Abstract

ABSTRAK Retinoblastoma merupakan keganasan pada mata yang sering terjadi pada anak-anak dengan insiden terjadinya berkisar antara 1 dalam 14.000 sampai 1 dalam 20.000 kelahiran. Diagnosis dan penanganan retinoblastoma yang terlambat menjadi salah satu permasalahan yang dapat mengakibatkan terjadinya metastasis ke bagian ekstraokuler, kehilangan penglihatan dan kematian. Pada negara berkembang, termasuk Indonesia penyebab keterlambatan diagnosis retinoblastoma berasal dari orang tua anak yang disebabkan oleh faktor ekonomi dan ketidaktahuan pada penyakit retinoblastoma serta kewaspadaan dari gejala klinis dari retinoblastoma. Kira-kira setengah dari populasi anak yang terdiagnosis retinoblastoma meninggal, diperkirakan karena baru terdiagnosis saat stadium lanjut. Oleh sebab itu perlu dilakukan upaya untuk meningkatkan kesadaran dan kewaspadaan orang tua tentang penyakit retinoblastoma. Penanganan awal retinoblastoma melalui deteksi dini tes lihat merah untuk melihat adanya leukokoria dapat dilakukan di fasilitas layanan kesehatan yang memadai dengan alat sederhana menggunakan oftalmoskop. Kata Kunci : Retinoblastoma, Tes Lihat Merah, Deteksi Dini  ABSTRACT Retinoblastoma is a common eye malignancy in children with an incidence ranging from 1 in 14,000 to 1 in 20,000 births. Delayed diagnosis and treatment of retinoblastoma is one of the problems that can lead to extraocular metastases, loss of vision, and death. In developing countries, including Indonesia, the cause of delay in the diagnosis of retinoblastoma comes from the child's parents due to economic factors and ignorance of retinoblastoma disease and awareness of clinical symptoms of retinoblastoma. Approximately half of the population of children diagnosed with retinoblastoma die, presumably because it was diagnosed at an advanced stage. Therefore, it is necessary to make efforts to increase the awareness and vigilance of parents about retinoblastoma disease. Early treatment of retinoblastoma through early detection, see red to see the presence of leukocoria can be done in adequate healthcare facilities with simple tools using an ophthalmoscope.  Keywords:  Retinoblastoma,  Red Reflex Test, Screening

Abstract

Introduction : Squamous cell carcinoma (SCC) is the second most common skin malignancy after basal cell carcinoma, with an increasing incidence worldwide. Found in various parts of the body, in the eye and periocular area, it may affect the skin of the conjunctiva, cornea, and eyelid. Surgical excision is the preferred mode of SCC treatment. After excision, reconstructive surgery is often required for the best functional and cosmetic results. Early diagnosis and treatment of tumors and follow up care for at least 5 years have a significant impact on patient survival. Case Illustration: Description of two cases squamous cell carcinoma that had undergone reconstructive biopsy and blepharoplasty procedure with glabellar rotation flap technique alone in one case and together with a cheek advancement flap in the second one. In both cases, conjunctival replacement by using lower labial mucosa. Discussion : In both cases tumor affects the margin of the upper eyelid, therefore flap reconstruction is necessary to maintain eyelid function and esthetics. The glabellar flap has the advantages of being a fast surgery, having a good vascular supply, and being able to fill deep abnormalities because it is a thick flap. A cheek lateral rotation flap was designed to avoid unnecessary facial distortion in second case. Conclusion : Glabellar rotation flap alone or together with cheek advancement flap was done to maintain function and physical aesthetic of eyelid. Additional cheek lateral rotation flap could reduce skin tension.

