Abstract

The Emergency Department (ED) is an essential unit within hospital services that plays a crucial role in handling patients experiencing emergency situations. The quality of care provided in the ED greatly influences patient safety, the satisfaction of patients’ families, and the overall reputation of the hospital. This study aimed to examine the quality of healthcare services in the Emergency Department of Permata Hati Hospital, Asahan Regency, in 2026 by applying the five SERVQUAL dimensions, namely tangible, reliability, responsiveness, assurance, and empathy. This research used a quantitative method with a cross-sectional study design. The population consisted of family members who accompanied patients in the Emergency Department of Permata Hati Hospital during the research period. A sample of 97 respondents was obtained through probability sampling using an accidental sampling technique. Data collection was conducted using a structured questionnaire, and the data were analyzed using univariate analysis, bivariate analysis with the chi-square test, and multivariate analysis through logistic regression. The findings indicated that the majority of respondents considered the quality of healthcare services in the Emergency Department to be good. The bivariate analysis showed that reliability, responsiveness, assurance, and empathy were significantly associated with the quality of healthcare services, while the tangible dimension did not demonstrate a significant relationship. Furthermore, the multivariate analysis identified responsiveness as the most influential factor affecting the quality of healthcare services in the Emergency Department. In summary, the quality of healthcare services in the Emergency Department of Permata Hati Hospital is determined by several dimensions of service quality, particularly the responsiveness of healthcare professionals. Therefore, hospital management is encouraged to improve service responsiveness, strengthen effective communication, and foster empathetic attitudes among healthcare staff in order to continuously enhance the quality of emergency care services.

Abstract

The implementation of hospital promotion strategies through public relations and brand management at F.L. Tobing General Hospital, Sibolga, is influenced by various inhibiting and supporting factors. The main inhibiting factors include limited marketing budgets, a lack of competent human resources in public relations and brand management, suboptimal use of digital media, intense competition from more aggressive private hospitals, and weak promotional effectiveness evaluation mechanisms. Meanwhile, supporting factors include F.L. Tobing General Hospital's status as a government referral hospital, support from local government policies, opportunities for collaboration with various institutions, and increasing management awareness of the importance of promotion and strengthening brand image to increase hospital competitiveness. This study used a descriptive qualitative approach with triangulation of data sources within the F.L. Tobing General Hospital's work area, involving one key informant and 11 triangulation informants, and data collection techniques through observation, interviews, and documentation. The results showed that the advertising strategy was still dominated by internal media such as banners, posters, and brochures. Inpatient sales promotion is conducted through non-tariff promotions due to regulations, including simplified BPJS and insurance administration, service package information, and supporting facilities. Personal selling through empathetic interactions with healthcare workers, oriented toward patient-centered care, is the most influential strategy in building trust and inpatient care decisions. Direct digital marketing through WhatsApp and a reservation system supports rapid information access and increases patient satisfaction and loyalty. Public relations strategies play a crucial role in building image, encouraging positive word of mouth, and increasing inpatient visits. Continuous improvement of interpersonal communication skills, optimization of digital media, and contextual health education for patients' families are recommended.

Abstract

In the era of globalization characterized by increasing competition, the demand for improved hospital service quality continues to rise. Patients as healthcare service users have become more selective and critical in evaluating the quality of services provided. Therefore, hospitals are required to continuously enhance service quality through innovation and effective management. Service quality is not only determined by the availability of facilities and technology, but is also significantly influenced by the implementation of management functions, including planning, organizing, actuating, and controlling (POAC). The optimal application of these management functions can promote a more efficient, integrated, and patient-oriented service system.  This study employed a quantitative approach with a cross-sectional design to analyze the relationship between POAC management functions and service quality at the Obstetrics Outpatient Clinic of Primaya Hospital Bekasi Utara. The study population consisted of 8,321 patients, with a sample of 90 respondents determined using the Slovin formula. Data analysis included univariate analysis, bivariate analysis using the chi-square test, and multivariate analysis using logistic regression.  The results showed that most respondents rated the implementation of management functions as good, including planning (66.7%), organizing (70.0%), actuating (65.6%), and controlling (74.4%). Service quality was also perceived as good by the majority of respondents (72.2%). Bivariate analysis indicated that all management function variables were significantly associated with service quality (p ≤ 0.001). Multivariate analysis revealed that planning, actuating, and controlling had significant effects on service quality, while organizing did not show a statistically significant effect. Among all variables, planning was identified as the most dominant factor.  In conclusion, the implementation of POAC management functions plays a crucial role in improving service quality, with planning as the most influential factor that needs to be continuously optimized.

Abstract

The implementation of Minimum Service Standards (MSS) serves as a policy instrument designed to ensure equitable quality of primary healthcare services. However, the implementation of MSS in first-level healthcare facilities continues to face various challenges, particularly those related to the readiness and capacity of supporting systems. This study aims to evaluate the implementation of MSS at Dandy Dinda Primary Clinic in Deli Serdang Regency, assess the availability of facilities and infrastructure, and identify the supporting and inhibiting factors influencing the achievement of MSS indicators. This study employed a qualitative approach with a case study design. Data were collected through in-depth interviews, direct observations, and document analysis involving clinic management and healthcare personnel. The data were analyzed thematically by linking empirical findings to systems theory and the Donabedian model, which encompasses input, process, and output components. The findings indicate that MSS implementation at Dandy Dinda Primary Clinic has been carried out; however, it has not yet reached an optimal level. In terms of input, information systems, human resource development, and the availability of facilities and infrastructure remain insufficient to fully support MSS implementation. In the process aspect, the application of standard operating procedures is inconsistent, and quality evaluation has not been conducted routinely nor in a data-driven manner. Meanwhile, in the output aspect, the achievement of MSS indicators varies and has not yet produced a significant impact on improving service quality and patient safety. This study concludes that the success of MSS implementation is more strongly influenced by the robustness of supporting systems rather than individual commitment alone. Therefore, strengthening information systems, continuously enhancing human resource capacity, and implementing follow-up action plans based on quality improvement cycles are essential to improve compliance with standards and achieve MSS indicators optimally.

