Abstract

Posterior tibial slope (PTS) has been proposed as a potential risk factor for ACL injury; however, studies that examined this relationship have provided inconclusive and sometimes contradictory results. Further characterization of this relationship may enable the medical community to identify individuals at greater risk for ACL injury and possibly characterize an anatomic target during surgical reconstruction. Medical records of 20 patients who underwent MRI of the knee and athroscopy procedure between August 2016 and March 2017 were reviewed. Patients were separated into 2 groups: a study group of those subjects who had undergone surgery for ACL injury (n =10) and a control group of patients diagnosed without ACL Injury (n = 10). Demographic data were collected, and MRI images from both groups were analyzed using protactor android imaging software to obtain PTS measurements. Data then analyzed using SPSS v20. Data analysis demonstrated that the ACL injury group had significantly greater values for PTS (14.4 SD ± 3.5) compared with controls (10.1 SD ± 2.6). After stepwise elimination of non significant variables, the final t test independent determined that PTS (p value 0.08; p < 0.1) had statistically significant relationships with ACL injury. INW ratio, age, and sex were not demonstrated to be significant predictors of ACL injury in this final model. This study showed a relationship between increased PTS and ACL injury, which corroborates the findings of previously published studies. INW ratio, sex, and age showed no significant relationship with ACL injury.

Abstract

Background: Osteoporosis prevalence in Indonesia is high. Risedronate commonly used for treating the osteoporosis. Unfortunately, long term therapy can cause hazardous adverse effect to the patient. Previous research in rat model osteoporosis, Zinc have a potential effect to prevent and rebuild bone structure with osteoblast stimulation in bone formation and osteoclast activity inhibition in bone resorption by increasing Osteoprotegerin production, that inhibit bond between RANKL and RANK.Purpose: To determine the effectiveness of Zink and Risedronate on bone turnover in osteoporotic model rats assessed the β-CrossLaps expression.Methods: This research is conducted as experimental comparative method used 25 female Galur Wistar rat that divided into Non-ovariectomy and Ovariectomy group to induce osteoporosis. The treatment is divided into Zinc-Risedronate, and Zinc, or Risedronate only. One Way Anova and Independent T-test was used to analyzed the significant difference between groups.Result: One Way Anova statistic test forβ-CrossLaps expression value in the 3rd Taken blood sample showed that p is less than 0.05 (p<0.05), which indicated that are significant difference in the mean between variable at 3rd Taken blood sample in all group. Meanwhile, Independent T-test was used to compare β-CrossLaps expression in 3rd taken blood sample between Ovari and Ovari+Zinc, Ovari and Ovari+Zinc+Rise showed p is less than 0.05 (p<0.05), which indicated that there are significant difference in the mean between variable 2 groups.Conclusion: Zinc-Risedronate administration will decrease bone turnover and β-CrossLaps expression more than Zinc or Risedronate only in rat model osteoporosis.

