Abstract

Fatigue adalah kondisi fisiologis yang ditandai dengan penurunan kapabilitas performa fisik dan mental akibat kehilangan tidur atau terjaga berkepanjangan, perubahan irama sirkadian dan beban kerja yang dapat mengganggu kewaspadaan dan kemampuan personil penerbangan dalam mengoperasikan pesawat atau tugas penerbangan dengan selamat. Insiden atau kecelakaan penerbangan dapat timbul akibat kesalahan di tingkat individu (penerbang atau awak pesawat lainnya), dimana fatigue menjadi salah satu faktor penyebab utama.International Civil Aviation Organization (ICAO) telah merekomendasikan Fatigue Risk Management System (FRMS) kepada negara-negara anggotanya, termasuk Indonesia. FRMS ini merupakan suatu sistem adalah suatu sistim pengawasan dan pengelolaan faktor-faktor risiko terkait kelelahan berbasis bukti, yang dilakukan secara berkesinambungan. Di Indonesia saat ini belum banyak maskapai yang menerapkan FRMS. Sebagai upaya lain untuk mengelola fatigue di penerbang, maka dibutuhkan panduan langkah-langkah praktis untuk mengelola faktor risiko fatigue di penerbang, khususnya terkait aktivitas fisik, aktivitas di waktu senggang atau istirahat dan pengaturan waktu tidur.Buku Panduan Praktis Pengelolaan Fatigue bagi Penerbang dengan Aktivitas Fisik, Latihan Fisik dan Waktu Tidur ditargetkan untuk menjadi acuan pengelolaan fatigue bagi penerbang, maskapai dan dapat menjadi rekomendasi bagi pemangku kebijakan untuk dapat dijadikan bagian dari prosedur keselamatan penerbangan di Indonesia.

Abstract

Kondisi pandemi Covid19, membuat perubahan dalam dunia penerbangan saat ini, penggunaan teknologi kamera dapat diaplikasikan untuk tindakan preventif terjadinya penularan penyakit ketika seseorang melakukan perjalanan menggunakan pesawat terbang. Seperti diketahui, pada tahun 2006 lebih dari 2 juta penumpang melakukan perjalanan menggunakan pesawat komersil. Teknologi yang digunakan adalah kamera dalam surveilans penerbangan. Didalam dunia penerbangan teknologi ini digunakan guna meningkatkan efisiensi dan menekan biaya operasional. Penggunaan yang paling sering dilakukan adalah pada sistem keamanan pesawat serta pada proses perawatan pesawat karena sifatnya yang tidak merusak material. Selain itu penggunaan kamera radiasi inframerah juga dapat digunakan pada sistem pengawasan didarat (bandara). Penggunaan kamera radiasi inframerah yang paling sering dilakukan dibandara adalah untuk mengecek suhu calon penumpang yang akan melakukan perjalanan.

Abstract

Kontaminan yang dihasilkan di dalam kabin merupakan hal yang paling penting untuk diperhatikan dan harus dikontrol dimana yang termasuk di dalamnya adalah bau badan manusia, aerosol mikroba, Volatile Organic Compound (VOC), gas karbon dioksida, dan gas karbon monoksida. Teknologi penyaring udara dalam penerbangan menggunakan HEPA dan Karbon Aktif. HEPA dalam penggunaannya telah dimanfaatkan secara luas dalam dunia kerja dan penerbangan untuk menjaga kualitas udara dalam ruangan. Dengan prinsip mekanisme kerjanya filter ini memiliki kelebihan dalam hal menyaring partikel yang berukuran sangat kecil. Karbon aktif dalam bentuk charcoal telah diketahui secara luas dalam efeknya untuk mengadsorpsi sejumlah besar zat toksik seperti VOC dan bau tidak sedap dalam gedung dan kabin.

Abstract

Background: The Swedish Occupational Fatigue Inventory (SOFI) is an instrument to assess a person’s fatigue, which is not limited to workers of a company, but can also be used for the general public. SOFI can determine fatigue from physical, cognitive and psychosocial conditions.1 This study aims to translate the instrument so that it can be used in Indonesia, then assess its validity and reliability.Methods: This study was conducted using a 10-step transcultural adaptation method from ISPOR followed by item validity and reliability tests on 224 workers at a geothermal company in Indonesia. Item-total correlation and Confirmatory Factor Analysis was performed using online version 0.14.3 of JASP statistic program. And for reliability test using SPSS tool.Results: Swedish Occupational Fatigue Inventory (SOFI) has been adapted into an Indonesian version through the ISPOR transcultural adaptation method. All items in the Indonesian version of SOFI which amounted to 20 items proved the validity and reliability of this questionnaire was very good (α-Cronbach 0.969) and the Confirmatory Factor Analysis test showed: Factor model (224, p < 0,001) = 337.649, CFI = 0,958, NFI = 0,924, RFI = 0,909, IFI = 0,958, TLI = 0,950, dan RMSEA = 0,070, 90% CI: 0,060 – 0,081. Fit indices results had acceptable model fits corresponding to the structure of the original instrument. Item-rest correlation range from 0.321 to 0.847, these results reveal that each item-rest correlation is extremely strong.Conclusion: The Indonesian version of SOFI is proven to have good validity and reliability as an instrument for assessing fatigue/work fatigue for workers in Indonesia.

