variable was ARI incidence. The sample size used was 33 case samples and 66 control samples with data inclusion and exclusion techniques. The instruments are questionnaires, roll meters, microtoise, and scales. Data were analyzed by univariate, bivariate, and multivariable with Chi Square technique and processed using SPSS. Result shows that birth weight (p value=0.00; OR=8.62; 95% CI=1.68-44.25) and house ventilation (p value=0.02; OR=3.63; 95% CI=1.13-11.61) correlated with ARI incidence in children under five. Factors that most correlated is birth weight (p value=0.02; OR adj=5.55; 95% CI=1.23-24.92). Suggestion in this research is to pay attention to pregnant women’s nutrition so that they don’t give birth with low birth weight and pay attention housing health.

Abstract

Background: COVID-19 has become a public health challenge in Sukoharjo Regency, as its cumulative cases reached 15,258 confirmed cases with 1,380 deaths (CFR 9.04%). Spatial and temporal analysis can provide an overview of the spatial and temporal factors associated with the disease and explain the analysis of the disease distribution in a population to clarify the transmission mechanism. Purpose: This study aimed to provide an overview of the spatial and temporal distribution of COVID-19 cases in the Sukoharjo Regency and determine the spatial autocorrelation of the distribution of COVID-19 cases in the Sukoharjo Regency. Methods: This was an observational study with an ecological design. The data used was Secondary data collected from the Health Office of Sukoharjo, with the population of all COVID-19 confirmed cases recorded by the Health Office of Sukoharjo from 2020 to 2021. The sample was 15,528 patients. Results: The highest number of COVID-19 cases in Sukoharjo Regency was found in the Baki Sub-district (242.23/10,000 people). In comparison, the lowest number of cases was found in the Polokarto Sub-district (114.60/10,000 people). The Sukoharjo Regency experienced two waves of COVID-19, and its peak occurred in July 2021. The results showed spatial dependence in the COVID-19 case distribution with a Moran'sI value of 0.36, z-score of 7.50, and p-value <0.01. Conclusion: The highest number of COVID-19 findings occurred in July 2021, and there was spatial autocorrelation in the distribution of COVID-19 cases in the Sukoharjo Regency with a clustered transmission pattern.

Abstract

Desa Ngabean merupakan salah satu desa di daerah urban yang sangat membutuhkan intervensi kesehatan, seperti IMD, ASI eksklusif, dan MP-ASI, terutama kepada masyarakat yang sebagian besar masih berpendapatan rendah. Tujuan dari kegiatan pengabdian ini adalah adanya peningkatan pengetahuan dan ketrampilan motivator ASI tentang pemberian IMD, ASI eksklusif, dan MP-ASI.Metode pelaksanaan kegiatan adalah pembentukan motivator ASI, pelatihan motivator ASI, pembuatan media bagi motivator ASI, penyusunan buku panduan bagi motivator ASI, dan simulasi motivator ASI dengan ibu hamil, ibu menyusui, dan ibu balita.Hasil dari kegiatan ini adalah terbentuknya motivator ASI disetiap dusun Desa Ngabean. Motivator ASI tersebut dilatih tentang pemberian makanan Bayi dan Anak (PMBA) kerjasama Tim pemngabdi dengan AIMI Jateng. Selain itu telah disusun media booklet dan buku panduan bagi motivator ASI. Simulasi peran motivator ASI untuk memotivasi ibu hamil dan ibu menyusui dalam memberikan ASI Ekslusif dan MP-ASI sesuai usia anak serta ASI sampai 2 tahun.