Abstract

Introduction. Reference value to determine the angle of lower extremity is only based on clinical measurement and radiological assessment, which is limited to tibiofemoral angle (TFA). Although this examination can estimate the lower extremity angle, it is not satisfactory for a comprehensive analysis. Material and Method. A descriptive study in RSUP Haji Adam Malik Hospital in August - September 2019 to measure mechanical lateral distal femoral angle (MLDFA), medial proximal tibia angle (MPTA), and mechanical axis deviation (MAD). Results. Thirty nine subjects were included in this study. The mean age was 26.77±4,65 years old (range: 22 to 39 years); 69,2% were male (n = 27) and 30,8% were female (n = 12). The average mechanical lateral distal femoral angle (MLDFA) was 87,93o±2,16o. The average medial proximal tibia angle (MPTA) was 86,28o±2,26o. The average mechanical axis deviation (MAD) was 1.56±1,48 mm. Our results of MLDFA and MPTA measurement, but not in MAD, are consistent with study conducted by Farr, et al. Conclusion. Our MLDFA, MPTA, but not MAD measurement results are similar to studies involving Caucasian population. Pendahuluan. Nilai acuan sudut pada ekstremitas bawah hanya berdasarkan pemeriksaan klinis dan radiologis, yang terbatas pada sudut tibiofemoral. Pemeriksaan sudut tibiofemoral (STF) tunggal tidak cukup untuk analisis komprehensif ekstremitas bawah. Bahan dan Cara. Penelitian deskriptif di RSUP Haji Adam Malik pada bulan Agustus – September 2019 untuk mengukur sudut mekanik lateral distal femur (SMLDF), sudut medial proksimal tibia (SMPT), dan deviasi aksis mekanik (DAM). Hasil. Sejumlah 39 subjek diteliti. Usia rata-rata 26,77 ± 4,65 tahun (22 - 39 tahun); 69,2% pria (n = 27) dan 30,8% wanita (n = 12). Nilai rata-rata sudut mekanik lateral distal femur (SMLDF) adalah 87,93º ± 2,16º. Nilai rata-rata sudut medial proksimal tibia (SMPT) adalah 86,28º ± 2,26º. Nilai rata–rata deviasi aksis mekanik (DAM) adalah 1.56 ± 1,48 mm. Pada penelitian ini, hasil pengukuran SMLDF dan SMPT sesuai hasil penelitian Farr, et al, tetapi hasil pengukuran DAM tidak sesuai. Simpulan. Nilai SMLDF dan SMPT pada penelitian ini tidak berbeda dengan penelitian pada populasi Kaukasia.

Abstract

Osteoporosis ditandai dengan pengurangan massa tulang dan gangguan mikro-arsitektur jaringan tulang, mengakibatkan tulang menjadi rapuh dan meningkatkan risiko fraktur. Terapi osteoporosis meliputi perubahan gaya hidup dan obat-obatan yang dapat meningkatkan kekuatan tulang. Zink berpotensi mengurangi proses penyerapan tulang dan merangsang pembentukan tulang. Penelitian lebih lanjut dibutuhkan untuk mengetahui efektivitas zink dalam tatalaksana osteoporosis. Osteoporosis is characterized by bone mass reduction and bone tissue micro-architectural disorders that result in brittle bone and increased risk of fracture. Osteoporosis therapy includes lifestyle changes and drugs to increase bone strength. Zinc has the potential to decrease bone resorption and stimulate bone formation. Further study are needed to determine the efectivity of zinc is osteoporosis therapy.

Abstract

Carrier screening for early detection of thalassemia in the general population needs a careful approach, especially in areas with limited health services. Various erythrocyte indices have been introduced as options for the first stage; however, the low-reliability value of these erythrocyte indices remains the problem. This study aimed to determine the most reliable index for screening beta-thalassemia carriers and distinguish it from iron deficiency anemia. A cross-sectional study was designed to explore thalassemia carrier status among medical students. Inter-rater reliability value of various indices was compared to Shine and Lal index. The Cohen's Kappa coefficient was calculated using SPSS v. 25.0. Among 320 respondents, 295 subjects were non-anemic and 25 were anemic subjects, whereas 105 subjects had low MCV and/or MCH values. Cohen's Kappa value showed moderate reliability results compared to Shine and Lal index for example Mentzer index (0.58), Ehsani index (0.57), Srivastava index (0.53), and Bordbar index (0.41), but showed very low-reliability results with Green and King index (0.04). New cut-off indices based on Kumar et al. were also compared, resulting in moderate reliability results. Since there was no Hb-electrophoresis test, the sensitivities and specificities of those indices could not be calculated. For this reason, a complete blood count can only be used for the early stages of screening for beta-thalassemia carriers, whereas Hb-electrophoresis and DNA tests were considered necessary to perform to confirm a diagnosis.

