Abstract

Background: Linear verrucous epidermal nevus is the most frequent variant of the epidermal nevus classification. Linear verrucous epidermal nevus is characterized by the proliferation of epithelium arranged in a configuration that follows Blaschko's line. Nevus unius lateris (NUL) is a variant of the verrucous epidermal nevus, which has a unilateral distribution of lesions. Lesions are usually found at birth or in the first year of life as brown to grey verrucous papules or papillomatous plaques. The management of NUL is challenging as the results are varied and there is a high risk of recurrence. Purpose: to report a case of NUL and its management, especially in children. Case: A 3-year-old girl presented with brownish spots and multiple small lumps on the left buttock that have extended to the left leg since she was 9-days-old. On dermatologic examination, there were numerous hyperpigmented verrucous papules and plaques along the Blaschko line over the affected area. In this case, the diagnosis of NUL, is based on clinical symptoms and dermoscopy examination showed multiple large brown oval or round structures with hyperpigmented brown border. The patient was treated with combination topical therapy of tretinoin 0.025%, corticosteroid desoxymethasone 0.25%, and urea 20% cream, and the lesion improved within four weeks. Discussion: Epidermal nevus is often cosmetically disturbing. The treatment is still challenging and various, including surgical and non-surgical, but none is ideal and could potentially recur over months or years. dst.

Abstract

Background: Purple sweet potato has been shown to contain anthocyanins, polyphenols, flavonoids, and tannins that act as antioxidants and have the potential to correct signs of aging in the skin, such as wrinkles, lack of hydration, and pigmentation. Purpose: to known the potential of 8% purple sweet potato extract for wrinkles, moisture, and elasticity. Methods: A one-group experimental study with pre and post-test assessment. The subject is women with photoaging aged 30-45 years. A pre-test was conducted by examining wrinkles using the Glogau scale and a simple A-one scanner, treatment with 8% purple sweet potato extract cream for 6 weeks, and then a post-test at the end of the 6th week to check wrinkles, moisture, and elasticity using a simple A-one scan. Statistical tests using the SPSS program, version 25.0. The analysis performed was descriptive statistical and comparative analysis using a paired T test.  Result: A total of 25 respondents with a mean age ± SD of 38.22 ± 4.23 years, the most Glogau scale in type 2 is 22 respondents (88%), the most sun exposure for 30 minutes per day is 14 respondents (56%). The degree of wrinkles decreased after treatment -4.960±1.353 [95% CI -7.681-(-2.239); P=0.001]. The degree of moisture increased after treatment 8.840±2.990% (95% CI 2.829-14.851; P=0.005. The degree of elasticity increased after treatment 14.840±4.623 (95% CI 5.544-24.136; P=0.002). Conclusion: 8% purple sweet potato extract can reduce the degree of wrinkles, improve the degree of skin moisture, and improve the degree of skin elasticity.

Abstract

Background Leprosy is a chronic progressive infectious disease. Phenolic glycolipid-1 (PGL-1) is an antigen of Mycobacterium leprae, which can trigger the host’s antibody response. Transforming growth factorβ (TGF-β) plays an immunosuppressive role when the host is exposed to PGL-1 antigen or other M. leprae antigens. This study aims to determine the correlation of TGF-β levels with IgM anti-PGL-1 levels in patients with multibacillary leprosy household contacts. Methods Observational analytical study with a cross-sectional approach. The study subjects consisted of 48 household contact subjects and 24 non-household contact subjects aged 15-65 years old who were selected by consecutive sampling based on inclusion and exclusion criteria. 3 mL of venous blood samples were taken and then examined for IgM anti-PGL-1 and TGF-β with the ELISA kit. Data analysis was carried out using SPSS version 23, and a p-value <0.05 was significant. Results The mean level of IgM anti-PGL-1 in the household contact group was 685.46 ± 290.79 u/mL, while in the non-household contact group was 345.50 ± 206.58 u/mL. The mean TGF-β level in household contact groups was 256.69 ± 127.41 pg/mL, while in the non-household contact group was 144.85 ± 36.73 pg/mL (p<0.001). This study found a moderate positive relationship (r=0.450, p<0.001) between levels of TGF-β and IgM anti-PGL-1 household contacts and non-household contacts group. Conclusions The mean level of IgM anti-PGL-1 and TGF-β in household contacts is higher than in non-household contacts, with a significant difference. There is a moderate positive significant relationship between levels of TGF-β and IgM anti-PGL-1 household contacts and non-household contacts group.

