Abstract

Healthy behavior carried out by nurses often does not produce good results due to several factors. The risks that arise due to not behaving healthy nurses can reduce the health condition of nurses and the quality of service. The purpose of this study was to identify the factors that can influence the behavior of healthy nurses. The research method used a cross-sectional design to 356 nurses at the General Hospital in Jakarta. The results of the analysis showed that there was a significant relationship between age 35 years (p = 0.038) and education D3 Nursing/Midwifery (p = 0.034) with healthy nurse behavior. In multivariate analysis, the most influential variables on the behavior of healthy nurses were physical activity (p<0.05; OR = 4.760, 95%CI), spiritual (p=0.211; OR = 1.456, 95%CI) and stress management (p< 0.05; OR = 4.549, 95%CI). Recommendations to improve the behavior of healthy nurses by making healthy nurse programs on a regular basis.

Abstract

Aplikasi optisizer pada smartphone digunakan untuk mengukur panjang badan dan estimasi berat badan pasien anak terutama pada kondisi darurat. Berdasarkan literatur review dari beberapa jurnal yang ditelaah, penelitian ini menggunakan desain uji klinis (clinical trial). Untuk memberikan gambaran tentang kelayakan dan keakuratan aplikasi optisizer pada smartphone dalam menghitung estimasi berat badan pasien anak. Penggunaan aplikasi optisizer pada smartphone memiliki hasil yang akurat, hal ini terbukti dengan melakukan perbandingan pada hasil pengukuran menggunakan smartphone dan data perkiraan berat badan anak yang didapat dari orang tua. Aplikasi optisizer dapat digunakan untuk menghitung estimasi berat badan pasien anak, sehingga hasil pengukuran ini dijadikan data utama untuk menentukan dosis obat yang akan diberikan, namun diharapkan untuk penelitian selanjutnya dapat membuat suatu pembaruan dengan adanya aplikasi yang tidak hanya mengukur berat badan pasien anak, namun hasil akhirnya juga dapat membantu dalam menghitung dosis obat dan penilaian obesitas pada anak.

