Abstract

Post-claim audits conducted by BPJS Kesehatan serve as a supervisory mechanism to ensure accountability in the management of the National Health Insurance (JKN) funds. However, audit practices that result in claim corrections or deductions may cause financial losses to hospitals and raise concerns regarding procedural and distributive justice. This study aims to analyze the implications of post-claim audits on hospital financial losses from the perspective of accountability and the principles of justice at RSU Grandmed. This research employed a mixed-method approach with a cross-sectional design involving 97 respondents selected through purposive sampling. Quantitative data were analyzed using multiple linear regression, examining variables such as transparency of the claim process, auditor accountability, tariff adequacy, timeliness of payment, and allocation of claim funds. The findings indicate that, partially, transparency of the claim process, tariff adequacy, timeliness of payment, and allocation of claim funds significantly influence the hospital’s financial condition, while auditor accountability was not significant in the bivariate analysis. However, simultaneously all variables demonstrated a significant effect on hospital financial losses and financial sustainability (R² = 0.693). These findings demonstrate that post-claim audits are not merely administrative mechanisms but have direct implications for hospital financial stability. Therefore, audit mechanisms must be implemented in a transparent, consistent, objective, and justice-oriented manner to maintain a balance between public accountability and the sustainability of health service delivery. Keywords: Indonesia Health Social Security Agency; post-claim audit; accountability; principles of justice; hospital financial loss.

Abstract

Queues occur when service demand exceeds facility capacity, causing patients to wait to receive care. Previous studies have shown that the implementation of a digital queuing system can reduce patient waiting times and improve efficiency as well as satisfaction with hospital services. This study aimed to analyze the effect of digital queuing service quality on the efficiency of patient waiting times at TK.IV 01.07.02 Binjai Hospital in 2025. This research used a quantitative method with a cross-sectional design. Sampling was conducted using the accidental sampling method. The sample in this study consisted of 113 patients who visited TK.IV 01.07.02 Binjai Hospital. The study was carried out at TK.IV 01.07.02 Binjai Hospital. The results showed that all dimensions of digital queuing service quality namely tangibles, reliability, responsiveness, assurance, and Empathy  had a significant effect on the efficiency of patient waiting times at TK.IV 01.07.02 Binjai Hospital in 2025 (p < 0.05). The tangibles aspect, such as the comfort of waiting rooms and digital facilities, helped accelerate service processes, while reliability and responsiveness improved the accuracy and speed of the system and staff in handling patients. The assurance aspect fostered patients’ sense of safety and trust in staff professionalism, while Empathy  encouraged humane communication and facilitated smoother service flow.Overall, improvements in all five aspects were proven to shorten waiting times and enhance service efficiency in hospital outpatient clinics. The conclusion of this study is that all dimensions of digital queuing service quality reliability, responsiveness, assurance, and Empathy  significantly affect the efficiency of patient waiting times at the hospital. Among the five aspects, Empathy  was identified as the most dominant factor in improving the efficiency of patient waiting times.

Abstract

The implementation of Minimum Service Standards (MSS) in the health sector is an obligation of local governments to ensure the provision of basic health services for the community. This study aims to analyze the implementation of MSS at Mersam Community Health Center, Batanghari Regency, in 2025, viewed from the aspects of communication, resources, disposition, and bureaucratic structure, as well as to identify the supporting and inhibiting factors. This research employed a qualitative approach with a descriptive method through interviews, observations, and documentation. The results indicate that the implementation of MSS has been carried out in accordance with existing regulations; however, it has not been fully optimal. The main obstacles include limited health personnel, inadequate facilities and infrastructure, high workload, and low public awareness of preventive services. Supporting factors include leadership commitment and support from the Health Office. The strategies implemented involve improving coordination, optimizing available resources, and strengthening public outreach. The findings emphasize the importance of strengthening resource capacity and enhancing cross-sectoral coordination to ensure that MSS implementation becomes more effective and sustainable in improving the quality of basic health services at the community health center level.

Abstract

Background: HIV/AIDS remains a serious global health problem because it attacks the immune system and can progress to AIDS without regular antiretroviral (ARV) therapy. Globally, more than 37 million people are living with HIV, and Indonesia is among the countries with relatively low ARV coverage, with only around 10% of people living with HIV/AIDS (PLWHA) receiving treatment. In North Sumatra, Tebing Tinggi City is one of the areas experiencing an increase in cases; however, the utilization of HIV testing services and adherence to ARV medication remain low. A preliminary study at RSUD Dr. H. Kumpulan Pane showed that lack of information, motivation, and behavioral skills caused some patients to be inconsistent in undergoing therapy. Objective: This study aimed to analyze adherence to ARV medication among PLWHA at RSUD Dr. H. Kumpulan Pane, Tebing Tinggi City, in 2025. Methods: This study used a quantitative design with a cross-sectional approach. Results: The results showed that information, motivation, and behavioral skills had a significant effect on adherence to ARV medication among PLWHA at RSUD Dr. H. Kumpulan Pane, Tebing Tinggi City, in 2025. The better the information received, the higher the patient’s motivation, and the better the behavioral skills in managing therapy, the higher the patient’s adherence to ARV treatment. Conclusion: Information, motivation, and behavioral skills are important determinants that contribute to the success of ARV therapy among PLWHA.

Abstract

BPJS Kesehatan Smart Claim implementation at Santa Elisabeth Hospital Medan has progressed to Star 4, including online pharmacy bridging or e-prescription integration. This study aimed to analyze the effect of this implementation on service efficiency and effectiveness. It used a qualitative approach with an intrinsic case study design. Eighteen informants were selected through purposive-homogeneous sampling. Data were collected from August to December 2025 through semi-structured in-depth interviews, limited participatory observation of the end-to-end service and claim workflow, and document review. The data were analyzed thematically using methodological and theoretical triangulation. The findings show that Smart Claim improves data access, supports partial paperless processes, and accelerates service and claim processes under certain conditions. The claim cycle is perceived to be shorter, and pharmacy processing becomes faster after e-prescription integration. Tangibles and responsiveness tend to improve, whereas reliability and assurance still fluctuate during network and bridging disruptions, slow devices, and data mismatch or mapping problems that trigger rework, cross-unit escalation, and pending or returned claims. This study concludes that Smart Claim improves service efficiency and effectiveness conditionally, depending on infrastructure stability, staff readiness, and claim data governance.