Abstract

Diabetic retinopathy is a complication of diabetes that is one of the top five causes of blindness in those over 50. The standard treatment is pan-retinal photocoagulation, which is effective but has established side effects. Anti-vascular endothelial growth factor (anti-VEGF) therapy becomes an alternative to avoid the side effects caused by laser therapy. This systematic review aims to know the effectiveness of anti-VEGF therapy compared to the pan-retinal photocoagulation laser therapy in patients with proliferative diabetic retinopathy. This review was carried out using a systematic review checklist on PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-Analyses). The articles reviewed were randomized controlled trial articles that met the inclusion and exclusion criteria. Based on inclusion and exclusion criteria, six articles were selected from a total of 215. Patients who received anti-VEGF medication had better visual acuity (positive values), whereas patients who received laser therapy had poorer visual acuity (negative values). Those results are because the laser directs light towards the retina, damaging photoreceptors and retinal cells as well as reducing visual acuity. On the contrary, anti-VEGF prevents damage to retinal endothelial cells and blood leaks in the vitreous by decreasing VEGF expression and thus resulting in improved visual acuity. Anti-VEGF proved to be a more practical alternative therapy in improving visual acuity than pan-retinal photocoagulation for patients with proliferative diabetic retinopathy.

Abstract

Pendahuluan: Kanker kolorektal adalah suatu tumor maligna yang muncul dari jaringan epitel dari kolon atau rektum. Kualitas hidup adalah suatu istilah yang membatasi suatu keburukan dan lebih menekankan pada komponen kebahagiaan dan kepuasan dalam hidup. Penelitian ini dilakukan untuk melihat gambaran kualitas hidup pasien kanker kolorektal dengan stoma temporer yang memuat transformasi fisik, psikososial, dan kecemasan terhadap citra tubuh pasien. Metode: Penelitian ini adalah penelitian analitik observasional dengan pendekatan cross sectional. Penentuan jumlah sampel menggunakan teknik simple random sampling dan ditemukan jumlah sampel sebanyak 22 pasien. Kualitas hidup pasien akan diukur menggunakan kueisoner EORTC QLQ CR-29 yang telah divalidasi dan diisi langsung oleh pasien dengan panduan peneliti. Data yang dikumpulkan diperiksa,dan diolah dengan bantuan program komputer, dianalisis dengan analisis sederhana dan disajikan dalam bentuk tabel dengan kategori baik, cukup, dan kurang. Hasil: Didapatkan hasil dari 22 sampel, 21 diantaranya menunjukan kualitas hidup baik dan 1 sampel dengan kualitas hidup cukup. Kesimpulan: Ditemukan gambaran kualitas hidup yang baik pada pasien kanker kolorektal dengan stoma temporer di RSUD dr. Moewardi Surakarta.

Abstract

Background: Conjunctival tumor patients tend to exhibit a compromised health-related quality of life (HR-QoL) as a consequence of their condition.Objectives: To explore the quality-of-life (QoL) of conjunctival tumor patients and its association with psychological distress in Indonesia.Methods: A cross-sectional study with simple random sampling was used. Data collection occurred from May 9 to August 31, 2024. The health-related dimensions of quality of life were measured utilizing the reliable RAND SF-36, while psychological distress was assessed with the Hospital Anxiety and Depression Scale (HADS). We performed statistical analyses using version 26 of IBM SPSS, with normality assessed via skewness.Results: A total of 260 conjunctival tumors completed health assessments, revealing significant psychological distress and the RAND SF 36 questionnaire. The mean age was 51, 52.3% male, and 86.2% married. Participants had an average HADS anxiety score of 9.52, with 60.8% exhibiting anxiety, notably higher in those with malignant tumors (mean score: 10.64) compared to benign tumors (mean score: 8.57, p<0.001). Depression scores averaged 6.97, with 37.7% showing potential mild disorders. Health-related quality of life (HR-\QoL) was appraised across various domains, alongside overall physical functioning, averaging 60.76; benign patients scored significantly higher (71.95) than malignant patients (47.50). Correlations indicated that higher psychological distress negatively impacted QoL, with anxiety showing a strong correlation coefficient of -0.433 and depression at -0.306 (both p<0.001).Conclusion: Psychological distress is prevalent in conjunctival tumor patients, adversely affecting their QoL. These findings highlight the necessity of targeted interventions to enhance both physical fitness and mental wellness—challenges in this population. Further research is essential to develop effective support strategies tailored to their needs.