Abstract

Osteoporosis is a condition of decreased bone mass density. Pharmacological management uses drugs that decrease bone resorption or increase bone formation. The most commonly used drugs are bisphosphonates. Risedronate is one of these bisphosphonates. It inhibits osteoclasts, resulting in osteoclast apoptosis. Denosumab is a human monoclonal antibody that inhibits receptor activator kappa-B ligand, inhibiting osteoclast activation. Both drugs are widely used. Research on the efficacy of these two mechanisms has yet to obtain conclusive results. β-cross-laps is a parameter that can be used to evaluate the effectiveness of therapy through monitoring the bone resorption process. This experimental study used female rats >9 weeks old and was conducted at the Pharmacology and Therapeutic Laboratory, Universitas Padjadjaran Bandung, Indonesia, from  June to September 2021. This study utilized a simple random sampling to allocate 24 experimental animals into three groups: control, risedronate, and denosumab. β-Crosslaps expression values before ovariectomy, post ovariectomy before receiving medication, and post ovariectomy and receiving medication was recorded and statistically analyzed using the SPSS version 24.0. The analysis of 24 samples revealed a statistically significant decrease in the median value of β-Crosslaps after ovariectomy in the denosumab group (p=0.036) when compared to the control group, whereas the decrease in the risedronate group was not significant (p=0.687). Administration of denosumab in rat models is more effective in reducing bone resorption compared to risedronate.

Abstract

Objective: Midfacial fracture is a fracture that separates maxilla from the middle part of face. Orbital floor fracture includes in midfacial fracture that difficult to treat. The aggravating factor of the treatment are depressed fractures and infection. We use ballooning technique which is inflated in the maxillary sinus to lift fracture segment of orbital floor and to prevent the orbital floor displaced to the maxillary sinus.Methods: There were multiple lacerations at left eye and nose region, post suturing at left zygoma and left cheek region. Intraoral findings there were multiple laceration, then the patient was examined a complete blood count, chest xray, cervical lateral examination, and 3D CT-Scan examination.Results: Examination of head and panoramic xray was performed and showed there was no bone discontinuity, contact and stable fracture segments. Examination of the 5th day post operative showed minimal pus drainage, leukocytes were within normal limits and other lab results were normal. Physical examination showed minimum scar, there was no dehiscence, symmetrical face and drain was performed at left zygoma region, symmetrical orbital, intact and stable occlusion.Conclusion: In this case, ballooning technique was successfully reduced the ruptured orbital floor and obtained good result.Keywords: Ballooning, Fixation, Open fracture, Orbital floor fracture.

Abstract

Kebocoran cairan dari terowongan atau portal merupakan masalah umum selama artroskopi lutut, yang berpotensi menyebabkan kekeruhan, gangguan visualisasi, dan waktu operasi yang lebih lama. Polytetrafluoroethylene (PTFE) adalah bahan yang dikenal karena sifatnya yang hidrofobik, tidak reaktif, tahan panas, dan tidak menyebabkan iritasi, sehingga banyak digunakan dalam aplikasi medis. Penelitian ini bertujuan untuk mengevaluasi efektivitas PTFE sebagai bahan penyumbat portal dan terowongan selama prosedur artroskopi. Penelitian eksperimental ini dilakukan pada 30 pasien yang didiagnosis mengalami cedera ligamen anterior cruciate (ACL), dan dijadwalkan menjalani artroskopi lutut di Departemen Ortopedi dan Traumatologi Rumah Sakit Hasan Sadikin (RSHS) Bandung, pada tahun 2024. Pasien dibagi menjadi dua kelompok: 15 orang menjalani operasi tanpa sumbat PTFE (Kelompok A) dan 15 orang dengan sumbat PTFE (Kelompok B). Usia rata-rata pasien adalah 24 tahun, dengan mayoritas berjenis kelamin laki-laki. Rata-rata penggunaan larutan NaCl pada Kelompok A dibandingkan Kelompok B adalah 13,3 vs. 7,767 liter. NaCl yang dikumpulkan melalui mesin hisap adalah 8,633 vs. 5,333 liter, dan NaCl yang ditemukan di lantai adalah 4,7 vs. 2,467 liter. Uji normalitas menunjukkan bahwa data terdistribusi normal (p > 0,05). Uji T tidak berpasangan menunjukkan perbedaan yang signifikan antara kelompok (p < 0,05). Penggunaan sumbat PTFE secara signifikan mengurangi kebocoran cairan dan meningkatkan efisiensi prosedur bedah. Keuntungan tambahan termasuk distensi sendi yang lebih baik, pengurangan kebutuhan akan torniket, dan efek tamponade terhadap perdarahan. Sumbat PTFE menawarkan alternatif yang hemat biaya dibandingkan dengan kanula silikon atau artroskop yang tidak memiliki fitur anti-balik cairan.