Abstract

Background: Brebes Regency was ranked first with the highest TB incidence in Central Java in 2020. In 2021 the trend of TB incidence increased and the trend of Success Rate decreased from the previous two years. The study aims to determine the determinants of successful treatment among new TB patients in Brebes Regency. Method: The study design used was cross-sectional with the subject of new drug-sensitive TB patients in SITB from January 2020 to June 2022. A sample of 565 respondents was taken by simple random sampling. Data were analyzed univariately and bivariately with chi-square and Fisher's tests as well as multivariably using logistic regression test. Results: Among 565 new TB patients included in this study, 65,1% had successful TB treatment outcomes. In multivariable logistic regression analysis, the categorical variable of pulmonary TB patients (AdjOR: 0,27; 95% CI: 0,08-0,85; p=0,03), ratio of doctors (Q2) (AdjOR: 0,39; 95% CI: 1,18-0,87; p=0,02), microscopic test status at diagnosis (AdjOR: 2,09; 95% CI: 1,08-4,06; p=0,28 ), duration of treatment Q1 (AdjOR: 9,40; 95% CI: 4,27-20,69; p<0,001), Q2 (AdjOR: 0,08; 95% CI: 0,01-0,43; p<0,001), Q3 (AdjOR: 0,05; 95% CI: 0,01-0,30; p<0,001), and negative smear microscopic test results at month 2 after initiation of treatment (AdjOR: 9,80; 95% CI: 2,10-45,57; p<0,001) has a significant relationship to the successful TB treatment. Conclusion: Category of pulmonary TB, ratio of doctors 4,4-8,8 per 100,000 population, microscopy test status at diagnosis, length of treatment less than 7 months 11 days-12 months, and negative smear microscopic test results at the end of the 2nd month after the start treatment were determinants of successful treatment among new TB patients in Brebes Regency.

Abstract

Background: Until now, COVID-19 is still a major global health problem. Even though Brebes Regency is not the regency with the highest number of COVID-19 cases in Central Java, Brebes Regency has not been spared from a spike in COVID-19 cases. The active case of COVID-19 in Brebes Regency in 2021 is 0.43% with a CFR of 10.98%. It is proven that active cases of COVID-19 have increased from the previous year which was only 0.05%. This research aimed to determine the factors associated with COVID-19 deaths in Brebes Regency. Method: The study design was a retrospective cohort using secondary data from Brebes Regency Health Office 2021. This research was conducted in September-November 2022. The number of samples studied was 10,374 samples. Data were analyzed using univariate, bivariate (chi-square and fisher), and multivariate (logistic regression) analysis. Results: The results showed that age ≥50 years (OR: 3.74; 95% CI: 3.28-4.26), symptoms of shortness of breath (OR: 2.84; 95% CI: 2.42-3, 34), heart disease (OR: 3.21; 95% CI: 1.29-7.99) and type of work as a health worker (OR: 0.30; 95% CI: 0.09-0.97) have significant relationship to the death of COVID-19 patients in Brebes Regency. Conclusion: Age ≥50, symptoms of shortness of breath, heart disease and type of work as a health worker increase the risk of death in COVID-19 patients in Brebes Regency.

Abstract

Globally, it is estimated that 10-20% of adolescents experience mental health problems and remains underdiagnosed and untreated. Mental health activities have not been active in Sekaran Public Health Center. Community partnership provide mental health education, training to overcome and prevent mental health problems, and to be peer educators by HOPE (Health Educated, Organized, Practiced, and Empowered) method. Twelve adolescents were screened by Strengths and Difficulties Questionnaire (SDQ). All youth cadres had normal difficulty scores, emotional symptoms, and hyperactivity. Two adolescents have threshold category in behavior problems, an adolescent has threshold category in peer problems, and an adolescent has abnormal category in proposocial behavior. There is a difference in mental health knowledge between before and after education is given (p=0,014). It is important for health facilities to regularly provide education and empowerment of adolescents in mental health. This HOPE method can be implemented for other health facilities.

Abstract

Kemiskinan merupakan akar dari penyebab masalah gizi kurang. Wilayah Gunung Brintik Kelurahan Randusari Kota Semarang merupakan wilyah kantung kemiskinan di pusat Kota Semarang dengan jumlah penduduk miskin sebesar 28.48% serta prevalensi gizi kurang sebesar 60%. Penelitian ini dilakukan untuk menganalisis faktor risiko gizi kurang serta menganalisis literasi gizi ibu sebagai efek modifikasi kejadian gizi kurang pada balita. Penelitian dilakukan dengan desain cross sectional melibatkan 97 balita di wilayah Gunung Brintik RW III dan IV Kelurahan  Randusari Kota Semarang. Pengumpulan data dilakukan dengan observasi dan wawancara responden ibu balita. Analisis bivariat dilakukan menggunakan uji chi square, sedangkan analisis multivariat menggunakan multiple logistic regression. Hasil penelitian menunjukkan terdapat 12 variabel yang memiliki asosiasi dengan kejadian gizi kurang pada balita, yaitu jenis kelamin, riwayat IMD, riwayat imunisasi, riwayat penyakit infeksi, asupan energi, asupan protein, asupan lemak, asupan karbohidrat, pendidikan ibu, pendidikan ayah, pendapatn keluarga, dan literasi gizi. Hasil analisis multivariat menunjukkan adanya interaksi antara pendapatan keluarga dengan literasi gizi sebagai variabel interaksi (nilai p: 0,044). OR adjusted pendapatan keluarga dan literasi gizi terhadap kejadian gizi kurang diperoleh OR = 2,37 (1,07-9,38). Keluarga yang mampu secara ekonomi, tetapi memiliki literasi gizi yang kurang berisiko sebesar 2,37 kali lebih besar untuk memiliki balita yang mengalami gizi kurang daripada keluarga yang mampu secara ekonomi dan memiliki literasi gizi yang baik. Dengan demikian, efek risiko pendapatan keluarga yang kurang terhadap kejadian gizi kurang termodifikasi oleh literasi gizi sebagai effect modifier.