Abstract

Background: Diabetes Mellitus (DM) is a metabolic disease characterized by an increase in blood glucose levels above normal values. This is caused by impaired glucose metabolism due to insulin deficiency, both relative and absolute. Diabetes that is often found in children is Diabetes Mellitus Type 1 (T1DM), this case is a disease that cannot be cured and requires lifelong treatment. It is estimated that every year 128,900 children and adolescents are diagnosed with Type 1 Diabetes Mellitus (T1DM). Globally in 2019 around 1,110,000 children and adolescents were diagnosed with T1DM. Nearly 80% of diabetes deaths occur in low- and middle-income countries. According to the World Health Organization (WHO), India is the country with the largest number in the world with more than 32 million diabetes mellitus patients and this number is predicted to increase to 79.4 million by 2030.Purpose: To reduce the risk of long-term complications, maintain HbA1C on target, and provide knowledge, education and skills to care for children with T1DM until adulthood independently.Method: This research uses a literature review design, namely collecting and summarizing various results from previous research to analyze literature that has been selected from various sources to form a conclusion on new ideas or ideas. The literature study selection process was adapted from Preferred Reporting Items for Systematic Reviews (PRISMA-ScR). The journals used in this study are journals that discuss topics with three keyword categories, namely, Insulin Pump, Diabetes Mellitus Type, and Pediatrics. Search for articles via online databases, including Scopus, ProQuest, Clinical Key Nursing, Sage, and Google Scholar, published in the last five years between 2019-2023.Results: The results of the study are based on a literature review that the author found that the use of insulin pumps is very effective in children and adolescents. In several developed countries, the use of insulin pumps is funded by the local government, but in several rural areas the use of this technology still faces many obstacles and Also, the price of an insulin pump is not cheap, which is an inhibiting factor for blood sugar instability for T1DM.Conclusion: Children with T1DM need insulin throughout their lives, the use of an insulin pump is very effective in controlling blood sugar. The use of an insulin pump can reduce punctures for daily blood sugar monitoring checks and is able to read the condition of glycemic levels in the blood equipped with a sensor. However, the high price of insulin pumps makes families think twice about buying insulin pumps. Insulin pumps can only be purchased by those with above average economic conditions. In developed countries such as Canada, the use of insulin pumps is funded by the local government. However, in developing countries, low ability and purchasing power combined with difficult access to technology mean that not all T1DM sufferers receive adequate treatment. In Indonesia, not many pediatric patients use insulin pumps, only patients from well-off families can use this technology. Apart from that, this insulin pump is not covered by insurance, either government insurance such as National Health Insurance (JKN) or private insurance. So this problem needs to be of concern to various elements, both health and government. Keywords: Child; Diabetes Mellitus Type 1 (T1DM); Insulin Pump; Therapy.Pendahuluan: Diabetes Mellitus (DM) merupakan penyakit metabolik yang ditandai dengan peningkatan kadar glukosa darah diatas nilai normal. Hal ini disebabkan oleh adanya gangguan metabolisme glukosa akibat kekurangan insulin baik secara relatif maupun absolut. Diabetes yang sering ditemui pada anak-anak adalah Diabetes Mellitus Tipe 1 (DMT1), kasus ini merupakan penyakit yang belum dapat disembuhkan dan memerlukan perawatan seumur hidup. Diperkirakan setiap tahun terdapat 128.900 anak-anak dan remaja di diagnosis Diabetes Melitus Tipe 1 (DMT1). Secara global pada tahun 2019 sekitar 1.110.000 anak-anak dan remaja didiagnosis DMT1. Hampir 80% kematian diabetes terjadi di negara berpenghasilan rendah dan menengah. Menurut World Health Organization (WHO), India adalah negara yang mempunyai jumlah terbesar di dunia dengan lebih dari 32 juta pasien diabetes mellitus dan jumlah ini diprediksikan meningkat menjadi  79.4 juta pada tahun 2030.Tujuan: Untuk menurunkan risiko komplikasi jangka panjang, mempertahankan HbA1C sesuai target, dan memberikan pengetahuan, pendidikan, serta keterampilan untuk merawat anak penderita DMT1 hingga dewasa secara mandiri.Metode: Penelitian ini menggunakan rancangan literature review yaitu melakukan pengumpulan dan merangkum berbagai hasil dari penelitian sebelumnya untuk menganalisis literatur- literatur yang telah dipilih dari berbagai sumber hingga menjadi sebuah satu kesimpulan ide atau gagasan baru.  Proses pemilihan studi literatur diadaptasi dari Preferred Reporting Items for Systematic Reviews (PRISMA-ScR). Jurnal yang digunakan dalam studi ini adalah jurnal-jurnal yang membahas topik dengan tiga kategori kata kunci yakni, Insulin Pump, Diabetes Mellitus Type, dan child. Penelusuran artikel melalui online database antara lain, Scopus, ProQuest, Clinical Key Nursing, Sage, dan Google Scholar yang diterbitkan lima tahun terakhir antara 2019-2023.Hasil: Hasil telaah berdasarkan literatur review yang penulis dapatkan Bahwa penggunaan insulin pump sangat efektif digunakan pada anak-anak dan remaja, di beberapa negara maju penggunaan insulin pump di biayai oleh pemerintah setempat, tetapi di beberapa daerah pedesaan penggunaan teknologi ini masih banyak mengalami kendala dan juga harga insulin pump yang tidak murah menjadi faktor penghambat ketidakstabilan gula darah untuk DMT1.Simpulan: Anak-anak dengan DMT1 membutuhkan insulin sepanjang hidup mereka, penggunaan insulin pump sangat efektif di dalam mengontrol gula darah. Penggunaan insulin pump mampu mengurangi tusukan untuk pemeriksaan monitoring gula darah harian dan mampu membaca kondisi kadar glikemik dalam darah dilengkapi dengan sensor. Tetapi harga insulin pump yang mahal membuat keluarga berpikir ulang membeli alat insulin pump, insulin pump hanya mampu dibeli oleh mereka yang mempunyai ekonomi diatas rata-rata. Di negara maju seperti Kanada penggunaan insulin pump didanai oleh pemerintah setempat. Namun di negara berkembang, kemampuan dan daya beli yang rendah ditambah sulitnya akses teknologi sehingga tidak semua penderita DMT1 mendapatkan perawatan yang memadai. Di Indonesia sendiri belum banyak pasien anak yang menggunakan insulin pump, hanya pasien yang berasal dari keluarga mampu yang dapat menggunakan teknologi tersebut. Selain itu, insulin pump ini tidak ditanggung oleh asuransi baik asuransi pemerintah seperti Jaminan Kesehatan Nasional (JKN) maupun asuransi swasta. Sehingga permasalahan ini perlu menjadi perhatian dari berbagai elemen baik kesehatan maupun pemerintahan.