Abstract

Active participation in BPJS is an important indicator of the successful implementation of the National Health Insurance (JKN) program. Although membership coverage is relatively high, inactive participants are still identified. This study aimed to analyze the influence of knowledge level, attitudes, and socialization on BPJS participation in the working area of Bandar Jaya Public Health Center, Lahat Regency.This study employed a quantitative approach with an analytical observational design and a cross-sectional method. The study population consisted of 9,400 household heads, with a sample of 97 respondents selected using simple random sampling. Data were collected through questionnaires and analyzed using the Chi-Square test and multiple logistic regression at a significance level of 0.05.The results showed that knowledge level was significantly associated with BPJS participation (p = 0.002), attitudes were significantly associated with BPJS participation (p = 0.001), and socialization was significantly associated with BPJS participation (p = 0.000). Multivariate analysis revealed that socialization was the most dominant factor influencing active BPJS participation (OR = 5.12; 95% CI = 1.89–13.86; p = 0.001), followed by attitudes (OR = 4.38; p = 0.004) and knowledge level (OR = 3.96; p = 0.011).It can be concluded that knowledge level, attitudes, and socialization significantly influence BPJS participation, with socialization as the most dominant factor. Therefore, strengthening sustainable and community-based socialization strategies is necessary to increase active BPJS participation.

Abstract

Healthcare professionalism is a fundamental foundation in improving service quality and patient safety in hospitals. This study aimed to analyze the influence of skills, responsibility, ethics, and communication on the professionalism of healthcare workers in the Pediatric Inpatient Unit of Lahat Regional General Hospital, Lahat Regency, South Sumatra, in 2025. This research employed an analytic quantitative design with a cross-sectional approach. The population consisted of 82 healthcare workers, with a sample of 68 respondents selected using stratified random sampling technique. Data were collected using structured questionnaires and analyzed through univariate, bivariate (Chi-Square test), and multivariate (multiple logistic regression) analyses.The results showed that the majority of respondents were categorized as professional (70.6%). Bivariate analysis indicated that skills (p = 0.001), responsibility (p = 0.000), ethics (p = 0.000), and communication (p = 0.018) had a significant influence on professionalism. Multivariate analysis revealed that ethics was the most dominant variable affecting professionalism (OR = 7.25; p = 0.004), followed by responsibility (OR = 5.59; p = 0.009) and skills (OR = 4.26; p = 0.021), while communication was not statistically significant after being controlled with other variables (p = 0.078).In conclusion, ethics, responsibility, and skills are the main factors influencing healthcare professionalism. Enhancing professional ethics training, strengthening responsibility, and continuously developing clinical skills are necessary to improve the quality of healthcare services at Lahat Regional General Hospital.

Abstract

Sistem pembiayaan yang digunakan oleh BPJS kesehatan yaitu sistem Indonesia Case Based Group’s (INA-CBGs) untuk biaya pengganti dari BPJS untuk klaim pembiayaan. Metode operasi caesar menjadi salah satu layanan BPJS yang mengalami peningkatan signifikan dari tahun ke tahun di mana operasi caesar di rumah sakit terus meningkat sehingga pembiayaan juga mengalami peningkatan. Tarif INA-CBGs adalah besaran pembayaran klaim oleh BPJS kesehatan kepada fasilitas kesehatan rujukan tingkat lanjut atas paket layanan yang didasarkan kepada pengelompokan diagnosis penyakit dan prosedur (PMK nomor 52, 2016). Tujuan dalam penelitian ini adalah untuk menganalisis perbandingan biaya rumah sakit dengan paket INA-CBGs pada pembayaran klaim sectio caesarea pasien BPJS di RSU Indah Kota Bagan Batu Riau Tahun 2025. Jenis penelitian ini dilakukan dengan menggunakan pendekatan kualitatif. Teknik pengambilan sampel dalam penelitian ini yaitu menggunakan informan. Teknik pengumpulan data yang utama dalam penelitian ini dilakukan melalui wawancara yang mendalam atau indept interview. Analisa data dalam penelitian ini menggunakan analisa naratif. Hasil analisa menunjukkan bahwa rata-rata biaya pelayanan tindakan Sectio Caesarea di RSU Indah Kota Bagan Batu Riau meliputi komponen obat-obatan, alat kesehatan, jasa medis, dan biaya administrasi. Total biaya aktual yang dikeluarkan rumah sakit lebih tinggi dibandingkan dengan tarif klaim yang diterima melalui sistem INA-CBGs, terutama pada kasus dengan komplikasi medis. Diharapkan Manajemen rumah sakit sebaiknya mengadakan evaluasi berkala terhadap perbedaan antara biaya aktual dan tarif klaim INA-CBGs.