Abstract

Latar Belakang: Gagal jantung kongestif menjadi suatu ancaman dan tantangan tersendiri bagi dunia kesehatan dikarenakan prevalensi dan kasus kematian yang terus meningkat. Pada tahun 2020, prevalensi kasus dan proporsi kematian akibat gagal jantung kongestif di RSUD dr. R. Goeteng Taroenadibrata Purbalingga yaitu 5,5% dan 10,6%. Tujuan penelitian ini adalah untuk mengetahui variabel-variabel yang mempengaruhi risiko kematian pasien gagal jantung kongestif. Metode: Penelitian ini merupakan penelitian analitik observasional dengan desain penelitian kohort-restrospektif. Sampel dalam penelitian ini adalah pasien dengan diagnosis utama gagal jantung kongestif yang tercatat dari bulan Januari-Desember 2020 dan memiliki data rekam medis yang lengkap. Total sebanyak 84 sampel digunakan dan terdiri atas 42 pasien gagal jantung kongestif meninggal dan 42 pasien gagal jantung kongestif membaik. Instrumen yang digunakan adalah lembar observasi. Proses analisis data dilakukan dengan aplikasi SPSS Statistic 20 menggunakan uji chi-square dan regresi logistik. Hasil: Terdapat 3 variabel yang secara signifikan tetap mempengaruhi risiko kematian pada gagal jantung kongestif bahkan setelah mempertimbangkan variabel lain, yaitu status anemia (p=0,01; AdjOR=3,80; IK 95%=1,31-11,04), denyut jantung (p=0,03; AdjOR=3,35; IK 95%=1,81-10,35), dan status trombositopenia (p=0,01; AdjOR=17,35; IK 95%=1,99-151,26). Kesimpulan: Kondisi trombositopenia pada pasien gagal jantung kongestif memiliki kontribusi paling kuat meningkatkan risiko kematian pasien gagal jantung kongestif.

Abstract

Jumlah kasus hipertensi di Kabupaten Klaten pada tahun 2019 tercacat sebanyak 134.312 kasus (Prevalence Rate 10,66%) kemudian turun menjadi 102.089 kasus (Prevalence Rate 8,10%) pada tahun 2020. Tujuan dari penelitian ini adalah untuk meramalkan jumlah kasus hipertensi di Kabupaten Klaten tahun 2021 – 2023. Jenis penelitian ini adalah penelitian data sekunder non-reaktif yang dilaksanakan pada bulan September 2021 s.d November 2021. Variabel bebas penelitian ini adalah data kasus hipertensi Kabupaten Klaten tahun 2017 – 2020 dengan data kasus hipertensi Kabupaten Klaten tahun 2021 – 2023 sebagai variabel terikat. Sampel dari penelitian ini adalah data kasus hipertensi bulanan yang tercatat oleh Dinas Kesehatan Kabupaten Klaten tahun 2017 – 2020. Hasil menunjukkan bahwa model ARIMA (0,1,1) layak digunakan untuk melaramalkan jumlah kasus hipertensi dengan memenuhi syarat asumsi stasioneritas data, p-value signifikan (<0,01), white noise, residual normal, MAPE layak (24,33%), namun tidak memiliki MS terkecil. Hasil peramalan menunjukkan adanya tren kenaikan kasus dengan rata-rata kasus tahun 2021, 2022, dan 2023 secara berturut-turut adalah 10.356 kasus, 11.206 kasus, dan 12. 227 kasus. Kesimpulan dari penelitian ini adalah model ARIMA (0,1,1) layak digunakan dalam peramalan kasus hipertensi di Kabupaten Klaten tahun 2021 – 2023.