Abstract

Hyperbilirubinemia is the 5th leading cause of neonatal morbidity. Neonatal jaundice occurs in approximately 60% of newborns. To ensure proper management for preventing and treating jaundice in neonates, an effective screening method is required. The gold standard of bilirubin measurement with Total Serum Bilirubin (TSB), which involves invasive procedures, that can be painfull and carry the risk of infection. The purpose of this study to describe the implementation of smartphone-based methods in the assessment of detection and periodic monitoring of jaundice in neonates, as a non-invasive methods alternative, indirectly reducing invasive procedures. The study employed a literatur review method and presents a literature review of 11 selected journals. It was concluded that the smartphone application-based bilirubin assessment showed a significant correlation with TSB and TcB values. Smartphone-based applications can be considered as a non-invasive screening tool for bilirubin monitoring in neonates. The jaundice screening method based smatphone is expected to be applied, developed and made an algorithm for its implementation in hospital services or outside the hospital.

Abstract

Background: The risk management program will run well with commitment and good management. This study aims to describe the experience of nurse managers in implementing risk management in hospitals. Research methodology: this study used a descriptive qualitative approach. The data collection technique used Focus Group Interviews with 12 nurse managers consisting of 1 Sub Installation Head, 7 Unit Heads, and 4 Team Leaders. Interviews were carried out three times, and each implementation consisted of four participants. Analysis using thematic content and consolidation for reporting qualitative research (COREQ) was used as a guide to research reporting. The research found three inhibiting factors in implementing risk management: 1) Busyness, 2) Lack of knowledge, and 3) Lack of structured monitoring and evaluation. Conclusion: The active participation of nurse managers in implementing risk management is very influential in the successful implementation of risk management in hospitals. Good time management, increasing the level of knowledge, and supported by a systematic risk management monitoring-evaluation system means that risk management will be carried out well

Abstract

Introduction: Improving patient safety attitudes will provide quality care and a safe environment and practice for patients. The attitude of nurses towards patient safety in hospitals still requires improvement and improvement efforts. The assessment of nurses’ attitudes toward patient safety can be used as a reference for preparing programs to maintain quality and patient safety in hospitals. Objectives: This study aimed to determine the relationship between head nurses’ nursing informatics competencies and nurses’ characteristics and attitudes towards patient safety. Methods: A cross-sectional study involving 251 nurses at a hospital in Jakarta was conducted. The Nursing Informatics Competency Assessment Tool and the Safety Attitudes Questionnaire with demographic information were used for data collection. Results: Overall, nurses’ attitudes towards patient safety showed a median value of 141 (80.18% of the maximum value), and the mean value of head nurses’ nursing informatics competency was 99.53 (66.35% of the maximum score). This study showed a significant relationship between head nurses’ nursing informatics competencies and nurses’ attitudes toward patient safety. Nursing informatics competencies, computer literacy, informatics literacy, and informatics management skill significantly influenced patient safety attitudes (p<0.05). Conclusion: Nursing informatics competence is the basis for nurses to respond to the development of health management information systems and technology in modern healthcare environments. Abstract