Abstract

Background: The Short Form 36 (SF-36) questionnaire was used to measure quality of life (QoL) in Indonesia for patients with various conditions. However, its validity and reliability, specifically for patients with conjunctival tumors, remain understudied. Given the limited research on health-related quality of life (HR-QoL) in this patient population, our research aims to fill this gap by validating the SF-36 specifically for conjunctival tumor patients. Method: This observational, cross-sectional study recruited patients with conjunctival tumors receiving treatment at the Department of Ophthalmology, Dr Moewardi Hospital, between May and July 2024, using consecutive sampling. Participants completed the SF-36 questionnaire in the Indonesian context. Validity was assessed through bivariate correlation analysis, examining the Pearson correlation value for each item's questionnaire with the total QoL score. Reliability was determined using scale reliability analysis, focusing on Cronbach's alpha coefficient. The data was processed using IBM SPSS version 26.Results: The Pearson correlation coefficients for the 36-item scores range from 0.345 to 0.797, indicating a moderate to significant correlation. The scale-level Cronbach's alpha for the comprehensive tool is 0.942, suggesting excellent internal consistency. The SF-36 demonstrated robust reliability across its eight domains, confirming its suitability for measuring QoL in this demographic. These findings confirm that the SF-36 questionnaire in the Indonesian context is highly reliable and has construct validity in conjunctival tumor patients.Conclusion: The SF-36 questionnaire in the Indonesian context exhibits a satisfactory reliability scale and construct validity in assessing the QoL of conjunctival tumor patients. Further research with a larger participant pool is recommended.

Abstract

Propolis is an herbal medicinal plant that contains caffeic acid phenethyl ester. This natural polyphenol compound acts as an antioxidant and can reduce blood glucose levels and increase body weight. This study aims to determine the effectiveness of Gunung Lawu propolis in lowering blood glucose levels and increasing body weight in diabetic rats. The Wistar rats were induced with STZ 45 mg/kg BW and nicotinamide 110 mg/kg BW as diabetic model rats. Research subjects were taken randomly and divided into five groups: (1) Normal, (2) DM with no propolis, (3) DM 14 days + propolis 100 mg/kg BW/day (P1), (4) DM 14 days + Propolis 200 mg/kg BW/day (P2), and (5) DM 0 day + Propolis 200 mg/kg BW/day (P3). Propolis extract was given orally once a day for 14 consecutive days (in groups 3 and 4) and 28 consecutive days (in group 5). Blood glucose levels in the treatment group (P1, P2, and P3) decreased significantly (P1: 115.28 ± 4.7 mg/dL; P2: 98.36 ± 4.8 mg/dL; and P3: 87.36 ± 4, 2 mg/dL) compared with that in the DM group (272.07 ± 3.9 mg/dL). The body weight of the treatment group (P1, P2, and P3) increased significantly (P1: 180.60 ± 5.7 g; P2: 180.60 ± 4.2 g; and P3: 208.00 ± 5.1 gr) compared with that in the DM group (160.00 ± 3.2 gr). In conclusion, Propolis is effective in reducing blood glucose levels and improving (increasing) body weight in diabetic model rats.

Abstract

BACKGROUND As quality of life (QoL) research continues to expand, there remains a study gap of less common malignancies such as conjunctival tumors. This study aimed to investigate the differences in QoL between Indonesian patients with benign and malignant conjunctival tumors. METHODS This cross-sectional study was conducted from May–September 2024 and included 273 patients diagnosed with conjunctival tumors. The participants provided informed consent and completed the RAND SF-36 questionnaire to assess their QoL. A consecutive sampling method was employed to adequately represent benign and malignant tumor types. Data were analyzed using IBM SPSS version 26 to identify QoL differences among the patients. RESULTS This study found that squamous cell carcinoma was the most frequently observed conjunctival tumor, accounting for 28.9% of the cases. Patients with benign tumors had significantly higher QoL scores (69.61) than those with malignant tumors (43.05). Key factors influencing QoL included tumor type (p<0.001), age (p<0.001), sex (p = 0.039), occupation (p = 0.027), residence (p = 0.044), income (p = 0.010), and comorbidities (p = 0.045). CONCLUSIONS The results show significant disparities in QoL between patients with benign and malignant conjunctival tumors, emphasizing the impact of sociodemographic and tumor type on patient well-being. Focused healthcare plans are needed to help improve the QoL of patients with conjunctival tumors. Prioritizing early detection and providing complete support to enhance patient outcomes in Indonesia are crucial.