Abstract

Background: Nurse has a risk to get an excessive weight nutritional status that can increase the risk of serious health problems, such as heart disease, hypertension and diabetes mellitus. The intervention of nutrition counseling and diet are expected to lose weight in people with excessive weight nutritional status. This study aimed to determine the effectiveness of nutrition counseling and the implementation of Rendah Energi, Seimbang dan Teratur (REST) diet on weight loss among hospital nurses with excessive weight nutritional status in the hospital.Methods: The study was conducted at Occupational Health Hospital of West Java with 22 nurses as study subjects with excessive weight nutritional status, implemented nutrition counseling intervention, a RESTTM Diet and also used the Bouchard questionnaire, OSI-RTM questionnaire and food record sheets.Results: The greatest average weight loss after receiving nutrition counseling and implementing a RESTTM Diet occurred in the 12th week of the last observation. It was 2.6 kg with 95% CI=1.3-3.9 kg. Based on the bivariate test showed gender had a significant effect on the average weight loss. The male nurses had greater average weight loss than female nurses (p=0.038). The average weight loss of male nurses was 3.1 ± 1.7kg and female nurses 1.6 ± 1.3kg. Meanwhile, the effect of occupational factors on weight loss after receiving nutrition counseling and implementing the RESTTM Diet did not show a significant effect, such as job position (p=0.948), the number of working hours (p=0.220), work shift schedule (p=0.692) and work stress (p=0.813).Conclusions: There was an effect of nutrition counseling and RESTTM Diet during 12 weeks on weight loss among the hospital nurses with excessive weight nutritional status in the hospital.

Abstract

Introduction: Pilots have job characteristics that are at risk of causing fatigue. Since fatigue is not a disease, it is important to manage the risk factors. We developed a self-assessment application to manage the fatigue risk for pilots, including physical activity and sleeping difficulties. This application aims to increase pilots’ awareness as one of the health maintenance measures.Methods: The application was developed in collaboration with Garuda Indonesia Training Centre (GITC) to ensure suitable educational content for pilots. The users were asked to fill in data inquiring about their flight characteristics, the Checklist Individual Strength-20 for fatigue screening, General Physical Activity Questionnaire for physical activity, and Jenkins Sleep Scale for sleep difficulties. Afterward, a recommendation will be given based on the received input.Results: The application was able to screen fatigue risks, physical activities, and sleeping difficulties, and provide recommendations based on the questionnaire that was filled in, all adjusted to the user’s work characteristics. The finalized version was then available for download as an Android-based application for the pilots in GITC.Conclusion: The FRAMES is an Android-based self-assessment application for pilots. With the use of this application, hopefully, fatigue can be detected early in pilots, hence counteract measures may be implemented sooner to mitigate fatigue.

Abstract

Psychosocial hazards are a major public health problem, so it is necessary to prevent and manage them so as not to cause mental and physical harm to workers. This study aims to determine changes in work stressors before and during the COVID-19 pandemic and related factors to workers in the PT.X Heavy equipment manufacturing industry. The study used a retrospective cohort design from 2018 and 2021 Medical Check-Up (MCU) data with a Stress Diagnosis Survey (SDS) questionnaire. Research was conducted from October 2021 until July 2022 with samples of 146 obtained. Bivariate analysis using proportion and mean test. The research found that the increase in moderate-severe stressors before and during the pandemic was role ambiguity, role conflict, quantitative workload, and career development. On the other hand, qualitative workloads and responsibility for others decreased during the pandemic. The most worsened was role ambiguity and responsibility for others. Based on the type of production and non-production work, there was a significant relationship to quantitative workload before the pandemic (p=0.043), and not significant during the pandemic. Furthermore, there is a significant relationship with qualitative workload during the pandemic (p = 0.043), and it was not significant before the pandemic. It can be concluded that the Pandemic conditions caused an increase and decrease in work stressors analysed in workers in the heavy equipment manufacturing industry.