This study aims to describe the implementation of planned changes in an effort to improve patient safety through optimizing monitoring and evaluation of the implementation of risk management countermeasures in inpatient rooms. The method used is to use a problem solving cycle approach which includes identifying problems, analysis and diagnosis, designing solutions, implementation and evaluation. The assessment was carried out by means of structured interviews, observation, and distributing questionnaires. Implementation of planned changes using Kurt Lewin's change model, namely unfreezing, movement and refreezing. The results of problem identification showed that the implementation of risk management in the inpatient unit was not optimal, so innovation was needed to motivate staff to implement patient safety by distributing online flyers and creating an instrument to monitor the implementation of risk management. In conclusion, monitoring using good monitoring instruments and distributing online flyers can increase nurses' understanding of implementing risk management so that patient safety can be realized. Abstract

Analysis on incomplete could lead to biased estimation when using standard statistical procedure since it ignores the missing observations. The disadvantage of ignoring missing data is that the researcher might not have enough data to conduct an analysis. The main objective of the study is to compare the performance between listwise deletion (LD), mean substitution (MS) and multiple imputation (MI) method in estimating parameters. The performance will be measured through bias, standard error and 95% confidence interval of interested estimates for handling missing data with 10% missing observations. A complete empirical data set was used and assumed as population data. Ten percent of total observations in the population ere set as missing arbitrarily by generating random numbers from a uniform distribution,  . Then, bias of parameter estimates and confidence interval of parameter estimates are calculated to compare the three methods. A Monte Carlo simulation was carried out to know the properties of missing data and investigated using simulated random numbers. Simulation of 1000 sampled data with 20, 50, and 100 observations and each sample is set to have 10% missing observations. Standard statistical analyses are run for each missing data and get the average of parameter estimates to calculate the bias and standard error of parameter estimates for every missing data method. The analysis was conducted by using SAS version 9.2. It was found that the MI method provided the smallest bias and standard error of parameter estimates and a narrower confidence interval compared to the LD and MS methods Meanwhile, the LD method gives a smaller bias of parameter estimates and standard error for small sample size of missing data. And, MS method is strongly recommended not to use for handling missing data because it will result in large bias and standard error of parameter estimates.

Abstract

Pendahuluan: Oksimetri menjadi alat penting dalam diagnosis dan manajemen keperawatan anak-anak yang menderita berbagai kondisi medis. Seiring dengan kemajuan teknologi medis, penggunaan oksimetri telah berkembang pesat dan ditemukan aplikasi yang semakin luas dalam praktik medis anak-anak. Tujuan: Untuk menyajikan tinjauan literatur terkini mengenai perkembangan penggunaan oksimetri dalam diagnosis dan manajemen keperawatan anak serta batasan dan potensi kesalahan dalam penggunaan oksimetri pada bayi dan anak-anak. Pembahasan: Berdasarkan analisis dan review 10 jurnal pilihan, dapat disimpulkan bahwa, oksimetri telah terbukti berkontribusi dalam diagnosis berbagai kondisi medis pada anak-anak. Tinjauan literatur ini juga menyoroti aplikasi oksimetri dalam pengelolaan terapi dan perawatan pasien anak. Namun, terdapat beberapa tantangan yang terkait dengan penggunaan oksimetri dalam praktik medis anak. Rekomendasi: Diharapkan penelitian selanjutnya dapat mengidentifikasi batasan dan tantangan dalam penggunaan oksimetri pada bayi dan anak-anak serta pengembangan teknologi oksimetri yang lebih canggih dan akurat untuk meningkatkan kualitas perawatan medis anak.

Abstract

This study aims to identify problems and develop nurse improvement strategies to speak up in IPCP. The method used in this research is an improvement program based on innovation development with a pilot project approach. The results showed that the characteristics of the respondents in the survey of nurse satisfaction in IPCP conducted during problem identification were that there were more female nurses (91%) than male nurses (9.3%), the age of the most respondents was between 21-30 years. (44%), and age 31 -40 years (33%) or productive age. The education level of most respondents is D3 (83%), the second is Nurses (23%). Respondent's competency level for PK I (25%), PK II (32%), PK III (45%), and PK IV (4%). Most of the respondents who participated in this survey were Associate Nurse (PA) at 91% and Primary Nurse (PP) at 14%. In conclusion, the speak-up method can improve nursing services, the image of the nursing profession, and the achievement of patient safety.