Abstract

Leprosy infection can cause anemia due to chronic disease (ACD). Multidrug therapy (MDT) for leprosy patients can overcome the infection. This study aims to determine the comparison in the incidence of ACD before and three months after MDT to leprosy patients at Prof. Dr. I G.N.G. Ngoerah Central General Hospital. A cross-sectional analytical observational study was conducted using secondary data from medical records of leprosy patients. A total of 142 samples were taken through total sampling. Data taken included gender, domicile, type of leprosy suffered, incidence of ACD, hemoglobin levels, and erythrocyte index before and three months after completing MDT leprosy therapy. Data analysis was carried out with SPSS version 21 using chi-square and paired t tests (dependent t-test), p value <0.05 means significant. The incidence of ACD was significantly higher before leprosy MDT therapy compared to 3 months after completing leprosy MDT therapy (49.3% vs. 16.2%; p value <0.001) with a prevalence ratio of 3.043 (2.021-4.584) CI95%. The mean hemoglobin levels, MCV, MCH, and MCHC levels 3 months after MDT leprosy therapy were significantly higher than before receiving MDT therapy (13.73 ± 1.47 vs. 12.40 ± 2.06 g/dL; p value <0.001), (88.18±7.22 vs. 84.07±9.55 fl; p value<0.001), (28.32±2.76 vs. 26.72±3.17 pg/cell; p value <0.001), (32.03±1.30 vs. 31.69±1.47%; p value<0.001). The incidence of ACD was significantly higher before MDT compared to 3 months after MDT. Mean hemoglobin, MCV, MCH, and MCHC levels were found to be significantly higher 3 months after MDT compared to before receiving MDT.

Abstract

ABSTRACT Acne vulgaris (AV) is a common disorder of the pilosebaceous, especially prevalent in adolescents. Acne vulgaris ranks among the top ten most prevalent diseases globally. Although AV may resolve spontaneously, sequelae such as scar formation can persist, causing aesthetic and psychological disturbances. There is an antioxidant to counteract acne-causing oxidative stress, vitamin E, which is an important antioxidant in the human body. Therefore, vitamin E in the skin can reduce the formation of squalene peroxide and prevent the occurrence of AV. This is an analytical observational study using a cross-sectional design. The investigation was conducted at the Dermatology and Venereology Clinic of Prof. Dr. I.G.N.G. Ngoerah Denpasar Hospital and the laboratory of the Faculty of Medicine, Udayana University, from February 2024 until the sample size was fulfilled. The sample consisted of patients aged 12-45 years with acne vulgaris who visited the clinic and were then selected through consecutive sampling according to the inclusion and exclusion criteria. In the non-AV group, the mean serum vitamin E level was 19.3 ± 0.99 µg/ml, whereas in the AV group, it was 4.49 ± 19.1 µg/ml, indicating a significant variance. There was a positive correlation between low vitamin E levels and the severity of AV. The linearity graph shows the direction of the relationship between serum vitamin E levels and the severity of AV, which leads to the lower right, indicating that the higher the severity of AV, the lower the serum vitamin E levels. The mean levels differed significantly between the non-AV and AV groups. Abstract

Introduction: Onychomycosis is a fungal infection of the nails that causes discoloration, thickening, and separation from the nail bed. Onychomycosis is caused by various organisms, most often dermatophytes of the genus Trichophyton. Other organisms include Candida, which is more common in fingernail infections and patients with chronic mucocutaneous candidiasis. Nondermatophyte molds are a less common cause in the general population. This study aimed to identify sex, age, classification, fungal culture, treatment, and incident of secondary infection in onychomycosis at Dermatology Outpatient Department at Sanglah General Hospital Denpasar Periods January 2016 – December 2017. Methods: Study design using descriptive retrospective model done by taking data from medical record in Dermatology Outpatient Department at Sanglah General Hospital Denpasar Periods 2016-2017. Result: There were 268 patients in Dermatovenereology Outpatient Department at Sanglah General Hospital Denpasar, 29 (10,82%) of whom were diagnosed with onychomycosis in Periods 2016-2017. According to age group, the most cases occur in 26-65 year age groups. Females were more frequently affected than males. The most classification finding is distal and lateral subungual onychomycosis and the most commonly isolated fungi were yeasts from the Candida species. The most commonly given therapy is a combination therapy of systemic antifungal with topical. Fluconazole is the most common drug that uses orally and ciclopirox as topical agent. Conclusions: Onychomycosis is one of the common fungal infection observed in patient attending dermatology outpatient department with the most classification finding is distal and lateral subungual onychomycosis. The most commonly isolated fungi were Candida sp.

Abstract

Superficial fungal infections are one of the most common skin infection infections. It is caused by fungal pathogens and is limited to the outer layer of the skin, hair, and nails. Superficial fungal infections include dermatophytosis, superficial candidiasis, and diseases caused by Malassezia spp. Most physicians diagnose and treat superficial fungal infections based solely on clinical appearance. But unfortunately, there are many other infectious and non-infectious diseases with a similar clinical appearance. Dermoscopy is a practical and noninvasive imaging method that allows magnifying clinical surface images that are normally invisible under a magnifying lens. In diagnosing superficial mycosis, dermoscopy was more sensitive and specificity than potassium chloride (KOH) and fungal culture. More understanding is needed about the use of dermoscopy in diagnosing superficial fungal infections in the hope of providing an overview of superficial fungal infections and dermoscopy, as well as dermoscopy features in some superficial fungal infections. This article describes dermoscopic findings in various superficial fungal infections on the skin, hair, and nails.