Abstract

Introduction: Based on Indonesia’s population data in 2018 and International Diabetes Federation in 2019, an estimated 1 in every 10 Indonesian workers have type 2 diabetes mellitus (T2DM). Diabetes is a chronic illness with costly implications, especially in working population. Workers with diabetes experience 12% reduction in productivity adjusted life years (PALY) and additional $USD 467 monthly average healthcare cost. This study aims to determine glycaemic control in workers with diabetes.Methods: Cross-sectional study utilizing laboratory medical records from 2015-2021 in an occupational clinic in Jakarta’s Central Business District is performed. Data collected through accessing in-clinic laboratory and client lists’ electronic medical records, is paired with their respective anthropometric parameters stored in paper archives. Criteria of Fasting Blood Glucose (FBG), lipid profile, HbA1c and Body Mass Index (BMI) are based on PERKENI 2019. Inclusion criteria are workers with prior history of T2DM who underwent HbA1c examination.Results: 2,796 employees who underwent HbA1c examination were found; 65.8% normal, 29.6% pre-diabetic, and 4.6% exceeded T2DM cut-off. Amongst 2,796 employees, 1,322 had their FBG examined; 62% normal, 33.1% had increased fasting glucose and 5% exceeded T2DM cut-off; another 1,316 also had their lipid profile examined, in which 80.5% had dyslipidaemia. From 2015-2021, 160 employees had a history of T2DM, of which 86 had matching laboratory medical records, with 42 of those undergoing HbA1c examination. Of 42 respondents with diabetes who underwent HbA1c examinations, 54.8% had sub-optimal glycaemic control. FBG is the only risk factor significantly correlated to optimal glycaemic control (p <0.05).Conclusion: Health programs and policies in the workplace enacted by management and health institutions who offer medical services may play a vital role in the improvement of prevention and intervention of diabetes amongst workers.

Abstract

Introduction: Jobs with shift work patterns, especially irregular ones, can disrupt circadian rhythms and sleep quality, which then impacts cognitive function. Despite its importance, research on cognitive domain vulnerabilities related to shift work patterns is still limited. Sleep quality and cognitive function are critical in the context of hospital health services, where decisions and actions must be taken quickly and precisely to support patient safety. Methods: This research is a cross-sectional observational analytical study to determine the relationship between shift work patterns and impaired cognitive function. To measure sleep quality, the Pittsburgh Sleep Quality Index Indonesian (PSQI-Ina) is used, while cognitive function and cognitive domains were measured using Oxford Cognitive Screen (OCS) Indonesian (OCS-Ina), a cognitive instrument for stroke patients, which has been validated. The minimum sample size in this study was 72 samples. Correlation, principal component analysis, demographic analysis, and regression were used to characterize the relationship between PSQI-Ina, OCS-Ina, and other research variables. Results: A total of 83 health workers met the inclusion criteria and were included in the research. The results showed that 16 respondents (19.3%) experienced impaired cognitive function in the Attention domain and 2 respondents (2.4%) experienced impairment in 2 cognitive domains (Attention and Number Management). The results of statistical analysis showed a significant relationship with health workers who had part-time jobs having a 12.8-fold risk (OR 12.8; 95% CI 1.7-91; p = 0.011) of experiencing cognitive impairment. Then health workers who experience poor sleep quality (with a PSQI value >5) have a 40.3 times greater risk (OR 40.3; 95% CI 2.2-708.1; p = 0.011) of experiencing cognitive impairment. Likewise, health workers with irregular shift work patterns have a 5.4 times risk of experiencing cognitive impairment (OR 5.4; 95% CI 0.1-26.6; p = 0.036). Conclusion: There is a relationship between shift work patterns and impaired cognitive function in the workplace. Hospitals should prioritize ergonomic shift work schedules, emphasizing speed and clockwise rotation, to support the well-being of their healthcare workers.

Abstract

Background: Breast cancer is a type of malignancy with the highest number of cases in the world and Indonesia. The C-reactive protein-albumin ratio (RCA) is a simple, feasible, and objective breast cancer serum marker representing inflammatory and nutritional status. There are not many studies regarding the relationship between RCA and breast cancer metastasis, especially in the advanced-stage case population in Indonesia.Method: Study with a cross-sectional design in 180 breast cancer patients stage III and IV at Cipto Mangunkusumo Hospital, who were diagnosed in 2018 until 2023. Comparative analysis of the RCA values between the groups with and without metastasis was carried out using the Chisquare test. All statistical test results are considered significant if the p-value is 0.05.Results: A receiver-operating characteristic curve (ROC) is a graphical analytical technique used to assess the effectiveness of a binary diagnostic classification method. The ROC area under the curve (AUC) value was 0.713 (IC 95%; 0.638–0.789) and significance 0.001, with the RCA cut-off value was 0.515 with sensitivity 74.4% and specificity 67.8%. The high RCA proportion was 53.3%. There is a significant relationship (p-value 0,001) between RCA level and the risk of metastasis in breast cancer patients. Liver and lung metastases of breast cancer are the most frequent locations. Conclusion: The CRP-albumin ratio has a significant relationship with the incidence of metastasis in breast cancer
